Depression, anxiety and stress levels of parents with children in the intensive care unit
Çocuğun hastalanarak yoğun bakım ünitesine yatması ebeveynlerin çocuğun sağlığıyla ilgili kaygı yaşaması ile birlikte günlük rutinlerini ve rollerini yerine getirmede zorlanmalarına ve bu süreçte duygusal olarak yıpranmalarına yol açabilmektedir. Tanımlayıcı türde yapılan bu araştırma ile çocuğu yoğun bakım ünitesinde yatan ebeveynlerin stres, anksiyete ve depresyon düzeylerinin incelenmesi amaçlanmıştır. Araştırmanın örneklemini Gazi Üniversitesi Tıp Fakültesi Hastanesi Çocuk Yoğun Bakım Ünitesi'nde yatan çocukların ebeveynleri oluşturmuştur (n= 152). Veriler, Çocuk ve Ebeveyn Bilgi Formu ve Depresyon-Anksiyete- Stres Ölçeği-21 kullanılarak, 04.12.2021-15.05.2022 tarihleri arasında toplanmıştır. Verilerin analizinde tanımlayıcı istatistikler, Cronbach Alfa, Kolmogrow Smirnow, Mann Whitney U, Kruskal Wallis H ve Pearson Korelasyon testi ile ileri analizler için Bonferroni testi kullanılmıştır. Araştırma kapsamdaki ebeveynlerin stres puan ortalaması 5,78±3,78, anksiyete puan ortalaması 5,56±3,84 ve depresyon puan ortalaması ise 6,64±3,85'tir. Ebeveynlerin anksiyete düzeyinin uğraş ve yakınlık durumuna göre; depresyon düzeyinin ise gelir durumu ve hastanede kaldıkları yere göre istatistiksel olarak anlamlı farklılık yarattığı tespit edilmiştir (p<0,05). Bu araştırma sonucunda çocuğu yoğun bakım ünitesinde yatan ebeveynlerin hafif düzeyde anksiyete ve depresyon yaşadıkları söylenebilir. Bu sonuçlar ışığında hemşirelerin yoğun bakım ünitesinde tedavi edilen çocuklar kadar, ebeveynlerini de kapsayan aile merkezli bütüncül bakım sunması önerilebilir.The fact that the child gets sick and stays in the intensive care unit can cause parents to have anxiety about the child's health, and to have difficulties in fulfilling their daily routines and roles, and to be emotionally worn out in this process. With this descriptive study, it was aimed to examine the stress, anxiety and depression levels of parents whose children were hospitalized in the intensive care unit. The sample of the study consisted of the parents of the children hospitalized in the Gazi University Medical Faculty Hospital Pediatric Intensive Care Unit (n= 152). Data were collected between 04.12.2021- 15.05.2022 using Child and Parent Information Form and Depression-Anxiety-Stress Scale-21. Descriptive statistics, Cronbach Alpha, Kolmogrow Smirnow, Mann Whitney U, Kruskal Wallis H and Pearson Correlation tests were used in the analysis of the data, and Bonferroni test was used for further analysis. The mean stress score of the parents within the scope of the study was 5.78±3.78, the mean anxiety score was 5.56±3.84, and the mean depression score was 6.64±3.85. According to the parents' anxiety level, occupation and closeness; it was determined that the level of depression created a statistically significant difference according to income status and hospital stay (p<0.05). As a result of this research, it can be said that parents whose children are hospitalized in the intensive care unit experience mild anxiety and depression. In the light of these results, it can be suggested that nurses provide family-centered holistic care, including their parents as well as children treated in the intensive care unit
Molecular characterization of antibiotic resistance genes in Enterobacteriaceae isolated from urinary tract infections (UTI)
