État des lieux de la dermatologie libérale en Auvergne en 2024 : élaboration d’une méthodologie pour la réalisation d’un état des lieux standardisé
CONTEXTE : l’accès aux soins dermatologiques se complexifie du fait de la tension croissante entre la baisse du nombre de dermatologues et la hausse des besoins de la population. Une des conséquences est celle du retard diagnostique préjudiciable pour le patient. La dermatologie libérale est pleinement concernée par ce phénomène.OBJECTIF : l’objectif de ce travail est faire un état des lieux de la dermatologie libérale en Auvergne en 2024 en collaboration avec l’Association des dermatologues vénéréologues libéraux de l’Auvergne Bourbonnais (ADELAB) puis de développer une méthodologie standardisée pour ce genre d’état des lieux qui pourrait être exportée aux autres territoires.MÉTHODE : deux formulaires ont été élaborés puis envoyés aux 42 dermatologues libéraux d’Auvergne. Un formulaire portait sur le « Profil médecin » et un autre « Consultation patient » devait être renseigné pour chaque consultation, anonymement, sur une semaine de juin 2024. L’élaboration de la méthodologie pour réaliser un état des lieux standardisé s’est appuyée sur cette expérience et une étude de la littérature.RÉSULTATS : trente quatre formulaires « Profil médecin » et 2486 formulaires « Consultation patient » ont été recueillis. Les résultats portent principalement sur la démographie médicale, la démographie de la patientèle et le parcours patient, tant logistique que médical. La méthodologie développée propose un nouveau formulaire « Profil médecin » ainsi qu’un tableur agrégeant les données des consultations d’une semaine type d’activité de chaque praticien.CONCLUSION : il est primordial de connaître le contexte dans lequel nous évoluons pour mieux l’appréhender, proposer des solutions adaptées et véhiculer des messages clés témoins des réalités locales. Notre travail a permis de dresser un état des lieux inédit de la dermatologie libérale en Auvergne. De cette expérience, une méthodologie qui pourrait être exportée dans les autres territoires ou dans le milieu hospitalier a été conçue
Facteurs prédictifs en scanner de la prise en charge chirurgicale des ethmoïdites en pédiatrie
CONTEXTE : l’ethmoïdite aiguë représente l’infection sinusienne la plus fréquente chez l’enfant, exposant à des complications orbitaires voire intracrâniennes. L’imagerie, en particulier la tomodensitométrie (TDM), est essentielle pour évaluer l’extension de l’infection et orienter la prise en charge thérapeutique. Toutefois, l’interprétation du scanner varie selon la spécialité et l’expérience du lecteur, pouvant influencer le choix entre traitement médical et recours à la chirurgie.OBJECTIF : identifier des critères scanographiques prédictifs d’une prise en charge chirurgicale des ethmoïdites compliquées de l’enfant, et évaluer leur reproductibilité interobservateurs.MÉTHODE : étude rétrospective monocentrique menée entre mai et novembre 2024, incluant 72 enfants ayant bénéficié d’une TDM pour suspicion d’ethmoïdite entre 2010 et 2024. Quatre évaluateurs (interne en radiologie, radiologue junior, radiologue sénior, ORL) ont relu chaque examen selon une grille standardisée. L’appréciation de la prise en charge (médicale vs chirurgicale) était également recueillie pour chaque lecture. La concordance interobservateurs a été évaluée pour chaque critère TDM ainsi que pour la stratégie thérapeutique proposée, comparée à celle effectivement réalisée.RÉSULTATS : les signes les plus fortement associés à une indication chirurgicale sont l’infiltration rétroseptale, l’exophtalmie, les anomalies des muscles oculomoteurs, la lyse de la lame papyracée et des cloisons ethmoïdales. Un volume d’abcès supérieur à 661 mm³ était significativement corrélé à la chirurgie, tout comme un diamètre transverse élevé. La concordance interobservateurs était satisfaisante pour plusieurs signes clés, de même que pour l’évaluation de la prise en charge globale.CONCLUSION : cette étude identifie des critères TDM fiables et reproductibles associés à la décision chirurgicale dans les ethmoïdites pédiatriques compliquées. Elle confirme le rôle central de la TDM dans une approche multidisciplinaire, et suggère l’intérêt d’un compte rendu structuré pour limiter la variabilité interindividuelle en contexte d’imagerie d’urgence pédiatrique

Oral exposure to micro- and nanoplastics generated from polyethylene terephthalate suppresses acute intestinal damage in vivo
