Understanding poor oral health among older adults in Türkiye: socioeconomic and healthcare access challenges
Background: Oral health is an integral part of overall well-being, and older individuals are particularly vulnerable due to age-related changes and barriers to healthcare access. Despite Türkiye’s comprehensive public health insurance system, significant disparities in oral health persist, highlighting the need for targeted research and interventions. This study assesses the oral health of people aged 65 and older in Türkiye and explores the factors linked to poor oral health, such as individual characteristics, lifestyle, socioeconomic status, social support, overall health, and access to healthcare services. Methods: A secondary analysis was performed using the cross-sectional 2022 Türkiye Health Survey, which gathered data from 3,144 individuals aged 65 and above. Oral health was self-reported, with poor oral health defined as participants rating their dental and gum condition as ‘bad’ or ‘very bad.’ Factors assessed included age, gender, education, social support, healthcare access, and the presence of chronic diseases. Results: Among the participants, 25.8% reported poor oral health. Key factors associated with this included being older, female, having lower levels of education, difficulty communicating in one’s mother tongue, experiencing limitations in daily activities, and facing cost-related barriers to dental care. Multivariable analysis found that cost related barriers and limitations in daily activities had the strongest impact on oral health. The study also highlighted the importance of social support in maintaining better oral health. Conclusions: Poor oral health in older adults in Türkiye is strongly influenced by cost-related barriers, limitations in daily activities, and lack of social support. Expanding public dental services, reducing out-of-pocket costs, and integrating oral health into chronic disease programs are critical steps. Providing culturally sensitive and mobile care options can also address mobility challenges and enhance oral health outcomes for the elderly. © 2025 Elsevier B.V., All rights reserved
Roy adaptasyon modeline temellendirilmiş damgalanmayı önleme psikoeğitiminin tip 1 diyabeti olan bireylerde damgalanma ve yaşam kalitesine etkisi
Amaç: Araştırmanın amacı, Roy Adaptasyon Modeline (RAM) temellendirilmiş damgalanmayı önleme psikoeğitiminin, Tip 1 Diyabeti olan bireylerde damgalanma ve yaşam kalitesi üzerine etkisini incelenmektir. Gereç-Yöntem: Araştırma, ön-test, son-test ve izlem ölçümlü, girişim ve kontrol gruplu, 2x3'lük split-plot desende randomize kontrollü deneysel bir çalışmadır. Araştırma, Mayıs 2024 - Şubat 2025 tarihleri arasında Ege Diyabetliler Derneği'ne kayıtlı 18 girişim 18 kontrol grbunda olan 36 birey ile yürütülmüştür. Araştırmada Tanıtıcı Bilgi Formu, Tip 1 Diyabet Damgalanma Değerlendirme Ölçeği, Kısa Form 36 ve hemşirelik sonuç kriterleri olmak üzere dört veri toplama aracı kullanılmıştır. Girişim grubuna 7 oturumdan oluşan Roy Adaptasyon Modeline Temellendirilmiş Damgalanmayı Önleme Grup Psikoeğitimi uyanırken, kontrol grubuna herhangi bir psikososyal müdahale uygulanmamıştır. Veriler tanımlayıcı istatistikler, bağımsız örneklem t testi, ANOVA testi, Mann Whitney U testi, Bonferroni analizi ve tekrarlı ölçümlerde iki yönlü varyans analizi ile değerlendirilmiştir. Güven aralığı %95, anlamlılık düzeyi p<0.05 kabul edilmiştir. Bulgular: Roy Adaptasyon Modeline Temellendirilmiş Damgalanmayı Önleme Grup Psikoeğitimi grubunun DDDÖ-1 toplam, "suçlama ve yargılama", "kimlik endişeleri" ve "farklı davranılma" alt boyutları son-test ve izlem testi puan ortalamaları, ön-test puanlarına göre istatistiksel olarak anlamlı düzeyde daha düşük olduğu belirlenmiştir (p<0,05). Girişim grubundaki bireylerin DDDÖ-1 suçlama ve yargılama alt boyutu puanlarının takip ölçümlerinin kontrol grubundakilere göre istatistiksel olarak anlamlı düzeyde daha düşük olduğu saptanmıştır (p<0,05). DDDÖ-1 toplam, "suçlama ve yargılama" ve "kimlik endişeleri" alt boyutları