    International audienceDans mon intervention, j’exposerai brièvement le dispositif institutionnel(CASNAV, direction d’établissement (lycée pro, CFA), équipepédagogique, équipe socio-éducative) qui dirige les jeunes allophones versles formations professionnelles. Puis, en m’appuyant sur l’enquêteethnographique menée dans le projet MATHALLO, je mettrai en exergue lesenjeux pédagogiques relatifs à la prise en compte des connaissances enmathématiques des jeunes non scolarisés antérieurement dans un contexte deformation dont la durée est extrêmement courte.     European Society of Cardiology quality indicators for the management of acute coronary syndrome

    International audienceAbstract gap for the treatment of acute coronary syndrome (ACS) is central to improving quality of care. Under the European Society of Cardiology (ESC) framework, we aimed to develop updated quality indicators (QIs) for the evaluation of quality of care and outcomes for patients with ACS. Methods A Working Group of experts including members of the ESC Clinical Practice Guidelines Task Force for ACS, Acute CardioVascular Care Association and European Association of Percutaneous Cardiovascular Interventions followed the ESC methodology for QI development. This methodology involved (i) the identification of the domains of ACS care for the diagnosis and management of ACS; (ii) the construction of candidate QIs through a systematic review of the literature; and (iii) the selection of the final set of QIs (using a modified Delphi method). Results Five domains of care for the diagnosis and management of ACS were identified: (1) structural framework and logistics, (2) in-hospital non-invasive care, (3) invasive strategy and periprocedural management, (4) secondary prevention interventions, and (5) outcomes. In total, 21 main QIs were selected, covering all five domains of care for the diagnosis and management of ACS. Conclusions This document defines five domains of ACS care, and provides 21 QIs for the diagnosis and management of ACS. The updated ESC QIs for ACS may be used for quality improvement initiatives

    Methicillin-Resistant Staphylococcus aureus Bacteremia Relapses Show Diverse Genomic Profiles but Convergence in Bacteremia-Associated Genes

    International audienceAbstract Background Recurrence of methicillin-resistant Staphylococcus aureus (MRSA) bacteremia is a high risk complication for patients. Distinguishing persistent lineages from new infections is not standardized across clinical studies. Methods We investigated factors contributing to recurrence of MRSA bacteremia among subjects in Philadelphia, Pennsylvania. Subject demographics and clinical history were collected and paired with whole-genome sequences of infection isolates. Recurrent bacteremia episodes were recorded and defined as relapse infections (same lineage) or new infections by genomic criteria, where a relapse contained isolates ≤ 25 single nucleotide polymorphisms (SNP) different, and by clinical criteria. All isolates were assessed for pairwise SNP distances, common mutations, and signatures of within-host adaptation using the McDonald-Kreitman test. Clusters of transmission between relapse-associated isolates and other subject lineages were identified. Results Among 411 sequential subjects with MRSA bacteremia, 32 experienced recurrent bacteremia episodes, with 24 subjects having exclusively relapse infections, 6 with infections exclusively from a new strain, and 2 patients with both relapse and new infections. No concordance between a genomic and a clinical definition of relapse was evident (Cohen κ = 0.18; confidence interval, −0.41). Recurrence-associated lineages exhibited signatures of positive selection (G test, < 0.01). Genes with SNPs occurring in multiple relapse lineages have roles in antibiotic resistance and virulence, including 5 lineages with mutations in mprF and 3 lineages with mutations in rpoB, which corresponded with evolved phenotypic changes in daptomycin and rifampin resistance. Conclusions Recurrent infections have a diverse strain background. Relapses can be readily distinguished from newly acquired infections using genomic sequencing but not clinical criteria

    <i>Euclid</i>: the potential of slitless infrared spectroscopy: a <i>z</i> = 5.4 quasar and new ultracool dwarfs

    International audienceWe demonstrate the potential of Euclid ’s slitless spectroscopy to discover high-redshift (z &gt; 5) quasars and their mainphotometric contaminant, ultracool dwarfs. Sensitive infrared spectroscopy from space is able to efficiently identify bothpopulations, as demonstrated by Euclid Near-Infrared Spectrometer and Photometer Red Grism (NISP RGE ) spectra of thenewly discovered z = 5.404 quasar EUCL J181530.01+652054.0, as well as several ultracool dwarfs in the Euclid Deep FieldNorth and the Euclid Early Release Observation field Abell 2764. The ultracool dwarfs were identified by cross-correlatingtheir spectra with templates. The quasar was identified by its strong and broad C III ] and Mg II emission lines in the NISPRGE 1206–1892 nm spectrum, and confirmed through optical spectroscopy from the Large Binocular Telescope. The NISPBlue Grism (NISP BGE ) 926–1366 nm spectrum confirms C IV and C III ] emission. NISP RGE can find bright quasars atz ≈ 5.5 and z 7, redshift ranges that are challenging for photometric selection due to contamination from ultracool dwarfs.EUCL J181530.01+652054.0 is a high-excitation, broad absorption line quasar detected at 144 MHz by the LOw-FrequencyArray (L144 = 4.0 × 1025 W Hz−1 ). The quasar has a bolometric luminosity of 3 × 1012 L and is powered by a 3.4 × 109 Mblack hole. The discovery of this bright quasar is noteworthy as fewer than one such object was expected in the ≈20 deg2 surveyed.This finding highlights the potential and effectiveness of NISP spectroscopy in identifying rare, luminous high-redshift quasars,previewing the census of these sources that Euclid’s slitless spectroscopy will deliver over about 14 000 deg2 of the sk
