Intubation of obese patients in the operating room with or without bag-mask ventilation: study protocol for a randomized multicenter trial (VENT OR NOT)
International audienceBackground: Almost two-thirds of the adults in Europe are overweight or obese. Obese patients are at higher risk of complications, especially during intubation. Therefore, the management of general anesthesia is crucial. Regarding intubation, there is no consensus among anesthesiologists between the two strategies: standard sequence intubation (i.e., with bag-mask ventilation between induction and intubation) vs. rapid sequence intubation (i.e., without bag-mask ventilation between induction and intubation) assuming that these patients are at high risk of inhalation and desaturation during apnea. This study aims to compare the incidence of intubation-related complications between these two strategies in severely obese patients undergoing elective surgery.Methods: The VENT OR NOT study is a prospective, multicenter, open-label, randomized trial including obese patients with a body mass index ≥ 35 kg/m2 requiring tracheal intubation before scheduled surgery. We will assess the superiority of the standard sequence intubation (\"VENT strategy\") compared with the rapid sequence intubation (\"NO VENT strategy\") to prevent intubation-related complications in 702 obese patients. Enrolment started in September 2023 in 13 French academic and nonacademic hospitals and is expected to finish in September 2025. The primary outcome is a composite criterion of complications occurring from the induction of general anesthesia up to 10 min after intubation including: oxygen desaturation < 95%, intubation failure on the first attempt, aspiration or regurgitation, and severe hypotension defined as a systolic blood pressure < 80 mmHg. The VENT OR NOT trial began after ethics committee approval has been obtained and is carried out according to the principles of the Declaration of Helsinki and the Good Clinical Practice guidelines.Discussion: The VENT OR NOT trial is the first randomized controlled trial designed to investigate whether the \"VENT strategy\" reduces the risk of intubation-related complications compared with the \"NO VENT strategy\" for elective surgery.Trial registration: ClinicalTrials.gov NCT05930678. Enhanced secretion of the amyotrophic lateral sclerosis ALS-associated misfolded TDP-43 mediated by the ER-ubiquitin specific peptidase USP19
International audienceProteinopathies, such as amyotrophic lateral sclerosis (ALS), are marked by the accumulation of misfolded proteins that disrupt cellular processes. Eukaryotic cells have developed protein quality control systems to eliminate these aberrant proteins, but these systems often fail to differentiate between normal and misfolded proteins. In ALS, pathological inclusions primarily composed of misfolded TDP-43 are a hallmark of the disease. Recently, a novel unconventional secretion process called misfolding-associated protein secretion (MAPS) has been discovered to selectively export misfolded proteins. USP19, an Endoplasmic Reticulum-associated ubiquitin peptidase, plays a crucial role in this process. In this study, we investigated the impact of ER-anchored USP19 on the secretion of misfolded TDP-43. Here we found that USP19 overexpression significantly promotes the secretion of soluble and aggregated misfolded TDP-43, requiring both ER anchoring and ubiquitin peptidase activity. Characterization of the cellular and molecular mechanisms involved in this process highlighted the importance of early autophagosomal and late endosomal/amphisomal compartments, while lysosomes did not play a key role. By using dominant-negative mutants and small interfering RNAs, we identified that USP19-mediated secretion of misfolded TDP-43 is modulated by key factors involved in cellular trafficking and secretion pathways, such as ATG7, the ESCRT-O HGS/HRS, the Rab GTPases RAB11A, RAB8A, and RAB27A, and the v-SNARE VAMP7. We also confirmed the crucial role of the DNAJC5/CSPα cochaperone. Overall, this study provides new insights into how cells manage the secretion of misfolded TDP-43 proteins and potentially opens new avenues for therapeutic interventions in ALS and related disorders
