Disease management self-awareness scale in patients with cardiac disease: Scale development, validity and reliability study
Bu araştırmanın amacı, kalp hastalarının hastalık yönetimi öz farkındalık düzeylerini belirlemeye yönelik geçerli ve güvenilir bir ölçüm aracı geliştirmektir. Metodolojik tasarımda planlanan bu araştırmanın örneklemini, İstanbul’da bulunan iki şehir hastanesi ve bir eğitim ve araştırma hastanesinin kardiyoloji servisinde tedavi gören, araştırmaya katılmaya gönüllü 621 kalp hastası oluşturmuştur. Araştırmanın verileri, 19/12/2023 – 31/03/2025 tarihleri arasında katılımcılar ile yüz yüze görüşme yapılarak toplanmıştır. Veri toplama sürecinde “Tanımlayıcı Özellikler Formu” ve 5’li Likert tipinde geliştirilen “Kalp Hastalarında Hastalık Yönetimi Öz Farkındalık Ölçeği” kullanılmıştır. Ölçeğin yapı geçerliği iki ayrı örneklem üzerinde incelenmiştir: Açımlayıcı Faktör Analizi (AFA) 344 kalp hastası üzerinde, Doğrulayıcı Faktör Analizi (DFA) ise 277 kalp hastası üzerinde gerçekleştirilmiştir. Kalp Hastalarında Hastalık Yönetimi Öz Farkındalık Ölçeği’nin Kaiser-Meyer-Olkin katsayısı değeri 0,94, Bartlett Küresellik Testi χ2(435) =6329,15 ve p≤0,001 olarak anlamlı bulunmuştur. Ölçeğin Kapsam Geçerliliği İndeksi 0,941 olduğu belirlenmiştir. Ölçeğin faktör yüklerinin 0,38-0,95 arasında değiştiği belirlenmiştir. Toplam açıkladığı varyans oranı %54,73 olan ölçek, 30 madde ve 4 faktör; Sağlık Sorumluluğu, Beslenme, Psikososyal Uyum ve Yaşam Tarzı Yönetimi şeklinde tanımlanmıştır. Ölçeğin güvenirliğine ilişkin toplam Cronbach Alpha katsayısı 0,89, alt boyutları için ise 0,62-0,87 arasında bulunmuştur. Ölçek Modelinin uyum ölçütleri iyiliği indeksi: X2/sd =2,23, RMSEA=0,067, CFI=0,95, RMR=0,067 ve SRMR=0,09 olarak bulunmuş ve modelin istatistiksel uyum gösterdiği tespit edilmiştir. Tüm alt boyutlar ile toplam ölçek puanı arasında anlamlı ve yüksek düzeyde korelasyonlar bulunmuştur (r=0,76-0,85). Sonuç olarak bu araştırmada geliştirilen ölçeğin, kalp hastalarının hastalık yönetimi öz farkındalık düzeyini ölçmede geçerli ve güvenilir olduğu sonucuna varılmıştır.The aim of this study was to develop a valid and reliable measurement tool to assess the self-awareness level of disease management in patients with cardiac disease. This methodological study was conducted with a sample of 621 voluntary patients receiving treatment in the cardiology departments of two city hospitals and one training and research hospital located in Istanbul. Data were collected through face-toface interviews with participants between December 19, 2023, and March 31, 2025. During the data collection process, a “Descriptive Characteristics Form” and a fivepoint Likert-type scale titled “Disease Management Self-Awareness Scale in Patients with Cardiac Disease” were utilized. Construct validity of the scale was examined on two separate samples: Exploratory Factor Analysis (EFA) was performed on 344 cardiac patients, while Confirmatory Factor Analysis (CFA) was conducted on a different group of 277 patients. The Kaiser-Meyer-Olkin (KMO) measure of sampling adequacy for the scale was found to be 0.94, and Bartlett’s Test of Sphericity was significant (χ²(435) = 6329.15, p ≤ 0.001). The Content Validity Index (CVI) of the scale was determined to be 0.941. Factor loadings ranged from 0.38 to 0.95. The scale explained 54.73% of the total variance and consisted of 30 items and four factors: Health Responsibility, Nutrition, Psychosocial Adaptation, and Lifestyle Management. The overall Cronbach’s Alpha coefficient for the scale was 0.89, and for the sub-dimensions, it ranged between 0.62 and 0.87. Fit indices for the scale model indicated acceptable model fit: χ²/df = 2.23, RMSEA = 0.067, CFI = 0.95, RMR = 0.067, and SRMR = 0.09. All subdimensions showed significant and strong correlations with the total scale score (r = 0.76–0.85). In conclusion, the findings suggest that the scale developed in this study is a valid and reliable measurement tool that can be used to assess the self-awareness of disease management in patients with cardiac disease
