    How different are self and nonself?

    Biological and artificial networks routinely make reliable distinctions between similar inputs, and the rules for making these distinctions are learned. In some ways, self/nonself discrimination in the immune system is similar, being both reliable and (partly) learned through thymic selection. In contrast to other examples, we show that the distributions of self and nonself peptides are nearly identical but strongly inhomogeneous. Reliable discrimination is possible only because self-peptides are a particular finite sample drawn out of this distribution, and T cells can target the spaces in between these samples. In conventional learning problems, this would constitute overfitting and lead to disaster. Here, the strong inhomogeneities imply instead that the immune system gains by targeting peptides which are similar to self, with maximum sensitivity for sequences just one or two substitutions away. This prediction from the structure of the underlying distribution in sequence space agrees, for example, with the observed responses to mutation derived cancer neoantigens

    Contexte : La réhabilitation améliorée après chirurgie (RAAC) repose sur une approche multimodale visant à réduire la morbi-mortalité, améliorer la récupération et le confort des patients. En chirurgie cardiaque sous circulation extracorporelle (CEC), l’ischémie-reperfusion, la réponse inflammatoire systémique et les risques hémorragiques rendent la récupération complexe. Traditionnellement, les patients restent 48h en réanimation après chirurgie cardiaque. Nous avons émis l’hypothèse qu’une sortie précoce, c’est-à-dire à 24h postopératoires, pouvait être réalisable et sûre dans une population sélectionnée.Méthode : Étude observationnelle prospective, monocentrique au sein du Centre Hospitalier Universitaire de la Timone, Marseille, France. 100 patients ont été inclus entre mai 2024 et mars 2025 selon des critères stricts : FEVG > 50 %, clairance rénale > 45 mL/min, score EuroSCORE II ≤ 5, chirurgie programmée en première position et se limitant à un mono-geste simple. Étaient exclus les chirurgies redux, les patients mineurs et les patients dans un état préopératoire précaire (endocardite active, HTAP sévère, insuffisance cardiaque droite, insuffisance respiratoire chronique, nécessité d’un support inotrope/vasopresseur). Le critère de jugement principal était la faisabilité d’une sortie de réanimation à 24h postopératoires. Les critères de jugement secondaires comprenaient les motifs de maintien en réanimation à 24 et 48h. Résultats : sur 764 patients CEC, 100 répondaient aux critères d’inclusion. 35% des patients étaient considérées comme sortant à 24h. Les non-sortants présentaient des durées plus longues de CEC (67 vs 60 min, p = 0,04) et de clampage aortique (49 vs 43 min, p = 0,02). Les complications hémorragiques étaient plus fréquentes (score composite positif : 28 % vs 6 %, p = 0,01). Les principaux motifs de maintien en réanimation à 24h étaient l’insuffisance rénale aiguë (24,4 %), les troubles du rythme (14,1 %), le recours aux catécholamines (12,8 %), les drains productifs (11,5 %) et l’oxygénodépendance (11,5 %). À J3, dominaient l’oxygénodépendance (30,6 %), les troubles du rythme (19,5 %) et le recours aux catécholamines (19,5 %). Les patients pris en charge pour un remplacement valvulaire présentaient plus de complication (p = 0,03). Dans notre étude, un seul patient sortant à 24h présentait une complication de novo à 48h empêchant sa sortie. Conclusion : la sortie précoce de réanimation après chirurgie cardiaque sous CEC est réalisable et sûre chez une population sélectionnée, mais reste limitée par les complications hémorragiques et les défaillances organiques précoces. L’optimisation de la prévention et de la prise en charge du risque hémorragique est la clé pour élargir la mise en œuvre de programmes RAAC sécurisés en chirurgie cardiaque. Des études multicentriques de plus grande ampleur sont nécessaires pour confirmer ces résultats

    Tensions et enjeux entre histoire de l'art et anthropologie. Réflexions à partir du cas d'Haïti

