The epidemiological signature of influenza B virus and its B/Victoria and B/Yamagata lineages in the 21st century
We describe the epidemiological characteristics, pattern of circulation, and geographical distribution of influenza B viruses and its lineages using data from the Global Influenza B Study. We included over 1.8 million influenza cases occurred in thirty-one countries during 2000-2018. We calculated the proportion of cases caused by influenza B and its lineages; determined the timing of influenza A and B epidemics; compared the age distribution of B/Victoria and B/Yamagata cases; and evaluated the frequency of lineage-level mismatch for the trivalent vaccine. The median proportion of influenza cases caused by influenza B virus was 23.4%, with a tendency (borderline statistical significance, p = 0.060) to be higher in tropical vs. temperate countries. Influenza B was the dominant virus type in about one every seven seasons. In temperate countries, influenza B epidemics occurred on average three weeks later than influenza A epidemics; no consistent pattern emerged in the tropics. The two B lineages caused a comparable proportion of influenza B cases globally, however the B/Yamagata was more frequent in temperate countries, and the B/Victoria in the tropics (p = 0.048). B/Yamagata patients were significantly older than B/Victoria patients in almost all countries. A lineage-level vaccine mismatch was observed in over 40% of seasons in temperate countries and in 30% of seasons in the tropics. The type B virus caused a substantial proportion of influenza infections globally in the 21st century, and its two virus lineages differed in terms of age and geographical distribution of patients. These findings will help inform health policy decisions aiming to reduce disease burden associated with seasonal influenza
Prevalence, antimicrobial resistance profile and comparison of methods for the isolation of salmonella in chicken liver from Argentina

Servicio de Antígenos y Antisueros; Argentina.This study was conducted to estimate the apparent prevalence of Salmonella spp. in chicken livers obtained from markets in Entre Ríos, Argentina, using two culture methods (preenrichment and direct selective agar plating). We also determined the antimicrobial resistance of Salmonella isolated strains and evaluated the performance of the two culture methods and selective-differential plating media used for Salmonella isolation. Of 666 chicken livers studied, 32 organs (4.8%) related to 4 poultry slaughterhouse companies were positive for Salmonella sp. using one or two culture methods. Fifty Salmonella strains were isolated from the positive liver samples and were typed into 3 serovars: S. ser. Schwarzengrund (78%), S. ser. Enteritidis (18%), and S. ser. Typhimurium 4(%). More than one Salmonella serovar was found in livers belonging to two chicken slaughterhouse companies. All strains were susceptible to all antibiotics tested, with the exception of erythromycin (100% resistant) and streptomycin (22% intermediate sensitivity). Overall, 32 (4.80%) and 3 (0.45%) of the chicken liver samples were positive for Salmonella sp. in preenrichment method and direct selective agar plating method, respectively; these percentages were significantly different (P=0.0001; kappa=0.16). There was also a statistical difference in relative accuracy, sensitivity and negative predictive value between the preenrichment method and the direct selective agar plating method; the first had greater values for these parameters than the direct selective agar plating method. These parameters were statistically different between MacConkey agar (MCA) and modified lysine iron (MLIA) in the two culture methods; the second had greater values than MCA for both culture methods. This study shows that even though serovars that are important for public health were isolated, the prevalence of Salmonella sp. is low in chicken livers from Entre Rios, Argentina. The isolated strains do not have multi-resistance patterns. Furthermore, the preenrichment method and MLIA are superior to the direct selective agar plating method and MCA for Salmonella sp. isolation from chicken liver samples, respectively

Instituto Nacional de Parasitología; Argentina.Chronic inflammation, as a consequence of the persistent infection with Trypanosoma cruzi, leads to continuous activation of the immune system in patients with chronic Chagas disease. We have previously shown that increased sera levels of soluble P-selectin are associated with the severity of the cardiomyopathy distinctive of chronic Chagas disease. In this study, we explored the expression of biomarkers of platelet and endothelial activation, tissue remodeling, and mediators of the coagulation cascade in patients at different clinical stages of chronic Chagas heart disease. The frequencies of activated platelets, measured by the expression of CD41a and CD62P were decreased in patients with chronic Chagas disease compared with those in uninfected subjects, with an inverse association with disease severity. Platelet activation in response to adenosine diphosphate was also decreased in T. cruzi-infected subjects. A major proportion of T. cruzi infected subjects showed increased serum levels of fibrinogen. Patients with severe cardiac dysfunction showed increased levels of endothelin-1 and normal values of procollagen I. In conclusion, chronic infection with T. cruzi induced hemostatic alterations, even in those patients who do not yet present cardiac symptoms
Brote de intoxicación alimentaria en un jardín de infantes de la provincia de Buenos Aires
On November 27, 2008, a foodborne disease outbreak associated with the consumption of chicken salad occurred in a kindergarten in the District of Hurlingham, Province of Buenos Aires. Thirty-seven children and 10 adults with gastrointestinal symptoms were affected. Five children were hospitalized with signs of dehydration, one of them requiring intensive care. Staphylococcus aureus subsp. aureus was isolated from the mentioned food in 4 out of 5 stool specimens from the patients, and in 3 out of 5 food handlers (nose of food handler #1, hands of food handlers #2 and 3). The isolates carried the genes coding for enterotoxins SEA and SED. The macrorestriction patterns showed 100% similarity by pulsed-field gel electrophoresis using the SmaI enzyme. A timely outbreak investigation allowed us to identify the causative agent of the food poisoning as well as the failures in food processing and to implement corrective measures