Investigation of changes in physical properties of denim fabric after bio-stoning and bio-polishing processes with cellulas enzyme obtained from mutant Bacillus subtilis IE3
Bu çalışmada, ticari ve Mutant Bacillus subtilis IE3 mutant bakterisinden elde edilen selülaz enzimlerinin denim kumaşlara uygulanan biyotaşlama ve biyoparlatma işlemlerinde kullanılma potansiyeli değerlendirilmiştir. Deneysel çalışmalarda, farklı pH, sıcaklık ve konsantrasyonlarda gerçekleştirilen biyotaşlama ve biyoparlatma işlemlerinin denim kumaşların fiziksel özelliklerine (ağırlık, kopma mukavemeti, kopma uzaması, pillingleşme, renk farklılığı vb.) etkileri karşılaştırmalı olarak incelenmiştir. Elde edilen veriler, mutant enzimin ticari selülazlara göre daha yüksek aktivite gösterdiğini ve kumaşlarda daha belirgin bir eskitme ve yüzey düzgünlüğü sağladığını ortaya koymuştur. Ayrıca, süreç boyunca su tüketiminin azalması ve çevresel etkilerin minimize edilmesi gibi sürdürülebilirlik açısından önemli avantajlar sunulmuştur. Bu bağlamda çalışma, tekstil endüstrisinde çevre dostu ve yenilikçi enzimatik terbiye tekniklerinin uygulanabilirliğini göstermektedir.In this study, the potential of using commercial cellulase enzymes and those obtained from Mutant Bacillus subtilis IE3 mutant bacteria in biostoning and biopolishing processes applied to denim fabrics was evaluated. For this purpose, the effects of biostoning and biopolishing processes carried out at different pH, temperature and enzyme concentrations on the physical properties (weight, tensile strength, elasticity, etc.) of denim fabrics were comparatively investigated. Comparative analysis demonstrated that the mutant enzyme exhibited superior performance over commercial cellulases, achieving more effective surface smoothness and faded appearance. Furthermore, the enzymatic process led to reduced water consumption and environmental impact, supporting its role as a sustainable alternative to conventional finishing methods. The results underscore the potential of bioengineered enzymes in textile finishing and their applicability in advancing eco-friendly innovations within the denim industry
Suppressing t(4;11) acute leukemia by lipopolymer nanoparticle delivery of sirna targeting <i>kmt2a: : Aff1</i> with enhanced extrahepatic delivery
Effective siRNA delivery in acute lymphoblastic leukemia (ALL) is limited by preferential hepatic accumulation. To address this, a lipopolymer (PEI-C) is developed by conjugating lipid to polyethylenimine and formulated lipopolymer nanoparticles (LPNPs) via complexation with siRNA. The siRNA delivery efficiency of LPNPs is evaluated in vitro in t(4;11)-positive ALL cells (RS4;11 and SEM) as well as "normal" peripheral blood mononuclear cells (PBMCs) from human donors and bone marrow stromal cells (BMSCs) from mice. Molecular effects are assessed by quantifying target mRNA silencing and downstream apoptosis. In vivo biodistribution and therapeutic efficacy are examined in mouse models. LPNPs demonstrated significantly higher siRNA uptake than commercial reagents in PBMCs and BMSCs, reaching siRNA uptakes of 87.2% and 93.0% in RS4;11 and SEM cells, respectively. Molecular analyses revealed effective silencing of KMT2A::AFF1 mRNA (approximate to 80% in RS4;11), accompanied by BCL2 downregulation and increased apoptosis. In vivo, LPNPs showed efficient siRNA biodistribution to leukemia-associated organs (spleen and bone marrow) and significantly reduced leukemia burden in a systemic RS4;11 xenograft mouse model and improved survival. These findings suggest that PEI-C-formulated LPNPs present a promising avenue for therapeutic siRNA delivery in ALL, effectively targeting leukemia-associated organs, and warrant further exploration in clinical studies
