Ring-Opening Polymerization of Hemoglobin.
Hemoglobin (Hb), an oxygen-carrying protein, has an α₂β₂ tetrameric structure that dissociates reversibly into two αβ dimers (α₂β₂ ⇄ 2αβ). We synthesized a cyclic Hb-ring monomer with two β subunits bound through a 10 kDa polyethylene glycol (PEG) chain. The monomer induced ring-opening polymerization to produce a supramolecular polymer via intersubunit interaction of αβ dimers of an Hb molecule at the PEG terminals. Both the ring-closed monomer and the ring-opened supramolecular polymer were then fixed covalently by intramolecular cross-linking of two β subunits. Quantification of fixed products at various monomer concentrations revealed the equilibrium constant (K), a ratio of propagation and depropagation rate constants, as 5.68 mM⁻¹. The average degree of polymerization (DP) increased proportionally, concomitantly with the initial monomer concentration. Hb polymer with DP = 13.2 (Mn = ca.1 MDa) was obtained by cross-linking at 2.33 mM. 冠動脈高度石灰化病変に対するスコアリングバルーンラクロスNon-Slip Elementを用いた冠動脈形成術-光干渉断層法による有用性の評価
Percutaneous coronary intervention (PCI) for heavily calcified lesions is challenging because these lesions are resistant to balloon dilatation and stenting. Lacrosse non-slip element (NSE) may have the potential to dilate heavily calcified lesions. We aimed to investigate predictors of successful lesion modification using Lacrosse NSE angioplasty via optical coherence tomography (OCT)-guided PCI. We investigated 32 patients with severe target lesion calcification treated with OCT-guided PCI. Successful lesion modification was defined as the complete fracture of calcification after Lacrosse NSE angioplasty. Before PCI, 172 segments with calcification were identified. After pre-dilatation using Lacrosse NSE, successful lesion modification was achieved in 117 segments (68.0%). Calcification was significantly thinner in successfully disrupted segments than in non-disrupted segments (p < 0.001). Calcification angle tended to be larger in disrupted than in non-disrupted segments (p = 0.08). Convex types were less frequently observed in disrupted than in non-disrupted segments (p < 0.001). At minimal lumen area sites, 26 segments (81.3%) were successfully modified. Similar to the overall results, the disrupted group had significantly thinner calcification than the non-disrupted group (p < 0.001). The angle of the calcified plaque was similar between the 2 groups (p = 0.39). Convex-type calcifications were less frequently observed in the disrupted group than in the non-disrupted group (p = 0.05). Receiver-operating characteristic curve analysis showed that calcification thickness < 565 μm was the best predictor of completely disrupted calcification. BACKGROUND: ADAMTS13 specifically cleaves the peptide bond between Y1605 and M1606 within the VWF-A2 domain. OBJECTIVE: The VWF contains ABO(H) blood group antigens, which may influence the susceptibility of VWF to ADAMTS13. METHODS: Using a unique monoclonal antibody recognizing the Y1605 residue, we have developed a sandwich ELISA to analyze the generation of a VWF-DP by ADAMTS13 quantitatively. RESULTS: Production of VWF-DP after exposure to four different degrees of high shear stress was validated in comparison to the reduction in high-molecular-weight multimers using VWF multimer analysis. In analysis of plasma from 259 healthy individuals, plasma levels of VWF antigen (VWF:Ag) were significantly lower in blood group O than in the other groups and were significantly correlated with plasma VWF-DP levels. The ratio between VWF-DP and VWF:Ag was significantly higher in blood group O than in blood groups A and AB. The ratio in blood group B was also significantly higher than those in A and AB, but did not differ from blood group O. Finally, to examine whether ABO(H) blood group antigens contributed to VWF cleavage, 82 plasma samples were exposed to high shear stress using a cone-plate shear stress inducer. The difference in the VWF-DP/VWF:Ag ratio before and after high shear stress in blood group O was significantly greater than those in groups A and AB. Current situation regarding maternity harassment of working pregnant women and the effects on their psychological and physical health.
本研究の目的は、マタニティハラスメントの実態を明らかにし、マタニテイハラスメントが産前産後の心理・身体的な健康に及ぼす影響を検討することである。妊娠期に就労経験のある初産婦を対象に、妊娠後期(妊娠28週以降)と産後早期(産後7日以内)に、縦断的に無記名自記式質問紙調査を用いて、コホート研究を実施した。295部を配布し、妊娠後期は134名(45.4%)、産後早期は44名(14.9%)から回答を得た。妊娠後期の対象者のうち55名(41.0%)がマタニティハラスメントを受けていた。マタニティハラスメントが職業性ストレス簡易調査票の心理的ストレス反応と身体的ストレス反応、妊娠期のエジンパラ産後うつ病自己評価票(Edinburgh Postnatal Depression Scale;以下、EPDS)、妊娠悪阻に影響していた。マタニテイハラスメントが産後早期のEPDSや在胎週数、児の出生体重には影響がみられなかった。就労妊婦が安全に安心して働き続けるためには、妊娠中のマタニティハラスメント予防・抑制への対策が重要であると示唆された。The aim of this study was to clarify the current situation of maternity harassment and to survey its effects on the psychological and physical health of women before and after childbirth. We recruited 295 primigravid women who were working in pregnancy. The questionnaires were distributed before childbirth (after the 28th week of pregnancy) and after childbirth (within 7days of giving birth). 134 women (45.4%) during pregnancy and 44 (14.9%) after childbirth anonymously answered questionnaire. Of those ansewers before childbirth, 55 (41.0%) had experienced maternity harassment, which promoted the psychological stress reaction, physical stress reaction (brief job stress questionnaire), higher scores on the Edinburgh Postnatal Depression Scale (EPDS) during pregnancy, and the prevalence of hyperemesis gravidarum.While, experiencing maternity harassment in pregnancy did not affect postpartum EPDS, gestational age, or birth weight. These results suggest that measures to prevent and eradicate maternity harassment are absolutely critical for allowing women to continue working safely and comfortably during pregnancy
