    Is gluteus medius injured in patients treated with a trochanter tip entry intramedullary nail? Clinical, electrophysiological and functional outcomes

    Objectives: This study aims to assess the functional outcomes, static-dynamic postural stability of patients and evaluate electromyographic activity of gluteus medius (GMed) muscle during gait and spatiotemporal parameters of gait in patients after antegrade intramedullary nailing (IMN) of femoral shaft fractures with trochanter tip entry. Patients and methods: Sixteen patients (15 males, 1 female; mean age 34.8 +/- 15.2 years; range, 18 to 58 years) who were treated with an antegrade trochanteric IMN between January 2009 and July 2013 and eight healthy male controls (mean age 39.3 +/- 9.8 years; range, 27 to 57 years) were included in this retrospective study. Muscle strength, static and dynamic postural stability and fall risk were evaluated. In addition, spatiotemporal parameters of gait were assessed using a validated wireless inertial sensing device and a wireless electromyography (EMG) device was used to measure electromyographic activity of GMed muscle in both groups. Results: There were significant differences in muscle strength, functional and dynamic balance test scores between operated and intact sides (one-leg hop test score p=0.009, balance test scores p 0.001-0.033). There were significant differences in functional and quality of life physical function test scores between groups (one-leg hop test score p=0.014). However, no significant differences were found in EMG results, dynamics balance, and statics postural stability test scores between groups (p>0.05). Conclusion: After isolated femur fracture, patients treated with antegrade trochanteric IMN demonstrated good static and dynamic stability and poor functional outcomes compared to controls. In conclusion, patients with antegrade trochanteric IMN have good balance but poor functional performance; however, further studies are needed to find out the primary reason for these results

    High-sensitive troponin T increase after hemodialysis is associated with left ventricular global longitudinal strain and ultrafiltration rate

    Background: Circulating troponin levels are both stable and higher in patients with end-stage renal disease, even in the absence of acute coronary syndrome. These patients commonly have underlying cardiac problems that frequently cause troponin elevation. The effect of hemodialysis (HD) on troponin levels has not been well elucidated. Thus, investigated herein is the relationship between the changes in troponin levels along with left ventricular deformation and volume depletion in patients with end-stage renal disease. Methods: Patients included were between 18 and 85 years of age and were receiving hemodialysis for at least 6 months. High sensitive cardiac troponin T (hs-cTnT) levels were studied in blood samples taken at the beginning and end of HD. Two-dimensional speckle tracking strain imaging was used to evaluate myocardial contractility. Results: Seventy patients (50.7 +/- 16.9 years of age, 27 women) were included in study. The mean volume of ultrafiltration was 3260 +/- 990 mL. A significant increase in circulating hs-cTnT levels was observed, as well as a prominent decrease in left ventricular global longitudinal strain (GLS) after HD (52.4 +/- 40.2 ng/L vs. 66.8 +/- 48.5 ng/L, p < 0.001 and 20.1 +/- 3.6\% vs. 16.8 +/- 3.8\% p < 0.001, respectively). Moreover, ultrafiltration rate and GLS were found as the strongest independent variables in relation to the relative increase in hs-cTnT. Conclusions: Hemodialysis can cause a significant increase in hsTnT. This can jeopardize the accuracy of clinical diagnoses based on hs-TnT measurements. GLS may be used as a determinant of this hs-TnT increase. The influence of HD on the cardiovascular system should be kept in mind to prevent unnecessary interventions

    Socioeconomic Factors and Sustainable Forage Crops Production in Turkey Aegean Region: A Multivariate Modeling

    The aim of this study is to evaluate the effect of farmers' socioeconomic characteristics on sustainable forage crop production after forage crop subsidies. One of the innovative aspects of the study is the simultaneous modeling of alfalfa, silage corn and vetch production efficiency. For this, the multivariate linear regression model was used. In this way, the effect of socioeconomic characteristics on production is more clearly demonstrated by taking into account the dependency structure between forage crop production. For the study, 487 farmers in the Aegean region, where fodder crops can be produced throughout the year, were interviewed face-to-face and data were collected through a questionnaire. According to the results obtained, it was determined that the width of the land had a positive effect on the increase in alfalfa, silage corn, and vetch production. The number of animals was found to have a positive effect on alfalfa and silage maize production growth. The production of alfalfa was higher in rural farmers, and vetch production was higher in urban farmers. Farmers who do not have non-agricultural income focused on vetch production, and farmers with non-agricultural income focused on alfalfa production. It was seen that the majority of the farmers participating in the study were primary school graduates and lived in rural areas. In general, a significant increase was observed in the production of silage corn, vetch, and alfalfa, respectively. After the subsidies, it was concluded that this increase in the production of silage corn, vetch, and alfalfa was not at a level to meet the amount of forage crops needed by animal husbandry

    Identifying Climate Literacy Competencies: A Delphi Study at the Middle School Level

    This study aimed to identify climate literacy competencies for middle school students. The study used a descriptive survey design. The data were collected using the Delphi method. Climate literacy competencies for middle school students were identified through Delphi questionnaires administered in three rounds. A panel of experts in social studies, geography education, and climate science participated in the Delphi applications. The number of experts was changed in each round. Four basic levels were defined for the selection of Delphi panelists. Each level was divided into two categories so that a more detailed and comprehensive approach to expert selection was used. Measures of central tendency (mean, median, and mode) and central dispersion (standard deviation and interquartile range) were used in the data analysis to reveal the general judgment of the panelists. Expert opinions were analyzed according to the criteria defined within the scope of the study and climate literacy competencies were identified for middle school students. A 70\% consensus was achieved on the entire Delphi procedure among the panel of experts. The results of the three-round Delphi exercise showed that climate literacy competencies that middle school students should have consist of six categories and the items thereof. Accordingly, these categories are ``concepts related to climate{''}, ``basic knowledge of climate{''}, ``knowledge of national and local climate{''}, ``relationship between climate and life{''}, ``skills{''}, and ``attitudes and values{''}