puan ortalamalarının zaman ve grup*zaman etkileşimlerinde anlamlı farklar bulunmuştur (p<0,05). Psikoeğitim grubunda KF-36 "genel sağlık algısı" alt ölçeği izlem puan ortalamalarının ön test ve son teste göre, "fiziksel rol güçlüğü", "enerji" alt ölçeği son test ve izlem testi puan ortalamalarının ön teste göre, "mental sağlık" ve "emosyonel rol güçlüğü" alt ölçeği izlem testi puan ortalamalarının ön teste göre istatistiksel olarak anlamlı düzeyde daha yüksek olduğu saptanmıştır (p<0,05). Girişim grubundaki bireylerin "enerji", emosyonel rol" ve "mental sağlık" alt ölçek puanlarının takip ölçümlerinin kontrol grubundakilere göre istatistiksel olarak anlamlı düzeyde daha yüksek olduğu bulunmuştur (p<0,05). KF-36 "fiziksel rol", "genel sağlık algısı", "enerji", emosyonel rol" ve "mental sağlık" alt ölçek puan ortalamaları zaman ve grup*zaman etkileşiminde anlamlı farklar bulunmuştur (p<0,05). RAM temelli damgalanmayı önleme psikoeğitim programının etki büyüklüğünün orta ila büyük düzeyde olduğu belirlenmiştir. Sonuç: RAM temelli damgalanmayı önleme psikoeğitim programı Tip 1 diyabet ile yaşamını sürdüren bireylerin damgalanma uyaranını azalttığı, damgalanma ile etkili baş etmesini sağladığı, fiziksel, benlik kavramı ve rol fonksiyonu alanlarına uyumu güçlendirdiği ancak karşılıklı bağlılığa uyuma yönelik kısıtlı bir etkisi olduğunu ortaya koymuştur.Objective: The aim of this study is to examine the effect of stigma prevention psychoeducation based on the Roy Adaptation Model on stigma and quality of life in individuals with Type 1 Diabetes. Materials and Methods: The study is a randomised controlled experimental study with pre-test, post-test, and follow-up measurements, intervention and control groups, and a 2x3 split-plot design. The study was conducted between May 2024 and February 2025 with 36 individuals in 18 intervention and 18 control groups registered with the Aegean Diabetes Association. Four data collection tools were used: the Research Information Form, the Type 1 Diabetes Stigma Assessment Scale, the Short Form 36, and Nursing Outcome Criteria. intervention group received seven sessions of stigma prevention group psychoeducation based on the Roy Adaptation Model, while the control group didn't received any psychosocial intervention. Data were analyzed using descriptive statistics at a 95% confidence interval and significance level of p<0.05, independent samples t-test, ANOVA test, Mann Whitney U test, Bonferroni analysis, and two-way analysis of variance for repeated measures. A confidence interval of 95% and a significance level of p<0.05 were accepted. Findings: RAM-based stigma prevention psychoeducation group's DSAS-1 total, "blame and judgement, "identity concerns" and "treated differently" subscale post-test and follow-up scores were found to be statistically significantly lower than pre-test scores (p<0.05). It was found that the follow-up measurements of the DDDÖ-1 blame and judgment subscale mean scores in the intervention group were statistically significantly lower than those in the control group (p<0.05) Significant differences were found in the time and group*time interactions of the mean scores of the DSAS-1 total, "blame and judgement," and "identity concerns" subdimensions (p<0.05). In RAM-based stigma prevention psychoeducation group, SF-36 "general perception of health"subscale follow-up mean scores were found to be statistically significantly higher than the post-test and pre-test, "role limitation due to physical problems" and "energy" subscale post-test and follow-up mean scores were found to be statistically significantly higher than pre-test, "mental health" and "role limitation due to emotional problems" subscales follow-up mean scores were found to be statistically significantly higher than the pre-test mean scores (p<0.05). It was found that the follow-up measurements of the "energy", "role limitation due to emotional problems" and "mental health" subscale mean scores of the intervention group were statistically significantly higher than those in the control group (p<0.05). Significant differences were found in the time and group*time interactions of the mean scores of SF-36 "role limitation due to physical problems", "general perception of health", "energy", "role limitation due to emotional problems" and "mental health" subscales (p<0.05). The effect size of the RAM-based stigma prevention psychoeducation programme was found to be moderate to large. Conclusion: The RAM-based stigma prevention psychoeducation programme has been shown to moderately to highly effective in reduce stigma stimuli, enable effective coping with stigma, and strengthen adaptation in the areas of physical, self-concept, and role functioning among individuals living with type 1 diabetes, but it had a limited effect on adaptation to interdependence
Losing a Mother Battling Cancer: Perspectives of an Academic Psychiatric Nurse
Purpose: This paper aimed to share the subjective experiences of an academic psychiatric nurse who lost her mother to terminal cancer and to reveal the impact of this loss on her professional identity and nursing practices. Background: The terminal cancer process affects not only patients but also their family members emotionally. Especially caregiving family members may experience emotional stages similar to the grieving process. Although psychiatric nurses develop emotional resilience through their professional knowledge and skills, they may struggle to balance their professional and personal identities when caring for their relatives. These overlapping roles can deepen the grieving process and directly influence coping mechanisms. Methods: This study is a first-person narrative based on my personal experiences as a psychiatric nurse caring for a terminally ill parent. Keypoint: This narrative offers a unique and profound perspective on the personal grief experience shaped by the intersection of caregiving roles. © 2025 Elsevier B.V., All rights reserved
Azerbaycan’da Safevi tarihçiliği
Bu tez, Azerbaycan'da Safevi tarih yazıcılığının başlangıcı ve gelişim aşamalarını incelemektedir. Çalışmada Safevi devletine ilişkin akademik yaklaşımların farklı dönemlerde nasıl değiştiği analiz edilmekte ve bu sürecin ideolojik, politik ve bilimsel çerçevede nasıl yapılandırıldığı ortaya konulmaktadır. Eserde, Azerbaycan Halk Cumhuriyeti döneminden başlayarak Sovyet dönemi ve bağımsızlık sonrası dönemlere kadar Safevi tarih yazıcılığına karşılaştırmalı bir yaklaşım sunulmaktadır. Sovyet döneminde bu konunun Marksist ideolojik görüşlerin etkisi altında seçici ve sınırlı bir biçimde ele alındığı gösterilmektedir. Bağımsızlık döneminde Safevilerin Azerbaycan tarihi açısından önemi, ulusal tarih yazımı düzeyinde yeniden değerlendirildi. Çalışmada ayrıca Safevilerin etnik ve dinsel kimliğine ilişkin farklı yaklaşımlar incelenmekte ve bu farklılıkların tarih yazımına etkisi analiz edilmektedir. Eserde Safevi devlet modeli, siyasi reformlar ve Şii ideolojisinin Azerbaycan tarih yazımındaki rolü ayrıntılı olarak incelenmektedir. Safeviler tarihinin sadece geçmişten ibaret olmadığını, aynı zamanda modern bir ulusal kimlik inşası açısından stratejik öneme sahip olduğunu hemen ortaya koymaktadır. Yazar, Azerbaycan ve yabancı kaynaklara dayanarak tarih yazımının yelpazesini genişletiyor ve mevcut kalıpları aşan yeni yaklaşımlar sunuyor. Araştırmada hem arşiv belgeleri hem de akademik literatür esas alındı. Bu yaklaşım, Azerbaycan tarih biliminin Safevi dönemini nasıl algıladığını ve gelecekteki araştırma yönlerini belirlemeye yardımcı olmaktadır. Tez genel olarak Safevi tarih yazımının Azerbaycan'ın ulusal tarih hafızasındaki yerini ve değişen akademik yaklaşımlar ışığında geçirdiği dönüşümü ele almaktadır.This thesis examines the stages of formation and development of Safavi historiography in Azerbaijan. The study analyzes how scientific approaches to the Safavi state changed in different periods and reveals how this process was built in an ideological, political and scientific framework. The work presents a comparative analysis of the approaches to Safavid historiography from the period of