    Measurement of the branching fraction ratio RKR_K at large dilepton invariant mass

    International audienceA test of lepton universality between muons and electrons is performed using B+K++B^+\to K^+\ell^+\ell^- decays (where =e\ell = e, μ\mu), in the dilepton invariant-mass-squared region above 14.3\,GeV2/c4^2/c^4. The data used for the measurement consists of beauty meson decays produced in proton-proton collisions, corresponding to an integrated luminosity of 99\,fb1^{-1}, collected by the LHCb experiment between 2011 and 2018. The ratio of branching fractions for B+K+μ+μB^+\to K^+\mu^+\mu^- and B+K+e+eB^+\to K^+e^+e^- decays is measured to be RK=1.080.09+0.11  (stat)  0.04+0.04  (syst)R_K = 1.08^{+0.11}_{-0.09}\;(\text{stat})\;^{+0.04}_{-0.04}\;(\text{syst}), which is consistent with the Standard Model prediction of unity. This constitutes the most precise test of lepton flavour universality using B+K++B^+\to K^+\ell^+\ell^- decays with dilepton invariant-mass-squared above the ψ(2S)\psi(2S) mass, whilst being the first of its kind at a hadron collider

    Introduction : les dermatoses neutrophiliques (DN) sont des dermatoses inflammatoires ayant pour caractéristique une infiltration de polynucléaires neutrophiles dans les différentes couches de la peau. L’association aux syndromes myélodysplasiques (SMD) et leucémies myélomonocytaires chroniques (LMMC), bien que souvent décrite, reste à préciser en termes de traitement et d’impact pronostic de la survenue d’une DN.Objectifs : décrire les caractéristiques clinico-biologiques des DN associées aux SMD, la réponse aux traitements et l’impact pronostic de leur survenue sur l’évolution de l’hémopathie.Patients et méthodes : les critères d’inclusions étaient : Âge > 18ans, DN prouvée histologiquement et SMD et/ou LMMC selon les critères OMS 2016. Les critères d’exclusions étaient les suivants : DN secondaire à une leucémie aigüe (LA), leukemia cutis, cause infectieuse, cancer < 5 ans, maladies inflammatoires chroniques de l’intestin, rhumatisme inflammatoire ou maladie auto-immune de > 5 ans, hémopathie préexistante de > 5 ans, DN réactionnelle (agent infectieux et médicaments sauf traitements de la myélodysplasie). La réponse aux traitements était analysée selon des critères d’évolution des lésions cutanées, de corticodépendance et de rechutes. Les témoins étaient recueillis dans le registre du groupe français des myélodysplasies et appariés sur l’âge, le sexe et le score R-IPSS.Résultats : 50 patients, de sexe masculin avec un âge médian au diagnostic de DN de 55ans ont été inclus. 60 % avaient une maladie auto-immune associée, et 30 % un syndrome VEXAS. L’infiltrat était de nature clonale. Le syndrome de sweet (SS) et le pyoderma gangrenosum (PG) étaient retrouvés dans 76 % et 16 % des cas. 41 patients ont été traité avec une corticothérapie et de la colchicine en 1ère et 2ème ligne chez 83 % et 24 % des patients. Les biothérapies et les JAK inhibiteurs étaient souvent associés en cas de syndrome VEXAS, principalement en 3ème et 4ème ligne, la meilleure réponse était observée avec les JAK inhibiteurs. 24 % ont reçu de l’azacytidine, efficace dans les trois sous-groupes. La durée médiane de suivi était de 1,5 ans. La mortalité était significativement plus élevée dans les groupes SMD (71 % vs 28 % ; p < 0,001), LMMC (71 vs 46 % ; p = 0,044), mais pas dans le groupe VEXAS. La transformation en LA était supérieure dans le groupe SMD (39 % vs 12 % ; p = 0,045), inférieure dans le groupe VEXAS (0 vs 13 % ; p < 0,001), et similaire dans le groupe LMMC (35 vs 22 % ; p = 0,466) Conclusion : les DN associées aux SMD/LMMC sont représentées par les SS et les PG. 1/3 des cas associées aux LMMC ont une rémission spontanée. Les lésions cutanées sont corticosensibles, mais avec une fréquence élevée de rechute et une forte dépendance aux corticoïdes, particulièrement dans le VEXAS. La dapsone et les dermocorticoïdes sont inefficaces. La colchicine semble être efficace et sure, sauf dans le VEXAS où les JAKi sont les plus efficaces à visée d’épargne cortisoniques. Les traitements ciblant la prolifération clonale comme les agents hypométhylants ou l’allogreffe de cellules souches sont les traitements les plus efficaces. Les dermatoses neutrophiliques sont associées à une plus grande mortalité dans les SMD et LMMC, et plus de transformation en LA dans les SMD. Elles sont associées à l’inverse à un meilleur pronostic dans les SMD associés au VEXA

