Development of Professional Competence and Competency Scale in Midwifery: A Methodological Study from Turkey
Background:The main scope of competence in midwifery is the skills and practices relevant to the profession. Competencies are used to set unique standards within disciplines and specializations, including educators, learners, and practitioners.Aim:This study was aimed at developing the Professional Competence and Competency Scale in Midwifery.Methods:This was a methodological and validation study. The scale consists of 26 items. The created items were administered to 362 midwives. According to the results of the exploratory and confirmatory factor analysis of the factor pattern for all the items, ideally, the scale has a two-dimensional distribution. Responses given to the items are rated on a five-point Likert-type scale.Results:According to the item-total test correlation, all the items were related to each other. The Cronbach's Alpha value was 0.953 for the overall Professional Competence and Competency Scale in Midwifery, 0.917 and 0.932 for its Professional Responsibilities and Communication Skills and Professional Knowledge, Skills and Practices sub-dimensions, respectively. In addition, the scale has construct validity.Conclusion:According to the findings of the validity and reliability study of The Professional Competence and Competency Scale in Midwifery, it is a valid, reliable tool, and it is discriminative in terms of measuring the desired quality
OSB olan bireylerin ebeveynlerindeki öz şefkat ve koşulsuz kendini kabul düzeyinin ebeveyn tükenmişliğine etkisi
Bu çalışmada OSB olan bireylerin ebeveynlerinin öz- şefkat ve koşulsuz kendini kabul düzeyinin ebeveyn tükenmişliğine etkisi incelenmiştir. Araştırma grubuna kolay ulaşılabilir örnekleme yöntemi ile ulaşılmıştır. Araştırmanın örneklemini OSB olan bireye sahip 204 ebeveyn oluşturmaktadır. Araştırma verileri Demografik Bilgi Formu, Anne Baba Tükenmişlik Ölçeği, Öz Anlayış Ölçeği ve Koşulsuz Kendini Kabul Ölçeği ile elde edilmiştir. Araştırma değişkenleri arasındaki ilişkiyi incelemek için Pearson Momentler Çarpımı Korelasyon Analizi kullanılmıştır. Araştırma sonucunda ebeveyn tükenmişliği ile koşulsuz kendini kabul arasında negatif yönde anlamlı ilişki saptanmıştır. Ebeveyn tükenmişliği ve öz -şefkat arasında negatif yönlü anlamlı ilişki saptanmıştır. Araştırma değişkenlerinden olan öz- şefkat ve koşulsuz kendini kabul arasında pozitif yönlü anlamlı ilişki saptanmıştır. Araştırmanın bulguları literatür doğrultusunda tartışılmış ve diğer çalışmalar için öneriler geliştirilmiştir.This study investigated the effect of self-compassion and unconditional self-acceptance levels of parents of individuals with autism spectrum disorder (ASD) on parental burnout. The research group was reached through a convenience sampling method, and the sample consisted of 204 parents of children with ASD. Research data were collected using the Demographic Information Form, the Parental Burnout Scale, the Self-Compassion Scale, and the Unconditional Self-Acceptance Scale. To examine the relationships among the research variables, Pearson Product-Moment Correlation Analysis was employed. The results revealed a significant negative relationship between parental burnout and unconditional self-acceptance. A significant negative relationship was also found between parental burnout and self-compassion. Furthermore, a significant positive relationship was determined between self-compassion and unconditional self-acceptance. The findings were discussed in line with the related literature, and suggestions for future research were provided
Advancing eco-friendly adsorption: a sustainable approach for rapid and efficient methylene blue removal
Synthetic dyes, particularly methylene blue (MB), pose significant environmental challenges due to their persistent presence in water bodies. This study introduces a sustainable polymer adsorbent, poly(LIM-co-DVB-co-AcM), synthesized using d-limonene and 4-acryloylmorpholine. Comprehensive characterization using FTIR, SEM, TGA, and BET analysis revealed its thermal stability, surface morphology, and adsorption properties. Adsorption experiments demonstrated optimal MB removal at pH 7.0 with a 20 mg dosage and a 60-min contact time. The Langmuir isotherm model showed monolayer adsorption with a capacity of 54.2 mg g-1, while kinetic studies confirmed the suitability of the pseudo-second-order model. Thermodynamic analysis indicated a spontaneous and exothermic process. Earthquake anxiety and sleep quality among adolescent survivors 9-12 months after the great earthquakes in Türkiye: a cross-sectional study
