Spectroscopic orbit and variability of the Be star V1624 Cyg = 28 Cyg
In recent years the idea, first formulated many decades ago, that the Be phenomenon could be causally related to the duplicity of Be stars, has been repeatedly reconsidered from various perspectives. It is important, therefore, to have reliable information on Be stars, which are confirmed members of binary systems. This study is devoted to V1624 Cyg = 28 Cygni, which was recently identified as a binary with a compact secondary. By measuring the radial velocities (RVs) of the wings of the H alpha emission line and using archival data and published RVs from the International Ultraviolet Explorer, we demonstrate that the Be primary moves in the 359. d26 orbit found recently from interferometry. Our preliminary radial-velocity solution leads to binary masses of 5.6 M, and 0.66 M. Moreover, we documented large and irregular spectral, brightness, and colour changes over a time interval of several decades to show that the object never completely lost its circumstellar matter
Utilizing nanoflower-like NiFe2O4 for dispersive micro solid phase extraction of copper at ultratrace level in food and water samples
Nanoflower-like NiFe2O4 (NFO-NF) was synthesized and employed to develop a dispersive micro solid phase extraction procedure for copper. NFO-NF was characterized by Fourier-transform infrared spectroscopy (FT-IR), scanning transmission electron microscopy (STEM), scanning electron microscopy-dispersive X-ray spectroscopy (SEM-EDX), field emission scanning electron microscopy (FE-SEM), X-ray diffraction (XRD). The optimal conditions for experimental parameters were pH: 8.0, adsorbent dose: 2 mg, both adsorption and desorption times: 60 s, sample volume: 30 mL, and eluent: 1.0 mL of 1.0 mol L−1 HNO₃. An effective enrichment factor (30-fold) for copper was obtained. The limit of detection and limit of quantitation of Cu (II) were 2.1 μg L−1, 8.5 μg kg-1, and 7.0 μg L−1, 25.4 μg kg−1 for water and food samples, respectively. The suggested method's accuracy was verified by certified reference materials with recoveries of higher than 90 %. The copper content of food and water samples was successfully determined by using presented method
Phenobarbital in HECW2-associated epileptic encephalopathy: unexpected clinical efficacy Фенобарбитал при HECW2-ассоциированной эпилептической энцефалопатии: неожиданная клиническая эффективность
Mutations in the HECW2 gene cause severe neurodevelopmental disorders, including epileptic encephalopathy, psychomotor delay, and drug-resistant seizures. The article presents a clinical case of a 2-year-old girl with epilepsy onset at 3 months, developmental regression, hypotonia, and dysmorphic features. Genetic testing revealed a heterozygous missense mutation in the HECW2 gene (c.3988C>T, p.Arg1330Trp). Electroencephalogram showed generalized, multifocal epileptiform discharges, and magnetic resonance imaging revealed external hydrocephalus. In treatment against the background of the ineffectiveness of carbamazepine, valproate and levetiracetam, significant seizure control was achieved with phenobarbital in combination with levetiracetam. This case highlights the challenges in treating HECW2-related epilepsy and suggests phenobarbital as a potential option for refractory seizures
The Impact of Frailty and Metabolic Syndrome on Mortality Among Older Adults: A Five-Year Prospective Cohort Study
Background: Frailty and metabolic syndrome (MetS) are common conditions in older adults and may share overlapping pathophysiological pathways that impact mortality. Objectives: We aimed to investigate the effect of frailty and MetS on mortality the older adults. Methods: This study included 1100 outpatients aged ≥60 years. We followed the participants for five years, during which 13.2% of them died. The status of MetS was assessed using the criteria established by the National Cholesterol Education Program Third Adult Treatment Panel. We evaluated frailty using the FRAIL scale. Results: Mean age was 71.57 ± 7.09 years. The frail group had a significantly higher mean age (73.13 ± 7.75) compared to the nonfrail (70.20 ± 6.23) and prefrail (70.86 ± 6.73) groups (P < 0.001). In all three groups, women made up the majority, but in the frail group (76.9%), there were significantly more women than other groups (P < 0.001). MetS was more prevalent in prefrail and frail groups compared to those who were nonfrail (nonfrail: %59.1, prefrail: %71, frail: %70.2) (P = 0.010). The frail group showed a higher frequency of cognitive impairment, depressive mood, malnutrition, and dependency. The overall mortality rate for the sample was 13.2%, as anticipated, the frail group has a significantly higher mortality rate (23.4%) compared to the other two groups (nonfrail: 5.3%, prefrail: 10.2%) (P < 0.001). The Cox proportional hazards model indicated that the frail group demonstrated an increased mortality risk over five years, even after adjusting for age, sex, and metabolic disorders (hazard ratio: 4.44, 95% confidence interval = 2.19-9.02, P < 0.001). Conclusions: Frailty was a more accurate predictor of mortality than MetS, regardless of age
