Dynamics of HIV-1 infected population acquired via different sexual contacts route: a case study of Turkey
This paper aims to study the transmission dynamics of HIV/AIDS in heterosexual, men having sex with men (MSM)/bisexuals and others in Turkey. Four equilibrium points were obtained which include disease free and endemic equilibrium points. The global stability analysis of the equilibria was carried out using the Lyapunov function which happens to depend on the basic reproduction number R-0. If R-0 1, the endemic equilibrium point is stable and epidemics will occur. We use raw data obtained from Kocaeli University, PCR Unit, Turkey to analyze and predict the trend of HIV/AIDS among heterosexuals, MSM/bisexual, and others. The basic reproduction number for heterosexuals, MSM/bisexuals, and others was found to be 1.08, 0.6719, and 0.050, respectively. This shows that the threat posed by HIV/AIDS among heterosexuals is greater than followed by MSM/bisexuals, and than the others. So, the relevant authorities should put priorities in containing the disease in order of their threat
Knowledge Level of the Primary Healthcare Providers on Chronic Obstructive Pulmonary Disease and Pulmonary Rehabilitation
Introduction. Awareness of the healthcare providers on chronic obstructive pulmonary disease (COPD), which is an important cause of mortality and morbidity in our country and all over the world, and on pulmonary rehabilitation (PR) which plays an important role in its nonpharmacological treatment will provide effectiveness in diagnosis and treatment of COPD. The present study aimed at determining knowledge level of the healthcare providers about COPD and PR. Materials and Methods. In this cross-sectional study, family practitioners and staff of home-care in central county of Manisa City were applied a questionnaire in order to determine their knowledge level on COPD and pulmonary rehabilitation during the in-service training on pulmonary rehabilitation, homecare services for the pulmonary diseases, and respiratory exercises. Results. 65.5% of the healthcare providers responded to the survey. Rate of those correctly knowing at least one of four items was 97.2%. No responder knew all items correctly. Average value for correct answers was 5.30 +/- 2.1 (range: 1-10). The physicians, men, and those working in family health centers had higher level of knowledge on COPD compared to nonphysician healthcare providers (p = 0.006), women (p = 0.002), and those working in other practices (p = 0.019), respectively. Conclusion. Knowledge level of the primary healthcare providers on COPD and PR remains inadequate. Dynamic postgraduate training on this topic will be useful in referring the patients to centers giving service for this condition
Pediatric inverted papilloma of the middle ear: Case report and review of the literature
Inverted papilloma of middle ear is a very rare clinical entity. It has been reported only in the adult population. We report a middle ear inverted papilloma in an 11-year-old male. To the best of our knowledge it is the first pediatric inverted papilloma of middle ear. The patient had a history of chronic suppurative otitis media and the lesion was found during preoperative evaluation for a tympanoplasty procedure. Total excision of the lesion and a tympanoplasty operation was accomplished. We reviewed the middle ear inverted papilloma cases regarding the age distribution, pathophysiology, treatment and prognosis. (C) 2014 Elsevier Ireland Ltd. All rights reserved
A karyological study on taxa of Coronopus DC section of Plantago L. genus (Plantaginaceae) in Turkey
In this study the chromosome number and morphology of five taxa of the Coronopus D.C. section - Plantago coronopus subsp. coronopus L., P. coronopus subsp. commutata (GUSS.) PLGER, P. crassifolia FORSSKAL, P. maritima L., and P. holosteum SCOP. (Plantaginaceae) - in Turkey were investigated using karyological techniques. The seeds of taxa were collected from natural habitats. The chromosome numbers are determined for four taxa (P. coronopus subsp. coronopus, P. coronopus subsp. commutata, P. crassifolia and P. maritima) as 2n = 4x =20 and one taxon (P. holosteum) as 2n = 2x = 12. The chromosome numbers and karyotype analysis of the species are reported for the first time for Turkey
Total tongue reconstruction with free functional gracilis muscle transplantation: A technical note and review of the literature
The tongue is one of the most difficult structures of the body to reconstruct if more than 85% to 90% of it has been resected, A functional neotongue requires both soft-tissue bulk and restoration of muscle function, The innervated gracilis muscle flap folded on itself was used to reconstruct a total glossectomy defect. The genioglossus muscle, which makes the greatest contribution to swallowing, and the mylohyoid muscle, which elevates the larynx to prevent aspiration, were simulated with the design and placement of the gracilis muscle, The muscle was placed and secured longitudinally from the remnant of the tongue base and pharynx to the mandible, and then folded on itself and attached to the hyoid bone. The obturator nerve to the gracilis was coapted with the hypoglossal nerve. The patient was able to resume oral feeding without aspiration, Electromyographic results demonstrated reinnervation of the flap with active elevation of the mouth base, The patient died 1 year postoperatively because of recurrence of her disease. This type of design and placement of gracilis muscle transplantation is recommended
Designing energy-efficient high-precision multi-pass turning processes via robust optimization and artificial intelligence
