Profile of Suaitabilit of BPJS Patient Drug Prescription With The National Formularium at Juata Community Health Center During The Periode of May-July 2024
Background: Compliance with the National Formulary not only improves the quality of health services in primary health facilities but also ensures the efficiency of treatment costs and the fulfillment of the right and safe drug needs for the community. This study supports efforts to strengthen the implementation of the National Formulary as a strategic guideline to increase access to quality health services, improve pharmaceutical governance, and encourage efficiency of the health budget. Objective: This study aims to assess the suitability of drug prescriptions for patients registered in the BPJS Kesehatan program in accordance with the National Formulary (Fornas) at the Juata Health Center during the period May to July 2024. Research Methods: The research design applied was non-experimental with a retrospective descriptive approach. A total of 300 BPJS patient prescriptions were selected as samples from a total population of 8704 prescriptions using the Slovin method with a precision level of 90%. Data were analyzed descriptively to calculate the percentage of prescription suitability based on prescription sheets and drug items. Results: The results showed that the suitability of prescriptions based on prescription forms and drug items reached 100% during the study period. Of the total 101 prescribed drug items (28 items in May, 41 items in June, and 32 items in July), all were based on the National Formulary. Conclusion: These results provide the conclusion that the Juata Health Center has met the standards of pharmaceutical services, with drug prescriptions that fully support rational, efficient, and effective drug use. The results of this study have significant relevance to national health policies, especially in the implementation of the National Health Insurance (JKN) program
Evaluation of Fondaparinux Use on Kidney Function in Hospitalized COVID-19 Patients with Decreased eGFR
Background: The COVID-19 pandemic has resulted in increased hospitalization rates among patients with multisystem complications, including coagulation disorders and decreased renal function. Fondaparinux, a synthetic anticoagulant with a lower risk of thrombocytopenia, is often used as an alternative to heparin. However, its use in patients with reduced glomerular filtration rate (eGFR) remains debatable, as its elimination is dependent on renal function. Objective: This study aims to evaluate the impact of fondaparinux on renal function, specifically examining changes in eGFR and serum creatinine levels in COVID-19 patients with impaired renal function during treatment at Bhayangkara South Surabaya Hospital. Methods: This research employed a retrospective observational quantitative approach, involving COVID-19 patients who received fondaparinux at the inpatient facility of Bhayangkara Hospital from June 2020 to October 2021. Data were collected from the medical records of patients during their hospitalization, including fondaparinux dosage and eGFR and serum creatinine values, as well as other relevant clinical parameters. Statistical analysis using the Wilcoxon test was conducted to assess significant differences in serum creatinine and eGFR values following fondaparinux administration. Results: Of the 535 patients analyzed, 56 medical records met the inclusion criteria. The results of the Wilcoxon test analysis showed that there was a significant increase in eGFR (p-value 0.012, CI 95%; p-value 0.043, CI 95%; p-value 0.000, CI 95%), a substantial decrease in SCr (p-value 0.035, CI 95%; p-value 0.043, CI 95%; p-value 0.000, CI 95%) based on each degree of COVID-19 severity. Conclusion: The use of fondaparinux may improve kidney function, as indicated by significant decrease in serum creatinine and an increase in eGFR. Furthermore, no bleeding incidents were observed during the use of fondaparinux
Evaluation Of Emergency Neonatal Obstetric Services (PONED) On The Degree Of Mother's Health In The Work Area Of The State Dolok Public Health Center, Silou District, Kahean, Simalungun Regency, In 2023