The Effect of Video Streaming With Virtual Reality Glasses During Arteriovenous Fistula Needle Insertion on Pain and Anxiety of Individuals Undergoing Hemodialysis Treatment
Introduction: Virtual reality headsets are increasingly used in medical procedures to help manage pain and anxiety. This study investigated the effect of a video played through a virtual reality headset on pain and anxiety levels during arteriovenous (AV) fistula needle insertion in patients undergoing hemodialysis. Methods: This randomized controlled trial included 80 patients from a hemodialysis ward, with 40 in the intervention group and 40 in the control group. Data were collected using a Personal Information Form (developed by the researcher based on relevant literature to gather descriptive and medical characteristics), the visual analogue scale (VAS) for pain severity, and the State–Trait Anxiety Inventory to measure anxiety levels. The intervention group utilized a virtual reality headset during the procedure. The intervention was applied during a single dialysis treatment. Findings: The mean pain score in the intervention (virtual reality headset) group was 6.2 ± 0.8, significantly lower than the control group's mean pain score of 8.3 ± 0.9 (p < 0.001). The mean State Anxiety Inventory score for the intervention (virtual reality headset) group was 41.9 ± 3.6, which was significantly lower than the control group's mean score of 64.6 ± 3.4 (p < 0.001). Conclusion: The results of this pilot study suggest that using a virtual reality headset significantly reduced both pain and anxiety in patients undergoing AV fistula needle insertion during hemodialysis. Further studies are indicated to evaluate the utility of virtual reality headset use over a prolonged period. Trial Registration: https://clinicaltrials.gov: NCT06776497
Ultra-Hypofractionated Radiotherapy for Early-Stage Glottic Cancer: Real-World Data From a Single Center
Background: In recent years, smaller-volume radiotherapy has attracted attention. It appears to be a safe and effective treatment for glottic laryngeal cancer. This study evaluated the results of ultra-hypofractionated radiotherapy for early glottic laryngeal cancer in unfit patients. Methods: This retrospective study analyzed 22 patients diagnosed with early glottic cancer between June 2017 and January 2021. The involved vocal cord was treated with 35–42.5 Gy in 5 fractions. Results: On a 59-month median follow-up, the 5-year overall survival and local control rate were 68.2% and 94.7%, respectively. However, the 5-year larynx preservation rate was 89.7%. Chondronecrosis was observed in one patient (4.5%), and soft tissue necrosis in one patient (4.5%). PTVD98 (> 41.08 Gy) was found to be statistically significant in surgery-needed events (HR: 22.4, 95% CI: 1.9–252.1, p = 0.01). Conclusion: Single-cord ultra-hypofractionated radiotherapy appears to be an effective treatment for local control. However, the risk of radionecrosis is higher at doses above 41.08 Gy
Genetic Algorithm-Based Hyperparameter Optimization of YOLOv11: Real-Time Defect Detection and Classification in Energy Cable Production Lines
The detection of defects emerging in manufacturing processes with high accuracy is crucial, and accurate classification of these defects will optimize the quality control processes and overall efficiency. In this study, a real-time prototype cable production line was first developed to create a dataset consisting of the defect types Fisheye, Dent, and Slit. Afterward, a deep learning (DL)-based detection and classification algorithm was optimized for real-time detection and classification of defects occurring on the surfaces of energy cables during the production process. The proposed approach stands out by incorporating a genetic algorithm (GA)-based hyperparameter optimization of the You Only Look Once (YOLOv11n) object detection algorithm. The performance of the optimized YOLOv11n was compared to that of the basic YOLOv11n and other YOLOv11 algorithms in terms of precision, recall, mAP@50, mAP@(50-95), F1-score, GFLOPS, and inference time metrics. The results obtained show that optimized YOLOv11n was able to provide better recall, mAP@50, F1-score, and inference time values. Optimized YOLOv11n produced the highest accuracy rates with the recall value of 91%, mAP@50 value of 94.7%, F1-score value of 90.9%, and inference time value of 1.2 ms. These values correspond to approximately a 3% increase in recall value, a 0.5% increase in mAP@50 value, a 1% increase in F1-score value, and a 0.1 ms decrease in inference time value. Consequently, it can be stated that GA-based hyperparameter optimization enhances the accuracy of DL-based defect detection