Essai émulé comparant deux stratégies thérapeutiques sur l’utilisation de dabrafénib+dramétinib ou de l’immunothérapie chez des patients en première ligne dans le mélanome avancé muté Braf, à partir du registre national Melbase : étude Eloquence
International audienceIntroductionLa survie globale (OS) du mélanome avancé Braf muté est actuellement de l’ordre de 50 % à 10 ans grâce aux thérapies ciblées associant l’inhibition de Braf et de Mek, ainsi qu’à l’immunothérapie (IT). Nous souhaitons confirmer les résultats de l’essai Dreamseq montrant la supériorité de la séquence ipilimumab + nivolumab en première ligne suivi de dabrafénib + tramétinib versus inversement en émulant un essai clinique utilisant la base de données Melbase (NCT02828202).Matériel et méthodesNous avons utilisé les données du registre prospectif Melbase. Les patients atteints d’un mélanome avancé muté Braf naïfs de traitement adjuvant, traités en 1ère ligne soit par IT soit dabrafénib + tramétinib de février 2015 à novembre 2022 ont été considérés pour l’étude. Nous avons émulé l’essai randomisé Dreamseq selon le cadre d’émulation d’un essai cible. Le groupe1 recevait la séquence IT suivie de dabrafénib + tramétinib en cas de progression versus la séquence inverse (groupe2). L’émulation de l’essai cible consistait à 1) appliquer les critères d’éligibilité ; 2) allouer les participants à chacune des stratégies en fonction du premier traitement reçu ; 3) corriger les facteurs de confusion initiaux par pondération par l’inverse de la probabilité de traitement ; 4) censurer artificiellement le suivi des participants en cas de déviation à la stratégie assignée pour l’analyse ; 5) corriger le biais de sélection temps-dépendant induit par cette censure artificielle par pondération par l’inverse de la probabilité de censure 6) analyser l’essai émulé. Le critère d’évaluation principal était l’OS (12, 24, 36 mois) et les critères d’évaluation secondaires étaient la survie sans progression (PFS), la meilleure réponse et la tolérance.RésultatsSur les 495 patients analysés, 480 (n = 229 groupe1 et n = 251 groupe2) ont été inclus dans l’analyse de pondération. Après pondération, le taux de l’OS à 12, 24 et 36 mois était de 83,7 %, 64,1 %, 53 % pour le groupe 1 et de 66,3 %, 39,6 %, 34,0 % pour le groupe 2 ; le taux de PFS était de 36,0 %, 26,4 %, 22,8 % (groupe 1) et de 30,4 %, 10,8 %, 7,2 % (groupe 2). La réponse objective de 1ère ligne était de 56,9 % (groupe1) et 62,7 % (groupe 2) de patients. Les effets indésirables associés tout grade confondu étaient plus fréquents dans le groupe1 (88,8 % et 74,9 % groupe 2). 2 patients sont décédés des suites d’une toxicité dans le groupe1 de 1ère ligne.DiscussionLe taux d’OS à 2 ans pour Dreamseq est de 71,8 % (équivalent du groupe1) et 51,5 % (équivalent du groupe2) soit une différence absolue de 20 % entre les deux bras. Les données de survie sont moins bonnes en vie réelle mais l’écart dans Melbase est au moins aussi important à 2 ans avec 24 %. La population en vie réelle n’est pas la même dans les études cliniques. Même si les facteurs de confusion initiaux entre les groupes sont corrigés, la comparaison entre l’essai Dreamseq et Eloquence peut être discutable.ConclusionLes résultats confirment que les patients qui ont commencé leur séquence de traitement par IT ont eu une meilleure survie
A longitudinal analysis of haemoglobin levels and major cardiovascular events
