    Evaluation of factors affecting the success of non invasive mechanical ventilation in acute cardiogenic pulmonary edema in the emergency department

    OBJECTIVES:The aim of this study was to evaluate the factors associated with non-invasive mechanical ventilation (NIMV) failure in acute cardiogenic pulmonary edema (ACPE) diagnosed in the emergency department. METHODS:This study was prospectively conducted at the Ege University Faculty of Medicine ED between February 19, 2021 and December 01, 2021. Patients who received NIMV with ACPE were included. Patients' clinical and laboratory parameters, treatments, NIMV mode, and settings were recorded. The primary endpoint was NIMV failure (intubation within 24 h). Secondary endpoints were early NIMV failure, early mortality (within 24 h), and in-hospital mortality. Early NIMV failure was defined as follows: if the patient had a respiratory rate of more than 25 per minute, oxygen saturation below 90%, PaCO2 >50 mmHg in blood gas, and pH 40.5, systolic blood pressure 5.2 mmol/L, base excess 3007 pg/mL (respectively area under the curve values; 0.723, 0.693, 0.739, 0.721, 0.690, 0.698, and 0.616). CONCLUSION:Signs of hypoperfusion such as low systolic blood pressure (5.2 mmol/L) are risk factors for NIMV failure.     This study investigated the adsorption efficiency of lanthanum (La) ions, a rare-earth element, using nanocrystalline cellulose (NCC)-reinforced carboxymethyl cellulose (CMC) biopolymers synthesized from textile waste. Microcrystalline cellulose (MCC) was derived via acid hydrolysis, followed by NCC synthesis through basic reaction. The characterization was performed using FT-IR, SEM, XPS and XRD analyses. Key adsorption parameters, including pH, initial concentration, agitation time, and temperature, were optimized. The adsorption process of La(III) ions onto the CMC/NCC nanocomposite follows the Langmuir isotherm model with a high adsorption capacity of 38.13 mg/g, indicating monolayer adsorption, while thermodynamic analysis reveals the process is endothermic, spontaneous at higher temperatures, and predominantly governed by physicochemical interactions. The CMC-NCC biopolymer presents an economical, effective, and eco-friendly alternative for lanthanum removal from aqueous solutions. © 2025 Elsevier B.V., All rights reserved

    Manisa ekolojisindeki bazı antepfıstığı çeşitlerinde döllenme biyolojisi üzerine çalışmalar

    Bu çalışma, Manisa/Demirci'de Siirt ve Uzun Antepfıstığı çeşitleri için en uygun tozlayıcı tiplerini belirlemeyi amaçlamıştır. Çalışmada, Atlı, Buttum, Uygur ve doğal Melengiç (Pistacia terebinthus L.) tipleri tozlayıcı olarak kullanılmıştır. Fenolojik gözlemlere göre, denemede kullanılan tüm tozlayıcı tipler ve dişi çeşitlerin çiçeklenme süresi 5-25 Nisan tarihleri arasında gözlenmiştir. Uzun çeşidi Uygur tipiyle, Siirt çeşidi ise sadece Buttum tipiyle çiçeklenme döneminde çakışmıştır. TTC testine göre çiçek tozlarının canlılık oranı Melengiç ve Buttum tiplerinde sırasıyla %90 ve %93, Atlı ve Uygur tiplerinde ise %81 ve %79 olarak tespit edilmiştir. IKI yöntemine göre, tüm tozlayıcı tiplerin çiçek tozu canlılık düzeyleri yüksek bulunmuştur. Çiçek tozu çimlenme oranlarının %45-%62 arasında değiştiği belirlenmiştir. Uzun çeşidinde Uygur tipiyle yapılan tozlama kombinasyonunda meyve döküm oranı düşük (%68) bulunmuş ve Uygur en iyi tozlayıcı olarak değerlendirilmiştir. Diğer yandan, serbest tozlama uygulamasında meyve döküm oranı en yüksek düzeyde (%85) tespit edilmiştir. Siirt çeşidinde ise meyve döküm oranları diğer çeşide göre daha yüksek olup, en düşük döküm oranı Buttum ile yapılan kombinasyonda gözlenmiştir (%78). Meyve tutumu açısından, Uzun çeşidinde tüm tozlayıcılar olumlu etki göstermiş, Uygur ve Melengiç en iyi sonuçları vermiştir. Siirt çeşidinde ise Buttum ve Atlı tozlayıcıları, diğer tozlayıcılara ve serbest tozlamaya göre daha yüksek meyve tutum oranları sağlamıştır.This study aimed to determine the most suitable pollinator types for the Siirt and Uzun pistachio varieties in the Demirci district of Manisa. Atlı, Buttum, Uygur, and the naturally growing Melengiç (Pistacia terebinthus L.) types were used as pollinators. According to phenological observations, the flowering periods of all pollinator types and female varieties occurred between April 5–25. The Uzun variety overlapped in flowering time with the Uygur type, while the Siirt variety overlapped only with the Buttum type. According to the TTC test, the pollen viability rates were 90% and 93% for Melengiç and Buttum types, respectively, while Atlı and Uygur types had viability rates of 81% and 79%. The IKI method showed that all pollinator types had high pollen viability levels. The pollen germination rates ranged between 45% and 62%. For the Uzun variety, the Uygur type had the lowest fruit drop rate (68%) and was evaluated as the best pollinator. On the other hand, the highest fruit drop rate (85%) was observed in the openpollination treatment. For the Siirt variety, fruit drop rates were higher compared to the Uzun variety, with the lowest drop rate observed in the Buttum combination (78%). In terms of fruit set, all pollinators significantly improved the fruit set rate for the Uzun variety, with Uygur and Melengiç producing the best results. For the Siirt variety, Buttum and Atlı pollinators achieved higher fruit set rates compared to the other pollinators and open-pollination treatment

