    Evaluation of pain and flexor reflex responses and their association with clinical parameters in patients with fibromyalgia

    Objectives: In the present study, we aimed to evaluate electrophysiological outcomes of flexor reflexes (FR) emerging in response to painful stimuli in fibromyalgia (FM) patients. Patients and methods: This study included a total of 21 female FM patients (mean age 36.8 +/- 8.0 years; range, 20 to 50 years) and 28 healthy female controls (mean age 37.3 +/- 8.8 years; range, 22 to 53 years). Clinical features of the patients, Fibromyalgia Impact Questionnaire (FIQ), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI) scores were recorded. Electrophysiological stimulation threshold, visual analog scale (VAS) scores of generated pain after each stimulation, current intensity, flexor responses obtained from muscles, and electrophysiological measurement of these responses were noted. Results: The threshold for perception of electrical current was higher and resistance level to maximum electrical current was lower in FM patients (p<0.05). The VAS scores of FM patients under maximum electrical current were higher than the control group (p<0.05), while the lowest current value in which FR response obtained in FM patients was lower (p<0.05). The latency was lower, amplitude was higher, and response area was wider in FM patients (p<0.05). No significant relationship between the FIQ, BDI, BAI scores and stimulant perception threshold levels was found in FM patients (p>0.05). Conclusion: Based on these findings, FR responses may be useful as an electrophysiological parameter in FM diagnosis

    Flood Analysıs In Çankırı Tatlıçay Stream

    Bu tez kapsamında Arc-GIS programı kullanılarak, çalışma alanı olarak belirlenen Çankırı ilinin merkezinden geçen Tatlıçay Deresi’nin havza parametreleri belirlenmiştir. Elde edilen havza parametrelerine uygun olarak seçilen taşkın hesaplama yöntemleri ile (DSİ Sentetik Yöntemi ve Bölgesel Taşkın Frekans Analizi) çeşitli tekerrürdeki taşkın debileri hesaplanmıştır. Taşkın analizleri için oluşturulan arazi modeli, 1/1000 ölçekteki hâlihazır harita Civil 3D programında işlenerek 3 boyutlu (3D) olarak oluşturulmuş, bu model yine Civil 3D vasıtası ile hidrolik analizlerin yapılacağı HEC-RAS programına aktarılmıştır. HEC-RAS programının son versiyonunda, yaygın olarak kullanılan bir boyutlu (1D) akıma ek olarak, son yıllarda gelişen teknoloji ve yüksek kaliteli veriye erişimin kolaylaşması ile iki boyutlu (2D) akım analizlerini de yapabilmektedir. Bu çalışma kapsamında Tatlıçay Deresi HEC-RAS programında 1D ve birleştirilmiş bir ve iki boyutlu (1D-2D) model olmak üzere iki farklı şekilde analiz edilmiştir. 1D modelde dere ve taşkın yatağı bir boyutlu,1D- 2D modelde ise dere yatağı bir boyutlu, taşkın yatakları iki boyutlu olarak analiz edilmektedir. 1D, 2D ve 1D-2D modellerinin veri gereksinimleri, kurulum aşamaları ve modellerden elde edilen sonuçlar karşılaştırılarak aradaki farklar ortaya konmuştur. Bu çalışmada ayrıca Tatlıçay Deresi üzerinde bulunan 8 adet köprü senaryolar oluşturularak kademeli olarak düzenlenmiş ve köprülerin taşkın anındaki etkisi irdelenmiştir. Sonuç olarak 1D model yatağın taşkın kapasitesinin bulunmasında yeterli olarak değerlendirilmiş, ancak ana yatak dışında yayılım alanlarının belirlenmesinde iki boyutlu (2D) analizin gerekli olduğu sonucuna varılmıştır. Bu sonuca ilave olarak, köprülerin taşkın anındaki olumsuz etkisi 1D analizde yeterli olarak gözlemlenemezken, köprülerin düzenlenmesiyle birlikte 1D-2D modelde elde edilen yayılım alanlarında 1D modele göre ciddi miktarda azalmalar belirlenmiştir.In this study, basin parameters of Tatlıçay Stream passing through the city center of Çankırı province is determined using the software programme Arc-GIS. Flood discharges with various reoccurrences are calculated by DSİ Synthetic Method and Regional Flood Frequency Analysis. The land model with a scale of 1/1000 is prepared using Civil 3D and transferred into HEC-RAS in order to calculate hydraulic analysis. With the latest version of HEC-RAS, 2D flood analysis can be done in addition to 1D flood analysis. Tatlıçay Stream is analyzed in two different ways such as 1D and united 1D and 2D models together using HEC-RAS. Stream and stream bed is analyzed in 1D model. Stream bed is modelled as one dimensional and floodplain is modelled as two dimensional in united 1D and 2D models. The differences between 1D, 2D and united 1D-2D models are compared according to the data requirements, installation stages and the results of the analysis. 8 bridges built on Tatlıçay Stream are also analyzed and their effect on the flood is discussed. As a result 1D model is considered to be sufficient to determine the flood capacity of the stream bed but it is concluded that 2D model is required to determine the propagation areas outside the main stream bed. In addition to this, the negative impact of bridges while flood occuring cannot be observed sufficiently in 1D analysis, with the arrangement of the bridges, significant reductions in the propagation areas obtained according to 1D model in the combined 1D-2D model were determine