International audienceBackground and Hypothesis Standard approach to anaemia in non-dialysis chronic kidney disease (CKD) does not account for potential age- or sex-specific related risks. We assessed differences in the association between haemoglobin and major cardiovascular events (MACE+) in men and women with CKD, by age groups. Methods Using 5-year longitudinal data from the CKD-REIN cohort, we studied patients with CKD stage 2–5 not treated with erythropoiesis stimulating agents (ESA). The main outcome was MACE+, defined as cardiovascular death, myocardial infarction, stroke or hospitalization for acute heart failure. Competing events were initiation of kidney replacement therapy and non-cardiovascular death. In each of the 4 predefined subgroups by sex and age (≤ 70 versus > 70 years at baseline), we estimated hazard ratios (HR) of current values of haemoglobin using a cause-specific Cox model adjusted for current values of glomerular filtration rate and transferrin saturation. All current values of biomarkers were first estimated in a multivariate-shared random effect joint model. Results Analyses considered 29 042 haemoglobin measurements from 2791 patients, and 364 MACE + . The association between current haemoglobin and log hazard of MACE + was linear in men and J-shape in women. For a haemoglobin of 10.5 g/dL, as compared to 11.5 g/dL, the hazard of MACE + at any time was increased by 60% in younger women (HR = 1.6, 95%CI 1.1–2.4), 70% in older women (HR = 1.7, 95%CI 1.3–2.4), 30% in younger men (HR = 1.3 95%CI 1.1–1.5) and 20% in older men (HR = 1.2 95%CI 1.1–1.4). Results were similar in the sensitivity analysis not censoring at the first ESA treatment. Conclusion Our longitudinal analysis in patients with CKD not on ESA therapy highlights a stronger association between anaemia and increased hazard of MACE + in women than in men. This sex-difference should inform the design of trials addressing anaemia correction in CKD
Mesure d'atténuation ultrasonore par ondes planes : Application à l’imagerie hépatique et rénale sur volontaires
Ultrasons biomédicaux - Imagerie et caractérisation des tissus mous; GAPSUS - Acoustique Physique, Sous-Marine et Ultra-SonoreNational audienceLes ultrasons médicaux sont utilisés à des fins diagnostics (échographie, Doppler) ou à des fins thérapeutiques (ultrasons focalisés, cavitation). Connaitre l’atténuation subie par les ultrasons est d’une importance majeure, pour déterminer les niveaux d’énergie nécessaires à des fins thérapeutiques mais également pour ouvrir une nouvelle voie de diagnostic. Dans cette étude, une sonde échographique était utilisée pour mesurer l’atténuation des milieux traversés par l’émission d’une onde plane. L’hypothèse sur laquelle se fonde la méthode est qu’une onde plane transmise doit être également proche d’une onde plane lors de sa rétrodiffusion. Cette hypothèse était testée à l’aide de simulations numériques réalisées sur la base de coupes histologiques humaines. Les expérimentations étaient réalisées à l’aide d’une sonde linéaire à 128 éléments, fonctionnant à 5,2 MHz. La séquence d’imagerie était composée d’une unique onde plane. Les acquisitions étaient réalisées sur des fantômes calibrés puis en imageant le tissu hépatique et le cortex rénal sur douze volontaires sains. Les résultats numériques permettaient de décrire et de vérifier les conditions nécessaires, par rapport à la longueur d’onde, sur la taille des diffuseurs (k0*a = 0.26 pour les hépatocytes) et sur le contraste d’impédance pour que l’onde rétrodiffusée soit assimilable à une onde plane. Pour les fantômes calibrés, l’écart entre les valeurs d’atténuation fournies par le constructeur et celles obtenues expérimentalement était inférieur à 4 %. Chez les volontaires sains, l’atténuation moyenne mesurée dans le foie et dans le cortex rénal était respectivement de 0.085 ± 0.023 Np/(cm.MHz) et de 0.065 ± 0.028 Np/(cm.MHz), soit un écart maximal de 14 % comparé à la littérature. Cette méthode permet donc de mesurer simplement l’atténuation, sans milieu de référence, aussi bien in vivo dans des contextes cliniques qu’in vitro en laboratoire et ouvre ainsi un champ d’application, aussi bien dans le domaine de la thérapie que du diagnostic