Bu tez çalışmasında idrar yolu enfeksiyonlarına sebep olan klinik Enterobacteriaceae izolatlarında beta laktam ve kinolon antibiyotiklerine karşı direnç oluşumunu sağlayan genlerin belirlenmesi ve bu genlerin konjugatif plazmitler üzerinde aktarılabilirliğinin gösterilmesi amaçlandı. İdrar Yolu Enfeksiyonu şikayeti ile başvuran hastalardan alınan idrar örneklerinden kültür yöntemleri ile izole edilmiş ve Vitek-2 otomatize sistem ile tanımlanmış yetmiş Gram-negatif Enterobacteriaceae izolatı Eylül-Aralık 2018 tarihleri arasında Kırşehir Ahi Evran Üniversitesi Eğitim ve Araştırma Hastanesi Tıbbi Mikrobiyoloji Laboratuvarından temin edildi. Gerçekleştirilen antibiyogram testleri sonucunda 13 Enterobacteriaceae izolatının çoklu ilaç direncine sahip olduğu tespit edildi ve bu izolatlar çalışmaya dahil edildi. İzolatların moleküler tanımlanması 16S rRNA gen dizileri kullanılarak gerçekleştirildi. Beta-laktam antibiyotiklerine direnç sağlayan plazmit aracılı ?-laktamaz (blaTEM, blaSHV, blaCTX-M1, blaCTX-M2, blaOXA-1, blaPER-1) ve kinolon antibiyotiklerine dirençsağlayan plazmit aracılı kinolon direnç genleri (aac(6?)-Ib-cr, qnrA, qnrB, qnrS) PCR yardımı ile belirlendi. Direnç genlerinin transfer edilebilirliğini belirlemek için konjugasyon deneyleri gerçekleştirildi. Moleküler tanımlama sonucunda izolatlar Proteus mirabilis (Pr3, 6, 8, 9, 11 ve 17), Citrobacter koseri (Pr4 ve Pr5), E. coli (Pr10, 12 ve 13), Proteus sp. (Pr19) ve Klebsiella pneumoniae (Pr20) olarak tanımlandı. İzolatların tamamının çoklu ilaç direncine sahip olduğu tespit edildi. Enterobacteriaceae izolatlarının tümü (%100) ?-laktam, ?-laktam/?-laktamaz inhibitörü, kinolon ve sülfonamid grubu antibiyotikler olan ampisilin, sulbaktam/ampisilin, nalidiksik asit ve trimethoprim/sülfametoksazol antibiyotiklerine dirençli olduğu tespit edildi. İzolatlardaki plazmit aracılı direnç genlerinin varlığı incelendiğinde, izolatların tümünün (%100) blaCTX-M1 ve aac(6?)-Ib-cr genleri bakımından pozitif olduğu tespit edildi. İzolatların %85'i blaTEM, %38'i blaCTX-M2, %8'i blaSHV pozitif bulundu. Hiçbir izolatda blaOXA-1 ve blaPER-1 genleri tespit edilemedi. Konjugasyon denyleri sonucunda, Proteus mirabilis Pr17 izolatının konjugatif bir plazmit içerdiği belirlendi. PCR çalışmaları göstermiştirki; blaTEM, blaCTX-M1, qnrA ve aac(6?)-Ib-cr direnç genleri ve Sınıf-1 integron gen kaset yapısının bu konjugatif plazmitte taşınmaktadır. Halk sağlığı açısından ilimizde moleküler yöntemler ile antibiyotik direncinin gösterilmesi, direnç mekanizmasının anlaşılması ve dirençli bakterilere karşı etkin korunma ve kontrol önlemlerinin geliştirilmesinde yardımcı olacağı düşünülmektedir.In this thesis, it was aimed to determine the genes that cause resistance to beta lactam and quinolone antibiotics in clinical Enterobacteriaceae isolates that cause urinary tract infections and to show the transferability of these genes on conjugative plasmids. Seventy Gram-negative Enterobacteriaceae isolates, which were identified with the Vitek-2 automated system and isolated by culture methods from the urine samples taken from the patients who applied with the complaint of Urinary Tract Infection, were obtained from the Medical Microbiology Laboratory of Kırşehir Ahi Evran University Training and Research Hospital between September and December 2018. As a result of antibiogram tests, 13 Enterobacteriaceae isolates were found to have multidrug resistance and these isolates were included in the study. Molecular identification of isolates was performed using 16S rRNA gene sequences. Plasmid-mediated ?-lactamase (blaTEM, blaSHV, blaCTX-M1, blaCTX-M2, blaOXA-1, blaPER-1) that provides resistance to beta-lactam antibiotics and plasmid-mediated quinolone resistance genes (aac(6)?-Ib-cr, qnrA, qnrB, qnrS) were determined by PCR. Conjugation experiments were performed to determine the transferability of resistance genes. As a result of molecular identification, the isolates were identified as Proteus mirabilis (Pr3, 6, 8, 9, 11 and 17), Citrobacter koseri (Pr4 and Pr5), E. coli (Pr10, 12 and 13), Proteus sp. (Pr19) and Klebsiella pneumoniae (Pr20). All of the isolates were found to have multidrug resistance. All Enterobacteriaceae isolates were found to be resistant to ampicillin, sulbactam/ampicillin, nalidixic acid and trimethoprim/sulfamethoxazole antibiotics, which are ?