Qualitative analysis of Current dietary behavior and Associated factors in Elderly individuals in need of care
application/pdfThis study provides, by means of group interviews, a qualitative analysis of the dietary behavior of elderly individuals in need of care and of the factors affecting their dietary behavior, and uses this analysis as basic information in discussing support methods for improving nutrition among elderly individuals in need of care. The study looks at eleven elderly individuals living at home who use preventive care services in their own home. The following factors in the dietary behavior of elderly individuals in need of care became clear: [menus tend to be lacking in variety], [they might not like what is put in front of them but they force themselves to eat it], [contents of meals that elderly people think are not good for them], [contents of meals that elderly people think are good for them], [they take less interest in what they eat], [they believe that supervising what they eat is equivalent to supervising their health], [they can't go shopping whenever they want to]. Factors which influence their diet are: [they are aware that their diet is bad, but their sense of values means that they are unable to change it], [their sense of values about food which they have no intention of changing], [influence on dietary behavior of reduction in bodily functions], [they cannot cope with the present food buying system and the change in products], [their financial situation affects their diet], [geographical and climatic conditions affect going shopping], [influence on diet of family composition]. The dietary behavior of elderly individuals in need of care must be supported on the basis of their current dietary situation and factors affecting their diet.本研究は要支援高齢者の食行動の現状と食行動に影響を及ぼしている内容をグループインタビューに\nより質的に分析し、要支援高齢者の栄養状態改善の支援方法を検討する基礎資料にすることを目的とし\nた。対象者は在宅にて介護予防サービスを利用して生活している在宅要支援高齢者11名である。要支援\n高齢者の食行動の現状は、【食事メニューの単一\n化】【出されたものは我慢して食べる】【高齢者が認識する体に良くない食事内容】【高齢者が認識する身体に良い食事内容】【食生活についての意欲が弱まる】【食事管理することは健康管理だと思っている】【買い物には自由にいけない】という内容が明らかになった。\nまた食生活に影響を及ぼしている要因として、【悪いと認識しているが変えない価値観】【変えるつもりはない食に対する価値観】【身体機能低下による食行動への影響】【現在の購買システムと商品の変化に対応できない】【経済状況が食生活に影響する】【買い物に影響を及ぼす地理的・気候的条件】【家族構成がもたらす食行動への影響】という内容が認められた。\n 要支援高齢者の食行動の現状と食行動に影響を及ぼす内容をふまえながら支援をしていくことが必要\nであると考える。journal articl
Differences between the Educational Background of Yogo Teachers and Health Diagnosis and Measures to Children with Mental Health Problems
application/pdfThis study aims to clarify differences in diagnosis and countermeasures of children with problems of mental health according to the licenses of Yogo Teachers. We carried out a questionnaire survey targeting Yogo Teachers in Niigata Prefecture and received answers from 651 subjects (response rate: 69.4%). After removing data with missing values, we conducted exploratory factor analysis and extracted the following factors: coordination; a Yogo Teacher’s diagnosis and countermeasures; relation with children; expertise; and uniqueness. Our test on mean differences according to the licenses of Yogo Teachers, the subscale scores of “expertise” and “uniqueness” of the group with nurses’ licenses were significantly higher than the group with teachers’ licenses.
We also conducted simultaneous analysis of several groups based on a model which we prepared by covariance structure analysis and examined the differences in diagnosis and countermeasures by Yogo Teachers according to their licenses. Our examination clarified that Yogo Teachers with teachers’ licenses weigh “relation with children” and those with nurses’ licenses emphasize “expertise” when “making nursing diagnosis and countermeasures children”.本研究の目的は、心の健康問題をもつ子どもの養護診断・対応における養護教諭の所有免許による相違を明らかにすることである。
新潟県の養護教諭を対象として質問紙調査を実施し、651人から回答があった(回収率69.4%)。欠損値のあるデータを除き、探索的因子分析から「連携」「養護診断・対応」「子どもとの関わり」「専門性」「捉え方の違い」の因子を抽出した。所有免許の違いによる平均値差の検定を行ったところ、看護師免許「有」の群が教員免許「有」の群よりも、「専門性」「捉え方の違い」の下位尺度得点が有意に高かった。前述の5つの因子において教職年数階級別平均値の差では、教職10~15年又は15~20年で教員免許「有」よりも看護師免許「有」の方が高かった。それ以降は両者とも同様の傾向が見られた。
共分散構造分析で作成したモデルをもとに多母集団同時分析を行い、養護診断・対応における所有免許による相違について考察した。「連携」「養護診断・対応」「子どもとの関わり」「専門性」においてのモデルでは、教員免許「有」の養護教諭はすべてにおいて有意なパスが得られた。看護師免許「有」は「子どもとの関わり」から「養護診断・対応」に有意なパスが得られなかった。日常の子どもの関わりから様子を把握し、意識的に「養護診断・対応」へつなげることが必要であることが示唆された。journal articl
Study of Structure of Disaster Social Work : Interview with Medical Social Workers in Fukushima Prefecture
application/pdfThis study attempted to gain a structured understanding of the social work done by disaster relief workers in areas affected by the Great East Japan Earthquake. The subjects were four medical social workers living in Fukushima prefecture, and a qualitative synthesis method was used in analyzing their responses during semi-structured interviews. Employees of medical organizations in Fukushima, the subjects were involved in disaster relief operations immediately after the earthquake. In addition to their regular full-time professional work, they also belonged to six other volunteer organizations, in both professional consulting and support capacities. They discovered during this relief work that a disparity of viewpoints and methods existed with respect to the work being provided, and that policy differences most prominently arose among their fellow professionals during times of interdisciplinary collaboration.
The issue also arose of a need for support for the support workers themselves, and also of what social workers could do to prepare for similar future disasters. The subjects found themselves dealing with a difficult, complicated situation requiring long-term relief work, due to the problems inherent to the Fukushima area.
The primary issues the subjects faced during their relief efforts consisted therefore of (1) differences between various professional disciplines with respect to their policies for providing relief work, (2) inadequate support systems for the relief workers themselves, and (3) the negative impact of vertical administrative governmental structures below the prefectural level.本研究は、東日本大震災被災地の医療ソーシャルワーカーによる災害ソーシャルワーク実践の全体像を明らかにし、構造として示すことを目的とした。対象者は、福島県の医療ソーシャルワーカー4名であった。半構造化面接で調査を行い、質的統合法を用いて分析した。その結果、福島県における災害ソーシャルワークを、【地震直後は所属する医療機関の職員として活動】を展開し、その後は通常業務の合間に【6団体相談支援専門職チームの一員としての活動を継続】していくうえでは【ソーシャルワークの視点や方法】を意識しながら【多職種協働で支援を展開していたが職種間で支援方針の相違】もあった。さらに、【支援者も被災者であり支援が必要】という新たな課題を抱えながら、【災害への備えとしてソーシャルワーカーができること】を地域で展開したり、【福島県特有の問題により複雑化・長期化する支援困難ケース】へ対応しているという構造として示した。これにより、災害ソーシャルワークの展開していくうえで医療ソーシャルワーカーが直面する課題として、①多職種間における支援方針の相違、②支援者支援システムの未整備、③縦割り行政の弊害、があることが分かった。journal articl