This study investigated the relationship between earthquake-related anxiety and sleep quality among adolescents affected by a major earthquake in T & uuml;rkiye. An analytical cross-sectional design was employed, involving 402 adolescents aged 12-18 years from Kahramanmara & scedil;, Kilis, Diyarbak & imath;r, Adana, Osmaniye, Gaziantep, & Scedil;anl & imath;urfa, Ad & imath;yaman, Malatya, and Hatay provinces who continued to live in Hatay. Participants were selected through purposive sampling. Data collection tools included the Adolescent Descriptive Characteristics Form, Earthquake Anxiety Scale, and Pittsburgh Sleep Quality Index. Data were evaluated using descriptive statistical analyses, chi-square test, and logistic regression analysis. The chi-square test was used to evaluate the categorical variables affecting earthquake anxiety and sleep quality. The relationship between the numerical scores of the two scales was calculated by Spearman's rho correlation coefficient. The mean age of the adolescents was 14.01 +/- 1.70 years, with 52.5% female participants and 98% living in containers. 65.7% reported sleeping less than 7 h per night. The mean Earthquake Anxiety Scale score was 119.40 +/- 28.13, and the mean Pittsburgh Sleep Quality Index score was 9.28 +/- 4.26. High earthquake anxiety was observed in 89.4% of adolescents with poor sleep quality (chi(2) = 38.884, p < 0.001). İngiliz parlamento kayıtları ışığında Akdeniz'de kriz, diplomasi, siyaset: 1861-1911
Akdeniz 1861-1911 yılları arasında pek çok siyasi dönüşüm, kriz ve çatışmaya sahne olmuştur. Bu dönem, Süveyş Kanalı'nın açılması (1869), Osmanlı Devleti'nin bölgedeki gücünün zayıflaması, İngiltere'nin Mısır'a müdahalesi (1882) ve İtalya'nın Trablusgarp'a saldırısı (1911) gibi önemli gelişmelerle şekillenmiştir. Çalışmanın temel amacı, İngiltere'nin Akdeniz'deki varlığının temel dinamiklerini anlamak ve bu dönemdeki siyasi söylemi ve karar alma süreçlerini İngiliz Parlamentosu tutanakları (Hansard) üzerinden analiz etmek ve bunların Akdeniz'deki diğer siyasal yapılar ve halklar üzerindeki etkilerini incelemektir. Araştırmada, nitel tarihsel analiz yöntemi kullanılmış ve parlamento tutanakları, dönemin diplomatik yazışmaları ve akademik literatür temel kaynaklar olarak incelenmiştir. Parlamento tartışmaları, İngiltere'nin Akdeniz stratejisinin şekillenmesinde hangi faktörlerin belirleyici olduğunu ve siyasetçilerin nasıl bir söylem geliştirdiğini anlamada önemli bir kaynak sağlamıştır. Çalışmanın bulguları, İngiltere'nin Akdeniz'deki stratejik varlığının deniz üstünlüğü, ekonomik çıkarların korunması ve Avrupa'daki güç dengelerinin yönetilmesi gibi çok boyutlu bir politikaya dayandığını göstermektedir. Akdeniz'deki siyasi-askeri pozisyonunun güçlendirilmesi, Süveyş Kanalı'nın güvenliğinin sağlanması ve Osmanlı Devleti'nin belirli ölçüde desteklenmesi, İngiltere'nin bölgedeki temel politikaları olmuştur. Parlamento kayıtları, İngiltere'nin bu eski denizdeki çıkarlarını korumak için izlediği politikaların iç politikadaki tartışmalarla nasıl şekillendiğini ve farklı hükümetler arasındaki politika farklılıklarını da gözler önüne sermektedir. Sonuç olarak, bu çalışma, İngiltere'nin Akdeniz politikasının sadece bölgesel bir mesele değil, küresel imparatorluk yönetiminin bir parçası olduğunu ortaya koymaktadır. Parlamento tutanaklarının incelenmesi, bu dönemde İngiliz dış politika kararlarının arkasındaki dinamikleri anlamada önemli bir içgörü sağlamıştır. Çalışma, birinci el tarih kaynakların uluslararası diplomasi araştırmalarındaki yerini vurgulamakta ve İngiltere'nin Akdeniz'deki rolünün tarihsel bir perspektiften analizine katkıda bulunmaktadır.The Mediterranean was the scene of many political transformations, crises and conflicts between 1861 and 1911. This period was shaped by important developments such as the opening of the Suez Canal (1869), the decline of the Ottoman Empire's power in the region, Britain's intervention in Egypt (1882) and Italy's attack on Tripolitania (1911). The main purpose of the study is to understand the basic dynamics of Britain's presence in the Mediterranean and to analyze the political discourse and decision-making processes during this period through the debates of the British Parliament (Hansard) and to reveal their effects