    Vital pulp therapy with two different calcium-silicate cements: two cases report

    It is a worthy aim to preserve the pulp vitality in teeth with immature root in order to continue the root's maturation. Traumatic injuries and caries are two main reasons for pulp exposure in immature teeth. This article describes two vital pulp therapies on two patients, a nine-year-old boy with traumatized central tooth and a 10-year-old boy with caries exposure in a premolar tooth. Both teeth had open apices in radiography. After removing inflamed pulp, the pulp cap was done using calcium silicate cements (CEM cement, MTA angelus) and then, the teeth were restored with filling material. In periodical follow-ups, both treated teeth remained functional, with complete root development and apex formation. Regarding to the potentiality of pulp healing in immature teeth, vital pulp therapy (VPT) can be an acceptable treatment. However, further clinical studies with longer follow-up periods are recommended

    Oxymetholone-Induced Acute Renal Failure: A Case Report

    Background: The prevalence of using anabolic steroids such as oxymetholone is increasing. This highlights the need for closely monitoring side effects of this drug. Acute renal failure (ARF) has been reported as a complication of rhabdomyolysis in anabolic steroids users. Case presentation: We present one 33-year-old man complaining of decreased urine volume, urine color change, and lower abdominal pain. He is engaged with a rare side effect of oxymetholone abuse. During assessments of potential medical issues associated with the intake of anabolic steroids, known side effects are known to be transient, but the need for appropriate interventions remains essential. Conclusions: Rhabdomyolysis due to drug use and the consequent acute kidney injury are among the lethal risks associated with anabolic steroid abuse. In most cases, the symptoms are extensive and often misleading. Therefore, detailed history taking, physical scrutiny, paraclinical testing, and early diagnosis are crucial for rhabdomyolysis patients

    A Review of the Indices of Mizaj-e-Meda (Temperament of Stomach) identification in Persian Medicine

    BACKGROUND AND OBJECTIVE: Digestive diseases are one of the problems of the health system in the world. From the perspective of Persian medicine, the stomach begins the second stage of digestion and its function plays an important role in human health or illness. The written sources of Persian medicine have been widely used to determine the health of the stomach. The purpose of this study is to summarize indices of Mizaj-e-Meda (Temperament of Stomach) identification to help better diagnose the status of health or illness of stomach. METHODS: In this review article, 11 valid Iranian medicine sources and scientific databases of Scopus, PubMed, Science Direct, Google Scholar, Elsevier, and web of science as well as Persian indices of SID, Iran Doc, Magiran were searched and the existing views were summarized. FINDINGS: In the sources of Persian medicine, the indices of different types of Mizaj (temperament) and Su-e-mizaj (abnormal temperament) have been proposed. Seventeen articles on the subject of gastric diseases in Persian medicine were found; one case merely examined gastric symptoms, while others only slightly referred to the indices. Some of the most important indices considered in these sources for identifying the type of Mizaj-e-Meda were the way of digestion, desire for food according to the type of Mizaj (temperament), thirst, appetite, burrow, moisture and taste of the mouth, tongue and mouth condition, flatulence, quality of waste matter, speed of food passage, sour stomach, burning stomach, stomach ache and stomach malmas (touch). CONCLUTIONS: Persian medicine sources have fully cited gastric indices, which can be used for a more effective diagnose and treatment and even prevention of gastric diseases, and can also be used for designing and validating standard scales for Mizaj-e-Meda (Temperament of Stomach) identification

    Comparison between the Plasma Levels of Long Noncoding RNA BDNF-AS in Patients with Alzheimerś disease and Healthy Subjects

    BACKGROUND AND OBJECTIVE: Diagnosis of Alzheimer's disease usually occurs when serious damages have occurred in the brain and common treatments are ineffective in preventing it. One of the RNAs involved in Alzheimer's disease is a long noncoding RNA, called BDNF antisense (BDNF-AS). The aim of this study is to determine the presence and compare the BDNF-AS levels in plasma of Alzheimer's patients and healthy subjects, and to evaluate its potential as a plasma marker for Alzheimer's disease. METHODS: In this case-control study, 30 patients with late-stage Alzheimer's disease and 30 healthy subjects without neurological disease who matched the patients in terms of age were selected by a specialist according to the criteria for clinical diagnosis of Alzheimer's disease and their intravenous blood samples were collected. The plasma of the blood samples was isolated and total plasma RNA was extracted. After cDNA synthesis, the presence of BDNF-AS in plasma was examined by PCR. Finally, the relative level of BDNF-AS transcripts in plasma samples of patients with Alzheimer's disease and healthy subjects was evaluated using Real Time PCR. FINDINGS: The results of this study showed that long noncoding RNA BDNF-AS was present in the plasma of patients and controls. Comparison of Real Time PCR data showed that BDNF-AS levels in the plasma of patients (0.107±0.021) showed significant increase compared to healthy subjects (0.039 ± 0.006). CONCLUSION: The results of this preliminary study indicate that the levels of long noncoding RNA BDNF-AS in plasma can be used as a blood/plasma marker for the diagnosis of Alzheimer's disease