Socio-economic life in Bursa through the eyes of Hacıbeyzade Ahmed Muhtar
Bursa, Osmanlı Devleti’nin ilk fethettiği bölgelerden biridir. İstanbul’a yakınlığı ve stratejik konumundan dolayı bu kente ayrı bir gözle bakılmış, devletin sonuna kadar değerinden bir şey kaybetmemiştir. Tarih araştırmalarında daha çok siyasi sahanın değerlendirildiği, sosyoekonomik perspektifin ise ikinci planda kaldığı söylenebilir. Hacıbeyzade Ahmed Muhtar tarafından kaleme alınan Bursa Sergisi Rehberi isimli eser, sosyoekonomik yapının ele alındığı nadir eserlerden biridir. Mezkûr çalışma bu nedenle değerlendirilmeye uygun bulunmuştur. Bu çalışmada Bursa’nın engin siyasi tarihine, idari, demografik ve coğrafi özelliklerine kısaca değinildikten sonra, bahsedilen eser temelinde şehrin sosyoekonomik yapısına odaklanılmıştır. Monografik olarak ele alınan Bursa şehrinin özgün ve farklı nitelikleri ön plana çıkarılmış, şehrin yıllar içinde geçirdiği ekonomik ve toplumsal süreç izah edilmiştir. 1923 yılına ait veriler ile önceki yıllara ait veriler mukayese edilmiş, Bursa’nın yeni kimliği ortaya konulmuştur.Bursa is one of the first regions conquered by the Ottoman Empire. Due to its proximity to Istanbul and its strategic location, this city was viewed with a special eye and did not lose any of its value until the end of the state. It can be said that in historical research, the political field is mostly evaluated and the socio-economic perspective remains in the background. The work titled Bursa Exhibition Guide written by Hacıbeyzade Ahmed Muhtar is one of the rare works that deals with the socio-economic structure. For this reason, the study was found suitable for evaluation. In this study, after briefly mentioning the vast political history, administrative, demographic and geographical characteristics of Bursa, the socio-economic structure of the city is focused on the basis of the mentioned work. The unique and different characteristics of the city of Bursa, which was handled monographically, were highlighted and the economic and social processes the city went through over the years were explained. Data from 1923 was compared with data from previous years, and Bursa’s new identity was revealed
Effect of atmospheric pressure plasma treatment on the wettability and aging behavior of metal surfaces
Atmospheric pressure plasma (APP) applications have started to come to the fore today because of their fast and stable applicability to material surfaces and many advantages compared to plasma applications made under vacuum. With APP, it is possible to improve the surface energies, adhesion behavior and surface mechanical properties of materials. However, the changes obtained are not permanent since the surface tends to return to its untreated original state after plasma treatment. In this study, the wettability of APP treated metal surfaces, aging behavior and storage conditions that can delay the return of the surface to its untreated initial state were investigated. While one type of material (H300LAD), two different plasma treatments (cold and hot) and three different plasma treatment numbers (1, 2 and 3 times) were determined as test parameters; two different ambient conditions such as air and water (for 1, 10 and 60 min) were chosen as the storage conditions that can delay the return of the plasma treated surfaces to their original state. In addition, the effects of temperature on the aging time were investigated for two different temperature values (-20 degrees C and 30 degrees C). The effects of the applied plasma type, plasma treatment number, the storage conditions in which the samples were kept and the temperature on the aging behavior were evaluated and the conditions that could delay the aging time were investigated. The results showed that aging is greatly affected by the type of plasma applied to the surfaces, the storage conditions in which the samples were kept and the temperature