    What kind of art history can be practised in a society that emerged from a colonial slave-owning system, such as Haiti ? Perspectives sought through an examination of the tensions between art history and anthropology reveal that the hegemony of fine arts still constitutes an obstacle to the full acceptance of the plurality of modes of representation. Promising avenues are emerging from studies on image and visual culture. This work and the debates that accompany it allow us to consider the class of objects (including fine arts) that can be grouped under the name of plastic creation as one of many centres of image production in a given community, presenting a diversity of forms of organisation and crystallisation in time and space. Such a proposition is crucial when we are interested in places where social groups from different visual worlds meet, clash and coexist, as in colonial situations. The case of the French colony of Saint-Domingue, studied from this perspective, demonstrates the heuristic potential of this approach. It leads to a re-examination of the entire history of the visual in colonial and post-colonial Haiti.Quelle histoire de l'art est-il possible de pratiquer depuis une société issue de la matrice coloniale esclavagiste, comme celle d'Haïti ? Des perspectives recherchées à travers une lecture des tensions entre histoire de l'art et anthropologie, il ressort que l'hégémonie des beaux-arts constitue aujourd'hui encore un blocage à l'acceptation pleine et entière de la pluralité des modes de figuration. Des voies prometteuses se dégagent des études sur l'image et la culture visuelle. Ces travaux et les débats qui les accompagnent permettent de considérer la classe d'objets (dont les beaux-arts) qu'on peut regrouper sous le nom de création plastique comme un foyer, parmi d'autres, de production d'images dans une communauté donnée et qui présente une diversité de formes d'organisation, de cristallisation dans le temps et dans l'espace. Une telle proposition est cruciale quand on s'intéresse aux lieux où se rencontrent, s'affrontent, cohabitent des groupes sociaux venant d'univers visuels différents, comme en situation coloniale. Le cas de la colonie française de Saint-Domingue étudié selon cette approche en montre les potentialités heuristiques. Il s'ensuit une remise en chantier de toute l'histoire du visuel dans l'Haïti coloniale et postcoloniale

    Introduction: According to the latest recommendations of the International Ankle Consortium (IAC), the assessment of psychological factors is essential in the decision to return to sport (RTS). The ALR-RSI questionnaire, derived from the ACL-RSI, allows the assessment of emotions, confidence in performance and fear of recurrence after ankle ligament surgery.Objective: The objective of this systematic review is to study the clinimetric properties of the ALR-RSI scale (reliability, validity), with the aim of clinical applicability of this score by physiotherapists in their professional practice.Method: Searches are carried out on seven databases (PubMed, Google Scholar, AMU University Library, ScienceDirect, SPORTDiscus, The Journal of Foot and Ankle Surgery, DiTA).Included studies must meet the determined eligibility criteria and provide data to answer the research question posed.Results: Three studies are included in the systematic review. The ALR-RSI scale has excellent internal consistency and repeatability. The risk of bias related to reliability (internal consistency, repeatability) is moderate to high. Convergent construct validity has a high correlation with other reference scores studied (AOFAS, Karlsson). The risk of bias for convergent construct validity is moderate. A meta-analysis is carried out, it indicates significant statistical heterogeneity for the repeatability outcome.Discussion: The ALR-RSI scale is reliable and valid for assessing the psychological fitness of athletes before their RTS after ankle ligament surgery. In addition, this scale is easy to apply by physiotherapists. Further studies are needed due to identified biases that impair the generalizability of the results, and to support the results on the clinimetric properties of this questionnaire.Introduction : d’après les dernières recommandations de l’International Ankle Consortium (IAC), l’évaluation des facteurs psychologiques est essentielle dans la décision du retour au sport (RTS). Le questionnaire ALR-RSI, dérivé de l’ACL-RSI, permet d’évaluer les émotions, la confiance dans la performance et la peur de la récidive, après une chirurgie ligamentaire de la cheville.Objectif : l’objectif de cette revue systématique est d’étudier les propriétés clinimétriques de l’échelle ALR-RSI (fiabilité, validité), dans un but d’applicabilité clinique de ce score par des kinésithérapeutes dans leur pratique professionnelle.Méthode : les recherches sont réalisées sur sept bases de données (PubMed, Google Scholar, Bibliothèque Universitaire AMU, ScienceDirect, SPORTDiscus, The Journal of Foot and Ankle Surgery, DiTA).Les études inclues doivent correspondre aux critères d’éligibilité déterminées, et apporter des données permettant de répondre à la question de recherche posée. Résultats : trois études sont inclues dans la revue systématique. L’échelle ALR-RSI bénéficie d’une excellente consistance interne et d’une excellente répétabilité. Le risque de biais relatif à la fiabilité (consistance interne, répétabilité) est modéré à fort. La validité par construit convergent présente une forte corrélation avec les autres scores de référence étudiés (AOFAS, Karlsson). Le risque de biais relatif à la validité par construit convergent est modéré. Une méta-analyse est réalisée, elle indique une hétérogénéité statistique importante pour le critère de jugement répétabilité.Discussion : l’échelle ALR-RSI est fiable et valide pour évaluer l’aptitude psychologique du sportif après une chirurgie ligamentaire de la cheville pour son RTS. De plus, cette échelle est facile d’application par les kinésithérapeutes. Des études complémentaires sont nécessaires en raison des biais identifiés qui altèrent la généralisation des résultats, et pour étayer les résultats sur les propriétés clinimétriques de ce questionnaire