    Frequency of Abdominal Obesity and Its Relationship with Blood Factors and Blood Pressure Indices in Candidates for Elective Angiography Referring to Heshmat Hospital in Rasht in 2016

    BACKGROUND AND OBJECTIVE: Considering the relationship between abdominal (central) obesity and chronic diseases and disorders of biochemical factors, as well as the role of ethnicity and geographical area on the relationship between obesity and chronic diseases, this study aims to determine the prevalence of abdominal obesity and its relationship with biochemical and blood pressure indices in candidates for angiography. METHODS: This cross-sectional study was performed on 610 subjects aged 25-75 years who were candidates for elective angiography and were referred to Heshmat Hospital in Guilan province in 2016. Data were collected by interview, medical records, blood pressure measurements and anthropometric indicators, including weight, height, body mass index and waist circumference. Fasting blood samples were used for biochemical indices. Abdominal obesity was determined based on waist circumference more than 102 cm in men and more than 88 cm in women. FINDINGS: In this study, 348 (57%) patients had abdominal obesity. 29% of men (101 patients) and 71% of women (248 patients) had abdominal obesity and there was a significant relationship between gender and abdominal obesity (p=0.0001). There was a significant different between patients with abdominal obesity and non-obese subjects in terms of total cholesterol (161±40 vs. 153±41 mg/dl, p=0.04), HDL cholesterol (44±8.6 vs. 43±8.2 mg/dl, p=0.01) and hematocrit (39±4.1 vs. 41±4.4 mg/dl, p=0.001). The variables of age, gender, and BMI were independent predictors of abdominal obesity. The incidence of abdominal obesity was 37 times higher in women and increased significantly with age. The chance of obesity increased by 1.25 per unit body mass index increase. CONCLUSION: The results of this study demonstrate the prevalence of abdominal obesity in candidates for elective angiography and its association with some blood factors

    The Relationship between Serum Zinc Level and Liver Elastrography Using Fibroscan in Non-Alcoholic Fatty Liver Patients

    BACKGROUND AND OBJECTIVE: Zinc is one of the essential elements for many vital functions of the body, including regulation of gene expression, anti-inflammatory and antioxidant activity, protein and nucleic acids metabolism. Since, many people develop advanced liver disease, including liver cirrhosis, they have zinc deficiency. The aim of this study was to investigate the relationship between liver stiffness and serum zinc levels in patients with non-alcoholic fatty liver disease. METHODS: This cross-sectional study was performed on 40 non-alcoholic fatty liver disease Patients referring to Imam Khomeini Hospital in Ahvaz in 1395. Serum zinc levels and liver Stiffness (based on METAVIR scale) were measured by fibroscan, and Age, sex, serum zinc level and liver fibroscan were recorded. Serum zinc level at different ages and scores of Liver stiffness was compared and analyzed. FINDINGS: This study was conducted on 40 patients with non-alcoholic fatty liver disease. The number of men was 26 patients (65%). Mean age of patients was 45.077±9.4 years, mean serum zinc level was 81.4±8.1 and mean liver stiffness was 6.5±2.1 kPa. Serum zinc level had a reverse and strong correlation with liver stiffness (p=0.0001, r=0.9). Also, with age increasing, liver stiffness increased (p=0.01, r=0.5) and serum zinc level decreased (p=0.01, r=0.5). CONCLUSION: According to findings of the present study, the serum zinc level significantly decreased with fibrosis progression in patients with non-alcoholic fatty liver

    Preparation and Evaluation of Rivax Protein Loading in Chitosan Nanoparticles

    BACKGROUND AND OBJECTIVE: Ricin toxin is a heterodimer glycoprotein which, due to its high toxicity, is used as a bioterrorism agent. Immunogenicity studies against ricin are now focused on two subunit vaccine candidates, including RiVax and RVEc. These studies have examined the vaccine candidate immunization as an alone and in combination with adjuvant, however, there is not a published study on the immunogenicity evaluation of the candidate vaccine through the delivery by nanoparticles. The aim of this study was preparation and evaluation of RiVax recombinant vaccine-loading in chitosan nanoparticles. METHODS: In this experimental study, After transferring the RiVax gene to the bacterium, inducing the expression and purification of the RiVax protein by affinity chromatography column, the RiVax protein was loaded with Ionic Gelation method in chitosan nanoparticles. Then, the properties of nanoparticles including size, morphology, loading percentage and release pattern of RiVax protein from nanoparticles and stability of this protein during acidic loading conditions in nanoparticles by SDS-PAGE were evaluated. Also, Immunization study were performed on 3 mouse groups (n=4/group) by RiVax protein, Nanoparticles containing protein and phosphate buffer. FINDING: The results of this study showed that the nanoparticles containing protein had a size of 178 nm and a Zeta potential of +27.8 MV and a polydispersity index of 0.193. Also, according to SDS-PAGE results, it was found that the RiVax recombinant protein was denatured during the process of preparing the chitosan nanoparticles. CONCLUSION: The results of this study showed that the RiVax protein has been unstable in acidic conditions for the production of chitosan nanoparticles and Ionic Gelation method is not suitable for loading this protein in chitosan nanoparticles