This paper suggests a novel robust formulation designed for optimizing the parameters of the turning process in an uncertain environment for the first time. The aim is to achieve the lowest energy consumption and highest precision. With this aim, the current paper considers uncertain parameters, objective functions, and constraints in the offered mathematical model. We proposed several uncertain models and validated the results in real-world case studies. In addition, several artificial intelligence-based solution techniques are designed to solve the complex nonlinear problem. We determined the most efficient solution approach by solving various test problems. Then, simulated several scenarios to demonstrate the robustness of our results. The results showed that the solutions provided by the offered model significantly reduce energy consumption in different setups. To ensure the reliability of the results, we carried out worst-case sensitivity analyses and found the most critical parameters. The results of the worst-case analyses indicated that the offered robust model is efficient and saves a significant amount of energy comparing to traditional models. It is shown that the provided solution by the presented robust formulation is reliable in all situations and results in the lowest energy and the best machining precision
Seasonal Trends and Interactions of Viral Pathogens in Children Presenting with Acute Respiratory Tract Infections in the Advancing Periods of SARS-Cov-2 Pandemic
Although various bacteria and viruses have been identified in the etiology of acute respiratory tract infections (ARI), 90% of acute ARIs that develop in children are of viral origin. The aim of this study was to investigate the seasonal trends and interactions between infectious agents and to determine the risk factors associated with ARI in children aged 1-15 years admitted to the Pediatric Emergency Department of Manisa Celal Bayar University Hospital in the advancing periods of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) pandemic. To determine the bacterial and viral agents, samples were taken from 314 patients attending to the hospital with symptoms suggestive for ARI, between 06/01/2021 and 05/31/2022. Viral and bacterial agents were identified by multiplex polymerase chain reaction (PCR) and automated identification system, respectively. Demographic data of the participants and possible risk fac- tors for ARI were recorded in the questionnaires. In the study, viral agents were detected in 77.3% of the children, and the most common infectious agent was rhinovirus/enterovirus (RV/EV) (36.3%), followed by influenza viruses (11.2%), and SARS-CoV-2 (10.5%). While RV/EV positivity was found to be higher in children with moderate and below average (p< 0.001) hand hygiene, influenza positivity was found higher in those attending school/preschool institution (p< 0.001) and whose mothers working full-time (p< 0.001). Respiratory syncytial virus positivity was associated with maternal smoking (p= 0.013) and home overcrowding (p= 0.014). Bacterial colonization was detected in 33 (11.6%) of 284 children whose swabs were taken for both bacterial and viral agents and the most frequently detected agents were Staphylococcus aureus (60.6%) and Pseudomonas aeruginosa (15.2%). Having siblings (p= 0.008) and maternal smoking (p= 0.012) were found to be associated with the detection of bacterial agents. In this study, in the advanced period of the pandemic, the most detected agents and seasonal characteristics were found to be similar to the pre-pandemic period. It is thought that knowing the regional etiology and risk factors will contribute to taking the necessary local control and protective measures
Two-phase mixture modeling of turbulent forced convective flow of water-silver nanofluid inside a rifled tube: hydrothermal characteristics and irreversibility behavior
This work aims to present a numerical evaluation of the turbulent forced convective flow of water-silver nanofluid (NF) inside a horizontal internally spiral-ribbed heat exchanger tube using the two-phase mixture model. The considered NF is prepared using an environmentally friendly method. The influences of the volume concentration of silver nanoadditives (phi), Reynolds number (Re), rib width (W), rib height (H) and rib pitch (P) on the convection coefficient, pressure loss, irreversibility in fluid friction ((S) over dot(g,f)) and heat transfer ((S) over dot(g,f)) and total irreversibility ((S) over dot(g,f)) are assessed. The outcomes disclosed that the hydrodynamic characteristics and irreversibility behavior of NF flow in the ribbed tube are better than that of the water flow in the plain tube. In addition, it was found that the best first-law performance belongs to the case phi = 1%, Re = 5000, W = 2 mm, H = 2 mm, P = 4 mm, while the best second-law performance occurs at phi = 0%, Re = 20,000, W = 2 mm, H = 1 mm, P = 5 mm
A Promising, Novel Radiosensitizer Nanodrug Complex for Oral Cavity Cancer: Cetuximab and Cisplatin-Conjugated Gold Nanoparticles
Background: Nanomedicine has provided promising tools for the imaging, diagnosis, and treatment of cancer. Gold nanoparticles (GNPs) may be useful in enhancing the efficacy of radiotherapy, such as radiosensitization, in cancer therapy. Aims: To develop a nanodrug complex containing cetuximab (C225, CTX) and cisplatin (CDDP) conjugated with GNPs and to investigate its cytotoxic effects on oral cavity cancer cells when combined with radiotherapy. Study Design: In vitro cell culture study. Methods: The GNPs were synthesized and successfully conjugated with cetuximab and cisplatin. Cell viability was monitored by the xCELLigence real-time cell analysis (RTCA) single-plate (SP) system in GNP-treated UPCI-SCC-131 cells for 48 hours. Cells with/without GNPs were irradiated with 6 MV X-rays, and colony formation was assayed to investigate the long-term effects of GNPs and the nanodrug complex after irradiation on radiotherapy-resistant oral cavity cancer cells. Results: The GNPs entered the tumor cells, and GNP-CDDP (P<.0001) and GNP-CDDP-CTX (P<.0001) were shown to cause a decrease in cell viability. GNP and GNP-CTX combined with radiotherapy led to greater reduction on UPCI-SCC-131 colony numbers, than radiation alone (P=.0369) and radiation with free CTX, with sensitizing enhancement ratios of 1 : 2 and 1 : 9, respectively. Conclusion: The cetuximab and cisplatin-conjugated gold nanodrug complex has a great potential to increase cytotoxicity and overcome resistance to radiotherapy, in the treatment of oral cavity cancer