    Sevrage tabagique a l'aide du dispositif Asalée : mise en évidence des éléments déterminants dans la réussite

    CONTEXTE : le tabac est un problème de santé publique majeur en France. Il constitue le premier facteur de risque de cancer du poumon. Le sevrage tabagique est complexe mais il existe des outils permettant de faciliter ce dernier dont le suivi des patients par les infirmières du dispositif Asalée.OBJECTIF : l’objectif principal de cette étude est d’analyser les différents éléments clefs jouant un rôle déterminant dans la réussite d'un sevrage définitif du tabac grâce à un suivi par le dispositif Asalée. L’objectif secondaire est d’analyser l’impact de la prescription des TNS par les IDSP.MÉTHODE : nous avons procédé à cela en réalisant une étude qualitative transversale, à partir d’entretiens individuels semi-dirigés auprès de patients sevrés sur le plan tabagique ayant bénéficié d’un suivi par une IDSP.RÉSULTATS : nous avons interrogé 15 patients dans la région Auvergne de mars à juillet 2024. Nous avons pu observer que le dispositif est encore méconnu malgré 20 ans d’existence. Dans un premier temps, les patients interrogés ont souligné que les éléments déterminants du suivi ayant permis leur sevrage étaient le soutien apporté par l’IDSP, la disponibilité, la personnalisation des consultations et les compétences des IDSP. Ensuite, nous avons pu souligner que la vision des patients vis-à-vis des TNS avait changé au cours du suivi grâce à une utilisation adaptée au cas par cas par les IDSP.CONCLUSION : le suivi par l’IDSP d’un patient en cours de sevrage tabagique joue un rôle déterminant dans la réussite de celui-ci. D’une part par l’adaptation de l’IDSP au patient, d’autre part par ses connaissances et enfin par sa formation à l’utilisation des TNS

    Mutational Analysis of Colistin-Resistant Pseudomonas aeruginosa Isolates: From Genomic Background to Antibiotic Resistance

    International audienceThis study analyzed eleven isolates of colistin-resistant Pseudomonas aeruginosa, originating from Portugal and Taiwan, which are associated with various pathologies. The results revealed significant genetic diversity among the isolates, with each exhibiting a distinct genetic profile. A prevalence of sequence type ST235 was observed, characterizing it as a high-risk clone, and serotyping indicated a predominance of type O11, associated with chronic respiratory infections in cystic fibrosis (CF) patients. The phylogenetic analysis demonstrated genetic diversity among the isolates, with distinct clades and complex evolutionary relationships. Additionally, transposable elements such as Tn3 and IS6 were identified in all isolates, highlighting their importance in the mobility of antibiotic resistance genes. An analysis of antimicrobial resistance profiles revealed pan-drug resistance in all isolates, with a high prevalence of genes conferring resistance to β-lactams and aminoglycosides. Furthermore, additional analyses revealed mutations in regulatory networks and specific loci previously implicated in colistin resistance, such as pmrA, cprS, phoO, and others, suggesting a possible contribution to the observed resistant phenotype. This study has a strong impact because it not only reveals the genetic diversity and resistance mechanisms in P. aeruginosa but also identifies mutations in regulatory genes associated with colistin resistance

    HALSR-Net: Improving CNN Segmentation of Cardiac Left Ventricle MRI with Hybrid Attention and Latent Space Reconstruction