    Fuzzy Sets in Strong Sheffer Stroke NMV-Algebra with Respect to a Triangular Norm

    In this paper, we explore the application of fuzzy set theory in the context of triangular norms, with a focus on strong Sheffer stroke NMV-algebras. We introduce the concepts of (Formula presented.) -fuzzy subalgebras and (Formula presented.) -fuzzy filters, analyze their properties, and provide several illustrative examples. Our study demonstrates that (Formula presented.) -fuzzy subalgebras and filters generalize classical subalgebras and filters, with level subsets preserving algebraic structures under t-norms. Notably, (Formula presented.) -fuzzy sets exhibit strong closure properties, and homomorphisms between SSNMV-algebras extend naturally to fuzzy settings. Furthermore, we examine the relationships between (Formula presented.) -fuzzy subalgebras (or filters) and their classical counterparts, as well as their corresponding level subsets and homomorphisms. These results contribute to refined uncertainty modeling in logical systems, with potential applications in fuzzy control and AI. © 2025 Elsevier B.V., All rights reserved

    Frequency, Severity and Impact of Pegylated Interferon Alpha–Associated Flares in Hepatitis D Infection

    We analysed the frequency, severity and impact of hepatitis flares in a large Phase 2 study investigating pegylated interferon-alfa-2a (PEG-IFNa) for the treatment of hepatitis D. In the HIDIT-II study, 120 patients were treated for 96 weeks with PEG-IFNa (180 ?g weekly) in combination with tenofovir disoproxil fumarate (TDF, 300 mg once daily) or placebo. Hepatitis flares were defined as ALT increases above 10 times the upper limit of normal or increases of more than 2.5-fold above baseline or nadir values. ALT flares occurred in 28 patients (23%) during treatment ( 1 log10 cop/ml) during subsequent study visits. Mean ALT levels early during treatment were higher in patients with HBsAg loss at follow-up Week 24. More than a third of hepatitis D patients undergoing PEG-IFNa therapy may experience ALT flares during or after treatment. ALT flares in this study posed no obvious safety risk to patients and should not lead to premature withdrawal from treatment.     Enhanced removal of methylene blue from water using mercaptosuccinic acid-functionalized PS-DVB-g-PGMA polymer

    In this study, a polystyrene-divinylbenzene-vinylbenzyl chloride (PS-DVB-VBC) polymer sorbent was synthesized and functionalized with glycidyl methacrylate (GMA) via surface-initiated polymerization, achieving an epoxy group content of 64%. The epoxy-functionalized polymer was further modified by reacting with mercaptosuccinic acid to enhance its adsorption performance for methylene blue (MB) removal from aqueous solutions. Characterization results confirmed the successful synthesis and functionalization of the sorbent, revealing a mesoporous structure with an average pore diameter of 4.8 nm and a BET surface area of 0.9144 m2 g- 1. Sorption experiments were conducted under varying conditions, including pH (2-12), initial MB concentrations (1-20 mg L- 1), sorbent dosages (10-50 mg), contact times (1-240 min), and temperatures (298-338 K). Kinetic analysis showed that the pseudo-second-order model (R2 = 0.9997) best described the adsorption process, while equilibrium data aligned well with the Langmuir isotherm (R2 = 0.9996), indicating monolayer adsorption with a maximum capacity of 6.27 mg g- 1. Thermodynamic studies confirmed a spontaneous and exothermic (Delta H degrees =-2.64 kJ mol-1) adsorption process. The polymer sorbent demonstrated excellent adsorption performance in ultrapure water (UPW) and tap water (TW), achieving over 95 % removal efficiency, while maintaining significant MB removal in industrial wastewaters (IWWs), despite the presence of competing contaminants.     Diagnosis of knee joint invasion in patients with osteosarcoma: the value of direct and indirect MRI findings

