Identificar especificamente as solicitudes que requiran extracción hospitalaria, para facilitar a obtención da cita da extracción, evitar erros e molestias aos pacientes.Sistematizar la solicitud de analíticas prescritas por los profesionales de la EOXI de Ferrol con el fin de que la extracción se realice en los centros de salud de referencia salvo las excepciones que se especifican en esta GUÍA y que se realizan en el Hospital. Identificar específicamente las solicitudes que requieran extracción hospitalaria, para facilitar la obtención de la cita de la extracción, evitar errores y molestias a los pacientes

    [es] Introducción: durante los últimos años, el cribado prenatal mediante test de ADNflc (ADN fetal libre circulante) basado en el test del ADN fetal en sangre materna ha surgido como una alternativa al cribado combinado del primer trimestre (CCPT), ya que se estima que podría tener una mayor capacidad de detección de las trisomías T21, T18 y T13. Inicialmente este test ha sido valorado fundamentalmente como prueba contingente dentro del cribado prenatal, pero recientemente varias guías lo recomiendan como prueba de cribado de primera línea, no existiendo actualmente un consenso a nivel nacional o internacional respecto de la estrategia de cribado más adecuada, ya que en la decisión de adoptar una u otra estrategia influyen muchos otros factores aparte de la validez diagnóstica y el coste. Con el objetivo de proceder a establecer un protocolo consensuado que defina los criterios de indicación de la prueba en todo el Sistema Nacional de Salud, la Dirección General de Cartera Básica de Servicios del SNS y Farmacia del Ministerio de Sanidad, Consumo y Bienestar Social ha solicitado un nuevo informe sobre el test de ADNflc a la Red Española de Evaluación de Tecnologías Sanitarias (RedETS). Objetivos: el principal objetivo es evaluar la seguridad y efectividad comparada de las principales estrategias de cribado con el test de ADN fetal para la detección de las trisomías 21 (T21), 18 (T18) y 13 (T13), estableciendo la validez diagnóstica (variables intermedias) y las implicaciones en términos de medidas de resultado pertinentes para la mujer embarazada y/o feto. Se pretende también analizar los costes y las principales cuestiones organizativas, éticas y sociales relacionadas con la implantación de las distintas estrategias de cribado a nivel nacional. Métodos: adaptación del informe de la Red Europea de Evaluación de Tecnologías Sanitarias (EUnetHTA) “Screening of fetal trisomies 21, 18 and 13 by non invasive prenatal testing”, realizado por la Unidad de Asesoramiento Científico-técnico de la Agencia Gallega del Conocimiento en Salud (Avalia-t; ACIS), en el 2018. Siguiendo las recomendaciones de la “Guía para la elaboración y adaptación de informes rápidos de evaluación de tecnologías sanitarias”, elaborado en el marco de la RedETS, se incorporaron los principales resultados de este informe y la información se complementó con datos nacionales en todos aquellos dominios en donde fue posible la contextualización. El informe final fue elaborado atendiendo a las consideraciones de un informe de “novo”. Resultados: respecto al T21, el meta-análisis sobre el test de ADNflc como prueba de primera línea en embarazos únicos (reemplazo total del CCPT), basado en 8 estudios sobre 136 544 mujeres embarazadas (885 aneuploides y 135 659 euploides), mostró una sensibilidad estimada del 99.3 % (IC 95 % 97.8 %-99.8 %) aplicando el modelo de efectos aleatorios. La especificidad fue del 99.9 % (95 % IC 99.8 %-99.9) excluyendo abortos, pérdidas fetales y resultados inciertos/fallidos. Según los datos de la revisión Cochrane, la sensibilidad y especificidad del test combinado del primer trimestre para un nivel de riesgo de 1:300 se estimaría en el 87.26 % (IC 95 % 85.18 %-89.09 %) y el 95.50 % (IC 95 % 94.86 %-96.05 %). Por tanto, si se hace una comparación indirecta, asumiendo una prevalencia de T21 de 24 en 10 000, el valor predictivo positivo del test de ADNflc como prueba primaria sería del 82.6 % frente al 4.4 % del test combinado, con cero falsos negativos en comparación con 0.03 %. En este hipotético escenario, se esperaría que la tasa de detección aumentase ligeramente (29 casos extra detectados por 100 000 mujeres) y que se redujese el número de pruebas invasivas. Si se asume que el 90 % de todas las mujeres con alto riesgo aceptasen realizarse una prueba invasiva, se estimaría una reducción considerable en el número de pruebas de confirmación (4 233 mujeres por 100 000 darían positivas con el test combinado frente a 259 con test de ADNflc). La sensibilidad estimada en base a 24 estudios (1408 aneuploides y 99 818 euploides) que evaluaron el test de ADNflc como prueba contingente fue del 99.2 % (95 % IC 98.59 %-99.56 %) y la especifidad del 99.95 % (95 % CI 99.93 %-99.96 %). Atendiendo a estos datos y a los de la Cochrane, si se asume que todas las mujeres con resultados positivos en el test combinado