-lactam, ?-lactam/?-lactamase inhibitor, quinolone and sulfonamide group antibiotics, respectively. When the presence of plasmid-mediated resistance genes was examined, it was determined that all of the isolates (100%) were positive for blaCTX-M1 and aac(6?)-Ib-cr genes. 85%, 38% and 8% of the isolates were positive for blaTEM, blaCTX-M2 and blaSHV gene, respectively. The blaOXA-1 and blaPER-1 genes were not detected in the isolates. As a result of conjugation experiments, it was determined that Proteus mirabilis Pr17 isolate contained a conjugative plasmid. PCR studies have shown that; the resistance genes blaTEM, blaCTX-M1, qnrA and aac(6?)-Ib-cr and the Class-1 integron gene cassette structure are carried in this conjugative plasmid. In terms of public health, it is thought that demonstrating antibiotic resistance with molecular methods in our province will help to understand the mechanism of resistance and to develop effective prevention and control measures against resistant bacteria
Examining the expectations of 7th grade students' parents from mathematics education in terms of anxiety, attitude and academic resilience
Öğrencilerin özelliklerini açıklayan birçok kuram bulunmasına rağmen, onların eğitiminde büyük rol sahibi olan velilerine yönelik araştırmalara da ihtiyaç duyulmaktadır. Bu yüzden öğrenci velilerinin matematik eğitimine ilişkin beklentilerin ortaya çıkarılmasının önemli olduğu söylenebilir. Bu beklentilerin ortaya çıkmasıyla birlikte belki de öğrencilerin düşünme süreçleri, hazırbulunuşluluk, öğrenme yaklaşımları, üst bilişsel farkındalıkları, matematik kaygı ve tutumları, akademik yılmazlıkları gibi çeşitli faktörler hakkında daha geniş bilgiler edinilebilir. Yapılan bu çalışmada ortaokul öğrenci velilerinin matematik eğitiminden beklentileri ile öğrencilerin matematiğe karşı kaygı ve tutumları arasındaki ilişki ortaya çıkarılmaya çalışılmıştır. İlişkisel tarama yönteminin kullanıldığı bu araştırmada toplam 917 tane 7. sınıf öğrencilerinden oluşan 391 erkek ve 526 kız öğrenci ve bu öğrencilerin velileri olan 721 tanesi anne, 184 tanesi baba ve 12 tanesinin diğer veli türü ile yapılmıştır. Araştırmada veri toplamı aracı olarak Aytekin, Baltacı, Altunkaya, Kıymaz ve Yıldız (2016) tarafından geliştirilen "Matematik Eğitimi Veli Beklenti Ölçeği", Bindak (2005) tarafından geliştirilen "Matematiksel Kaygı Ölçeği", Önal (2013) tarafından geliştirilen "Matematiksel Tutum Ölçeği", Ricketts, Engelhard ve Chang (2015) tarafından geliştirilen ve bu ölçeği Türk kültürüne Pekdemir, Yazıcı, Altun, Tosun (2019) tarafından uyarlanan "Matematiksel Yılmazlık Ölçeği" olmak üzere 4 ölçek kullanılmıştır. Toplanan veriler SPSS ile analiz edilmiştir. Araştırmanın sonucunda öğrencilerin matematik kaygıları ile velilerin kavramsal anlama ve aktif katılım beklentisi arasında pozitif anlamlı ilişki olduğu tespit edilirken, velilerin öğretmen merkezli ve kural odaklı eğitim beklentileri arasında negatif yönlü istatistiki olarak önemli olmayan bir ilişki tespit edilmiştir. Yine matematiksel tutum alt boyutlarından olan ilgi ve kaygı alt boyutları ile velilerin kavramsal anlama ve aktif katılım beklentisi arasındaki ilişkinin pozitif yönlü ve istatistiki olarak önemli olduğu belirlenmiştir. Velilerinin olumlu tutum ve davranış kazandırma beklentileri ile matematiğe yönelik tutumun alt boyutlarından matematik dersine olan ilgileri arasında pozitif anlamlı ilişki tespit edilirken, matematik dersine olan kaygı, çalışma tutumu ve gereklilik tutumları arasındaki ilişkinin istatistiki olarak önemli olmadığı sonucuna