    Evaluation of pelvic floor rehabilitation in the prevention of low anterior resection syndrome: Study protocol of the CONTICARE trial

    International audienceAbstract Aim Bowel dysfunction following sphincter‐preserving rectal resection for cancer, commonly referred to as low anterior resection syndrome (LARS), significantly impacts patients' quality of life. Preventing this condition is essential for healthcare teams. Postoperative pelvic floor rehabilitation, including anal biofeedback therapy, has shown potential in alleviating established LARS symptoms. This trial aims to evaluate the effectiveness of pelvic floor rehabilitation prior to bowel continuity restoration in preventing LARS in patients undergoing sphincter‐preserving rectal resection for cancer. Methods CONTICARE is a national multicentre randomized trial. Patients who have undergone total mesorectal excision with sphincter preservation and a defunctioning stoma ( n = 174; 87 per arm) will be randomly assigned to either the rehabilitation or control group before stoma closure. The rehabilitation group will receive systematic pelvic floor rehabilitation, comprising four sessions before and six sessions after stoma closure, following a standardized approach. The control group will receive standard follow‐up care, which includes symptom‐based therapy after ileostomy closure. The primary outcome measure will be the severity of LARS, assessed using the dedicated LARS score at 6 months. Comparisons of faecal incontinence symptoms, quality of life and complications related to biofeedback therapy will also be evaluated at 6 weeks, 6 months and 1 year between the two groups. Conclusion Pelvic floor rehabilitation has the potential to enhance symptom management and quality of life for patients following rectal resection by preventing LARS. The combination of anal exercises and biofeedback therapy, which has been extensively studied without reported adverse effects, suggests that the anticipated benefits outweigh any potential risks.     Des plans aux vestiges : redonner corps aux tranchées de 1870-1871!

    International audienceL’identification des lignes de défense de la guerre de 1870-1871 constitue un défi majeur pourl’archéologie, car elles sont souvent difficiles à localiser et dépourvues de mobilier caractéristique. Leurrepérage nécessite une superposition avec d’anciennes cartographies militaires, comme cela a été fait àOrmes (45), où une tranchée liée aux combats de 1870 a été découverte.Les sources disponibles incluent des récits contemporains et des études historiques, mais leur précisionvarie : certains sont très généraux et manquent de précision (Fig 1), d’autres ne couvrent qu’un secteurlimité, laissant parfois dans l’ombre les emprises étudiées par les archéologues.Les archives du Service historique de la défense à Vincennes, notamment la série L et les documents durégiment du Génie, fournissent des informations précieuses sur la localisation et la nature desaménagements défensifs. Ces documents ont les mêmes limites que les précédents.Certains plans non directement liés au conflit s’avèrent être des sources inattendues mais cruciales. Ainsi,un plan de Tavers de 1888, réalisé par l’instituteur Abel Adam pour répertorier les parcelles viticoles,mentionne clairement des tranchées du 9 décembre 1870 (combats de Beaugency-Cravant). Cesdocuments illustrent l’intérêt d’une approche pluridisciplinaire croisant archives militaires, cartographiecivile et archéologie de terrain.L’ensemble de ces recherches met en évidence la complexité d’identification des vestiges de 1870-1871.Les sources écrites restent limitées et imprécises, et les vestiges eux-mêmes sont difficilementcaractérisables sans une méthodologie adaptée, incluant l’usage de détecteurs de métaux. Il est doncessentiel d’engager une réflexion sur le recensement et l’étude de ces structures. Ces aménagements, sansdoute les seuls témoins matériels accessibles à l’archéologie dans la région, subissent une érosionconstante due aux aménagements modernes. Leur préservation et leur analyse doivent devenir une prioritépour mieux comprendre cette période de l’histoire militaire

    International audienceThis chapter presents an overview of the tools, approaches and recent results of numerical modeling in the field of 3D concrete printing. After summarizing digital tools for geometric modeling and virtual printing, the chapter deals the state of the art in digital modeling of the 3D printing process. In the context of the construction sector's move toward digitalization, the integration of BIM (building information modeling) with 3D printing activities has become a challenge for many research efforts. The developments and applications of numerical tools in the 3D printing process for concrete structures have demonstrated the effectiveness of numerical modeling in this field. The stochastic optimization procedure could be widely useful for establishing design regulations for structures built using 3D printing. On a large scale, the interest of numerical modeling lies in predicting failure modes such as elastic buckling and plastic collapse of the structure, phenomena commonly observed during printing

    On stability of one-dimensional Hughes' dynamics with affine costs

    We investigate stability issues for the one-dimensional variant of the celebrated Hughes model for pedestrian evacuation. The cost function is assumed to be affine, which is a setting where existence of solutions with BV loc in space regularity, away from the so-called turning curve, was recently established. We provide a uniqueness result for solutions having the special property that agents never cross the turning curve (which implies that they are BV globally). In the same setting, continuous dependence of solutions on the cost parameter is highlighted. On the other hand, numerical simulations using the manyparticle approximation of the model, with more general initial conditions that allow the support of the solutions to intersect the turning curve, demonstrate the strong sensitivity of the evacuation time to the same cost parameter; this instability arises from interactions between agents and the turning curve.</div

Il se trouve en rive gauche du cours de la Borne, un affluent de la Loire. Le site est protégé des vents d'ouest par une avancée du relief correspondant à l'extrémité d'une ancienne coulée volcanique descendant du volcan de la Denise qui surplombe la vallée de la Borne. Le site présente plusieurs locus correspondant à différents types de vestiges de plusieurs périodes chronologiques, numérotés de 1 à 4.