se realizarían la prueba de ADN fetal, se estimaría que la sensibilidad de la estrategia contingente (CCPT + test de ADNflc) sería del 86.8 % (95 % IC 82.2 %-90.4 %). La especifidad se estima en el 100 % (95 % CI 99.9 %-100 %). Por tanto, el valor predictivo positivo sería del 99.1 % (95 % CI 96.7 %-99.7 %) y el valor predictivo negativo (VPN) del 100 % (95 % CI 99.9 %-100 %). En este hipotético escenario, la estrategia contingente resultaría en dos falsos negativos adicionales por cada 100  000 mujeres cribadas. Asumiendo que se confirman un 90 % de todos los positivos, se estimaría que 189 mujeres realizarían pruebas invasivas con el cribado contingente frente a las 4238 después del cribado combinado. Las dos estrategias de cribado que incluyen el test de ADNflc (solo test ADNflc o CCPT+test ADNflc) presentan ventajas e inconvenientes cuando se comparan entre sí. El empleo del test de ADNflc como prueba única de cribado reduciría el número de casos no detectados en relación al cribado contingente (2 versus 288 in 100  000 mujeres cribadas) pero resultaría en un mayor número de pruebas invasivas. Como prueba contingente, no aumentaría la tasa de detección, sino que probablemente exista riesgo de que se pierdan algunos casos, pero se evitaría un porcentaje importante de pruebas invasivas. Los costes totales estimados para una población de 100 000 mujeres embarazadas, asumiendo una prevalencia de T21 de 24 por 10 000 habitantes y una frecuencia de muerte fetal del 1 %, se estimaría en 9 739 077 € para el cribado combinado, en 8 68 2160 para el cribado con el test ADNflc como prueba contingente (CCPT+test ADNflc) y 28 764 629 € para el test de ADNflc como prueba de primera línea. Para el T18, la sensibilidad del test de ADNflc como prueba primaria calculada en base al modelo bivariante de efectos aleatorios, excluyendo abortos, pérdidas fetales y resultados inciertos/fallidos se estimó en el 97.4 % (IC 95 % 94.4 %-98.8 %) y la especificidad en el 99.90 % (IC95 % 99.87 %-99.97 %). No obstante, el modelo proporciona estimaciones poco fiables debido a la escasez de datos y al pequeño número de casos aneuploides (234 aneuploides versus 13 5405 euploides). Debido a este hecho y a la baja calidad de la evidencia no se consideró oportuno ningún tipo de modelaje. En cuanto a la T13, la sensibilidad estimada a través del modelo bivariante de efectos aleatorios fue del 98.8 % (IC95 % 1.41 %-100 %) y la especificidad del 99.9 % (IC95 % 99.94 %-99.97 %). No obstante, el modelo fue inestable debido al gran número de celdas con resultados en blanco. Debido a este hecho y a la baja calidad de la evidencia no se consideró oportuno ningún tipo de modelaje. Discusión: la calidad de la evidencia de los estudios sobre T21 que evalúan el uso del test de ADNflc como prueba de primera o segunda línea fue moderada para sensibilidad y baja para especificidad (GRADE). La principal limitación de los estudios reside en el alto riesgo de sesgos. El seguimiento fue incompleto en todos los estudios y los dos estudios que más contribuyen a los resultados en el caso del test de ADNflc como prueba de primera línea, dado su tamaño muestral, muestran pérdidas del 16.4 % y del 23 %. Asimismo, la verificación de los casos negativos del test de ADNflc se realiza en base a los registros, bases de datos médicas o entrevistas, arrojando importante incertidumbre al respecto de la fiabilidad de los resultados de especificidad. La aplicabilidad de las estimaciones del valor predictivo positivo y valor predictivo negativo también es dudosa debido a que la prevalencia de T21 en los estudios no es representativa de aquella en la población general. El hecho de haber excluido abortos, pérdidas fetales y casos con resultados inciertos/fallidos pudo haber ocasionado una sobreestimación de la especificidad tanto en la población general como en la población de alto riesgo. En general los estudios sobre T18 y T13 presentan poco poder estadístico debido al pequeño tamaño muestral y esto explica el que no se encuentren FP o FN para ninguna de estas aneuploidías. Conclusiones: 1) la evidencia apunta a que la sensibilidad del test de ADNflc para la detección del T21 es significativamente mayor que la del test combinado cuando se emplea como prueba de cribado de primera línea y a que el reemplazo podría reducir el número de pruebas invasivas innecesarias. No obstante, existe incertidumbre al respecto de la especificidad de estas pruebas debido a la inapropiada verificación de los casos negativos. Asimismo, la aplicabilidad de las estimaciones de VPP y VPN es limitada debido a que la prevalencia de T21 en los estudios no es representativa de aquella en la población general; 2) la evidencia también apunta a que el uso del test de ADNflc como prueba contingente también podría reducir sustancialmente el número de pruebas invasivas, aunque este dato necesita ser confirmado con datos reales de la práctica clínica. La falta de verificación de los casos fallidos y resultados inciertos podría contribuir a cambiar este resultado; 3) la baja calidad de la evidencia existente sobre T18 y T13 no permite extraer conclusiones definitivas al respecto de estas trisomías; 4) existe incertidumbre respecto a la exactitud diagnóstica del test de ADNflc en embarazos gemelares; 5) se necesitan estudios comparativos prospectivos para evaluar la efectividad de las distintas estrategias, evaluando la detección de todas las anomalías, interrupciones voluntarias de embarazo, abortos y otras variables relacionadas con el paciente. A día de hoy, existen importantes incertidumbres al respecto de cual es la mejor estrategia de cribado.