International audienceMicro- and nanoplastics are emerging environmental contaminants with increasing evidence of their presence in human tissues, particularly via oral exposure. However, toxicological assessments often rely on unrealistically high doses of spherical plastic beads, which do not reflect real-world conditions. In this study, micro- and nanoplastics were generated from polyethylene terephthalate (PET-MNPs) by pin-on-microtextured disc system under ultraviolet irradiation, mimicking their natural fragmentation process in the environment. These PET-MNPs exhibit irregular morphologies, closely resembling those of MNPs found in the environment. To assess their biological impact, mice were orally exposed to environmentally relevant doses of PET-MNPs prior to induction of dextran sulfate sodium-induced acute colitis. Contrary to expectations, exposure to PET-MNPs suppressed disease activity during acute colitis. Histological analysis consistently demonstrated that PET-MNPs reduced acute intestinal damage without affecting the recovery phase. Moreover, RNA-seq analysis revealed that PET-MNPs downregulated key inflammatory pathways, including JAK-STAT, NF-kappaB, cytokine and chemokine signaling, particularly during the acute phase of colitis. These findings indicate that PET-MNPs downregulates acute immune responses in the gut, a site of oral immune tolerance. This is the first in vivo disease model study applying environmentally realistic PET-MNPs, highlighting the complex, non-linear effects of micro- and nanoplastic exposure on intestinal immune regulation
The Viscosity of Multiphase Lava: New Insights from Integrating Laboratory and Field Measurements
International audienceLaboratory measurements on remelted rocks are the standard technique for characterizing the rheology of lava at eruptive temperatures. This approach enables precise measurements but struggles to recreate natural emplacement conditions. Particularly, lavas erupt as multiphase suspensions (melt + crystals + bubbles), but current laboratory methods cannot retain bubbles, and experiments are limited to two-phase (melt+crystals) suspensions. The oxygen fugacity of lavas is known to influence the kinetics of crystallization, and despite its importance, few studies have considered this factor to date. The only available technique to measure the three-phase viscosity of natural lava is through in-situ measurements while it is flowing. Here, we present the first study that integrates viscosity data from laboratory-derived single (melt) and two-phase (melt+crystals) measurements with data from three-phase (melt + crystals + bubbles) field measurements, using the 2023 Litli-Hrútur eruption in Iceland as a case study. We present a rheological characterization of the remelted lava during crystallization at thermomechanical equilibrium and disequilibrium at log fO 2 = -8.7, and deformation rates of 3.3 s -1 . These bubble-free laboratory experiments were designed to overlap with field temperatures (1150-1165 C), enabling direct comparison with the in-situ three-phase field viscosities. We find laboratory and field data overlap in viscosity space (10 2.5 to 10 4.5 Pa s) and at cooling rates of 0.5-1 C/min. Isothermal experiments successfully recreate crystal phases and volumes comparable to natural samples (~20-50 vol.% crystals). Metagenome-scale metabolic modelling for the characterization of cross-feeding interactions in freshwater cyanobacteria-associated microbial communities
International audienceFavored by global changes, freshwater cyanobacterial harmful blooms (HCBs) generate increasing ecological, economical and public health challenges. Microcystis, one the most pervasive genera of cyanobacteria, grows within a phycosphere where specialized interactions with its microbiome occurs, and are suspected to influence bloom appearance and its potential toxicity.Through metagenomic, metabolomic and metabolic modelling, we characterized twelve Microcystis phycosphere cultured after isolation from a French pond. Metagenomic revealed that associated bacteria introduce new functions to the phycosphere, while functional redundancy within and across communities remains. Metabolic reaction presence in Microcystis is consistent with their genospecies, whereas community-levelmetabolic landscape diverges cyanobacteria’s phylogeny. On the other hand, metabolomic results lean on metabolic output led by cyanobacteria. Metabolic modelling and identification of toxic secondary metabolites biosynthetic gene cluster further highlighted differences between phycosphere metabolic capabilities and the importance of manual curation of secondary metabolism in GSMNs. These findings deepen understanding of Microcystis’ phycosphere functioning, demonstrate the relevance of multi-omics systems biology approaches, and lay the ground for further characterisation of freshwater HCB’s microbial interactions and inter-species complementarity