on other political structures and peoples in the Mediterranean. The study employs a qualitative historical analysis method and examines parliamentary proceedings, diplomatic correspondence of the period and academic literature as the main sources. Parliamentary debates provided an important source for understanding which factors were decisive in shaping Britain's Mediterranean strategy and how politicians developed a discourse. The findings of the study show that Britain's strategic presence in the Mediterranean is based on a multidimensional policy of maritime supremacy, protection of economic interests and managing the balance of power in Europe. Strengthening its politico-military position in the Mediterranean, securing the Suez Canal and supporting the Ottoman Empire to a certain extent were Britain's main policies in the region. Parliamentary records also reveal how Britain's policies to protect its interests in this ancient sea were shaped by domestic political debates and policy differences between different governments. As a result, this study reveals that Britain's Mediterranean policy was not just a regional issue, but part of global imperial rule. The analysis of the Hansard records provides important insights into the dynamics behind British foreign policy decisions during this period. The study highlights the importance of historical primary sources in the study of international diplomacy and contributes to the analysis of Britain's role in the Mediterranean from a historical perspective
OPTIModel scoring for metastatic bone tumors: A data-driven tool for surgical planning and prognosis
Objectives: This study aims to evaluate the predictive accuracy and clinical applicability of the OPTIModel Scoring System in Turkish patients with long bone diaphyseal metastases and to compare its survival predictions and surgical recommendations with real-world outcomes. Patients and methods: Between April 2013 and June 2021, a total of 108 patients (52 males, 56 females; mean age: 64.5 +/- 10.6 years; range, 45 to 83 years) with histologically confirmed metastatic bone disease affecting the diaphysis of long bones and who underwent surgical treatment were retrospectively analyzed. The OPTIModel Scoring System was applied to estimate survival and guide surgical planning. The prognostic accuracy of the model was statistically analyzed. The patients were stratified into low-, intermediate-, and high-risk groups based on their predicted survival. Results: The most common primary tumors were breast (24.1%), lung (23.1%), and multiple myeloma (13.0%). The femur (46.3%) was the most frequently affected site. The OPTIModel-predicted median survival was 10 months (95% confidence interval [CI]: 7.9-12.1), comparable to the real-world median survival of 13 months (95% CI: 10.8-15.2). Agreement between OPTIModel's surgical recommendations and real-world procedures was substantial (kappa [kappa]=0.74, 95% CI: 0.63-0.85, p<0.001). The Kaplan-Meier analysis revealed no significant difference between predicted and real-world survival distributions (p=0.126). The ROC analysis yielded an area under the curve [AUC] of 0.61 (95% CI: 0.52-0.70), indicating moderate prognostic accuracy. Conclusion: The OPTIModel Scoring System demonstrated substantial agreement with real-world surgical decisions and moderate accuracy in survival prediction for Turkish patients with metastatic long bone diaphyseal tumors. While the model provides valuable insights for clinical decision-making, further refinements incorporating emerging oncological advancements and biomarker-based risk stratification are needed. Future multi-ethnic validation studies are essential to enhance its applicability in diverse populations
Sosyal medyada yeşil reklam uygulamalarının tüketici farkındalığı üzerindeki etkisi