Successful treatment of temporal bone langerhans cell histiocytosis with oral steroids: A case report
Langerhans cell histiocytosis (LCH) is a rare disease; treatment recommendations are based on organ involvement and the extent of the disease. We present a patient with a solitary lesion of LCH with BRAF-V600K mutation which was successfully managed with systemic steroids and followed up for a long time. A 39-year-old man presented with persistent right otitis media with effusion despite treatment. LCH was diagnosed by punch biopsy, and temporal computed tomography (CT) showed an invasive, enhancing soft tissue mass extending from the malleolus to the epitympanic recess. Due to the patient’s young age and the localization of the lesion, radiation and surgery were not considered. The patient was treated with systemic methylprednisolone. The patient’s symptoms resolved after treatment and the tumor regressed completely. The case had a good prognosis in the 5-year period. This is the first report of a LCH case with the BRAF-V600K mutation, highlighting a unique aspect of the disease. The successful therapy of the temporal bone LCH with systemic steroids alone, underscores the potential effect of this treatment for the management of unifocal lesions in critical locations. Further studies and case reports are needed to expand our understanding of LCH and explore optimal therapeutic approaches for different mutation subtypes

Türk Anayasa Mahkemesine bireysel başvuruda kabul edilebilirlik kriterleri arasında idarî ve yargısal başvuru yollarının tamamıyla tüketilmesi şartı bulunmaktadır. Bireysel başvuru kapsamındaki ihlal iddiası hem medeni hem idarî hem de cezaî yargılamaya konu olabilir. Birden fazla başvuru yolunun bulunduğu böylesi bir durumda nasıl bir yol izleneceği ise belirsizdir. Bu çalışmada, birden fazla başvuru yolu bulunması durumunda bu yolların tamamının tüketilmesinin gerekip gerekmeyeceği incelenecektir. Türk Anayasa Mahkemesinin (AYM) bu hususa ilişkin kararları hukukî güvenlik ilkesi çerçevesinde tartışılacaktır. Avrupa İnsan Hakları Mahkemesi (AİHM) kendisine yapılan bireysel başvurularda kullanılabilecek etkili hukuk yolu sayısı birden fazla ise bunlardan bir tanesinin tüketilmiş olmasını kural olarak yeterli görmektedir. Bu yollardan hangisinin tüketileceğinin ise başvurucunun takdirinde olduğunu belirtmektedir. AYM kural olarak başvuru yollarından bir tanesinin son merciye kadar tüketilmesi gerektiğini ifade etse de her olayın kendine özgü şartları çerçevesinde değerlendirme yapmakta ve kurala çokça istisna getirmektedir. Bu noktada verilen kararlar hukukî güvenlik ilkesiyle çelişebilmektedir. Nitekim hukukî güvenlik ilkesi 1982 Anayasası madde 2’de düzenlenen hukuk devleti ilkesi kapsamında AYM tarafından benimsenmesi gereken bir ilkedir. Bu ilke; hukuk kurallarında belirlilik, öngörülebilirlik ve istikrarın sağlanmasını, idarenin eylem ve işlemlerinin bu kurallara uygun olmasını, kişilerin devlete güven duymalarını ve hukuk tarafından bu güvenin korunmasını gerekli kılar. AYM’nin her olayın kendine özgü şartları çerçevesinde değerlendirme yapması bireysel başvuruda bulunacak kişiler için hukukî öngörülebilirliği zedeleyebilmektedir.AYM’ye göre olağan başvuru yollarının tüketilmiş olması için söz konusu başvuru yolu, başvurucu bakımından “makul başarı şansı” sunabilmelidir. Çözüm sağlayabilecek mahiyette olmalı, kullanılabilir bir yol olması gerektiği gibi etkili bir yol da olmalıdır. Mahkeme, kasıtlı fiillerin olduğu yaşama hakkı, işkence yasağı gibi insan hakkı ihlali iddialarında durumlarda ceza yargılaması yolunun tüketilmesini yeterli bulmakta ayrıca tazminat davası açılmasına gerek olmadığını belirtmektedir. Tehlikeli faaliyetler sonucu meydana gelen olaylarda kamu görevlilerinin öngörebilecek olmalarına rağmen tedbir almadıkları “muhakeme hatasını veya dikkatsizliği aşan” bir ihmalin bulunduğu olaylarda devletin etkin soruşturma yapma yükümlülüğü olduğunu belirtmiştir. Şeref ve itibara saygı hakkı bakımından yapılan