L'objectif de ce colloque a été d'étudier le poids qu'occupe désormais la durabilité dans notre droit économique et d'observer si, à son contact, ce dernier évolue, s'améliore, voire se pervertit. Divers textes la mentionnent, notamment des directives européennes ou des textes de soft law. Surtout, les principales branches du droit économique semblent touchées par sa percée : droit des sociétés, droit financier, droit des contrats, droit de la concurrence, droit de la consommation, droit pénal, et procédure civile.Les interventions de spécialistes de haut niveau (universitaires, magistrat de la Cour de cassation ou membre de l'Autorité de la concurrence) permettent d'éclairer le public sur un mot encore bien mystérieux, mais d'ores et déjà bien enraciné dans notre Droit

Offshore Record of Explosive Volcanic Eruptions in the Southern Part of the Panamá Basin During the Past 10 Myr: 1. Tephrostratigraphy, Cross‐Correlations and Geochemical Characterization
co-auteur étrangerInternational audienceNorthern Andean volcanism is characterized by an intense Quaternary activity, whose onshore deposits have partly covered Mio‐Pliocene products associated with the early development of the arc, making it difficult to obtain an exhaustive catalog of past eruptions. To improve our knowledge of the largest eruptions that occurred in the Northern Andean arc, we analyzed several cores from drilling sites off Ecuador to seek tephra records. We characterize for the first time the mineralogical and geochemical characteristics of tephra beds recorded in the southern part of the Panamá Basin in the sediments of DSDP and ODP drilling Sites 504, 677, 678, 1238, 1239 and 1240. We show that products of at least 27 major eruptions from the Northern Andes have reached the Pacific Ocean since the Early Pliocene, and we have correlated 11 of them between several drilling sites. Products of the oldest volcanism had mainly rhyolitic compositions belonging to a High‐K calc‐alkaline magmatic series, whereas magmas display more heterogeneous SiO 2 and K 2 O contents from the beginning of the Pleistocene. Correlations established in this work allow us to provide new temporal constraints to age models of sedimentary sequences of Sites 677, 1238 and 1240 constructed based on biostratigraphy. In addition, we show that sediments of ODP Site 1240, the closest to the Galápagos islands, recorded several Pleistocene rhyolitic eruptions associated with the hotspot's activity, possibly revealing past oceanic ridge‐hotspot interactions
Development of White Light-Emitting Textiles Using Inorganic Phosphor-Loaded Photopolymers for Potential Lighting Applications
International audienceThe development of flexible textile-based lighting systems remains challenging, usually requiring the combination of light-emitting diodes (LEDs) with polymer optical fibers to achieve 2 large emission areas. However, the overall emission remains point-based, leading to possible glare issues and shadowed zones. In this context, the present work proposes the development of light-emitting textiles that uniformly distribute white light when excited by a blue LED source. Simple cotton substrates were coated with a flexible photoluminescent phosphor-loaded polymer composite. The yellow and/or red phosphors were efficiently incorporated into the polymer matrix via a fast and easy-to-implement UV-induced polymerization reaction, conducted in air and in the absence of any toxic organic solvents. Under blue light, the yellow phosphor-coated textile produced cold white light but the color temperature was easily tuned to warmer nuances by adjusting the amount of red-emitting phosphor, with high color rendering indexes (>70) and luminance values over 500 cd/m². Stability and structural measurements were then carried out to assess possible scale-up options. The versatility of the polymer matrix also expanded the possible applications to 2D surface functionalization and 3D printing