    International audienceThe Port-Cros National Park (PCNP) is a terrestrial and marine protected area. Its governance requires a 10-year Scientific Strategy to define the scientific objectives and priorities for all disciplines.• The Scientific Strategy adopted for 2023-2032 promotes an evolution-focused or "evocentric" approach, that is a deliberate conservation of the evolutionary potential and opportunity of non-humans beyond the services they provide humans with (Sarrazin and Lecomte, 2016).• Humans are a component within the (eco)system, contributing, like other species, to the main ecosystem functions (competition, habitat shape, etc.). They benefit from services of other living species and have to manage natural resources in long-term perspective. All species -including humans -play a role in the evolution process.• The scientific goal of the PCNP is to understand these underlying processes, to identify their tangible manifestations and to endeavour to guarantee their functioning.• The challenge is now to translate this ethical evocentric approach into its operational management declination. The conservation timeline we must consider is two-fold: the functions of the ecosystem at short -midterm, and their evolutionary trajectories at long-term.• This poster shows the steps forward, based on the assets of the PCNP 60-year life and ecosystem-based management, and a dedicated workshop.</div

    Étude de cohorte rétrospective monocentrique de patients atteints de pneumopathies interstitielles diffuses associées à un cancer bronchique : description et analyse centré sur les patients traités chirurgicalement

