Relationship Between Duodenal Histopathology and Strong Positive Tissue Transglutaminase Antibodies in Children with Celiac Disease
Introduction: In celiac disease (CD) strong positive tissue transglutaminase antibody (TTGA) levels (> 100 U/A) have been shown to almost always indicate villous atrophy. The aim of this study is to determine the sufficiency of >= 100 U/A Ig A type TTGA levels for diagnosis of CD. Materials and Methods: Results from duodenum biopsy performed due to positive TTGA in 197 children were retrospectively examined. IgA TTGA levels had a positive value of > 18 U/A. Increases of 5 times or more than this threshold value (>= 100 U/A) are accepted as strong positivity. CD diagnosis was made according to ESPGHAN criteria. A modified Marsh stage >= 2 was accepted as significant for CD. Results: Of the cases, 129 were female (65.5%) and 68 were male (34.5%). Duodenum histopathology was compatible with Marsh 0 for 1 case (0.5%), Marsh 2 for 17 cases (8.6%), Marsh 3a for 41 (20.8%), Marsh 3b for 81 (41.4%) and Marsh 3c for 57 (28.9%). The TTGA levels of 64 of the 197 cases (32.5%) were >= 100 U/A. In cases with strong positivity for TTGA the duodenum histology was compatible with Marsh 3 (villous atrophy) for 63 and Marsh 0 (normal histology) for 1 case (type 1 diabetic and asymptomatic for CD). For Marsh 3c TTGA levels >= 100 U/A had a sensitivity of 85.96% (95% CI: 74.2-93.7%), specificity of 89.29% (95% CI: 82.9-93.8%), positive predictive value of 76.56% (95% CI: 64.3-86.2%) and negative predictive value of 93.9% (95% CI: 88.4-97.3%). Conclusions: This study showed that positive IgA TTGA levels (>= 100 U/A) were almost always accompanied by Marsh 3 duodenal histopathological changes. Diagnosis of CD without biopsy may miss certain accompanying diseases, however in some cases with advanced examinations CD may be diagnosed by pediatric gastroenterology specialists without endoscopy

The Edinburgh feeding evaluation in dementia (EdFED) scale:A Turkish validity and reliability study
Aim: The present study aimed to adapt the Edinburgh Feeding Evaluation in Dementia (EdFED) Scale to Turkish. Method: This methodological study was reported using STARD (The STAndards for Reporting of Diagnostic Accuracy). The sample included 200 people with dementia. The data were obtained using the Personal Information Form, the Edinburgh Feeding Evaluation in Dementia Scale, and the Mini Nutritional Assessment Test. For the Turkish adaptation of the scale, construct validity (confirmatory factor analysis), criterion validity (concurrent scale validity) and reliability analysis (Cronbach's alpha coefficient, item-total score correlation) were performed, respectively. Results: The three-factor model in the original scale was verified. The content validity index was 0.95. The Cronbach's alpha coefficient factors were as follows: ( Indicators of Patient Difficulty' alpha = 0.81, 'Patient's Need for Assistance' alpha = 0.79 and 'Indicators of Feeding Difficulty' alpha = 0.64, respectively. When the CFA fit indexes were examined, the model fit values were good. The three-factor structure was verified, compared with the original model and was compatible. No modification was needed in the model. Conclusions: The Edinburgh Feeding Evaluation in Dementia Scale Turkish version provides reliable and valid measures of feeding difficulties in people with dementia. It has satisfactory psychometric properties and is suitable to use in clinical practice. Implications for practice: Feeding is one of the most neglected subjects in caring for people with dementia. Screening is recommended to evaluate feeding and malnutrition, but, to our knowledge, there is no tool/scale to evaluate the feeding of the dementia patient in Turkish. The EdFED scale can serve healthcare professionals and caregivers as a practical tool for feeding difficulties in people with dementia
FT-Raman, FT-IR, UV spectra and DFT and ab initio calculations on monomeric and dimeric structures of 3,5-pyridinedicarboxylic acid
The FT-IR and FT-Raman spectra of 3,5-pyridinedicarboxylic acid (dinicotinic acid, C7H5NO4) in the solid phase were recorded. The fundamental vibrational wavenumbers, intensities of vibrational bands and the optimized geometrical parameters of the compound are evaluated using HF and DFT (B3LYP) methods with 6-311G(d) basis set. The difference between the observed and scaled wavenumber values of most of the fundamentals is very small. The computed dimer parameters also show agreement with experimental data. The complete assignments were performed on the basis of the total energy distribution (TED). Stability of the molecule arising from hyperconjugative interactions, charge delocalization have been analyzed using natural bond orbital (NBO) analysis. UV-visible spectrum of the compound was recorded in the region 200-400 nm and the electronic properties HOMO and LUMO energies were measured by time-dependent TD-DFT approach. Thermodynamic properties (heat capacity, entropy and enthalpy) of the title compound at different temperatures were calculated. (c) 2012 Elsevier B.V. All rights reserved
Using the Satisfaction with Life Scale in People with Parkinson's Disease: A Validation Study in Different Europe Countries