Yaşanan çevresel sorunlar ve tüketici beklentilerindeki değişim, sürdürülebilirlik kavramının önem kazanmasına ve bu kavramın işletmelerin iletişim stratejilerinde daha fazla yer bulmasına yol açmıştır. Bu bağlamda yürütülen araştırmada, sosyal medya platformlarında gerçekleştirilen yeşil reklam uygulamalarının, başta tüketici farkındalığı olmak üzere, satın alma davranışı ve müşteri tatmini üzerindeki etkileri incelenmiştir. Araştırmaya, sosyal medyayı aktif olarak kullanan ve 18-65 yaş aralığında yer alan 151 katılımcı dahil edilmiştir. Katılımcılar Çavuşoğlu (2021) tarafından uyarlanıp geliştirilen anket formunu yanıtlamışlardır. Elde edilen veriler, Smart PLS 4.1.1.1 programı kullanılarak analiz edilmiştir. Araştırma bulgularına göre, sosyal medyada yer alan yeşil reklam içerikleri markanın çevre dostu imajını ve marka farkındalığını artırdığı görülmüştür. Ancak bu içeriklerin doğrudan yeşil satın alma davranışını ve müşteri tatminini artırmadığı sonucuna ulaşılmıştır. Buna karşın, yeşil marka farkındalığı üzerinden satın alma davranışının olumlu yönde etkilendiği, sosyal medyadaki yeşil reklamla motive edilen satın alma davranışının tüketici tatmini artırdığı ve genel tüketici farkındalığının müşteri tatmini üzerinde pozitif etkiler yarattığı görülmüştür. Demografik bulgular incelendiğinde, kadın katılımcıların sosyal medyada uygulanan yeşil reklamlara daha olumlu yaklaştıkları dikkat çekmiştir.The growing environmental issues and shifting consumer expectations have led to an increased emphasis on the concept of sustainability and its integration into corporate communication strategies. Within this context, the present study investigates the effects of green advertising practices on social media platforms, particularly focusing on consumer awareness, purchasing behavior, and customer satisfaction. A total of 151 participants, aged between 18 and 65 and active users of social media, were included in the study. Participants responded to a questionnaire adapted and developed by Çavu`o lu (2021). The data collected were analyzed using the Smart PLS 4.1.1.1 software. According to the findings, green advertising content on social media enhances both the brand9s environmentally friendly image and brand awareness. However, it was found that such content does not directly influence green purchasing behavior or customer satisfaction. Nevertheless, green brand awareness was shown to positively impact purchasing behavior, and purchase behavior stimulated by green advertising on social media contributed to increased customer satisfaction. Furthermore, general consumer awareness was found to have a positive effect on customer satisfaction. In terms of demographic findings, female participants were observed to exhibit a more favorable attitude toward green advertisements on social media
Antifungal prophylaxis in high-risk patients with hematologic malignancy: a comparison of practice and guidelines in Türkiye
Background/aim: Primary antifungal prophylaxis (AFP) is as considered the gold standard therapy for patients with hematologic malignancies based on the available guidelines. The aim behind this study was to investigate the level of guideline knowledge and the translation of guideline recommendations into real-life settings among physicians in Türkiye. Materials and methods: In this prospective, observational study, the physicians’ knowledge of ?FP guideline recommendations for patients with acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) undergoing first remission-induction (Group I) or graft-versus-host-disease (GvHD) after allogeneic stem cell transplant (Group II) was compared with clinical practice via an 11-item multiple-choice questionnaire evaluating the patients in whom AFP was initiated, the timing, the AFP duration, and the drugs used. Results: The mean patient age was 48.3 years and 79% had AML/MDS. AFP was initiated in 75.3% of Group I patients on the first day of induction chemotherapy before baseline and in 26.2% of Group II patients at baseline. For 98.8% of the Group I patients, the physician’s plan was to continue AFP until recovery from neutropenia and complete remission (CR), whereas it was to be used during immunosuppression in Group II. The median (IQR) duration of AFP was 0.8 (0.5–1.9) and 5.5 (2.4–8.1) months for Groups I and II, respectively. Breakthrough invasive fungal infections (bIFIs) were observed in 35.8% and 14.3% of Groups I and II, respectively. The mean survey scores were 8.5 ± 2.3 for the AML/MDS patient group and 9.9 ± 3.2 for the GvHD patient group. Most of the surveyed sites adhered to the guidelines from the European Conference on Infections in Leukemia (80.6%), followed by the fever and neutropenia guidelines of the Infectious Diseases Society of America (51.6%). Posaconazole was the drug of choice in both groups (96.8% in Group I vs. 71% in Groups II). Conclusion: The data indicate an inconsistency between real-life clinical practice and the guideline recommendations for primary AFP and bIFI management in high-risk patients with hematologic malignancies in Türkiye. © 2025 Elsevier B.V., All rights reserved