    International audienceAccurate cardiac MRI segmentation is vital for detailed cardiac analysis, yet the manual process is laborintensiveand prone to variability. Despite advancements in MRI technology, there remains a significant need for automated methods that can reliably and efficiently segment cardiac structures. This paper introducesHALSR-Net, a novel multi-level segmentation architecture designed to improve the accuracy and reproducibilityof cardiac segmentation from Cine-MRI acquisitions, focusing on the left ventricle (LV). The methodologyconsists of two main phases: first, the extraction of the region of interest (ROI) using a regression model thataccurately predicts the location of a bounding box around the LV; second, the semantic segmentation step basedon HALSR-Net architecture. This architecture incorporates a Hybrid Attention Pooling Module (HAPM) that merges attention and pooling mechanisms to enhance feature extraction and capture contextual information. Additionally, a reconstruction module leverages latent space features to further improve segmentation accuracy. Experiments conducted on an in-house clinical dataset and two public datasets (ACDC and LVQuan19) demonstrate that HALSR-Net outperforms state-of-the-art architectures, achieving up to 98% accuracy and F1-score for the segmentation of the LV cavity and myocardium. The proposed approach effectively addresses the limitations of existing methods, offering a more accurate and robust solution for cardiac MRI segmentation, thereby likely to improve cardiac function analysis and patient care

    Rare Ketose Synthesis from Glycolaldehyde by C2 Elongation of Aldoses Catalyzed by Transketolase Variants

    International audienceThe enzyme thiamine diphosphate-dependent transketolase (TK) efficiently catalyzes the C2 elongation of aldoses using β-hydroxypyruvate (HPA) as a nucleophile for the synthesis of Cn+2 ketoses by stereoselective C–C bond formation. This synthetic approach is flawed by the tedious, low-yield chemical route to HPA. We report here an alternative transketolase-catalyzed reaction, with glycolaldehyde (GoA) as a nucleophile, for the efficient upgrading of several aldoses (C3–C5) as electrophiles, eliminating the release of carbon dioxide from the commonly used HPA. We found that the syn-acyloin condensation of GoA catalyzed by wild-type Escherichia coli transketolase (wt-TKeco) carried out under an inert atmosphere reduced byproduct formation and potentiated erythrulose (ERY) formation, giving ERY with a 78% isolated yield from 25 g·L–1 of GoA. The same reaction conditions were extended to the cross-acyloin condensation of GoA as a nucleophile with different aldoses as electrophiles, d-glyceraldehyde, l-glyceraldehyde, l-threose, d-ribose, and 5-deoxy-d-ribose, in stoichiometric amounts. Following this one-pot procedure, the corresponding Cn+2 ketoses of biological interest, d-xylulose, l-ribulose, l-sorbose, d-sedoheptulose, and 7-deoxy-d-sedoheptulose were obtained with moderate-to-high yields (38–89%) and high diastereoselectivities, showing that the TK-catalyzed reaction equilibrium was most often shifted toward the expected products. TK-catalyzed cross-acyloin reaction from GoA removes the need for HPA in the reaction, affording efficient atom economy, with product yields similar to or higher than obtained with HPA as a nucleophile

    Q fever as an anthropological prism for revealing how farmers live with microbes

    International audienceTo develop effective public health management strategies, it is necessary to account for the viewpoints of all stakeholders. Thus, anthropological approaches can potentially inform strategies for preventing and managing zoonotic diseases. Here, we use Q fever as a starting point for exploring how small ruminant farmers perceive the reality of microbes by disentangling the farmers’ often subtle relationships with their livestock, disease, and the world in general. We found that livestock farmers feel like they exist in the borderlands between two worlds: the non-naturalistic World A, characterised by long timespans and complex relationships with non-humans, and the naturalistic World B, characterised by short timespans and the control of non-humans. The occurrence of diseases leads to tension and shifts between the worlds, depending on how much farmers entrust World B with health risk management and relations with non-humans. Significant or complete delegation of these responsibilities may result in a sense of unease and dispossession, particularly when World B fails to provide productive solutions. Whether farmers view Q fever as mysterious and threatening is also highly dependent on the degree of health risk delegation. Overall, the agent that causes Q fever is perceived in one of two ways: as a fearsome pathogen or a normal denizen in the farm’s ecosystem. These results have implications beyond Q fever and clearly illustrate the concept of the “microbial turn”, which emphasizes the plurality and ambivalence of the relationships between humans and microbes