Background. Overall, people with chronic illnesses have lower life satisfaction compared to nonclinical populations. The objective of this international study was to examine the psychometric properties of the Satisfaction with Life Scale (SWLS) in patients with Parkinson's disease (PD). Methods. PD patients (n = 350) were recruited and interviewed at different specialized services in the United Kingdom, Spain, Czech Republic, Italy, and The Netherlands. A questionnaire set including a measure of life satisfaction, quality of life (QoL), self-reported health and disability status, and sociodemographic information was used. Acceptability, reliability, and validity were examined. Results. The internal consistency was good (alpha = 0.81). The scale structure was satisfactory (comparative 'fa index = 0.99; root mean square error of approximation = 0.08). The SWLS was able to discriminate between healthy and unhealthy, disabled and nondisabled, and those perceiving a more severe impact of the disability on their lives. Concurrent validity using multiple linear regression models confirmed associations between SWLS and QoL and age. Conclusions. This study is the first to report on the use of the SWLS in PD patients in different European countries. It is a useful tool in assessing satisfaction with life in PD patients through the continuum of care
Effect of olive mill waste addition on the properties of porous fired clay bricks using Taguchi method
Production of porous clay bricks lightened by adding olive mill waste as a pore making additive was investigated. Factors influencing the brick manufacturing process were analyzed by an experimental design, Taguchi method, to find out the most favorable conditions for the production of bricks. The optimum process conditions for brick preparation were investigated by studying the effects of mixture ratios (0, 5 and 10 wt%) and firing temperatures (850, 950 and 1050 degrees C) on the physical, thermal and mechanical properties of the bricks. Apparent density, bulk density, apparent porosity, water absorption, compressive strength, thermal conductivity, microstructure and crystalline phase formations of the fired brick samples were measured. It was found that the use of 10% waste addition reduced the bulk density of the samples up to 1.45 g/cm(3). As the porosities increased from 30.8 to 47.0%, the compressive strengths decreased from 36.9 to 10.26 MPa at firing temperature of 950 degrees C. The thermal conductivities of samples fired at the same temperature showed a decrease of 31% from 0.638 to 0.436 W/mK, which is hopeful for heat insulation in the buildings. Increasing of the firing temperature also affected their mechanical and physical properties. This study showed that the olive mill waste could be used as a pore maker in brick production. The clinical assessment of lung involvement in patients with Still's disease, results from the multicentre international AIDA Network Still's Disease Registry
Objectives To assess the lung involvement in patients with Still's disease, an inflammatory disease assessing both children and adults. To exploit possible associated factors for parenchymal lung involvement in these patients.Methods A multicentre observational study was arranged assessing consecutive patients with Still's disease characterized by the lung involvement among those included in the AIDA (AutoInflammatory Disease Alliance) Network Still's Disease Registry. Still's disease-lung involvement was defined by the presence of pleuritis, parenchymal features, acute respiratory distress syndrome (ARDS) and/or pulmonary arterial hypertension.Results In total, 90 patients with Still's disease and lung involvement were assessed (mean age 36.3 +/- 17.8 years, 35.6% male sex). Among them, 13.3% of patients were paediatrics. These patients with lung involvement mainly showed pleuritis in 72.2% of cases, parenchymal features in 34.4%, ARDS in 9.5% and pulmonary arterial hypertension in 2.3%. After that we focused on patients characterised by parenchymal lung involvement, which is an emergent issue of clinical concern. These patients with parenchymal lung disease were significantly characterized by sore throat, pericarditis and higher values of systemic score than others. Finally, the administration of both IL-1 or IL-6 inhibitors was not associated with the presence of parenchymal lung involvement.Conclusion The clinical characteristics of patients with Still's disease and lung involvement were described in the AIDA network. We also provided a clinical profile of patients with parenchymal lung involvement considering its prognostic relevance. Although providing a clinical landscape of these patients, further studies are needed to fully clarify this issue
Oral administration of ruxolitinib in psoriasis vulgaris: A case report of plaque psoriasis accompanied by myelofibrosis secondary to polisitemia vera successfully treated with oral ruxolitinib
Psoriasis is a chronic inflammatory skin disease characterized by keratinocyte hyperproliferation and immune cell infiltration. Various therapies have been discovered for psoriasis, including topical treatments, phototherapy, conventional systemic agents such as methotrexate, retinoids and ciclosporine, as well as biologics. Janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway inhibitors targeting Tumor Necrosis Factor alpha (TNF-α), interleukin (IL)-23 and IL-17 can be effective in psoriasis. Ruxolitinib is a US Food and Drug Administration-approved first-generation Janus kinase inhibitor for polycythemia vera, myelofibrosis, and acute graft-versus-host disease. Ruxolitinib cream has been investigated in various dermatologic diseases, including atopic dermatitis, vitiligo, psoriasis, and alopecia areata. However, there is limited data on the efficacy of oral ruxolitinib in patients with psoriasis vulgaris. Here, we report a patient diagnosed with myelofibrosis coexisting with psoriasis vulgaris, successfully treated with oral ruxolitinib. © 2024 Japanese Dermatological Association
