    Diabetic Foot Osteomyelitis in a Child: A Rare Complication of Type 1 Diabetes Mellitus Subject — A Case Report

    Background: Diabetic foot infections (DFIs) are serious complications of diabetes mellitus, leading to significant morbidity, prolonged hospital stays, and potential limb amputation. Diabetic foot osteomyelitis (DFO) is a severe complication that may develop insidiously, often without overt signs of infection due to underlying neuropathy and vascular insufficiency. Early diagnosis and intervention are essential for preventing severe outcomes. Case report: The case of an 11-year-old girl with poorly controlled type 1 diabetes mellitus who developed osteomyelitis-associated diabetic foot following trauma to both ankles has been presented. She presented with fever, pain, swelling, and restricted movement in both ankles. Imaging studies, including MRI, confirmed the diagnosis of DFO. Methicillin-susceptible Staphylococcus aureus was identified in purulent fluid drained from the affected areas. Following aggressive antibiotic therapy and surgical intervention, the patient showed significant clinical improvement and was discharged after 40 days of hospitalization. Conclusions: Diabetic foot osteomyelitis is a rare but serious condition in the pediatric population, often under-recognized due to its subtle presentation. This case highlights the importance of early imaging and multidisciplinary management to prevent severe complications, including amputation. As the prevalence of diabetes increases in children, awareness of DFO in this age group is critical for timely diagnosis and effective treatment. Diabetic foot osteomyelitis in children is a challenging condition that requires prompt diagnosis and a comprehensive treatment approach. Coordinated care involving early imaging, surgical intervention, and targeted antibiotic therapy is key to preventing severe complications and improving outcomes in pediatric patients with diabetes. © 2025 Elsevier B.V., All rights reserved

    Recognizing malnutrition in adults with critical illness: Guidance statements from the Global Leadership Initiative on Malnutrition

    BackgroundPatients with critical illness may present with disease-related malnutrition upon intensive care unit (ICU) admission. They are at risk of development and progression of malnutrition over the disease trajectory because of inflammation, dysregulated metabolism, and challenges with feeding.MethodsThe Global Leadership Initiative on Malnutrition (GLIM) convened a panel of 36 clinical nutrition experts to develop consensus-based guidance statements addressing the diagnosis of malnutrition during critical illness using a modified Delphi approach with a requirement of >= 75% agreement.Results(1) To identify pre-existing malnutrition, we suggest evaluation within 48 h of ICU admission when feasible (100% agreement) or within 4 days (94% agreement). (2) To identify the development and progression of malnutrition, we suggest re-evaluation of all patients every 7-10 days (97% agreement). (3) To identify progressive loss of muscle mass, we suggest evaluation of muscle mass as soon as feasible (92% agreement) and again after 7-10 days (89% agreement). (4) To identify the development and progression of malnutrition before and after ICU discharge, we suggest re-evaluating nutrition status before ICU discharge and during clinical visits that follow (100% agreement).ConclusionResearch using consistent etiologic and phenotypic variables offers great potential to assess the efficacy of nutrition interventions for critically ill patients with malnutrition. Assessment of these variables during and beyond the ICU stay will clarify the trajectory of malnutrition and enable exploration of impactful treatment modalities at each juncture. GLIM offers a diagnostic approach that can be used to identify malnutrition in critically ill patients