[en] Introduction: during the past years, the non-invasive prenatal screening based on the analysis of cell free fetal DNA from maternal blood of pregnant women, has emerged as an alternative to first trimester combined screening (FCT), because of its alledged greater detection of trisomies T21, T18 and t13. Initially, these tests were offered as a contingent test within the prenatal screening strategy, but recently various guidelines recommend its use as a first-line screening test. To date, there is no consensus at the national or international level with respect to the most appropriate screening strategy, as the decision to adopt a given strategy can depend on many factors, apart from the diagnostic accuracy and cost. With the aim of establishing a consensual protocol that defines the indication criteria for the whole National Health Care System (NHS), the Directorate General of Basic Health Portfolio of the NHS and Pharmacy of the Ministry of Health, Consumption and Social Welfare, has requested a new HTA assessment of cfDNA from the Spanish Network of Health Technology Assessment Agencies (RedETS). Objectives: the main objective is to assess the comparative safety and effectiveness of the main prenatal screening strategies including cfDNA for the detection of trisomies 21 (T21), 18 (T18) y 13 (T13), establishing the diagnostic accuracy (intermediate variables), as well as the implications in terms of relevant outcomes for the pregnant women and/or fetus. The report also aims to analyse the costs and the main organizational, ethical and social issues related to the implantation of the different screening strategies at the national level. Methods: adaptation of the European Network of Health Technology Assessment (EUnetHTA) report “Screening of fetal trisomies 21, 18 and 13 by non invasive prenatal screening” carried out by the health technology assessment unit of the Galician Agency for Health Knowledge Management (Avalia-t; ACIS) in 2018. In line with the recommendations of the “guideline for elaboration and adaptation of rapid health technology assessment” developed within the framework of the RedETS, the report incorporates the main results of the assessment and complements existing information with national data wherenever contextualization is possible. Results: the meta-analysis of the diagnostic accuracy of NIPT as a primary testing method for T21 in singleton pregnancies (total replacement of FCT), calculated based on 136 544 pregnant women (885 aneuploid and 135659 euploid cases), yielded a pooled estimate of sensitivity (S) of 99.3 [95 %CI 97.8 to 99.8], using the bivariate effect model. The specificity (Sp) was 99.9 [95 %CI 99.8 to 99.9] excluding miscarriages, fetal losses and uncertaing/ indeterminate/no call results. Based on the accuracy data of the Cochrane review, the estimated pooled sensitivity of FCT for the risk level of 1:300 is estimated to be 87.26 [95 %-CI 85.18 to 89.09] and the estimated pooled specificity is 95.50 % [95 %CI 94.86 to 96.05]. Therefore, if an indirect comparison is carried out, assuming a prevalence of T21 of 24:10  000 pregnant women (EUROCAT data), the positive predictive value of NIPT as a primary test would be 82.6 % in comparison to 4.4 % for the FCT, with cero false negatives versus 0.03 %. In this hypothetical scenario, the detection rate would increase slightly (29 extra cases detected in 100 000 screened women) and the number of invasive test would be reduced. If we assume that around 90 % of all of the women that would test positive would undergoe invasive testing, a substantial reduction in invasive testing would be expected (4233 out of 100 000 screened women would undergoe invasive testing with the FCT strategy versus 259 with NIPT as a primary test). The pooled sensitivity, estimated based on twenty four studies (1408 aneuploidy cases and 99818 euploid cases) which provided data on the accuracy of NIPT as a second tier test was 99.21 %, 95 %CI 98.59 to 99.56 and Sp reached 99.95 %, 95 %CI 99.93 to 99.96. Based on this data and on the accuracy data of the Cochrane review, assuming that all women testing positive with FCT would undergo NIPT testing, the estimated sensitivity of the add-on strategy (FCT + NIPT) for a hypothetical cohort of 10 000 would be 86.8 % [95 % CI 82.2 %-90.4 %]. Estimated specificity would be 100 % [95 % CI 99.9 %-100 %]. The positive predictive value would be 99.1 % [95 % CI 96.7 %-99.7 %] and the negative predictive value 100 % [95 % CI 99.9 %-100 %]. In this hypothetical scenario de contingent strategy would lead to two additional false negatives in 100 000 screened women. Assuming that 90 % of the women testing positive would undergo confirmation, 189 would be prone to invasive procedures with contingent screening in comparison to 4238 with FCT. Both NIPT strategies (NIPT only or FCT+NIPT) show advantages and inconveniences when they are compared against each other. The use of NIPT as a sole screening test would lead to an important reduction in the number of non detected cases in comparison to contingent screening (2 versus 1000 screened women) but would in turn occassionate a greater number of invasive tests. As a contingent test, the detection rate will not increase with regards to FCT, on the contrary, there is a possibility of missing some cases, although less invasive testing is expected. The total estimated costs for a population of 100 000 pregnant women, assuming a prevalence of T21 of 24:10 000 would be 9 739 077 € for the FCT strategy, 8682160 for the contingent strategy (FCT+NIPT) and 28 764 629 € for the NIPT only strategy. For T18, the pooled sensitivity of NIPT as a primary screening test calculated applying the bivariate random-effect model, was estimated to be 96.86 % [95 %CI 88.35 % to 99.21 %] and pooled Specificity was 99.97 [95 %CI 99.93 to 99.98]. However, the model was unreliable due to the lack of variability in the specificity and the great number of aneuploidy cases (234 versus 135405). Due to this fact and the low quality of the evidence, it was not considered appropriate to do any modelling. Discussion: the quality of evidence of the studies on T21 that assess the use of NIPT as a primary or secondary testing method was moderate for sensitivity and low for specificity (GRADE approach). The main limitation of the studies was the high risk of bias. The follow up was incomplete in all of the studies and the two studies that contribute mostly to the results of NIPT as a first tier test, given their sample size, showed losses of 16.4 % and 23 %. In addition, the verification of the negative results of the cfDNA test was based on registries, medical databases or interviews, raising important uncertainties regarding the reliability of the specificity results. The applicability of the estimations of the positive predictive value and negative predictive was also doubtful given that the prevalence of T21 in the included studies was not representative of that found in the general population. The exclusion of miscarriages, fetal lossess and uncertain/no call results could have led to an overestimation of the specificity for both the general and high-risk population. In general, the studies on T18 and T13 have very low statistical power due to the small simple size and this can explain the lack of false positives or false negatives for these aneuploidies. Conclusions: 1) existing evidence supports that the sensitivity for T21 is significantly higher for NIPT than for FCT, when used as a first tier screening test, and that the replacement of FCT by NIPT would lead to a considerable reduction in unnecessary invasive testing. The generalisability of the positive predictive value and negative predictive value is limited by the fact that the prevalence of T21 was not representative of that found in general pregnant population; 2) available data also suggests that the use of NIPT as an add-on to combined testing for high risk T21 population screening could also lead to substantial reductions in unnecessary invasive. However, this results needs to be confirmed with real world data. The performance of the test (test failures, uncertain results) and the uptake of NIPT screening are amongst the factors that could contribute to change this ratio in real practice. 3) the low quality of the evidence for T18 and T13 does not allow for drawing conclusions on these trisomies for any of the screening pathways. 4) uncertainty remains regarding the diagnostic accuracy of NIPT testing in twin pregnancies. 5) Appropriately designed prospective comparative studies are required in order to be able to assess the performance of the different test strategies, taking into account detection of all abnormalities, abortions, miscarriages and other patient related outcomes.     