Background: Mortality rates over time can describe the state and development of public health. Maternal Mortality Rate is one of the important indicators in determining the public health. Improving the health status of mothers and children (KIA) is still one of the main priorities in health development in Indonesia, due to the still high Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in Indonesia.Objectives: to evaluate Basic Emergency Obstetric Neonatal Services (PONED) on Maternal Health Levels in the working area of ??the Dolok State Health Center, Silou Kahean District, Simalungun Regency in 2023.Research method: qualitative method with a qualitative descriptive approach that does not provide treatment to research subjects with the aim of obtaining a detailed picture of the subjects and using an in-depth interview method with informants that begins with observation and is accompanied by documentation to obtain research data.Results: The informant said that the supporting health equipment for PONED was available but not yet complete, while the equipment at the Dolok Public Health Center was mostly available and there were several equipment that did not function properly. The availability of human resources and facilities was good, but it required more attention for virgins in terms of function.Conclusion: The number of those who understand PONED at the Dolok Public Health Center is still lacking, the facilities and infrastructure at the PONED health center are quite adequate, and the implementation of obstetric and neonatal emergencies, PONED health workers are not on duty for 24 hours. Training and efforts to fulfill the needs of medical devices await realization
The Relationship Between Mother's Knowledge and Attitude About Family Lactation Management and Exclusive Breastfeeding for Infants Aged 0-6 Months at RSU Eshum Medan
During the first six months of life, newborns are only given breast milk, which is called exclusive breastfeeding. Breast milk helps improve the baby's immunity, meets the baby's nutritional needs, and reduces mortality and the risk of disease in them. This study aims to determine the relationship between maternal knowledge and attitudes about family lactation management with exclusive breastfeeding in infants aged 0-6 months at RSU Eshum Medan. This study used a quantitative descriptive correlation design using a cross-sectional design. The population of this study was 104 breastfeeding mothers at Eshmun General Hospital, with 64 samples selected using purposive sampling techniques. Data analysis was carried out univariately and bivariately using the Chi Square test. The results showed that the majority of respondents had good knowledge about family lactation management, namely 33 people (51.6%), the majority of respondents had a positive attitude about family lactation management, namely 48 people (75%) and the majority of respondents gave exclusive breastfeeding to newborns, namely 47 people (73.4%). The results of statistical tests showed that there was a significant relationship between maternal knowledge about family lactation management and exclusive breastfeeding in infants aged 0-6 months (p = 0.000) and there was a significant relationship between maternal attitudes about family lactation management and exclusive breastfeeding in infants aged 0-6 months (p = 0.000). Therefore, Eshmun Hospital is expected to improve health education by teaching parents about the benefits of exclusive breastfeeding and effective lactation management practices
The Effect Of Core Stability Exercise And Telephysiotherapy On The Walking Balance Of Post-Ischemic Stroke Patients
Stroke remains a major global health concern, ranking as the second leading cause of death and the third leading cause of disability worldwide. Among the common residual effects faced by stroke survivors is impaired walking balance. To address this issue, this study investigated the effectiveness of core stability exercises in improving walking balance, as measured by the Timed Up and Go (TUG) test. The study involved 12 participants aged between 40 and 70 years and adopted a case study approach. A quasi-experimental one-group pre-test and post-test design was used. The dependent variable was walking balance in post-ischemic stroke patients, while the independent variable was the implementation of core stability exercises delivered through tele-physiotherapy.Statistical analysis using the Shapiro-Wilk test revealed a significant improvement in walking balance after the intervention. The results showed a pre-intervention mean TUG score of 17.500 (SD = 5.351) and a post-intervention mean of 12.333 (SD = 3.228), with a significance value of 0.000 (df = 12). These findings indicate a statistically significant difference in performance before and after the intervention.