ulaşılmıştır. Bir diğer faktör olan akademik yılmazlık ile velilerin kavramsal anlama ve aktif katılım beklentisi ile olumlu tutum ve davranış kazandırma beklentisi arasında pozitif yönlü anlamlı ilişki tespit edilmiştir. Velilerin öğretmen merkezli ve kural odaklı eğitim beklentisi ile akademik yılmazlık arasındaki ilişkinin istatistiki olarak önemli olmadığı sonucuna ulaşılmıştır.Although there are many theories explaining the characteristics of students, there is also a need for research on parents who have a great role in their education. Therefore, it can be said that it is important to reveal the expectations of student's parents about mathematics education. With the emergence of these expectations, perhaps more extensive information can be abtained about various factors such as students thinking processes, readiness, learning approaches, metacognitive awareness, mathematics anxiety and attitudes, academic resilience. In this study, it was tried to reveal the relationship between the expectations of the parents of secondary school students from mathematics education and the student's anxiety and attitudes towards mathematics. In this study, it was tried to reveal the relationship between the expectations of the parents of secondary school students from mathematics education and the student's anxiety and attitudes towards mathematics. The research, which the relational screening method was used, was conducted with a total of 917 students and their parents, 721 of them were mothers, 187 of them were fathers and 12 of them were other types of parents. There were 4 scales used which are Mathematics Education Data Expectation Scale, developed by Aytekin, Baltaci, Altunkaya and Kiymaz (2016) as data collection means in the research, "Mathematical Anxiety Scale" developed by Bindak (2005), "Mathematical Attitude Scale developed by Onal (2013), developed by Ricketts, Engelhard and Chang (2015) and this scale was adapted to Turkish culture by Pekdemir, Yazici, Altun and Tosun (2019) "Mathematical Resilience Scale". Collected data were analyzed with SPSS. At the end of the study, it was determined that there was a positive significant relationship between students math anxiety and the conceptual understanding of data and the expectation of active participation, while as a negative statistically insignificant relationship was found between the parents teacher centered and rule oriented education expectations. The relationship between interest and anxiety sub-dimensions, which are sub-dimensions of mathematical attitude and parent's conceptual understanding and active participation expectation is positive and statistically significant. While a positive significant relationship was detected between parent's expectations of gaining positive attitudes and behaviours and their interest in mathematics lesson that is one of the sub-dimensions of attitude towars mathematics, it was concluded that the relationship between anxiety, study attitude and necessity attitudes towards the mathematic lesson was not statistically significant. A positive and significant relationship was found between academic resilience, another factor and parent's conceptual understanding and active participation expectation. It was concluded that the relationship between parents expectation of teacher centered and rule based education and academic resilience was not statistically significant
Factors influencing of quality of life in adolescent idiopathic scoliosis
