Association between moral intelligence, burnout and quality of nursing care
Background: Providing quality care is of the fundamental elements of holistic nursing practice, and burnout and moral intelligence of nurses be mentioned as the important factors influencing the quality of nursing care. The present study was conducted to investigate the relationship between moral intelligence, burnout, and the quality of nursing care. Methods: This descriptive-correlative study was conducted on 125 nurses working in Sari-based Educational hospitals affiliated to Mazandaran University of Medical Sciences, Iran, between June and August 2020. The sample was selected via random sampling. The data were collected by the Maslach Burnout Inventory, Lennick and Kiel Moral Intelligence Scale, and Quality of Patient Care Scale. The data were analyzed by SPSS-21 and Amos-24. Results: A direct and significant relationship was found between the quality of nursing care and moral intelligence (r = 0.285, p = 0.001). Quality of care had an inverse relationship with subscales of frequency of burnout including emotional exhaustion (r = �0.369, p < 0.001) and depersonalization (r = �0.471, p < 0.001) and also, a direct relationship with personal accomplishment (r = 0.226, p = 0.011). The findings also showed an inverse relationship between quality of care and subscales of the intensity of burnout. Amos software yielded results that demonstrated moral intelligence as a robust mediator between burnout and the quality of care. Conclusion: The findings implied the necessity for more attention to moral intelligence as a mediator in order to come up with properly managing the personality traits influencing the nurses� burnout reduction, which can ultimately lead to improved quality of nursing care. © The Author(s) 2021
The critical role of mesenchymal stromal/stem cell therapy in COVID-19 patients: An updated review
New coronavirus disease 2019 (COVID-19), as a pandemic disaster, has drawn the attention of researchers in various fields to discover suitable therapeutic approaches for the management of COVID-19 patients. Currently, there are many worries about the rapid spread of COVID-19; there is no approved treatment for this infectious disease, despite many efforts to develop therapeutic procedures for COVID-19. Emerging evidence shows that mesenchymal stromal/stem cell (MSC) therapy can be a suitable option for the management of COVID-19. These cells have many biological features (including the potential of differentiation, high safety and effectiveness, secretion of trophic factors and immunoregulatory features) that make them suitable for the treatment of various diseases. However, some studies have questioned the positive role of MSC therapy in the treatment of COVID-19. Accordingly, in this paper, we will focus on the therapeutic impacts of MSCs and their critical role in cytokine storm of COVID-19 patients. © 2021 John Wiley & Sons Lt
A new method on kerma estimation in mammography screenings
Background: Given the extensive use and preferred diagnostic method in common mammography tests for screening and diagnosis of breast cancer, there is concern about the increased dose absorbed by the patient due to the sensitivity of the breast tissue. Objective: This study aims to evaluate the entrance surface air kerma (ESAK) before irradiation to the patient through its estimation. Material and Methods: In this descriptive paper, firstly, a phantom was used to measure some data, including ESAK, Kvp, mAs, HVL, and type of filter/target. Secondly, the MultiLayer Perceptron (MLP) neural network model was trained with Levenberg-Marquardt (LM) backpropagation training algorithm and finally, ESAK was estimated. Results: Based on results obtained from the program in different neuron num-bers, it was found that the number of 35 neurons is the most optimal value, offering a regression coefficient of 95.7. The Mean Squared Error (MSE) for all data was 0.437 mGy and accounting for 4.8 of the output range changes, predicting 95.2 accuracy in the present research. Conclusion: Using neural networks in ESAK prediction, the method proposed in the present research leads to the possible ESAK estimation of patients before X-Ray. The results suggested that the regression coefficient represented 4.3 difference between the kerma measured by solid-state dosimeter in the radiation field and the value predicted in the research. In comparison with the Monte-Carlo simulation method, this method has better accuracy. © 2021, Shriaz University of Medical Sciences. All rights reserved
Investigation of thermal comfort changes using summer simmer index (Ssi): A case study in different climates of iran
Objective: This study aimed to assess the thermal comfort trend using a metrological parameter based on the Summer Simmer Index (SSI), for different climates of Iran between 1985 and 2014. Methods: This is a Time-series study. The new SSI was calculated using metrological data in a 30 year-period for illustrating the changes to the thermal comfort or discomfort level through summer months in different climates of Iran. Mann-Kendall test and Sen's Slope were used to compare the upward or downward trend of the index during this period. Results: The worst thermal condition was observed in the southern and central regions of the country. A few stations were put in comfort zone (77�SSI<83°F). The fluctuations of SSI were not considerable during the study period (P<0.001). However, a commonly upward trend was observed, indicating rising temperature. This initial assessment, which can be reported daily along with other atmospheric parameters at meteorological stations, could play a significant role in reducing the heat-related complications among exposed individuals and a basis for taking appropriate protective measures. Conclusion: Based on the results obtained in this study, which has been investigated in Iran for a long period of time, considering intrinsic features of the SSI index, such as ease of calculation and interpretation and also the possibility of calculating the index using daily reported meteorological data, the use of SSI as a screening index of thermal stress in order to adopt preventive policies in outdoor settings in climates of Iran is recommended. © 2021 Asghari et al

Protective effects of rutin and chlorogenic acid against antihypoxic conditions in mice