Comparison of cage versus floor rearing systems on broiler performance, welfare and behavioral parameters
This study was carried out to determine the effects of the floor type (litter rearing and sledge cage system) on the performance, welfare, and behavior parameters of broilers. A total of 240 one-day-old Ross 308 male broilers were reared in two different rearing systems; sledge cage system (CS) and litter floor system (FS) until 42 days of age. The final body weight and body weight gain between 1-42 days of age tended to be higher in FS than in the CS system (P=0.060). The cumulative feed consumption was similar between the rearing systems at 1-21, 1-28, 1-35, and 1-42 days of age periods (P>0.05). The feed conversion ratio was lower in FS than in the CS system (P0.05). The tonic immobility duration and induction number were found similar in both rearing systems (P>0.05). The higher eating, scratching, wing clap-body shaking, spot pecking; and lesser sleeping behavior were observed in FS than in CS (P0.05). The wing-leg stretching, feather maintenance, and pecking each other behavior of broilers were found similar in both rearing systems (P>0.05). The aggressive pecking behavior tended to be observed more in FS than in the CS system (P=0.062). It has been determined that broilers had better performance, welfare, and behavior parameters in the floor-rearing system when the litter condition is good
Effect of different water and nitrogen levels on watermelon yield
Su ve azot (N), karpuz bitkisinin vejetatif ve generatif gelişimini etkileyen temel belirleyicilerdir. Bu çalışmada, farklı sulama ve azot seviyeleri altında Bursa ekolojik koşullarında yetiştirilen karpuz bitkisinin su-verim ilişkilerinin belirlenmesi amaçlanmıştır. A sınıfı buharlaşma kabından buharlaşan su miktarına farklı bitki-kap katsayıları (kpc) uygulanarak dört sulama seviyesi (IR-100, IR-75, IR-50 ve IR-25) ve beş azot oranı (N, kg ha-1) (N0, N75, N150, N225 ve N300) deneme konusu olarak ele alınmıştır. Araştırmada, bölünmüş parseller deneme deseni kullanılmıştır. Deneme konularına 201 ile 550 mm arasında sulama suyu uygulanırken bitki su tüketimi 275 ile 634 mm arasında değişiklik göstermiştir. En yüksek meyve verimi, IR-100 konusundan elde edilmiştir. Sulama seviyelerindeki azalmayla birlikte verim de azalmıştır. Meyve verimi, tek meyve ağırlığı ve meyve boyutu 225 kg ha-1 azot seviyesine kadar artış göstermiştir. Sulama ve azot interaksiyonu, meyve verimi üzerinde istatistiksel olarak önemli (p<0.01) etki göstermiştir. En yüksek verim ve verim bileşenleri IR-100 sulama konusu altında N225 ve N150 azot oranlarında elde edilirken ikinci en yüksek verim IR75×N225 interaksiyonundan elde edilmiştir. Düşük sulama seviyeleri ve 225 kg ha-1’a kadar artan azot oranlarında daha yüksek su üretkenliği saptanmıştır. Sonuç olarak, yüksek düzeyde karpuz verimi elde etmek ve su verimliliğini artırmak için A sınıfı kaptan buharlaşan su miktarına kpc=0,75 katsayısı uygulanarak sulama yapılması ve 225 kg ha-1 azot oranı önerilebilir. Bu koşullar sonucunda, uygulanan sulama suyu miktarı, bitki su tüketimi, meyve verimi ve su verimliliği değerleri sırasıyla 434 mm, 537 mm, 100,23 t ha-1 ve 18,47 kg m-3 olarak belirlenmiştir.Water and nitrogen (N) are the main determinants affecting watermelon plants' vegetative