Background: Adolescent idiopathic scoliosis (AIS) is the lateral and horizontal deformity of the vertebral column which occurs idiopathically during adolescence. The aim of this study is to identify independent predictors of quality of life in AIS patients. Methods: In total, 31 adolescent patients diagnosed with AIS aged between 10 and 18 years old were included in the study. The scoliosis severity was determined for each patient according to the Cobb method, and their scoliosis perception using the Walter Reed Visual Assessment Scale, a pain assessment was conducted based on the Visual Analog Scale, quality of life using the Scoliosis Research Society-22 questionnaire, and depression level according to the Children's Depression Scale. Multiple Linear Regression analysis was then performed in order to determine the independent determinants of health-related quality of life. Findings: According to the Linear Regression analysis results, children's depression scale, walter reed visual assessment scale, cobb, and anterior trunk rotation explained 52.7% of the variance as independent determinants of SRS-22. Interpretation: The study examined the determinants affecting the quality of life in AIS patients. The results of the study showed that scoliosis severity, perception of cosmetic deformity, degree of rotation, and depression level to be predictors of quality of life in AIS patients. “This trail registered with NCT05242601.” © 202
Results from a cervical cancer screening program in Samsun, Turkey
Background: Cervical cancer is a preventable disease. This study aimed to share the results of the national cervical cancer screening program performed in primary health care institutions in Samsun between 2015 and 2019. Methods: Women aged 30–65 years who were screened for cervical cancer in screening centers of Samsun between January 01, 2015, and December 31, 2019, were included in this descriptive study. The data were obtained from the automation program of the “National Human Papilloma Virus (HPV) Laboratory Application” used by the Provincial Directorate of Health Cancer Unit through filtering the completion time of the tests, and all results were evaluated without sampling. Thus, data were presented using descriptive statistics. Results: The mean age of 89,302 women included in the cervical cancer screening program was 45.9 ± 9.0 years. Of the samples obtained from the participants, 1.0% were determined as insufficient material, 94.1% as HPV-negative, and 4.9% as HPV-positive. The most common HPV genotypes were 16, 51, 31, and 52. Of the 4337 HPV-positive women, 74.7% of the pap smear results were negative (including infection, 36.5%), and the most common premalignant lesions were atypical squamous cells of undetermined significance in 7.1% and low-grade squamous intraepithelial lesions in 6.9%. HPV 16/18 was also observed in 31.7% of HPV-positive women. Seven hundred ninety-five women were referred to a specialist physician for further examination and treatment within the scope of the screening algorithm. Conclusion: Detecting HPV-positivity by reaching more women within the national cervical cancer screening program’s scope is vital in fighting against this disease. The effectiveness of cancer screening programs should be increased by ensuring community participation through awareness activities. © 2022, The Author(s)
Intracranial lipomas: Clinical and radiological analysis of 153 cases and review of the literature
Aim: It is aimed to describe the localization distribution of lipomas in cases diagnosed with intracranial lipoma, HU value measurements in CT examinations, and point value measurements in MRI and discuss them in the light of the literature. Material and Methods: Cases diagnosed with intracranial lipoma were recorded retrospectively by scanning the radiology database and polyclinic records. The localization of the lipomas was recorded by measuring the HU value on CT imaging and spot value measurements on MRI T2 sequence imaging. The same data were recorded and compared in control radiological examinations.
