    膜乳化デバイスによるエマルションを用いたウサギVX2肝腫瘍モデノレへの肝動脈化学塞栓術における抗腫瘍効果、薬物動態の検討

    Purpose: To evaluate the phamacokinetics of epirubicin in conventional transarterial chemoembolization using a developed pumping emulsification device with a microporous glass membrane in VX2 rabbits. Materials and methods: Epirubicin solution (10 mg/mL) was mixed with ethiodized oil (1:2 ratio) using the device or 3-way stopcock. Forty-eight rabbits with VX2 liver tumor implanted 2 weeks prior to transarterial chemoembolization were divided into 2 groups: a device group (n = 24) and a 3-way-stopcock group (n = 24). Next, 0.5 mL of emulsion was injected into the hepatic artery, followed by embolization using 100-300-μm microspheres. The serum epirubicin concentrations (immediately after, 5 minutes after, and 10 minutes after) and the tumor epirubicin concentrations (20 minutes after and 48 hours after) were measured after transarterial chemoembolization. Histopathologic evaluation was performed with a fluorescence microscope. Results: The area under the curve and maximum concentrations of epirubicin in plasma were 0.45 ± 0.18 μg min/mL and 0.13 ± 0.06 μg/mL, respectively, in the device group and 0.71 ± 0.45 μg min/mL and 0.22 ± 0.17 μg/mL, respectively, in the 3-way-stopcock group (P = .013 and P = .021, respectively). The mean epirubicin concentrations in VX2 tumors at 48 hours in the device group and the 3-way-stopcock group were 13.7 ± 6.71 and 7.72 ± 3.26 μg/g tissue, respectively (P = .013). The tumor necrosis ratios at 48 hours were 62 ± 11% in the device group and 51 ± 13% in the 3-way-stopcock group (P = .039). Conclusions: Conventional transarterial chemoembolization using the pumping emulsification device significantly improved the pharmacokinetics of epirubicin compared to the current standard technique using a 3-way stopcock.博士(医学)・乙第1443号・令和元年12月5日Copyright © 2019 SIR.     中大脳動脈瘤手術における術中MEPの一過性増大に関する検討

    Objective: To study clinical significance of augmentation of intraoperative motor evoked potentials (MEPs) during direct open surgery for middle cerebral artery (MCA) aneurysms. Methods: Between 2009 and 2017, 134 MCA aneurysm surgeries were performed with intraoperative MEP monitoring. The frequency and cause of augmentation with >50% increase of MEP amplitude from baseline were studied. Factors associated with MEP augmentation were investigated. Results: MEP augmentation was demonstrated in 9 patients. All 9 events were observed just after application of the temporary clip to the parent artery. The ratio of the maximum amplitude to baseline was 2.6 ± 1.1 at an mean of 2.4 ± 1.1 minutes after parent artery occlusion. Ten patients who did not show MEP augmentation after parent artery occlusion were compared with the patients showing MEP augmentation. The distance of the temporary clip point from the midline was smaller in patients with MEP augmentation compared with patients without MEP augmentation (P = 0.033). Conclusions: MEP augmentation was thought to be an early ischemic sign preceding a significant decrease in MEPs during MCA aneurysm surgery. Transient augmentation of MEPs was more frequently observed in cases with a temporary clip applied to the more proximal part of the MCA.博士(医学)・乙第1445号・令和元年12月5日© 2019 Elsevier Inc.     Effect of Glucose Loading on Cancer Growth and Skeletal Muscle Atrophy.

    Skeletal muscle volume is reported to be associated with prognosis of cancer patients. Maintenance of skeletal muscle is an essential issue in cancer treatment. Glucose metabolism plays a pivotal role in maintaining skeletal muscle; however, cancer cells also require glucose as a main energy source. We here examined the effect of glucose loading on tumor growth and muscle maintenance using a mouse subcutaneous tumor model. CT26 murine colon cancer cells were inoculated into the subcutaneous tissues in syngenetic BALB/c mice, which were treated with water, 10% glucose or 50% glucose by free drinking for 2 weeks. Blood sugar was not different among the groups. Tumor sizes were larger in the glucose-treated groups than those in the control group. Protein content in the muscle was not different among the groups, whereas the levels of SDS-soluble myosin light chain 1 protein were higher in the glucosetreated groups than those in the control group. These findings suggest that glucose loading enhances muscle differentiation as significant as tumor growth