    The Eastern Mediterranean is a region characterized by complex geopolitical dynamics, historical rivalries, and rich hydrocarbon resources. Numerous factors such as energy security, great power competition, international security concerns, military initiatives and collaborations, and the rise of non-state actors significantly influence the region. The purpose of this study is to examine the strategic, political, and military objectives and military initiatives and collaborations of states that aim to increase their military and economic influence in the field of energy in the Eastern Mediterranean in the light of developments in the region. In this context, the study evaluates the hydrocarbon resource discoveries in the region, strategic, political, and military dynamics, and collaborations. In the study, case study and document analysis methods were used within the scope of qualitative research methods. The Eastern Mediterranean is of geopolitical importance with its strategic transit routes and energy resources. The rich natural gas reserves discovered in the region have attracted the attention of global and regional actors and increased Exclusive Economic Zone disputes. Energy collaborations and military partnerships have developed among the countries in the region. The Eastern Mediterranean continues to be a focal point for energy security and geopolitical competition. Military and diplomatic initiatives among regional countries will shape the long-term balance. To resolve regional disputes, it is essential to increase diplomatic efforts among nations, reduce military tensions, and foster economic cooperation.Doğu Akdeniz, karmaşık jeopolitik dinamikler, tarihi rekabetler, zengin hidrokarbon kaynakları ile karakterize edilen bir bölgedir. Enerji güvenliği, büyük güç rekabeti, uluslararası güvenlik endişeleri, askeri girişimler ve iş birlikleri, devlet dışı aktörlerin yükselişi gibi çok sayıda faktör bölgede etkisini göstermektedir. Bu çalışmanın amacı, Doğu Akdeniz’de enerji alanında askeri ve ekonomik etkilerini artırma amacı güden devletlerin stratejik, politik ve askeri hedefleri ile askeri girişimleri ve iş birliklerini bölgedeki gelişmeler ışığında ele almaktır. Bu bağlamda çalışma, bölgedeki hidrokarbon kaynak keşifleri, stratejik, politik ve askeri dinamikleri ve iş birliklerini değerlendirmektedir. Çalışmada nitel araştırma yöntemleri kapsamında durum çalışması (case study) ve doküman analizi (document analysis) metotlarından yararlanılmıştır. Doğu Akdeniz, stratejik geçiş yolları ve enerji kaynaklarıyla jeopolitik önem taşımaktadır. Bölgede keşfedilen zengin doğal gaz rezervleri, küresel ve bölgesel aktörlerin ilgisini çekmiş ve Münhasır Ekonomik Bölge anlaşmazlıklarını artırmıştır.     Coordination problems experienced by volunteer healthcare providers in disasters: Challenges and risks

    This study aimed to explain the coordination problems that were experienced by volunteer healthcare providers in the acute period of disasters, with a special focus on the February 6 Kahramanmaraş Earthquakes. The population comprised of volunteer healthcare providers in the earthquake area participating in the health service in the first 72 hours after the Kahramanmaraş Earthquakes. A phenomenological type of study and criterion sampling method were used, and 12 people participated in the study. Data were collected through semistructured in-depth interviews. The transcripts were evaluated via the content analysis technique, and the process was from codes to categories and from categories to themes. This study observed that the organizational problems during the transfer caused loss of time and motivation. Experiencing disasters provided an advantage in adaptation to the area, whereas the lack of experience, organization, adaptation, and physical needs were obstructive in professional roles and in orientation. Individuals created reactive solutions to problems. There were differences in thought, approach, and behavior in coordination between healthcare providers from different regions. Healthcare providers experienced significant coordination problems in the earthquakes centered in Kahramanmaraş.     Bioactive Phytochemicals from Carrot (Daucus carota) By-Products

    Carrot (Daucus carota subsp. sativus) is a root vegetable with total production, including turnips, reaching approximately 41 million tons in 2020. Carrots have high nutritional value and provide a rich source of bioactive compounds that significantly benefit human health, including carotenoids, vitamins, and minerals. The popularity of carrot consumption is on the rise, attributed to its recognition as a natural antioxidant source that may help prevent chronic diseases, along with its industrial advantages. Following the harvest, carrots that fail to meet the consumer market’s quality standards and size and shape requirements are discarded and often repurposed as cattle feed. This plant is also utilized across various industries, with carrot peels, tops (green leafy parts), pulp, seed oil, pomace, and residue after juice extraction considered by-products. The primary bioactive phytochemicals in Daucus carota by-products include carotenoids, polyacetylenes, phenolic compounds, dietary fiber, and minerals, predominantly extracted from carrot peels and tops. Due to these valuable components, Daucus carota by-products are preferred in food, animal feed, pharmaceutical and cosmetic industries, and agriculture.     The effect of shared decision-making on the conflict and regret in menopause symptoms management: an interventional study