Using yeast-based probiotic as dietary supplement during transition period to improve performance of dairy cows
A study was conducted to investigate the effect of live Saccharomyces cerevisiae yeast supplementation on dairy cow dry matter intake (DMI), milk production, milk composition and somatic cell count (SCC), rumen pH, and serum parameters (glucose, urea, alanine aminotransferase; ALT and aspartate aminotransferase; AST) during transition. Totally 24 multiparous dairy cows were randomly assigned to 3 groups (eight cows/group) fed the same diets supplemented without probiotics (Control group) or with probiotics at the rate of 15 or 30 g live yeast culture/cow daily for 5 weeks (transition period; from 2 weeks’ pre-partum to 3 weeks’ post-partum) in a completely randomised design. Dry matter intake (DMI) and milk yield were recorded daily and the milk composition, somatic cell count (SCC), rumen pH, and serum parameters were measured every week. A higher DMI and greater average daily milk yield were found in Saccharomyces cerevisiae treated groups as compared to the control groups (P0.05) effect of both yeast levels on milk yield and quality, which suggests the use of a lower level of 15 g/d in practice. © 2025 Elsevier B.V., All rights reserved
Mitapivat in adults with non-transfusion-dependent ?-thalassaemia or ?-thalassaemia (ENERGIZE): a phase 3, international, randomised, double-blind, placebo-controlled trial
Background Non-transfusion-dependent (NTD) thalassaemia is characterised by ineffective erythropoiesis and haemolytic anaemia, leading to long-term complications, poor quality of life, and early mortality. No oral disease-modifying therapies are approved for f3-thalassaemia and no agents are approved for alpha-thalassaemia. The objective of this study was to evaluate the efficacy and safety of mitapivat, an oral activator of pyruvate kinase, in adults with NTD alpha-thalassaemia or NTD f3-thalassaemia. Methods ENERGIZE is a phase 3, double-blind, randomised, placebo-controlled trial followed by an open-label extension conducted at 70 hospitals in 18 countries globally. Participants had to be aged 18 years or older with NTD alpha-thalassaemia or NTD f3-thalassaemia and haemoglobin concentrations of 10 g/dL or lower. Participants were randomly assigned 2:1 to mitapivat or placebo (100 mg orally twice a day for 24 weeks) via a central interactive response technology system using block randomisation, stratified by baseline haemoglobin concentration and thalassaemia genotype. Everyone was masked to the patients' treatment assignment until the study was unblinded for the analysis of the primary endpoint. The primary endpoint was haemoglobin response (>= 10 g/dL increase from baseline in mean haemoglobin concentration from week 12 through week 24), analysed in all patients who were randomly assigned. Safety was analysed in all patients who received at least one dose of study treatment. This study is registered with ClinicalTrials.gov, number NCT04770753, and is active but not recruiting. Findings Between Nov 8, 2021, and March 31, 2023, 235 patients were screened, of whom 194 were enrolled (123 [63%] were female and 71 [37%] were male). 130 patients were randomly assigned to mitapivat and 64 patients to placebo and formed the full analysis set. One patient in each group was randomly assigned but not given treatment and was therefore excluded from the safety analysis set (mitapivat 129 patients and placebo 63 patients). Seven patients in the mitapivat group and one patient in the placebo group discontinued treatment before the end of the 24-week double-blind period. 55 (42%) of 130 patients in the mitapivat group had a haemoglobin response versus one (2%) of 64 in the placebo group (least-squares mean difference 41% [95% CI 32-50], two-sided p<00001). Adverse events were reported in 107 (83%) of 129 patients who received mitapivat and 50 (79%) of 63 patients who received placebo. The most commonly reported adverse events with mitapivat were headache (29 [22%] of 129 patients in the mitapivat group vs six [10%] of 63 in the placebo group), initial insomnia (18 [14%] vs three [5%]), nausea (15 [12%] vs five [8%]), and upper respiratory tract infection (14 [11%] vs four [6%]). No deaths were reported. Interpretation Mitapivat could be a new oral treatment for adults with NTD alpha-thalassaemia or NTD f3-thalassaemia by increasing haemoglobin concentration and improving fatigue.