the Azerbaijan Democratic Republic to the Soviet period and the stages after independence. It is shown that during the Soviet period, this topic was approached selectively and limitedly under the influence of Marxist ideological views. During the period of independence, the importance of the Safavid for Azerbaijani history was reassessed in the field of national historiography. The study also examines different approaches to the ethnic and religious identity of the Safavid and analyzes the impact of these differences on the writing of history. The work studies in detail the statehood model of the Safavid, political reforms and the role of Shiite ideology in Azerbaijani historiography. The thesis shows that the history of the Safavid is not only about the past, but also has strategic importance in terms of the construction of modern national identity. The author expands the spectrum of historiography based on local and foreign sources and presents new approaches that go beyond existing stereotypes. The research is based on both archival materials and academic literature. This approach helps to determine how Azerbaijani historical science perceives the Safavi era and future research directions. In general, the thesis covers the place of Safavi historiography in the national historical memory of Azerbaijan and its transformation against the background of changing scientific approaches
EĞİTİMDE ÜSTBİLİŞSEL DÜZENLEME ENVANTERİ: ÖĞRENCİ VERSİYONU’NUN (EÜDE: ÖĞRENCİ-KENDİ FORMU) GELİŞTİRİLME ÇALIŞMASI
Sayısız araştırma, öğrencilerin bir bilgiyi öğrenmeleri kadar bu bilgileri hakkında bilgilere sahip olmalarının ve bunları düzenlemelerinin, diğer bir ifadeyle üstbilişsel performanslarının, akademik başarılarında oldukça kritik bir öneme sahip olduğunu işaret etmektedir. 120K850 No.lu TÜBİTAK projesi kapsamında desteklenen bu çalışmada, standart bir öğretim bağlamındaki iki temel grubun (öğretmen ve öğrencilerin) üstbilişsel “hedef”, “izleme” ve “kontrol” süreçlerini kavramsallaştıran ve “eğitimde çok düzeyli üstbilişsel düzenleme modeli” olarak alan yazına sunulan modele uygun olarak “Eğitimde Üstbilişsel Düzenleme Envanteri (EÜDE): Öğrenci Versiyonu”nun geliştirilmesi amaçlanmıştır. Bu amaçla, mevcut ilişkili ölçeklerden yararlanılmış ve buna ek olarak araştırmacılar tarafından eklenmiş ölçek maddeleri, 24 ilden toplam 10190 5, 6 ve 7. sınıf öğrencisine tamamı çevrimiçi olarak üç aşamada uygulanmıştır. Oluşturulan 33 maddelik ölçek, ilk aşamada 6178 öğrenciye uygulanmıştır. Açımlayıcı faktör analizi (AFA) sonrasında 20 madde içeren ölçek, ikinci aşamada 2845 öğrenciye ve son aşamada AFA, madde ve test parametrelerine göre belirlenerek elde edilen 12 maddelik ölçek 1527 öğrenciye uygulanmıştır. Son aşamada elde edilen 12 maddelik nihai ölçek için doğrulayıcı faktör analizi (DFA) yapılmıştır. AFA ve DFA sonuçlarına göre, öğrencilerin üstbilişsel düzenleme yapısındaki üstbilişsel “hedef”, “izleme” ve “kontrol” alt boyutlarına ait maddeler belirlenebilmiştir. Güvenirlik analizi sonuçları da geliştirilen ölçeğin iç tutarlı olduğunu göstermiştir
2050’ye doğru dünya ekonomisi: Kapsayıcı gıda güvenliği modelleri üzerine bir değerlendirme
Yeterli beslenme ve gıda güvenliği tüm insanların en temel yaşam hakkı olarak kabul edilmektedir. Dünya Gıda Zirvesi (1996)'da gıda güvenliğinin tanımı, tüm insanların her zaman aktif ve sağlıklı bir yaşam için beslenme ihtiyaçlarını ve gıda tercihlerini karşılayan yeterli, güvenli ve besleyici gıdaya fiziksel ve ekonomik imkanlarla erişebilmesi olarak yapılmıştır. Sanayi devrimi ile üretim ve tüketim faaliyetleri hızlı bir şekilde artmış, ancak bu artış zamanla iklim krizi, ormansızlaşma, okyanus asitlenmesi, arazi ve toprak kayıpları, kimyasal kirlilik gibi gezegensel sınırların aşılmasına neden olarak, ekosistem üzerinde büyük kayıplara yol açmıştır. Dolayısıyla küresel çapta yaşanılan eşitsizlikler ve çevresel yıkımların yarattığı baskı gıda güvenliğini özellikle dezavantajlı bölgelerde yaşayanlar için sürdürülemez bir boyuta taşımıştır. Bu tez çalışmasında iklim krizinin gıda