The Impact of Household Preparedness and Work-Family Conflict on Medic-Firefighter Ability and Willingness to Respond for Duty in a Disaster
Get PDFThere is a general assumption in disaster planning that emergency responders will respond for duty in any emergency or disaster. Role abandonment by medic-firefighters could impede an organization’s ability to effectively manage the impact of a disaster. This study investigated household preparedness and work-family conflict levels, identified as primary variables affecting ability and willingness to respond in the literature. The aim was to determine if these factors impacted the ability and willingness of medic-firefighters in a coastal Virginia fire department to respond during disaster scenarios. The results showed that emergency preparedness levels were low, and that there were moderate levels of work-family conflict. The study participants were given three disaster scenarios and asked separately their ability and willingness to respond. In response to questions on ability, the majority of participants reported that they would have the ability to respond to two of the three disaster scenarios and the majority would be willing to respond to all scenarios. There was no correlation found between ability and willingness to respond and levels of preparedness or work-family conflict. While household preparedness and work-family conflict are important considerations, they may not be the primary drivers of response behavior within this professional context. Organizations need to understand the factors that influence response behavior in a specific context, so interventions and policies can be better tailored to enhance the readiness and effectiveness of emergency responders

Standardization of Chlorhexidine Gluconate Bathing: A Quality Improvement Project in Central Line Bloodstream Infection Prevention
Get PDFBackground: Central Line-Associated Bloodstream Infections (CLABSIs) are severe healthcare-related infections that can lead to sepsis, organ failure, and even death. Several healthcare agencies provide guidelines to prevent CLABSIs, with Chlorhexidine Gluconate Bathing (CHGB) as one of the preventive measures for individuals with central venous lines (CVL).
Purpose: The Doctor of Nursing Practice (DNP) project aims to standardize CHGB procedures for maintaining CVLs on a specific unit for CLABSI prevention through evidence-based practice.
Methods: A quality improvement project was implemented at a Central Alabama hospital to standardize CHGB procedures for CVL patients utilizing a CLABSI unit champion monitoring system to establish guidelines. Over eight weeks, the DNP student audited 28 individuals admitted to the specified unit with CVL.
Results: The daily CHGB across the identified ward remains consistent, indicating effective implementation. As a result of the intervention, no CLABSIs were identified during the 8-week Quality Improvement implementation period, demonstrating the effectiveness of the standardized CHGB protocol.
Conclusion: The project evaluation confirmed the efficacy of daily CHGB, highlighting their potential to reduce CLABSI rates, and recommended their widespread use to improve patient outcomes
A Quality Improvement Project: Improving the Clinical Practice for Assessing Hemoglobin A1C on Admission in Diabetic Ketoacidosis Patients Admitted to the Intensive Care Unit
Get PDFBackground: Diabetic ketoacidosis (DKA) is a life-threatening problem that affects people with diabetes. Early treatment and adherence to guidelines are crucial in preventing DKA. The American Diabetes Association (ADA) publishes annual guidelines for effective DKA management, and the Education Recognition Program National Benchmarks program can help maintain adherence to the published guidelines. Improving the intensive care unit (ICU) admission process enhances the quality of care to DKA patients and improves progress towards national benchmarks.
Purpose: This quality improvement project aimed to improve Hemoglobin A1C (HbA1c) evaluations in DKA patients admitted to the ICU.
Methods: The intervention of triggering HbA1c orders on patients admitted to the ICU with DKA was supported by Roy\u27s Adaptation Model theoretical framework and the Leveraging Resources Model methodology. To implement this, a new HbA1c order was created in the electronic health record (EHR) to be triggered by providers when DKA patients are admitted to the ICU. The project compared no-HbA1c orders vs triggered HbA1c orders.
Results: The EHR prompt improved orders for HbA1c tests for patients with DKA admitted to the ICU, leading to a 2.5% increase in ADA compliance within three months.
Conclusion: Adding a prompt to the electronic health record is a cost-effective and efficient strategy for meeting national benchmarks in DKA management.

A Tale of Two Turtles: Sympatry of Two Lotic Musk Turtle Species in the Pascagoula River Drainage
Get PDFCompetition and hybridization between closely related species have remained topics of interest for decades. The ranges of the Razorback Musk Turtle (Sternotherus carinatus) and Stripe-necked Musk Turtle (Sternotherus peltifer) are mostly non-overlapping, but they are sympatric in south and central Mississippi, USA. In allopatry, each species fills the niche of a lotic, bottom-walking musk turtle and each can range in habitat from small, montane headwater streams to large, muddy bayous in the coastal plain. Using environmental, morphological, and genetic data, we assessed whether the two species exhibit different habitat associations, differ in morphology, and maintain reproductive isolation in sympatry. The two species differed significantly in habitat association in sympatry, with the Stripe-necked Musk Turtle occurring primarily in small streams or headwater stream habitats and the Razorback Musk Turtles frequenting larger riverine habitats. The two species occurred in syntopy in intermediate habitats, and while we detected three individuals with mixed ancestry, hybridization is likely precluded by both pre- and postzygotic barriers, notably the significant differences in body size between species and the reversed sexual size dimorphism exhibited by each species

Dr. Millican ended her tenure at JSU in the 1980s as Dean of the College of Library Science and Instructional Media, but she was also a dorm matron, counselor, and a professor of Education earlier in her career