bireysel başvurularda başvuru yollarının tüketilmiş olması için tazminat davası açılmış olmasını gerekli bulmaktadır. AYM’nin bireysel başvuruda olağan başvuru yollarının tüketilmesi kriterini uygularken her somut olayı farklı değerlendirmesi hukukî öngörülebilirlik ve belirliliğin sağlanmasını zorlaştırmaktadır. Çalışmada bu içtihatlar hukukî güvenlik ilkesi çerçevesinde bir değerlendirmeye tâbi tutulacaktır.Among the admissibility criteria for individual application to the Turkish Constitutional Court is the requirement that all administrative and judicial remedies must have been exhausted. A violation claim within the scope of an individual application may be subject to civil, administrative and criminal proceedings. In such a situation with more than one remedy, it is unclear how to proceed. In this study, it will be analyzed whether it is necessary to exhaust all of these remedies in case there is more than one remedy. The decisions of the Turkish Constitutional Court (AYM) on this issue will be discussed within the framework of the principle of legal certainty. The European Court of Human Rights (ECtHR) considers it sufficient, as a rule, if there is more than one effective remedy in individual applications. It states that it is at the discretion of the applicant which of these remedies to exhaust. Although the Tukish Constitutional Court states as a rule that one of the remedies must be exhausted until the last authority, it evaluates each case within the framework of its specific circumstances and makes many exceptions to the rule. The decisions rendered at this point may conflict with the principle of legal certainty. The principle of legal certainty is a principle that must be adopted by the Constitutional Court within the scope of the rule of law principle regulated in Article 2 of the 1982 Constitution. This principle requires that certainty, predictability and stability are ensured in the rules of law, that the actions and transactions of the administration comply with these rules, that individuals have confidence in the state and that this confidence is protected by the law. The Constitutional Court's assessment within the framework of the specific circumstances of each case may damage the legal predictability of the persons who will file an individual application.According to the Turkish Constitutional Court, ordinary remedies must be exhausted when a legal remedy offers a reasonable chance of success. It must be capable of providing a solution and must be an available and effective remedy. In cases of alleged violations of human rights such as the right to life and the prohibition of torture where there are intentional acts, the Court finds it sufficient to exhaust the criminal proceedings and states that there is no need to file a lawsuit for compensation. The Court stated that the state has an obligation to conduct an effective investigation in cases where there is a circumtance that “beyond a mere error or medical negligence, beyond an error of judgment or negligence” in which public officials do not take measures despite the fact that they could have foreseen in the events that occur as a result of dangerous activities. In individual applications concerning the protection of reputation, the Court finds it necessary to have filed a lawsuit for compensation in order for the remedies to be exhausted. In Işıl Yaykır application, the Court stated that the remedy of compensation should also be exhausted in case of material and moral damage caused to another person by negligence. In the Mehmet Seyfi Oktay application, the Court stated that it is not sufficient to exhaust the criminal proceedings in applications made within the scope of the right to respect for private life due to defamation and that a compensation case must also be filed. In the Sinem Hun application, the Court stated that in the case of hate speech, it is sufficient to exhaust only the criminal proceedings. In some of its decisions regarding administrative jurisdiction, it expects both an action for annulment and a full remedy action to have been filed. In the İsmail Akçayoğlu application, it stated that the rejection of the annulment action does not prevent the applicant from being compensated for his damages through a full remedy action. The fact that the Constitutional Court evaluates each concrete case differently when applying the criterion of exhaustion of ordinary remedies in individual application makes it difficult to ensure legal predictability and certainty. In this study, these jurisprudence will be evaluated in the context of the principle of legal certainty.</p