Os primeiros cigarros electrónicos aparecen no mercado chinés no ano 2003 e en 2006 introdúcense en Europa e Estados Unidos. En España a venda de cigarros electrónicos comeza en 2006 a través de Internet. En Galicia disponse de información sobre o uso de cigarros electrónicos desde 2014 a través do Sistema de Información sobre Condutas de Risco (SICRI). O uso de cigarros electrónicos e de tabaco sen combustión é, englobal, minoritario entre os galegos. Aínda que na poboación de 16 a 24 anos apreciase un aumento na prevalencia no uso de cigarros electrónicos. As medidas de vixilancia do consumo, e mesmo dos efectos sobre a saúde, deben manterse e sería recomendable estudar polo miúdo o uso en menores de 16 anos.Los primeros cigarrillos electrónicos aparecen en el mercado chino en el año 2003 y en 2006 se introducen en Europa y Estados Unidos. En España la venta de cigarrillos electrónicos comienza en 2006 a través de Internet. En Galicia se dispone de información sobre su uso desde 2014 a través del Sistema de Información sobre Conductas de Riesgo (SICRI). El uso de cigarrillos electrónicos y de tabaco sin combustión es, en global, minoritario entre los gallegos. Aunque en la población de 16 a 24 años se aprecia un aumento en la prevalencia en su uso. Las medidas de vigilancia de consumo, y también de los efectos sobre la salud, deben mantenerse y sería recomendable estudiar detalladamente el uso en menores de 16 años

    Molecular docking and machine learning analysis of Abemaciclib in colon cancer

    BACKGROUND: The main challenge in cancer research is the identification of different omic variables that present a prognostic value and personalised diagnosis for each tumour. The fact that the diagnosis is personalised opens the doors to the design and discovery of new specific treatments for each patient. In this context, this work offers new ways to reuse existing databases and work to create added value in research. Three published signatures with significante prognostic value in Colon Adenocarcinoma (COAD) were indentified. These signatures were combined in a new meta-signature and validated with main Machine Learning (ML) and conventional statistical techniques. In addition, a drug repurposing experiment was carried out through Molecular Docking (MD) methodology in order to identify new potential treatments in COAD. RESULTS: The prognostic potential of the signature was validated by means of ML algorithms and differential gene expression analysis. The results obtained supported the possibility that this meta-signature could harbor genes of interest for the prognosis and treatment of COAD. We studied drug repurposing following a molecular docking (MD) analysis, where the different protein data bank (PDB) structures of the genes of the meta-signature (in total 155) were confronted with 81 anti-cancer drugs approved by the FDA. We observed four interactions of interest: GLTP - Nilotinib, PTPRN - Venetoclax, VEGFA - Venetoclax and FABP6 - Abemaciclib. The FABP6 gene and its role within different metabolic pathways were studied in tumour and normal tissue and we observed the capability of the FABP6 gene to be a therapeutic target. Our in silico results showed a significant specificity of the union of the protein products of the FABP6 gene as well as the known action of Abemaciclib as an inhibitor of the CDK4/6 protein and therefore, of the cell cycle. CONCLUSIONS: The results of our ML and differential expression experiments have first shown the FABP6 gene as a possible new cancer biomarker due to its specificity in colonic tumour tissue and no expression in healthy adjacent tissue. Next, the MD analysis showed that the drug Abemaciclib characteristic affinity for the different protein structures of the FABP6 gene. Therefore, in silico experiments have shown a new opportunity that should be validated experimentally, thus helping to reduce the cost and speed of drug screening. For these reasons, we propose the validation of the drug Abemaciclib for the treatment of colon cancer

    Knowledge, Attitudes, and Intentions towards HIV Pre-Exposure Prophylaxis among Nursing Students in Spain

    Human immunodeficiency virus (HIV) infection is one of the main causes of morbidity and mortality worldwide. Among the preventive approaches proposed to control this disease is pre-exposure prophylaxis (PrEP), whose effectiveness depends on the medication adherence. The aim of the present study was to determine the knowledge and attitudes about PrEP among a sample of Spanish nursing students as well as their intentions of receiving it in case it was indicated. An observational cross-sectional descriptive study was carried out. A total of 570 nursing students from the University of Santiago de Compostela (Spain), >/=18 years old and of both sexes were invited to self-complete a questionnaire between February and March 2020. A total of 352 students decided to participate in the study. Participants had low knowledge [overall knowledge score 1(0-2)] and a neutral attitude towards PrEP. The intention of receiving PrEP improved significantly after the completion of the questionnaire and the administration of information about PrEP (p = 0.039; before: 23.58% and after: 93.77%). Nursing staff play an important role in the prevention of sexually transmitted diseases, so their training in preventive strategies, such as PrEP, could help to reduce the incidence of new cases of HIV infection