We report a case of a 9-year-old female mongrel dog with lesions in the prescapular lymph nodes. The histopathology of a lymph node sample showed flexuous septate hyphae, and a sterile mold grew in culture from that specimen. DNA sequencing of the ITS region allowed us to identify the fungus as Tropicoporus tropicalis. The dog was treated with itraconazole, but it was euthanized six months later due to an unfavorable clinical outcome. Tropicoporus tropicalis is an infrequent pathogen of pets, and the use of molecular tools is needed for its identification. Animal infections due to T. tropicalis were not previously been reported in Argentina

Servicio Fisiopatogenia; Argentina.Shiga toxin-producing Escherichia coli (STEC) are a heterogeneous group of foodborne pathogens causing a broad spectrum of human disease, from uncomplicated diarrhea to hemolytic uremic syndrome (HUS). In this study, we report an HUS case associated with an O59:NM[H19] strain, harboring stx2a, iha, lpfAO26, lpfAO113 genes associated with STEC, and aatA, aap, pic, sigA, agg4A genes associated with enteroaggregative E. coli (EAEC), named Stx-EAEC. The strain showed low toxicity on Vero cells, and was resistant to streptomycin and trimethoprim/sulfonamides. The child carried the bacteria for more than 100 days. Since the large outbreak associated with Stx-EAEC O104:H4, many strains with similar profiles have been described. In Germany, an O59:NM[H19] strain, with comparable characteristics to the Argentine strain, was isolated from a bloody diarrhea case. In Argentina, this is the first report of an HUS case associated with a Stx-EAEC infection, and represents a new challenge for the surveillance system

Comparación de las características moleculares de aislamientos de Neisseria gonorrhoeae con alta y baja resistencia a la azitromicina en Argentina
Introducción y Objetivos: Actualmente, la azitromicina (AZM) en combinación con una cefalosporina de espectro extendido (CEE) se recomienda como terapia empírica de primera línea para el tratamiento de la gonorrea. Sin embargo, la emergencia de aislamientos con baja (CIM: 2-8 μg/ml) y alta (CIM: >=256 μg/ml) resistencia a AZM, sumado a la presencia de aislamientos con resistencia acompañante a CEE, pone en riesgo la efectividad de esta estrategia de tratamiento. En Argentina, un aislamiento con elevada resistencia a azitromicina fue observado en el año 2001. Sin embargo, hasta el año 2018, aislamientos con elevada resistencia a azitromicina no han sido reportados en nuestro medio. El objetivo de este estudio es comparar las características moleculares de aislamientos de N. gonorrhoeae con baja y elevada resistencia a AZM.
Materiales y Métodos: Se estudiaron 21 aislamientos de N. gonorrhoeae colectados por el PROVSAG en el periodo 2005–2016 (n=19, CIM: 2-16 μg/ml), y 2001 y 2018 (n=2, CIM: >=256 μg/ml). El estudio molecular se realizó a través de la secuenciación de genoma completo de todos los aislamientos con la plataforma Miseq (Illumina) usando la química V2. Se realizó el análisis bioinformático donde se estudiaron los principales determinantes de resistencia a AZM (mtrR, ARNr 23S), y genotipificación con NG-MAST y MLST de la secuencia.
Resultados: Aislamientos con baja resistencia a AZM fueron localizados en las provincias de Córdoba (42,1%; n=8), Buenos Aires (52,6%; n=10) y Santa Fe (5,3%; n=1). Sin embargo, ambos aislamientos con alta resistencia a azitromicina se localizaron en la provincia de Buenos Aires. El análisis molecular del ARNr 23S mostró la presencia de la mutación C2611T y A2059G en el 73,7% (14/19) y 100% (2/2) de los aislamientos con baja y alta resistencia a AZM, respectivamente. El gen mtrR reveló un cambio de aminoácido en la posición 45 (G—D) en el 42.8% (9/21) de los aislamientos, deleción de una adenina (A-) en la secuencia invertida del promotor en el 33,3% (7/21), G45G/A- en el 4,8% (1/21) y 9.5% (n=2) mostraron un alelo mosaico (N. meningitidis-like). El análisis de genotipificación mostró relaciones clonales entre aislamientos con baja y elevada resistencia a AZM. El NG-MAST ST696 y MLST ST1580 estuvo presente en el 28,6% (n=5, CIM 4 μg/ml; n=1, >=256 μg/ml) de los aislamientos. Además, dos aislamientos (n=1, CIM 2 μg/ml; n=1, CIM >=256 μg/ml) mostraron el MLST ST9363, y NG-MAST STs (8241 y 3935, respectivamente) con >=99% de identidad en las secuencias porB y tbpB.
Conclusiones: En Argentina, un aislamiento con alta resistencia a AZM es evidenciado después de 15 años. Los aislamientos reportados con baja y alta resistencia a AZM estuvieron circunscriptos a una acotada región geográfica de nuestro país. El estudio de epidemiología molecular mostró aislamientos con una alta relación clonal. Actualmente, en nuestro país se recomienda la utilización de AZM para el tratamiento de C. trachomatis y M. genitalium, aun ante la sospecha y sin confirmación en todos los casos. Los hallazgos obtenidos en este estudio sostienen la necesidad de fortalecer la vigilancia para la detección temprana de aislamientos resistentes y en consecuencia la elaboración de estrategias de salud pública para el control de la diseminación de estos aislamientos