In conclusion, the null hypothesis (H?) is rejected and the alternative hypothesis (H?) is accepted, demonstrating that core stability exercises delivered via tele-physiotherapy significantly improve walking balance in post-ischemic stroke patients at Budhi Dharma Clinic, Kreshna Foundation
Implementation of Occupational Safety for Waste Processing Officers for Hazardous and Toxic Materials (B3)
The implementation of occupational safety principles plays a very important role for officers who handle B3 medical waste, considering that they interact directly with hazardous and toxic materials. This effort aims to create a safe, healthy, and pollution-free work environment, so as to minimize the occurrence of work accidents and health problems due to exposure in the workplace. This study uses a qualitative approach with the aim of exploring in depth the implementation of occupational safety in B3 medical waste processing officers at the Pagar Merbau Health Ceter, Deli Serdang Regency. There were three informants in this study, and data were obtained through observation and in-depth interviews. The results of the study showed that the implementation of occupational safety in the field has not been running optimally. One of the main obstacles is the lack of consistency of officers in using Personal Protective Equipment (PPE). Based on these results, it is recommended that K3 officers be more intensive in providing direction and supervision of the use of PPE and the implementation of work procedures according to applicable standards. This is important to ensure the safety and comfort of work for officers involved in the management of B3 medical waste
The Influence of the Risk Reporting and Evaluation System on the Performance of Health Service Units
Background: Improving the quality of health services highly depends on the effectiveness of the reporting system and risk evaluation. These systems support early problem detection and encourage data-driven decision-making to enhance the performance of healthcare service units. Objective: This study aims to examine the effect of reporting systems and risk evaluation on the performance of healthcare service units in selected primary and referral healthcare facilities. Methods: This research utilized a descriptive-analytical method with a quantitative approach. Data were collected through questionnaires and interviews with 60 respondents from various healthcare units. Multiple linear regression analysis was applied using SPSS version 25. Results: The findings show that the reporting system significantly affects the performance of healthcare service units (p<0.05). Risk evaluation also has a positive impact, although to a lesser extent. Supporting factors such as reporting culture, managerial support, and continuous training were also identified as critical to improving the system's effectiveness. Conclusion: Reporting systems and risk evaluation significantly influence the performance of healthcare service units. Strengthening reporting culture and developing structured evaluation systems are necessary to improve overall service quality
Evaluation of the Effectiveness of Risk Management in Addressing Patient Safety Incidents
Background: Patient safety incidents are a serious issue affecting healthcare quality and public trust in hospitals. Effective risk management implementation is crucial to reduce adverse events. Objective: This study aimed to evaluate the effectiveness of risk management in mitigating patient safety incidents at Granmed Hospital, Lubuk Pakam. Methods: The study was conducted from September 2023 to February 2024 using a descriptive quantitative and qualitative approach. Data were collected from 50 healthcare worker respondents via questionnaires, incident report reviews, and in-depth interviews. Results: Findings showed that 78% of respondents believed risk management systems were well implemented, especially in risk identification and analysis stages. However, incident reporting was still low (43%), due to fear of punishment and lack of training. Document analysis revealed a 27% decrease in reported incidents compared to the previous year. Conclusion: Risk management at Granmed Hospital is relatively effective in reducing patient safety incidents. Nevertheless, there is a need to improve the safety culture and develop a more transparent reporting system
The Effect of Organizational Culture and Internal Supervision on the Performance of Health Workers
Background: Health worker performance is a key element in the success of primary healthcare services at Puskesmas. A strong organizational culture and effective internal supervision are believed to enhance motivation, productivity, and service quality among health workers. Objective: This study aims to analyze the effect of organizational culture and internal supervision on the performance of health workers at Bandar Khalifah Health Center, Percut Sei Tuan District. Methods: This research employed an analytical quantitative approach with a cross-sectional design. The population consisted of all active health workers (n=58), selected through total sampling. Data were collected via structured questionnaires and analyzed using Spearman correlation and multiple logistic regression. Results: Findings indicated that organizational culture and internal supervision simultaneously had a significant influence on health worker performance (p<0.05), explaining 63.7% of performance variation. Organizational culture showed a greater effect (Exp(B)=5.66) compared to internal supervision (Exp(B)=3.64). Conclusion: Positive organizational culture and structured internal supervision significantly improve health worker performance. It is recommended that management enhance teamwork values, provide leadership training, and ensure consistent, participative supervision practices

Breast milk (ASI) is the optimal nutrition for infants up to six months, benefiting both babies and mothers. This study examined the relationship between maternal knowledge and husband’s support with breastfeeding success among mothers of 0–6-month-old infants in the Langsa City Health Center UPTD Work Area. Using a cross-sectional design, 62 respondents were selected through purposive sampling. Data were analyzed using chi-square tests. Most mothers had either poor or good knowledge (33.9%) and received low support from their husbands (64.5%). Breastfeeding was unsuccessful in 72.6% of cases. Maternal knowledge (p=0.002) and husband’s support (p=0.001) were significantly associated with breastfeeding success. In conclusion, maternal knowledge and husband’s support influence successful breastfeeding in the study populatio