Almost all plants contain polyphenols. Literature shows that polyphenols exhibit many biological activities. Little has known about their protective effects against hypoxia-induced lethality. The protective effects of rutin (1) and chlorogenic acid (2) against hypoxia conditions in mice were determined by three different experimental models. Antihypoxic activity was especially pronounced in asphytic hypoxia. Both compounds (1&2) showed statistically significant (p>0.05) activities respect to the control. Compound (1) significantly prolonged the latency for death with respect to control (39.20±8.70 vs. 13.20±2.58min, p<0.001). Compound (1) was the most effective compound in circulatory hypoxia. It significantly prolonged the latency for death with respect to control (14.44±2.82 vs. 9.82±0.79 min, p<0.01). On the other hand, Chlorogenic acid (2) at a dose of 100 mg kg-1 kept mice alive for 12.76±1.30min (p>0.05). Challenges facing the health system in responding to mining accidents: The case of the zemestan-yurt mine explosion in iran (2017)
Background: Occupational accidents cause death and disabilities among millions of workers around the world. The mining industry is among the riskiest occupations in this respect. This study aims to investigate interventions made and challenges faced by the health system in response to the Zemestan-Yurt Mine explosion in the Golestan Province, Iran, 2017. Materials and Methods: In this article, being a field report, data were collected through direct observations and interviews with first responders involved in the response process, and by checking national and local reports from other organizations. Result: On Wednesday, May 4, 2017, at 11:58 AM, an explosion occurred in the Yurt coal mine, Golestan Province, in which 126 people were affected directly or indirectly. In fact, 43 workers were killed immediately after the explosion of a coal wagon, 55 were dispatched to hospitals, and 28 received outpatient health services. In this incident, more than 100 teams from various organizations, including the health system, arrived at the scene and carried out relief and rescue operations. Conclusions: The results of the current study indicate that the health system does not have specific instructions, training plans, as well as well-trained and prepared personnel for managing mine-related accidents. Besides, interactions are not integrated and coordinated among disaster management response teams. Therefore, developing preparedness instructions for responding to disasters, forming rapid response teams and training them for industrial accidents, as well as performing joint exercises and drills with responsible organizations can be effective in improving responses to such accidents. © The Author(s) 2021; All rights reserved. Efficacy and safety of rituximab therapy in patients with systemic sclerosis disease (SSc): systematic review and meta-analysis
The clinical benefits of rituximab in systemic sclerosis (SSc) are still contentious. The present meta-analysis aimed to systematically assess rituximab�s safety and efficacy profile in SSc patients. A systematic online query was performed in PubMed, Scopus, Web of Science, and Embase. The studies on the application of rituximab for patients with SSc were reviewed comprehensively for over two years. In terms of efficacy profile, mRSS, MS, LVEF, sPAP, FVC, DLCO, TLC, FEV, DAS, severity activity, HAQ-DI and SF36 were assessed for organ involvement and quality of life. The level of biological and immunological markers was also evaluated in SSc patients treated with RTX. In total, 24 studies met the criteria. Although they did not have a high quality, they were free from heterogeneity and publication bias. The pooled results revealed a long-term improvement in mRSS and MS. HAQ-DI was improved to 0.78 after 12 months, and DAS was significantly reduced to 0.33, 0.23, and 0.24 following 6, 12, and 24 months of treatment, respectively (p = 0.00 for both parameters). The rest of the parameters remained stable over time in patients with SSc. The pooled analysis of these patients demonstrated that the induction of death, cancer, infection, and infusion were 9, 5, 18 and 10, respectively. Based on the pooled results of this meta-analysis, rituximab improves skin score and disease indices and stabilizes organ involvement in SSc patients. Rituximab seems to possess reasonable safety, similar to previous data from other autoimmune diseases. © 2021, International League of Associations for Rheumatology (ILAR)

Hyperglycemia results in decreased immune cell infiltration and increased viral load in the lung in a mouse model of RSV infection
Respiratory Syncytial virus (RSV) infection is a feared disease in vulnerable populations with impaired immune responses. There is currently no vaccine against RSV and young children along with elderly people are at increased risk of severe or sometimes life-threatening RSV infection. Hyperglycemia with immunomodulatory patterns can impact on infectious disease outcomes and immune system responses in diabetic patients. Even though research continues to uncover the complex mechanisms underlying RSV immunopathogenesis and diabetes mellitus disease separately, limited information is available about interaction between these two phenomena. Here, we evaluated the influence of hyperglycemia as the hallmark of diabetes mellitus disease on the pathogenesis and immunopathogenesis of RSV in a mouse model. In this experiment, hyperglycemia was induced by intraperitoneal injection of Streptozotocin (STZ), and after diabetes confirmation, mice were infected with RSV-A2, and the immune responses were followed for 5 days until the mice were sacrificed. Analyses on airway immune cell influx, T-Lymphocyte subtypes, cytokines secretion, lung histopathology, and viral load were conducted. Our results showed that hyperglycemia resulted in reduced lung immune cells infiltration totally and it was associated with decreased pathological damage of the lung. Following RSV infection in hyperglycemic mice, the ratio of CD4/CD8 T-Lymphocytes due to CD8+ depletion, increased. Furthermore, the level of IFN-γ and IL-17A cytokines decreased, whereas IL-10 showed an upward trend and the viral load increased in hyperglycemic mice compared with normoglycemic mice. In conclusion, these findings indicate that hyperglycemia can ameliorate and downregulate RSV-induced inflammatory and antiviral responses, and result in increment of viral load. © 2021 Elsevier Lt