    The Relationship between Atopic Dermatitis and Premenstrual Syndrome in Women Referred to the Skin Clinic of Hamadan

    BACKGROUND AND OBJECTIVE: One of the factors affecting dermatophyte is hormonal factors, in which atopic dermatitis is more common in adult women. Since one of the psychotic problems regarding reproductive performance of women is premenstrual syndrome, in this study, the association between premenstrual syndrome and atopic dermatitis in reproductive age women was investigated. METHODS: This case-control study was performed on 82 women aged between 15 and 49 years old in two groups of atopic dermatitis referring to Skin Clinic of Sina Hospital in Tehran and control group including healthy women for atopic dermatitis. Demographic questionnaire and (PSST) Premenstrual Symptoms Screening Test (with score 42-0) were completed and evaluated by both groups. FINDINGS: The mean age of subjects in the case group was 17.17 ± 4.45 and in the control group was 27.29 ± 8.03 years. The results showed by controlling the possible confounding factors as age, occupation and marital status the probability of having moderate or severe premenstrual syndrome in patients with atopic dermatitis is 13.26 (OR= 13.26, 95% CI: 3.31, 53.07), as compared with healthy subjects. CONCLUSION: The results showed that there is a strong relationship between moderate to severe menstrual syndrome and atopic dermatitis in women

    The Relationship between Uterine Endometrial Neovascularization and Pelvic Pain Intensity

    BACKGROUND AND OBJECTIVE: Pelvic pain is one of the most important symptoms of endometriosis. There is evidence that high blood flow to endometrioma is associated with more pelvic pain, but this has not been completely proven. Therefore, the present study was conducted to evaluate the relationship between uterine endometrial neovascularization and pelvic pain intensity. METHODS: In this cross-sectional study, 76 patients with ovarian endometrioma (based on the final diagnostic pathology) were divided into two groups of mild/moderate pelvic pain (VAS lower than 66) and severe pelvic pain (VAS 66 to 100) in terms of pelvic pain intensity (using the visual analog scale). Patients underwent transvaginal doppler ultrasound prior to surgery, and endometrial neovascularization, as well as resistive index (RI) and pulsatility index (PI) in the artery of the endometrioma cyst wall were measured and were compared between two groups of mild/moderate pelvic pain and severe pelvic pain. FINDINGS: Moderate and severe neovascularization (50%) in patients with severe pelvic pain was significantly higher than patients with mild/moderate pelvic pain (13%) (p=0.005). The mean RI in the group with severe pelvic pain (0.59±0.05) was lower than the mild/moderate group (0.66±0.06) (p<0.001). The mean PI in patients with pelvic pain (1.07±0.13) was lower than patients with mild/moderate pelvic pain (1.14±0.15) (p=0.03). CONCLUSION: Severe neovascularization and low RI and PI indices in transvaginal doppler ultrasound of ovarian endometrioma indicate high activity of endometrium and is associated with severe pelvic pain in patients

    A Comparison of the Effect of Ondansetron and Propofol on Intrathecal Opioid-Induced itch in Elective Cesarean Section

    BACKGROUND AND OBJECTIVE: Itch is one of the most common and uncomfortable side effects of neuraxial anesthesia. Its incidence is higher especially in cesarean section. The aim of this study was to compare the effect of ondansetron and propofol at doses lower than the hypnotic dose on the treatment of itch induced by intrathecal fentanyl in cesarean section. METHODS: In this prospective study, 90 patients with American Society of Anesthesiologists (ASA) class I and class II with an average age of 30 years and first time cesarean section underwent intra-spinal anesthesia with 25 μg fentanyl and 10 mg bupivacaine 0.5%. Women were randomly divided into two groups of 45. One group received 4 mg ondansetron and another group received 10 mg propofol at first and then 10 μg / kg / min through infusion. The incidence and severity of itch were recorded on the basis of the visual scoring system during operation and during recovery. FINDINGS:The highest incidence of itch was 30 to 60 minutes after injection. The incidence of itch during surgery was 2.22% and 7.26%, and during recovery was 7.6% and 8.8%, in the ondansetron and propofol groups, respectively (p=0.5, p=0.4). The mean itch severity scores during surgery were 1.85±0.69 and 1.66±0.81, and during the recovery were 1.33±0.57 and 1.25±0.5, in the ondansetron and propofol groups, respectively (p=0.65) (p=0.84). CONCLUSION: Based on the results of this study, ondansetron and propofol at doses lower than the hypnotic dose were well tolerated.