güvenliği üzerindeki riskleri analiz edilmiştir. Bu bağlamda Dünya Bankası, Birleşmiş Milletler Tarım ve Gıda Örgütü(FAO) ve Küresel Atmosfer Araştırmaları için Emisyon Veritabanı (EDGAR) tarafından elde edilen verilerle panel veri analizi yapılmıştır. Analizde 2000-2020 yılları aralığında 21 yılı kapsayan 154 ülkeye ait panel veri seti kullanılmıştır. Tüm ülkeler ve gelir durumuna göre (Dünya Bankası tarafından) sınıflandırılan ülke gruplarından oluşan altı ayrı panel veri seti Genelleştirilmiş Momentler Metodu (GMM) yöntemi ile analiz edilmiştir. Analiz sonucu elde edilen bulgular iklim değişikliğinin gıda güvenliği üzerinde riskleri arttırdığı ve ülke gelir grupları için gelir azaldıkça gıda güvensizliğinin çok daha ciddi boyutlarda yükseldiğini göstermektedir.Adequate nutrition and food security are considered the most fundamental right of all people to live. The definition of food security at the World Food Summit (1996) was made as the physical and economic access of all people to sufficient, safe and nutritious food that meets their nutritional needs and food preferences for an active and healthy life at all times. Production and consumption activities increased rapidly with the industrial revolution, but this increase caused great losses in the ecosystem by exceeding the planetary boundaries such as climate crisis, deforestation, ocean acidification, land and soil losses, and chemical pollution. Therefore, the pressure created by global inequalities and environmental destructions has brought food security to an unsustainable level, especially for those living in disadvantaged regions. In this thesis, the risks of the climate crisis on food security were analyzed. In this context, panel data analysis was conducted with data obtained from the World Bank, the Food and Agriculture Organization of the United Nations (FAO) and the Global Emissions Database for Atmospheric Research (EDGAR). The analysis used a panel dataset for 154 countries covering 21 years between 2000-2020. Six separate panel data sets consisting of all countries and country groups classified by income status (by the World Bank) were analyzed using the Generalized Method of Moments (GMM). The findings of the analysis show that climate change increases the risks to food security and that food insecurity increases more seriously as income decreases for country income groups
Strategies for Sustainability and Cost Optimization in Corneal Transplantation: From Surgeons' Perspective
The main purpose of this review is to provide an overview of surgical strategies that can be implemented in keratoplasty to maximize resource utilization and enhance sustainability. To achieve this, we conducted a thorough search of PubMed to identify articles on sustainability and cost-effectiveness in surgical settings, as well as studies comparing the cost-effectiveness of different keratoplasty techniques. Our review shows that both penetrating keratoplasty and lamellar techniques are cost-effective. However, lamellar techniques offer greater long-term sustainability and cost efficiency in addition to improving patient vision. For corneal transplantation surgeries, strategies such as reducing operating room time, properly educating the surgical team, reusing instruments like trephines and punches, using surgical materials economically, and selecting the appropriate surgical technique are recommended to enhance sustainability and reduce costs. The strategies outlined could contribute to more sustainable practices in keratoplasty procedures. In conclusion, although ensuring the economical use of surgical materials is beneficial for improving sustainability and reducing costs during surgery, utmost care should be taken to preserve safety and effectiveness while taking measures to reduce costs, and a balance should be achieved between sustainability and patient safety
Kentsel mavi-yeşil altyapıda ekosistem servislerinin hesaplanması: Karşıyaka, İzmir