and generative growth. This study aimed to determine the water-yield relationships of watermelon plants grown in Bursa ecological conditions under different irrigation and N levels. Four irrigation levels (IR-100, IR-75, IR-50, and IR-25) and five N ratios (kg ha-1) (N0, N75, N150, N225 and N300) were considered as experimental treatments. In the study, a split-plot experimental design was used. While irrigation water was applied to the treatments between 201 and 550 mm, actual crop evapotranspiration varied between 275 and 634 mm. Among the irrigation treatments, the IR-100 achieved the highest fruit yield. With the decrease in irrigation levels, the yield also decreased. Fruit yield, weight, and size increased to a N level of 225 kg ha-1. Irrigation and N interaction showed a statistically significant effect (p<0.01) on fruit yield. While the highest yield and yield components were obtained at N225 and N150 N rates under IR-100 treatment, the second highest yield was obtained from the IR75×N225 interaction. Relatively higher water productivity was found at low irrigation levels (IR-25 and IR-50) and increasing N rates up to 225 kg ha-1. As a result, to obtain high watermelon yield and increase water productivity, IR-75 irrigation treatment and a N rate of 225 kg ha-1. As a result of these conditions, the amount of applied irrigation water, actual evapotranspiration, fruit yield, and water productivity values were determined as 434 mm, 537 mm, 100,23 t ha-1, and 18,47 kg m-3, respectivel
The effect of dehydroepiandrosterone on GNRH regulation in a premature ovarian ıinsufficiency model: Hypothalamic peptide-receptor expression profile
Prematür over yetmezliği (POY), 40 yaşından önce ovaryum fonksiyonlarının bozulmasıyla karakterize edilen ve infertiliteye yol açan önemli bir klinik durumdur. Dehidroepiandrosteron (DHEA), POY hastalarında ovaryum fonksiyonlarını ve in vitro fertilizasyon (IVF) başarı oranlarını artırma potansiyeline sahiptir. Bu çalışmanın amacı, POY modeli oluşturulan sıçanlarda DHEA’nın hipotalamus-hipofiz-gonad (HPG) ekseni üzerindeki etkilerini ve bu etkinin altında yatan mekanizmaları ortaya koymaktır. Bu amaçla, 49 dişi Sprague-Dawley sıçan kontrol (n=7), VCD (n=7), VCD+DHEA (n=7), VCD+DMSO (n=7), DMSO (n=7), DMSO+DHEA (n=7) ve DMSO+DMSO (n=7) olmak üzere yedi gruba ayrıldı. POY modeli, 15 gün süreyle 160 mg/kg subkutan VCD enjeksiyonu ile oluşturuldu. Normal over rezervi grubuna ise sadece DMSO uygulandı. POY modelinin doğrulanması için 1. ve 15. günlerde anti-müllerian hormon (AMH) düzeyleri, enzyme-linked immunosorbent assay (ELISA) yöntemiyle ölçüldü. Daha sonra, belirli gruplara 45 gün boyunca 60 mg/kg/sc dozunda DHEA verildi. Deney sonunda sıçanlar sakrifiye edilerek hipotalamus dokuları izole edilerek, gerçek zamanlı polimeraz zincir reaksiyonu (RT-PCR) yöntemiyle nöropeptid ve reseptörlerine ait gen ekspresyon düzeyleri analiz edildi. POY modelinde hipotalamusta; kisspeptin, galanin, melanokortin, leptin, glutamat gibi uyarıcı nöropeptidlerin düzeyleri azalırken, nöropeptid Y, dinorfin, RFRP, GABA gibi baskılayıcı nöropeptidlerin ifadesinde artış gözlemlendi. DHEA tedavisinin, bu dengesizliği tersine çevirerek uyarıcıları artırdığı, baskılayıcıları azalttığı görüldü. Sonuç olarak; VCD kaynaklı POY sıçan modelinde HPG ekseninde meydana gelen değişimlerin, DHEA ile düzenlenebileceği bulundu. DHEA’nın HPG ekseni üzerinde direkt etki gösteren nöropeptidler üzerinde etkili olduğu, ancak melanokortin, leptin, dinorfin, RFRP gibi bazı metabolik bağlantılı peptid ve reseptörlerde etkisiz kaldığı görüldü. Bulgular, DHEA’nın hipotalamik nöropeptid sistemleri üzerinde seçici etkiler gösterdiğini ve GnRH salınımı ile ilgili bazı yolları iyileştirebildiğini ortaya koyarak POY tedavisinde