    Les pneumopathies interstitielles diffuses (PID) sont des maladies pulmonaires chroniques, rares et sévères. Parmi elles, la fibrose pulmonaire idiopathique (FPI) est la plus fréquemment associée au cancer bronchopulmonaire (CBP). CBP et PID partagent des facteurs de risque communs (inflammation chronique, tabac, remaniements tissulaires et altérations génétiques). La prévalence du CBP chez les patients atteints de PID varie largement (2,7 % à 48 %). La coexistence CBP-PID représente un défi clinique majeur, du fait du risques compétitifs entre la survenue d’une exacerbation aiguës (EA) de PID avec une mortalité atteignant jusqu’à 50 % et le pronostic plus sombre d'une pathologie cancéreuse traitée de manière sous optimale. Nous avons mené deux études rétrospectives monocentriques, à l’Assistance Publique–Hôpitaux de Marseille. La première (2010-2025), a inclus 102 patients atteints de CBP-PID. Les caractéristiques démographiques, cliniques, fonctionnelles, histologiques, moléculaires, ainsi que les événements respiratoires aigus (ERA), les traitements et la concordance avec les recommandations RCP ont été recueillies. La survie globale (SG) constituait le critère principal d’évaluation. La SG médiane de la cohorte était de 17.7 mois, avec une SG à 5 ans de 12.7 %. En analyse multivariée, deux facteurs se sont avérés indépendamment associés à la survie : le coefficient de transfert du monoxyde de carbone (KCO), dont chaque augmentation de 1 % réduisait le risque de décès de 3 %, et le performance status (PS). Nos résultats suggèrent un profil moléculaire distinct, dominé par les mutations de TP53, KRAS et les altérations de la voie PI3K. Les ERA, l'intention curative, la concordance RCP montraient des associations en analyse univariée. La seconde étude (2006-2023), a inclus 43 patients opérés pour un CBP-PID et 4073 patients ayant bénéficié d’une résection pour CBP sans PID, identifiés par la base de données chirurgicale française Epithor. La SG était abaissée dans le groupe CBP-PID sans atteindre le seuil de significativité (45 mois versus 84 mois, p = 0.068). Les patients sélectionnés pour la chirurgie présentaient un faible taux de complications et d’ERA, probablement en raison d’un processus de sélection rigoureux dans un centre expert. Ces résultats soulignent la vulnérabilité des patients CBP-PID et les limites d'application des recommandations standards du CBP du fait du risque élevé d'évènements respiratoires. Si la chirurgie est confirmée comme la principale option curative pour des patients sélectionnés, pour les autres la prise en charge repose largement sur la chimiothérapie, avec des bénéfices limités. L'intégration de paramètres comme le KCO et le PS sont ici confirmés comme ayant un impact pronostic important permettant de guider la décision thérapeutique

    Introduction : les pneumopathies aiguës communautaires (PAC) sévères représentent une cause majeure de morbi-mortalité, et nécessitent la mise en place d’une antibiothérapie probabiliste rapide et adaptée. Le choix de la bêtalactamine initiale, entre l’association amoxicilline/acide clavulanique (AMC) et une céphalosporine de 3ᵉ génération (C3G), reste débattu. Objectif : décrire la sensibilité à l’AMC et aux C3G des PAC sévères, documentées à Streptococcus pneumoniæ, Hæmophilus influenzæ et/ou Staphylococcus aureus, et analyser l’adéquation de l’antibiothérapie empirique. Méthodes : Étude rétrospective monocentrique incluant les patients admis en réanimation entre janvier 2018 et novembre 2022 pour une PAC sévère (incluant les pneumopathies d’inhalation communautaires), avec documentation microbiologique des germes cités plus haut. Les données cliniques, microbiologiques et thérapeutiques ont été recueillies et analysées. Résultats : Parmi les 104 patients inclus (âge médian 64 [48–71] ans ; ratio Femme/Homme 1/2), la sensibilité à l’AMC était de 81,5 % pour Streptococcus pneumoniæ, 78 % pour Hæmophilus influenzæ et 100 % pour Staphylococcus aureus. La sensibilité aux C3G était respectivement de 96,3 %, 95,1 % et 100 %. L’AMC a été prescrite dans 59 % des cas, appropriée dans 86,9 % des situations. Les C3G, utilisées dans 20 % des cas, étaient majoritairement non justifiées. La désescalade antibiotique a été réalisée dans environ un tiers des cas. La mortalité semble davantage liée à la gravité initiale qu’au choix empirique d’antibiothérapie. Conclusion : l’AMC semble être une option antibiotique pertinente dans le traitement probabiliste des PAC sévères. Les C3G apparaissent souvent surutilisées, soulignant la nécessité de nouveaux travaux pour optimiser les prescriptions afin de limiter la pression de sélection antibiotique

    Automatic Segmentation of Primary Central Nervous System Lymphoma at Clinical Routine Postcontrast T1-weighted MRI