Objective: This study aimed to calculate the regulating ecosystem services such as carbon storage, air pollution, runoff retention, and urban heat island reduction provided by the blue-green infrastructure of Karşıyaka, Izmir (Türkiye). Material and Method: Regulating ecosystem services provided by urban blue-green infrastructure were calculated with remote sensing techniques and geographic information system. Data preparation and analyzes were performed in ArcGIS 10.8 software. Results: The findings showed that the carbon storage potential of blue-green infrastructure was 2.45 kg C m2 while the particulate matter removal potential was 4.73 g/m2. The total rate of runoff in the BGI has been calculated to be 0.008 m3/m2. The urban heat island intensity index showed that the hot islands were located in the middle and north, and the cool islands were located in the south of the city. Conclusion: It has been determined that the blue-green infrastructure in Karşıyaka has predominantly low values in terms of the regulating ecosystem services evaluated. It has been concluded that the blue-green infrastructure needs to be improved to boost ecosystem services and urban resilience. © 2025 Elsevier B.V., All rights reserved
Innovative Integration of Duckweed Proteins: Enhancing the Nutritional and Textural Properties of Snack Bars, Roll Breads, and Muffins Enhancing Baked Goods With Duckweed Protein
ABSTRACT: Duckweed, a fast-growing aquatic plant rich in high-quality protein, has emerged as a sustainable and alternative vegan protein source for food applications. This study aimed to evaluate the effects of duckweed protein concentrate (DPC) incorporation on the physical, chemical, and textural properties of snack bars, roll breads, and muffins. Based on preliminary optimization studies, DPC was added at levels of 12.5% in snack bars, 5% in roll breads, and 10% in muffins. Its inclusion significantly decreased the lightness (L*) values of all products, with the most pronounced reduction observed in roll bread crumb (54.5%) and snack bars (34.8%). Protein contents increased by 41%, 28%, and 12% in snack bars, roll breads, and muffins, respectively, improving their nutritional profiles. Chlorophyll content rose substantially—up to 18.8-fold in roll breads—along with notable increases in total phenolic and flavonoid contents, enhancing antioxidant capacity. A significant increase in hardness was observed in all products, especially in snack bars. These results suggest that DPC can be effectively used as a functional ingredient in bakery and snack products to boost their nutritional and functional value. Practical Application: Duckweed protein concentrate can be used to enhance the nutritional profile of bakery and snack products by increasing protein, chlorophyll, and antioxidant contents. Its incorporation offers a sustainable plant-based protein alternative, making it a promising ingredient for the development of functional food products. © 2025 Elsevier B.V., All rights reserved
New records of rarely reported species in the Mediterranean Sea (March 2025)
This collective study presents information on fifteen (15) species recorded across five countries (Greece, Italy, Slovenia, Spain, and Turkiye) and four (4) ecoregions, ranging from the Alboran Sea to the Aegean Sea. The recorded species belong to three phyla: Mollusca (3 species), Arthropoda (3 species), and Chordata (9 species). Among mollusks, Martadoris mediterranea is reported for the first time in the Aegean Sea at two different locations, while Opisthoteuthis calypso and Octopoteuthis sicula are newly recorded in the western Mediterranean. Regarding arthropods, Ocypode cursor is reported from the Tyrrhenian coast of Sicily, Pachygrapsus transversus is recorded for the first time in the Tyrrhenian Sea, and Cancer pagurus in the Sea of Marmara. Among chordates, Lobotes surinamensis and Zu cristatus are newly recorded in the Alboran Sea, while Callionymus fasciatus and Symphodus doderleini are reported from Slovenian coastal waters. Tripterygion delaisi and Gymnothorax unicolor are documented for the first time in the waters of the Sea of Marmara and Crete, respectively.