DHEA’nın olası mekanizmasına ışık tutmakta ve alandaki bilgi birikimine katkı sunmaktadır.Premature ovarian insufficiency (POI) is a condition characterized by the deterioration of ovarian function before the age of 40 and is one of the causes of infertility. Dehydroepiandrosterone (DHEA) improves both ovarian function and in vitro fertilization (IVF) outcomes in POF patients. The aim of this study was to determine the effect and mechanism of DHEA on the hypothalamic-pituitary-gonadal (HPG) axis in POF cases. In this study, 49 female Sprague-Dawley rats were divided into 7 groups as control (n=7), 4-Vinylcyclohexene diepoxide (VCD) (n=7), VCD+DHEA (n=7), VCD+dimethyl sulfoxide (DMSO) (n=7), DMSO (n=7), DMSO+DHEA (n=7) and DMSO+DMSO (n=7). While no treatment was applied to the control group, VCD groups were injected with a dose of 160 mg/kg/sc for 15 days for the POI model. To establish the normal ovarian reserve group, only DMSO was administered for the same period. To confirm the POI model, serum anti-mullerian hormone (AMH) levels of rats were measured using the enzyme-linked immunosorbent assay (ELISA) method on days 1 and 15. Following VCD and DMSO administration, DHEA was injected with a dose of 60 mg/kg/sc to certain groups for 45 days. At the end of the experiment, the hypothalamuses of the sacrificed rats were isolated and the gene expression levels of excitatory and inhibitory neuropeptides and their receptors were analyzed in these tissues by real-time polymerase chain reaction (RT-PCR). In the hypothalamus in the POI model; while the levels of excitatory neuropeptides such as kisspeptin, galanin, melanocortin, leptin, glutamate decreased, an increase in the expression of inhibitory neuropeptides such as neuropeptide Y, dynorphin, RFRP, and GABA was observed. It was observed that DHEA treatment reversed this imbalance by increasing excitatory and decreasing inhibitory neuropeptides. As a result; it was found that the changes in the HPG axis in the VCD-induced POI rat model could be regulated by DHEA. It was observed that DHEA was effective on neuropeptides that directly affect the HPG axis, but was ineffective on some metabolic-related peptides like melanocortin, leptin, dynorphin, RFRP and their receptors. The findings shed light on the possible mechanism of DHEA in the treatment of POI by revealing that DHEA has selective effects on hypothalamic neuropeptide systems and can improve some pathways related to GnRH release, thus contributing to the knowledge in the field
Evaluation of outdoor thermal comfort with algorithm applications: Bursa Uludağ University Faculty of Agriculture sample
Termal konfor, bireylerin çevresel koşullara ilişkin fizyolojik ve psikolojik memnuniyet durumu olarak tanımlanmaktadır. Bu durum, sıcaklık, nem, rüzgar hızı gibi çevresel faktörlerin yanı sıra bireyin giyim ve metabolizma düzeyi gibi kişisel özelliklerle de ilişkilidir. Dış mekânlarda yetersiz termal konfor koşulları, insanların sağlık, verimlilik ve genel yaşam kalitesi üzerinde olumsuz etkiler yaratabilmektedir. Bu bağlamda, termal konforun analizi ve iyileştirilmesi, daha yaşanabilir ve sürdürülebilir mekânlar tasarlamak için büyük önem taşımaktadır. Bursa Uludağ Üniversitesi Ziraat Fakültesi’nin dış mekânları, mevcut termal konfor koşullarının değerlendirilmesi ve iyileştirilmesi amacıyla bir araştırma alanı olarak seçilmiştir. Rhino, Grasshopper ve Ladybug yazılımlarının entegre kullanımıyla, alanın fiziksel özellikleri ve çevresel verileri analiz edilmiştir. Sıcaklık dağılımı, güneş radyasyonu ve rüzgar yönlendirmesi gibi parametreler incelenerek, dış mekânların termal performansını etkileyen faktörler detaylı bir şekilde değerlendirilmiştir. Gerçekleştirilen analizler ve algoritmik modelleme yöntemleri sayesinde, mevcut