A Case of Metaplastic Breast Cancer with Rapid Growth in an Elderly Women
No full textJournal Article症例は85歳，女性．2015年10月，他疾患にて受診中の近医にて左乳房腫瘤を指摘され当院紹介受診した．視触診にて左乳房C領域に腫瘍径1.8×1.6 cm大，弾性硬，境界比較的明瞭な腫瘤を認めた．病理診断では軟骨化生を伴う低分化腺癌を認め，免疫組織染色で，ER，PgR，HER2が陰性，Cytokeratin AE1/AE3，Vimentin陽性で化生癌（骨・軟骨化生を伴う癌）と診断した．画像診断では腋窩リンパ節および遠隔転移は認めなかった．患者および家族に病状を説明したところ高齢を理由に治療を拒否されたため外来にて経過観察となった．初診から6ヶ月後頃より腫瘤の増大を自覚するも受診せず，8ヶ月後に疼痛，皮膚の発赤が出現したため再診となった．再診時左乳房の腫瘤は7.0×5.5 cm大に増大し皮膚の発赤と浮腫状の肥厚を認めた．疼痛の増強も認めたためご本人の了承を得て初診より9ヵ月後に手術（Bt+Ax（II））を施行した．患者の希望にて術後化学療法は施行せず，術後4年経過し外来定期観察中である．journal articl
Pulmonary Metastases from a Tracheal Adenoid Cystic Carcinoma 14 Years after Resection
No full textJournal ArticleWe present a rare case of primary tracheal adenoid cystic carcinoma (ACC) in which pulmonary metastases developed 14 years after the initial surgery. A 69-year-old woman underwent a radical tracheal resection with tracheoplasty for tracheal ACC. At a checkup 14 years later, computed tomography revealed three solid nodules in the lungs. We performed video-assisted thoracoscopic lung resection of the bilateral masses. Histologically, we observed ACC that was compatible with the previously resected primary lesion. The patient has had no recurrence 24 months after metastasectomy (198 months after the initial surgery).journal articl
Coughing-induced Pulmonary Barotrauma after Pulmonary Lobectomy
No full textJournal ArticleA left upper lobectomy was performed in an 80-year-old man with lung cancer. There was no air leakage after surgery, but on the evening of the next day severe subcutaneous emphysema and a massive air leakage developed after he had suffered bouts of coughing. In response to his worsening symptoms, he underwent surgery, which identified a subpleural pocket-like tear on the remaining lower lobe. There had been no evidence of emphysematous changes in his preoperative findings. This case demonstrates the need to consider reinforcing the pulmonary surface facing a large dead space to avoid coughing-induced pulmonary barotrauma.journal articl

No full textDepartmental Bulletin PaperWe can find variety of premise forest surrounding a residence in Japan. There are several places to remain\nthe landscape harmonizing these premise forest and traditional wooden houses in Gunma. The premise forest is\ncalled“ Kashigune” in Gunma. We conducted a field survey and interview to the residents who have the Kashigune\nalong the old street in Ookubo, Yoshioka town.\n We found the actual condition of Kashigune. The Kashigune is consisted of Oak wood and lined up north\nof the site to prevent a fire and strong wind from the mountain. The highest one is 8 meters. Most of the residents\nask a private company to prune Kashigune. There are several reasons to keep Kashigune not only functional\nreasons such as preventing a fire and strong wind but also receiving inheritance and legacy from the\nancestor. There are several problems to maintain Kashigune such as pruning cost and burden of cleaning up\nfallen leaves in the autumn. The policy to keep beautiful landscape with premise forest and wooden houses is\nexpected by the local government. Furthermore, it is necessary that many people recognize the landscape as precious\nlegacy in the town. It is important to have opportunities to know kashigune through education at school\nand outside school events.departmental bulletin pape