Results: A total of 153 cases were diagnosed with intracranial lipoma, and falx lipoma (n:72) was the most common. Intra-calvarial lipoma, which was not defined in the literature, was detected in 21 cases. In CT examinations, the mean HU value at the time of initial diagnosis was -57.91 +/- -12.81; In the examination of MRI T2 sequences, it was recorded as 1384.23 +/- 101.62. In control radiological examinations, no statistically significant difference was detected between HU values in CT examinations and point values in MRI examinations. Discussion: It was determined that intracranial lipomas did not differ in their quantitative radiological examinations. Epilepsy resistance to medical treatment is observed in almost all cortical lipomas. Association with central nervous system anomalies is seen only with callosal lipomas
Superior cluneal nerve entrapment neuropathy due to lower crossed syndrome: A case with low back pain [Alt çapraz sendroma bağlı superior kluneal sinir tuzaklanma nöropatisi: Bel ağrılı bir olgu sunumu]
The superior cluneal nerve (SCN) is a sensory nerve known to be originated from the dorsal rami of the lower thoracic and lumbar nerve roots. One of the overlooked causes of low back pain (LBP) is the SCN Entrapment Neuropathy (SCNEN). SCNEN may also be associated with SCN stretching due to lumbar movement and the poor body posture through an increase in the paravertebral muscle tonus. A 59-year-old female patient presented with chronic LBP localized on the right iliac crest and radiating to the right buttock, groin, and leg. She had increased lumbar lordosis and anterior pelvic tilt. She had a tender point over the right iliac crest, and the pain was radiating to the buttock and posterolateral thigh (Tinel sign +). She was diagnosed with lower crossed syndrome and SCNEN, and a therapeutic nerve block was performed. Clinicians should consider SCNEN as a possible diagnosis of LBP
Editing video cases to facilitate preservice teachers’ self-reflection on their instructional decisions
This study examines preservice teachers’ (PTs’) video case (VC) editing process to enhance their self-reflection on instructional decisions in teaching experiences. We apply the triple-cycle process, including teaching experiences, video editing, and peer discussion. Each PT edited three teaching experiences and discussed a total of forty-eight VCs. While editing VCs, PTs tended to notice, describe, identify the reason, judge, express emotions, analyse, and generalize with any other situations. The results indicated that while editing their teaching experiences, PTs improved self-reflection on their instructional decisions, which increased from defining to analysing and generalizing the instructional decisions. Editing video-cases improve PT’s self-reflection on their instructional decisions. Viewing and discussing peers’ experiences enrich PT’s teaching repertoire. PT tend to focus on how to teach including classroom management and teaching methods. Editing and discussing video-cases enhance PT’s noticing skills from both the teacher’s and the student’s perspectives. Thereby, editing video-cases and viewing and discussing peers’ experiences improved PT’s self-reflection and the ability to notice through the eyes of both the student and the teacher. Ecological degradation and non-carcinogenic health risks of potential toxic elements: a GIS-based spatial analysis for Doganci Dam (Turkey)
This study was carried out to determine the ecological degradation and non-carcinogenic health risks at Doğancı Dam, Bursa, Turkey. Potentially toxic element (PTE) concentrations (ppm) were as follows: Fe (55.030)>Al (27.220)>Mn (1053)>Cr (181)>Ni (180)>Zn (95)>Cu (62)>As (17)>Pb (11)>Cd (0.20)>Hg (0.108). As, Pb, Cd, and Hg were enriched anthropogenically, while other PTEs were of natural origin. The contamination severity index (CSI) indicated a moderate PTE contamination in the dam, mostly due to lithogenic efects. According to the modifed hazard quotient (mHQ), ecological risk was identifed at the level of extreme severity for Ni of lithological origin, of high severity for Cr of considerable severity for As of anthropogenic origin, and of moderate severity for Cu. According to the ecological contamination index (ECI), the dam had an ecological risk of a slight-to-moderate contamination. Health risk index showed no non-carcinogenic health risks in the dam. Mining, highways, and agricultural activities were identifed as the primary anthropogenic drivers to be monitored. Genetic diversity and population structure of Salix alba across river systems in Turkey and their importance in conservation management
Background: Salix alba is a pioneer species of river ecosystems throughout Turkey. Its genetic
diversity and population structure across these ecosystems is currently unknown.
Aims: We investigated genetic diversity in Turkish S. alba to assess factors likely to shape the
genetic structure of the species and to assist with conservation recommendations.
Methods: Six hundred and forty-six individuals from 10 major river systems in Turkey were
genotyped using 15 microsatellite markers. Between one and five sub-populations were
sampled from each river system with 23 sub-populations sampled in total.
Results: Populations contained moderately high levels of genetic diversity. Five genetic groups
were detected by Bayesian clustering, with samples from particular river systems mainly
assigned to particular genetic groups. This revealed a geographic structure, also detected by
principal coordinate analysis, showing that particular river system populations in different parts
of Turkey were genetically similar to each other but different from those in other parts of the
country. Conclusion: Genetic isolation caused by geographic distance (in part) and natural barriers
among river systems appear to have shaped the genetic structure of populations. The results