termal konfor sorunlarının giderilmesine yönelik iyileştirme önerileri geliştirilmiştir. Elde edilen bulgular, hem dış mekân termal konforunun artırılmasına yönelik etkili çözümler sunmuş hem de sürdürülebilir tasarım yaklaşımlarına katkı sağlamıştır. Bu yöntemlerin uygulanabilirliği, benzer çevresel analizlerin ve tasarım iyileştirmelerinin farklı alanlara da taşınabileceğini göstermektedir.Thermal comfort is defined as the state of physiological and psychological satisfaction of individuals regarding environmental conditions. This state is influenced by environmental factors such as temperature, humidity, and wind speed, as well as personal attributes like clothing and metabolic rate. Inadequate thermal comfort conditions in outdoor spaces can negatively impact people's health, productivity, and overall quality of life. Therefore, analyzing and improving thermal comfort is crucial for designing more livable and sustainable spaces. The outdoor areas of Bursa Uludağ University Faculty of Agriculture were selected as the research site to evaluate and improve existing thermal comfort conditions. Using integrated applications of Rhino, Grasshopper, and Ladybug software, analyses were conducted based on the physical characteristics of the site and environmental data. Parameters such as temperature distribution, solar radiation, and wind direction were examined to comprehensively evaluate the factors affecting the thermal performance of outdoor spaces. Through these analyses and algorithmic modeling methods, improvement proposals were developed to address current thermal comfort issues. The findings not only provide effective solutions for enhancing outdoor thermal comfort but also contribute to sustainable design approaches. The applicability of these methods demonstrates the potential for extending similar environmental analyses and design improvements to other locations
Comparing scoring systems and biomarkers for mortality in fournier gangrene patients
Objective We aimed to compare scoring systems and peripheral inflammatory biomarkers in estimating mortality among Fournier's gangrene (FG) patients. Background Multiple scoring systems and biomarkers are utilized to assess FG severity and predict mortality. However, these indices have complexities, limitations, and inconsistencies. Therefore, alternative methods and parameters are investigated. Materials and Methods Each patient's clinical and laboratory data are well documented at hospitalization, Fournier's gangrene severity index (FGSI), Uludag FGSI (UFGSI), simplified FGSI (SFGSI), and age-adjusted Charlson comorbidity index (ACCI) scores and peripheral blood parameters and ratios were calculated. Patients were compared in two groups as deceased and survived ones. Results Among the 160 patients, 35 (21.9%) died. Diabetes was the most prevalent comorbidity. Deceased patients had significantly higher FGSI, UFGSI, ACCI, and SFGSI scores compared to the survivors (p < 0.001). Among the prognostic parameters, only the red cell distribution width (RDW) value was statistically significantly higher in the non-surviving group (p < 0.05). Multivariate analysis identified age, elevated blood urea nitrogen, symptom duration, disease extent, and malignancy as independent prognostic factors. Conclusion FGSI, UFGSI, SFGSI, and ACCI are reliable prognostic scores in FG and aren't superior to each other. Age, blood urea nitrogen levels, duration of symptoms, disease extent, and coexisting malignancy were found to be independent prognostic factors. Given its rapid and cost-effective nature, RDW should be considered alongside other established parameters in future prognostic models. A new scoring method that includes these variables would be practical and worth testing