    Background/aim: Osteosarcoma is the most common primary malignant bone tumor in adolescents, and the evaluation of joint invasion with MRI is important for treatment planning. This study aimed to investigate the diagnostic value of MRI findings (direct and indirect) for joint invasion in patients diagnosed with osteosarcoma of the knee region. Materials and methods: The MRI evaluations of 50 knee osteosarcoma patients who underwent surgical resection between 2006 and 2018 were reviewed retrospectively by two radiologists and an orthopedic oncologist. The presence of intrasynovial tumor tissue, intra-articular destruction of cartilage or bone, and invasion of the capsular and cruciate ligament insertions were evaluated as direct findings in the diagnosis of joint invasion on MRI. Indirect findings included tumor size, adjacent epiphyseal bone signal changes- bone marrow infiltration and edema, synovial contrast enhancement, and joint effusion. These findings were scored separately on a 5-point Likert scale and statistically compared with histopathologic results Results: The mean age of the patients was 22 years and the gender distribution was 21 females and 29 males. The best predictors for joint invasion were direct visualization of capsular insertion invasion (p < 0.05) and destruction of intraarticular bone (p < 0.05). MRI findings with statistically significant sensitivity and specificity: intrasynovial tumor tissue specificity 76%, sensitivity 58%; intra-articular cartilage destruction specificity 84%, sensitivity 56%; intra-articular bone destruction sensitivity 84%, specificity 48%; capsular insertion invasion sensitivity 92%, specificity 48%. Synovial effusion and contrast enhancement were the most sensitive indirect signs but lacked specificity. Conclusion: Joint invasion by osteosarcoma can reliably be assessed on preoperative MR images with high sensitivity and specificity. Particularly direct visualization of intrasynovial tumor tissue, capsular insertion invasion, and destruction of intraarticular bone and cartilage, a combination of highly specific direct signs was valuable, while indirect signs were less predictive and specific

    Safety and Effectiveness of Image-Guided Therapies for Giant Hepatic Hemangiomas: A Systematic Review and Meta-Analysis of 2,617 Patients

    Purpose: To evaluate the safety and effectiveness of image-guided therapies—including transarterial chemoembolization (TACE), radiofrequency (RF) ablation, microwave ablation, and percutaneous sclerotherapy—for the treatment of giant hepatic hemangiomas (GHHs). Materials and Methods: A comprehensive search was performed across PubMed, Scopus, and Web of Science, including studies with ?5 patients that reported image-guided treatment of GHHs. Primary outcomes included technical success, adverse events (AEs; classified according to the Society of Interventional Radiology [SIR] system), clinical success (symptom relief without additional intervention), and radiologic success (?50% size reduction and/or lack of enhancement on follow-up imaging). A subgroup analysis was performed for GHHs of ?10 cm. Outcomes were analyzed using a random-effect meta-analysis. Results: Twenty-eight studies (2,617 patients; 32.5% men; mean age, 46.1 years [SD ± 3.2]) with 2,996 GHHs, ranging from 4 to 30 cm, were included. Of these, 22 were noncomparative, and 6 compared either 2 image-guided therapies or surgery, reporting outcomes for TACE (n = 13), RF ablation (n = 7), microwave ablation (n = 6), and percutaneous sclerotherapy (n = 4). The pooled technical success rate was 99.9%. Grade 2–4 AEs occurred in 1.64%, with TACE having the lowest rate (0.2%) and RF ablation the highest (2.1%). Clinical success at final follow-up was 99.9%, while radiological success was 85.7%. Grade 2–4 AEs were significantly higher in the subanalysis of GHHs of ?10 cm (10.6%; P < .001), despite similar technical success and radiological and clinical outcomes. Conclusions: Image-guided therapies are safe and effective for GHHs, achieving high technical, clinical, and radiological success with minimal Grade 2–4 AEs. However, for GHHs ?10 cm, AE rates were higher. © 2025 Elsevier B.V., All rights reserved

    Synthesis of bio-based solvents derived from sorbitol and stearic acid for the separation of Th, U, and REEs: a solvent extraction study

    Solvent extraction is a widely used method for separating metals such as Th, U, and REEs. However, using excessive amounts of petroleum-derived solvents significantly increases the carbon footprint of the solvent extraction. In this study, bio-based solvents were synthesized in ester form using sorbitol and stearic acid. The extraction and separation performance of the synthesized bio-based solvents were evaluated for Th, U, and rare earth elements (REEs). Among the synthesized bio-based solvents, SSA2 demonstrated superior separation efficiency for Th compared to conventional extractants such as Cyanex 272 and D2EHPA.