    Evaluation of a two-image technique consisting of an axial and a coronal image generated by using the rib-flattening application: effect on reading time and diagnostic validity

    Background/aim: When reading a chest CT, a radiologist needs to evaluate each rib one by one due to complex curvy shape, which makes reporting a tiresome and time-consuming task. A new curved planar reformat application that flattens ribs on a single plane may find a place in the radiology reporting room. This study aimed to evaluate the effect of a two-image set created by using the rib-flattening application on the performance of a radiologist in detecting sclerotic rib lesions in cancer patients. Materials and methods: The local Institutional Review Board approved this retrospective study. Two radiologists with different experience levels reviewed chest CT examinations of 106 patients (76 men, 30 women). We divided the patients into group A (n = 54), reviewed by a standard method, and group B (n = 52), reviewed by a standard method and the two-image set created on the rib-flattening application. Reading times, validity indices, and agreement levels with reference data were evaluated for both readers. Results: The median reading time of the junior examiner significantly decreased with the rib-flattening method (160.5 s vs. 70.0 s; P < 0.001). Diagnostic indices of the senior reader were improved significantly at per patient level (group A, AUC: 0.867; group B, AUC: 0.982; P = 0.046). The new method showed better agreement levels (kappa: 0.69 to 0.96) than the general method (kappa: 0.53 to 0.91). Conclusion: Based on improved agreement levels, reading times, and diagnostic validity indices we conclude that a two-image set consisting of an axial and a coronal flattened-rib image may be used in conjunction with an ordinary exam

    NAUTICAL FESTIVALS IN THE SECOND HALF OF THE 19TH CENTURY: BUYUKADA (PRINKIPO) REGATTAS AND THE TRANSFORMATION OF THE ISLAND

    In the 19th century, specific times of the year were named accordingly to seasonal changes and / or the activities individuals performed in their daily (bad season, theatrical season, carnival season and beautiful season). Regattas, which were maritime / nautical festivals seen in cities such as London and Marseille, were important urban actions of the beautiful season of Istanbul. These festivals functioned as urban social events that span a whole day. The first regatta in Istanbul was organized in 1859 with the attempts and under the patronage of British Ambassador Sir Henry Lytton Bulwer in different parts of the city such as Moda and Buyukdere, yet in time regattas became signature events of Buyukada. Regattas attracted the attention of all the citizens, especially the public officials, diplomats and bankers, namely the high society of Istanbul. Of the periodicals published in the 19th century, the turnout in the festivals in the 1870s was about 10,000 people. This very high level of participation brings into question the existence and extent of the effect of these events that could have transformed the places where these particular social events took place. From this perspective, such a transformative effect of an urban action can be investigated through the Buyukada regattas. There are no studies on the regattas of Buyukada and Istanbul in particular. However, the periodicals such as Journal de Constantinople, Levant Herald, Le Monde Illustre, Levant Herald and Eastern Express, Servet-i Funun, Malumat and memoirs written by the individuals like Samuel S. Cox and Lady Hornby who travelled Istanbul in the 19th century present primary evidence about the events. By applying these findings onto the Buyukada Map of Antoine Raymund, published in 1912, this paper analyses regattas' major effects on the formation, transformation and modernization of the urban space in the 19th century Istanbul

    3D printing and spine surgery

    Rapid prototyping (RP), also known as three-dimensional printing (3DP), allows the rapid conversion of anatomical images into physical components by the use of special printers. This novel technology has also become a promising innovation for spine surgery. As a result of the developments in 3DP technology, production speeds have increased, and costs have decreased. This technological development can be used extensively in different parts of spine surgery such as preoperative planning, surgical simulations, patient-clinician communication, education, intraoperative guidance, and even implantable devices. However, similar to other emerging technologies, the usage of RP in spine surgery has various drawbacks that are needed to be addressed through further studies