    Introduction: Menopausal women often face challenges in choosing appropriate interventions to mitigate distressing menopause symptoms, primarily due to concerns about treatment suitability. Therefore, the present study aimed to assess the effect of shared decision-making (SDM) on conflict and regret in menopause symptoms management. Method: This interventional study was conducted on 44 menopausal women aged ? 45 years in Tabriz, Iran, from September 2020 to January 2021. Subjects were randomly assigned to the intervention (n = 22), which received SDM-based counseling along with a decision aid (DA) booklet, and control (n = 22) groups. The data were collected using a socio-demographic and obstetric characteristics checklist, awareness of the menopause symptoms management strategies, decisional conflict, and regret. The intergroup differences were compared employing an independent t-test and ANCOVA. Results: The mean score of decision conflict [Mean difference (MD): -2.07, 95% CI: -29.65 to -11.92, P = 0.001] and decision regret [MD: -1.25, 95% CI: -1.64 to 0.64, P = 0.03] significantly reduced in the intervention group compared to the control group after the intervention. Additionally, a statistically significant increase in the mean score of women’s awareness was observed in the intervention group compared to the control group following the intervention [(Hormone Therapy: MD: 3.38, 95% CI: 1.01 to 5.57, P = 0.006) (Natural Products: MD: 2.44, 95% CI: 0.93 to 3.94, P = 0.002)]. Conclusion: The results indicated the effect of SDM-based counseling on improving women’s awareness of the menopause symptoms management strategies and reducing the decisional conflict and regret. © 2025 Elsevier B.V., All rights reserved

    Enhancing biomass and lipid productivities of Haematococcus pluvialis for industrial raw materials products

    For biofuels and nutraceuticals, the green microalga Haematococcus pluvialis (Chlorophyceae) is a prospective source of biomass and lipids. This study examined how biomass production and lipid accumulation were affected by temperature (10 °C, 20 °C, and 30 °C) and potassium nitrate (KNO?) concentrations (0.41 g/L, 0.31 g/L, 0.21 g/L, 0.10 g/L, and 0). The findings showed that the largest biomass (0.665 ± 0.200 g/L) was produced at a potassium nitrate concentration of 0.21 g/L at 20 °C, whereas the highest lipid content (46.31 ± 0.026% dry weight) was produced at a temperature without nitrate. Notably, a balanced result was obtained with a modest nitrate content (0.10 g/L) at 20 °C, yielding significant biomass (0.560 ± 0.136 g/L) and lipids (40.30 ± 0.012% dry weight). These results highlight how crucial it is to optimize cultivation settings in order to increase H. pluvialis's dual productivity, offering important new information for its industrial-scale use. By adjusting growing conditions, this research helps meet the need for renewable resources worldwide by promoting the production of high-value bioproducts and sustainable, commercially viable algae-based biofuels. © 2025 Elsevier B.V., All rights reserved

    Adaptive EKF algorithm with novel fading memory: Design and validation

    In this paper, an adaptive extended Kalman filter (EKF) with a novel fading memory is proposed and validated through its application to the state estimation of an induction motor (IM). A standard EKF (SEKF) observer requires a stochastic system with complete dynamic or measurement equations to achieve estimations. However, in practice, these equations are often partially known or entirely unknown, which degrades the performance of the SEKF. To address this issue, an EKF observer with a novel fading memory is proposed and applied to the state estimation problem of an IM. To validate its effectiveness, the proposed adaptive fading EKF (AFEKF) is experimentally compared to the SEKF and an existing AFEKF from different perspectives. The results confirm that the proposed AFEKF achieves better estimation performance with reduced computational complexity compared to the existing AFEKF approach

    Design and optimization of lamivudine-loaded nanostructured lipid carriers: Improved lipid screening for effective drug delivery

    NeuroAIDS, a neurological complication of HIV, remains a challenge due to the restricted penetration of antiretroviral (ARV) drugs across the blood-brain barrier (BBB). Lamivudine (LAM), a nucleoside reverse transcriptase inhibitor, exhibits limited CNS bioavailability, necessitating novel drug delivery strategies. This study focuses on the development and optimization of a nanostructured lipid carrier (NLC) system for nose-to-brain delivery of LAM, aiming to enhance its therapeutic potential. A systematic formulation approach was employed, including lipid screening, excipient compatibility assessments, and optimization via Box-Behnken design. NLCs were prepared using hot homogenization and ultrasonication methods, ensuring optimal physicochemical properties. The optimized NLC formulation demonstrated a particle size of 112.6 +/- 0.8 nm, PDI of 0.191 +/- 0.009, zeta potential of -31.9 +/- 0.8 mV, and encapsulation efficiency of 49.9 +/- 0.4 %, indicating a stable and homogenous nanosystem. In vitro release exhibited a biphasic profile, with similar to 85 % of the drug released within 4 h and similar to 90 % by 7 h, supporting sustained release behavior. Ex vivo permeation studies across bovine nasal mucosa revealed a cumulative permeation of similar to 16 mg/cm(2) over 6 h, significantly outperforming the conventional LAM solution. The developed NLC formulation provides a promising strategy for improving LAM's bioavailability and CNS targeting NeuroAIDS treatment.
