Examining Psychosocial Processes of change in Students‘ Daily Lives during COVID-19: An Ecological Momentary Assessment
Background
The scientific purpose of diagnoses of psychiatric syndromes unfortunately no longer leads to further useful knowledge. Questions regarding etiology, understanding trajectories of pathological processes, and identifying and understanding clinically relevant responses to interventions, often remain unanswered. Consequently, the "protocols for syndromes'' approach to evidence-based psychological interventions inadequately meets the standards of scientific progressiveness (Ciarrochi et al., 2022). With regard to the study of evidence-based psychological interventions, there has been an increased focus in recent years on the direct study of change processes associated with therapy goals (Hofmann & Hayes, 2019). Process-Based Therapy (PBT; Hayes & Hofmann, 2018) offers an alternative model to the "protocols for syndromes'' approach in line with the change in focus. PBT focuses on biopsychosocial processes of change, which have been shown in the course of research to be functionally important means of achieving therapy goals and consequently relevant to individual therapy (Hofmann et al., 2021). However, this change in focus requires new, more integrative and idiographic methods for measuring and analyzing change processes at high temporal density. The Process-Based Assessment Tool (PBAT, Ciarrochi et al., 2022) represents a good start. It provides a means of measuring and analyzing change processes at high temporal density and allows clinicians and researchers to use individual items or groups of items for the purpose of individual-focused research and practice.
The PBAT assesses nine dimensions of psychosocial processes identified through the Extended-Evolutionary Meta-Model of PBT: affect, cognition, attention, social connection, motivation, overt behavior, physical health behaviors, variation, retention. Based on the general principles of dynamic systems theories (Ram & Gerstorf, 2009; Ford & Lerner, 1992) and as proposed by the PBT approach (Hayes & Hofmann, 2018), considering each of these dimensions as a dynamical process that change over time can bring about novel understandings for clinical interventions. Effective interventions take place by focusing on enacting change at the momentary level and on the relevant dynamic characteristics of change (Biswas-Diener et al., 2011). Additionally, change enactment needs to be considered in relation to the specific person and how individuals differ from themselves and from others over time (Nesselroade, 1991). Previous research has identified a set of three meaningful dynamic characteristics based on dynamic process model parameters (Ram & Gerstorf, 2009) that can examine dynamic profiles unique to each person (Hamaker, Ceulemans, Grasman, & Tuerlinckx, 2015): (1) intra-individual variability (IIV)--how much fluctuation a person demonstrates on each dimension over time; (2) baseline levels--the average level of a person’s experience on each dimension; (3) inertia or regulation--how quickly a person returns to their baseline levels. Using these dynamic characteristics we can recognize and understand key processes of change in PBT dimensions as they pertain to clinical outcomes.
Aims and Methods
The aim of this study is to examine within-person temporal dynamic characteristics of the PBAT psychosocial processes of change in relation to more stable clinically relevant outcomes. Specifically, we will examine each dimension of PBAT and its patterns of change over momentary time-scales in terms of three dynamic characteristics (intra-individual variability, baseline levels, and inertia). We then explore for each PBAT dimension which characteristics of temporal change are most predictive of relevant clinical outcomes. For this purpose, we will target young adult student populations during COVID-19 who may potentially be experiencing distress and discomfort due to changes in their living circumstances and educational experience as a result of the pandemic.
Procedure. Through an Ecological Momentary Assessment (EMA; Shiffman et al., 2008) design we plan to collect longitudinal data in the everyday life of students. To this end, we will conduct a 14-day study in which we will measure PBAT psychological processes as well as other parameters of well-being (e.g., Core Affect, psychological distress, sleep) five times per day using a semi-random schedule. These momentary assessments will be scheduled within participants’ self-reported waking hours, with smartphone-based prompts delivered randomly within three-hour intervals throughout the day and at least 30 minutes apart. Furthermore, throughout the 14-day EMA period, biometric data will be collected via fitness trackers/smartwatches (e.g.,Garmin) for objective measures of psychosocial processes. One day before and after the 14-day EMA period, participants will be invited to a short (30-minute) in-person lab visit for instructions as well as completing a battery of questionnaires on trait clinical outcomes as well as demographic information. Furthermore, before the study begins, functional magnetic resonance imaging (fMRI) will be conducted to measure brain activities for the purpose of examining brain function correlates of psychosocial processes. An initial pilot study with 60 participants will inform the researchers on the feasibility of the design of the study. After any adjustments, the main study will be conducted with 110 participants.
Timeline
The preparation phase for the questionnaire study began on 01.03.2021. After a positive ethics vote (duration approx. one month) and final programming of the study, the pilot phase including recruitment and survey will follow (approx. 01.06.2022), which is scheduled for two weeks. After possible changes, the main study begins (approx. 01.07.2022) for the duration of two weeks. On 15.07.2022 the evaluation phase begins accordingly