    International audiencePurpose To develop and validate a deep learning model for automatic segmentation of primary central nervous system lymphoma (PCNSL) at postcontrast T1-weighted MRI. Materials and Methods Data were retrospectively collected from patients with pathologically proven immunocompetent PCNSL between September 2010 and February 2022. Postcontrast T1-weighted MRI scans were used to train and validate a deep learning model based on the nnU-Net framework. Manual segmentation by neuroradiologists served as the reference standard. The model was trained using an internal dataset from a single center and tested on both internal and external test sets from seven additional centers. Performance was assessed using Dice score, mean average surface distance, and F1 score. Statistical comparisons were performed using Mann-Whitney U test and bootstrap resampling for CIs. Results The study included 135 patients (68 female, 66 male, and one of unspecified sex; internal dataset: mean age ±SD, 67.0 years ± 12.0; external dataset: mean age, 75.5 years ± 13.6). The model achieved a mean Dice score of 0.84 (95% CI: 0.79, 0.88) on the internal test set (n = 44) and 0.88 (95% CI: 0.84, 0.91) on the external test set (n = 48), with no evidence of a difference between test sets (P = .59). Performance varied by lesion type; accuracy was highest in homogeneous discrete lesions, and performance was slightly decreased when numerous poorly defined infracentimetric lesions occurred. Strong volumetric correlation was observed between automatic and manual segmentations (internal: r = 0.99, P &lt; .001; external: r = 0.98, P &lt; .001). Conclusion A deep learning model achieved accurate and robust automatic segmentation of PCNSL across multiple clinical centers with different MRI acquisition parameters

    Transition metal nanolines grown on a functionalized silver substrate

    International audienceThe self-organized growth of 3d transition metal nanolines on the Ag(110) surface functionalized with Si nanoribbon arrays is investigated by means of scanning tunneling microscopy. Deposition of Fe, Co and Ni on the nanoscale template at room temperature leads to the formation of high densities of identical and regularly spaced nanolines with the same crystallographic orientation. The length of these nanolines is governed by the diffusion process of the transition metal atoms into the Si nanoribbons and varies with the chemical nature of the deposited metal. We show that the transition metal nanolines adopt the same dimer structure and the atomic dimensions are accurately extracted from highresolution scanning tunneling microscopy images. At coverages higher than a monolayer, the transition metal growth reproduces the one-dimensional pattern of the Si template.Our study highlights the potential of Si nanoribbon templates to guide the 3d transition metal growth to form high densities of well-defined one-dimensional nanostructures

    Anomalous self-diffusion in tungsten and molybdenum: Exonerating the di-vacancy contribution and the key role of interatomic interaction

    International audienceThis study completes the long-standing effort to understand experimental measurements of self-diffusion in body-centered cubic (bcc) metals. Over the past decade, a growing consensus has emerged regarding the attribution of the anomalous non-Arrhenius behavior to the anharmonicity of atomic interactions. This work advances the understanding of two fundamental issues: (i) the free energy contributions from small vacancy clusters are quantitatively accounted for, leading to the conclusion that the di-vacancy contribution is negligible; and (ii) quantitative agreement between theory and experiment can only be achieved when employing interatomic interactions of a quality beyond standard exchange-correlation functionals, such as those provided by recent meta-generalized gradient approximations (meta-GGAs). The present conclusions are made possible thanks to an accurate characterization of the free energy landscape of metals that goes beyond the perfect bulk and simple defects, such as mono-vacancies. Moreover, the present treatment is able to provide the free energy landscape not only for the absolute minimum of defect configurations, but also for other local minima. These achievements make use of advanced Bayesian sampling techniques applied to the calculation of the formation free energy and the optimal solution through averaged force projection for migration free energy, utilizing high-precision, data-driven force fields at ab initio accuracy. We emphasize the importance of atomic interactions, as produced by the exchange-correlation functional underlying the data-driven force fields, by investigating two machine-learning potentials constructed using standard GGAs. Corrections to formation energies at zero temperature, arising from differences between the standard GGA and a recent meta-GGA functional, are shown to be the primary source of discrepancies between theoretical predictions and experimental values. The workflow developed in this study paves the way for the comprehensive study of diffusion in materials and enables automated, systematic investigations of complex energetic landscapes
