    Missed opportunities for early infant HIV diagnosis: Results of a national study in South Africa

    Background: Services to diagnose early infant HIV infection should be offered at the 6-week immunization visit. Despite high 6-week immunization attendance, the coverage of early infant diagnosis (EID) is low in many sub-Saharan countries. We explored reasons for such missed opportunities at 6-week immunization visits. Methods: We used data from 2 cross-sectional surveys conducted in 2010 in South Africa. A national assessment was undertaken among randomly selected public facilities (n 625) to ascertain procedures for EID. A subsample of these facilities (n 565) was revisited to assess the HIV status of 4- to 8-week-old infants receiving 6-week immunization. We examined potential missed opportunities for EID. We used logistic regression to assess factors influencing maternal intention to report for EID at 6-week immunization visits. Results: EID services were available in \u3e95% of facilities and 72% of immunization service points (ISPs). The majority (68%) of ISPs provide EID for infants with reported or documented (on infant\u27s Road-to-Health Chart/booklet - iRtHC) HIV exposure. Only 9% of ISPs offered provider-initiated counseling and testing for infants of undocumented/unknown HIV exposure. Interviews with self-reported HIV-positive mothers at ISPs revealed that only 55% had their HIV status documented on their iRtHC and 35% intended to request EID during 6-week immunization. Maternal nonreporting for EID was associated with fear of discrimination, poor adherence to antiretrovirals, and inadequate knowledge about mother-to-child HIV transmission. Conclusions: Missed opportunities for EID were attributed to poor documentation of HIV status on iRtHC, inadequate maternal knowledge about mother-to-child HIV transmission, fear of discrimination, and the lack of provider-initiated counseling and testing service for undocumented, unknown, or undeclared HIV-exposed infants

    On the analytic solution for the steady drainage of magnetohydrodynamic (MHD) Sisko fluid film down a vertical belt

    This paper presents an analytic study for the steady drainage of magnetohydrodynamic (MHD) Sisko fluid film down a vertical belt. The fluid film is assumed to be electrically conducting in the presence of a uniform transverse magnetic field. An analytic solution for the resulting non linear ordinary differential equation is obtained using the Adomian decomposition method. The effects of various available parameters especially the Hartmann number are observed on the velocity profile, shear stress and vorticity vector to get a physical insight of the problem. Furthermore, the shear thinning and shear thickening characteristics of the Sisko fluid are discussed. The physical quantities discussed for the Sisko fluid film have also been discussed for the Newtonian fluid film and comparison between them made

    Outlier Detection and a Method of Adjustment for the Iranian Manufacturing Establishment Survey Data

    The role and importance of the industrial sector in the economic development necessitate the need to collect and to analyze accurate and timely data for exact planning. As the occurrence of outliers in establishment surveys are common due to the structure of the economy, the evaluation of survey data by identifying and investigating outliers, prior to the release of data, is necessary. In this paper, different robust multivariate outlier detection methods based on the Mahalanobis distance with blocked adaptive computationally efficient outlier nominators algorithm, minimum volume ellipsoid estimator, minimum covariance determinant estimator and Stahel-Donoho estimator are used in the context of a real dataset. Also some univariate outlier detection methods such as Hadi and Simonoff’s method, and Hidiroglou-Barthelot’s method for periodic manufacturing surveys are applied. The real data set is extracted from the Iranian Manufacturing Establishment Survey. These data are collected each year by the Statistical Center of Iran using sampling weights. In this paper, in addition to comparing different multivariate and univariate robust outlier detection methods, a new empirical method for reducing the effect of outliers based on the value modification method is introduced and applied on some important variables such as input and output. In this paper, a new four-step algorithm is introduced to adjust the input and output values of the manufacturing establishments which are under-reported or over-reported. A simulation study for investigating the performance of our method is also presented

    An M/G/1 Queue with Server Breakdown and Multiple Working Vavation

    This paper deals with the steady state behavior of an M=G=1 multiple working vacation queue with server breakdown. The server works with different service times rather than completely stopping service during a vacation. Both service times in a vacation period and in a regular service period are assumed to be generally distributed random variables. The system may breakdown at random and repair time is arbitrary. Further, just after completion of a customer’s service the server may take a multiple working vacation. Supplementary variable technique is employed to find the probability generating function for the number of customers in the system. The mean number of customers in the system is calculated. Some particular cases of interest are discussed. Numerical results are also presented

    On the stability of a Pexiderized functional equation in intuitionistic fuzzy Banach spaces

    During the last few decades several researchers have been devoted to establishing stability of different kinds of functional equations, differential equations, functional differential equations, fractional differential equations, etc. under different sufficient conditions in different spaces like Banach spaces, Banach modules, fuzzy Banach spaces etc. In this paper, we remain confined in the discussion of stability of functional equations in intuitionistic fuzzy Banach spaces. Ulam was the first person who introduced an open question concerning the stability of a group homomorphism in an international conference. Thereafter several researchers have replied and are still replying to this open question in different contexts. The objective of the present paper is to determine the Hyers-Ulam-Rassias type stability concerning the Pexiderized functional equation in intuitionistic fuzzy Banach spaces.     In the title calcium levulinate complex, [Ca(C5H7O3)2(H2O)2]n, the Ca2+ ion lies on a twofold rotation axis and is octacoordinated by two aqua ligands and six O atoms from four symmetry-related carboxylate ligands, giving a distorted square-antiprismatic coordination stereochemistry [Ca - O bond-length range = 2.355 (1)-2.599 (1) Å]. The levulinate ligands act both in a bidentate carboxyl O,O′-chelate mode and in a bridging mode through one carboxyl O atom with an inversion-related Ca2+ atom, giving a Ca⋯Ca separation of 4.0326 (7) Å. A coordination polymeric chain structure is generated, extending along the c-axial direction. The coordinating water molecules act as double donors and participate in intra-chain O - H⋯O hydrogen bonds with carboxyl O atoms, and in inter-chain O - H⋯O hydrogen bonds with carbonyl O atoms, thus forming an overall three-dimensional structure
