Abolhassan Ahmad bin Mohammad Tabari Taranji; The famous Iranian Physician
Tabaristan (current Mazandaran), in northern Iran and the south of the Caspian Sea, is considered as one of the scientific and medical centers in both scientific and Islamic worlds. The emergence of numerous scholars in this area indicates the centrality of science in this amazing geography. One of the greatest men of this land is Abolhassan Ahmad bin Mohammad Tabari Taranji. Information about him is very limited in historical sources but he is recognized for writing a scientific book on al-mu'alaja al-buqratiya. Moreover, he has also played a role in the political system of Buyid dynasty and Abbasid Caliphate. In this research, his life, his scientific experiences and his medical works are discussed
Anticonvulsant effects of hesperetin in animal model of pentylenetetrazole-induced-seizures
Background and objective: Hesperetin as the main flavonoid in citrus possesses various pharmacological properties including anti-oxidant and anti-inflammatory effects. In this study, the effects of hesperetin on seizures behavior and its function on total antioxidant capacity and lipid peroxidation has been investigated in pentylenetetrazol (PTZ)-induced seizures model.
Materials and methods: In this experimental study, thirty-five mice were divided into 5 experimental groups as control, saline and hesperetin at doses of 10, 20 or 50 mg/kg. Animals received orally the related interventions for 7 days. On day 7, 30 minutes after oral gavage, convulsion was induced by single intraperitoneal (i.p.) injection of PTZ at dose of 60 mg/kg. After recording of convulsion behaviors including latency to myoclonic jerks, latency and duration of generalized tonic-clonic seizures, time to death, measuring of Ferric Reducing Antioxidant Power (FRAP) and Thiobarbituric acid reactive substances (TBARS) carried out in hippocampus tissues.
Findings: Pretreatment with hesperetin at dose of 50 mg/kg significantly increased the latency of myoclonic jerks (hesperetin 50: P=0.0323) and generalized tonic-clonic seizures (hesperetin 10: P= 0.0003, hesperetin 20: 0.0017, hesperetin 50: P=0.0040). Hesperetin application at dose of 10 mg/kg significantly reduced the levels of TBARS compared to control group. Any significant difference in FRAP levels was not observed between different experimental groups.
Conclusion: The results of study indicate that hesperetin might be effective as supplementary treatment in epilepsy disorder
A Case Report of Primary Angiitis of the Central Nervous System
BACKGROUND AND OBJECTIVE: Primary angiitis of the central nervous system (PACNS) is an inflammatory vasculitis with very low frequency and prevalence. It is not clear why the inflammatory process of this disease is limited to cerebrovascular disease without systemic manifestations. A case of primary angiitis of the central nervous system with cerebrovascular manifestations is reported here.
CASE REPORT: The patient is a 50-year-old woman with headache, nausea, vomiting, and visual symptoms without clear motor impairment. Normocytic normochromic anemias tests and function tests of kidney, liver and thyroid and serology of infectious diseases were reported to be normal. The patient underwent brain imaging, and due to multiple lesions in both cerebral hemispheres and cerebellum, she underwent biopsy with a probable diagnosis of cerebral vasculitis. After histopathologic confirmation, the patient was discharged with glucocorticoid and cyclophosphamide therapy, and the symptoms subsided after one year follow-up.
CONCLUSION: Based on the results of this case report, some general and common symptoms of the primary angiitis of the central nervous system should also be considered
Digital Library in Web of Science: Analyzing World Scientific Papers Emphasizing Iranś rank
Background and aim: Digital libraries have been attracted by many countries around the world in recent years, and many scientific outputs regarding to digital libraries have been published. The aim of this study was to analyze the scientific papers of the world in the field of digital libraries based on quantitative and qualitative indicators with focus on Iran's rank. Material and methods: This descriptive-analytical study with a scientometric approach was evaluated 5637 papers indexed in Web of Science (WoS) by the end of 2016. The HistCite software was used for data analysis and VOSviewer software was applied for drawing the maps related to thematic and co-authorship analysis. Findings: The result indicates that the scientific papers of the world have had an increasing trend in this field. H-index of articles was 93 and 18 in the world and Middle East countries, respectively. The United States of America (USA) had the highest number of articles (2221) with h-index 81. Iran had h-index 7. It was found that single-authored paper had the highest frequency among global papers. Scientific collaborative coefficient was 0.44 and 0.54 for global and Iranian researchers during 35 years, respectively. Conclusion: The research results suggest that most scientific articles of digital libraries have been published by several countries, with the USA topping the list. In the Middle East, Iran has the largest share in science production with 41 articles
Effect of dietary patterns on oxidative stress in Patiants with metabolic syndrome: Tehran Lipid and Glucose Study
Background: Metabolic syndrome is a prevalent condition with dramatic rising trend worldwide. Single dietary factors, such as omega-3 fatty acids consumption protect body against oxidative damage by reinforcement of dietary total antioxidant capacity but the combination of all dietary components may be more effective when studied as integrated dietary patterns. This present study was designed to assess the association between different dietary patterns and oxidative stress in a population of Tehranian adults suffering from metabolic syndrome.
Methods: Dietary data were collected using a validated 147-item semi-quantitative FFQ with a standard serving size. Factor analysis method was used to derive dietary patterns. Blood analysis and anthropometric measurements were also obtained. Oxidative stress was assesses using serum levels of Malondialdehyde (MDA) and total antioxidant capacity (TAC).
Results: The regression coefficient for TAC and MDA with different quintiles of dietary patterns, adjusted for potential confounder in model 3 reveal a significant positive association between healthy pattern and serum TAC levels (β=0.244, p=0.008) and also between serum MDA levels and the unhealthy pattern (β=0.387, p=0.0001). On the other hand, a significant negative association found between serum TAC levels (β=-0.289, p=0.001) and the unhealthy pattern, a relationship also noted between serum MDA levels and the healthy dietary pattern (β=-0.273, p=0.002).
Conclusions: Our findings suggest that following a healthy pattern filled with fruits and vegetables ameliorates oxidative stress status and on the contrary, attachment to an unhealthy pattern, characterized by higher intakes of fast foods and processed foods, aggravated the oxidative stress levels in Tehranian individuals suffering from metabolic syndrome
Prevalence of Musculoskeletal Pains and the Demographic Factors Related to the Pain Locations in Patients with Chronic Trauma
BACKGROUND AND OBJECTIVE: Chronic trauma can be a risk factor for most of the musculoskeletal disorders. Also, identifying the related factors can help to control it better. Therefore, this study aimed to evaluate the prevalence of chronic musculoskeletal pains and the relation between their locations and demographic factors.
METHODS: This cross-sectional study was performed on 1000 traumatic patients who referred to orthopedics clinic of Shahid Beheshti hospital during 2016-2017. Demographic characteristics (age, sex, occupation, education, weight and height) and clinical information of the patients were obtained.
FINDINGS: Of the 1,000 patients referred, 223 (22.3%) had chronic pain. The most common site of pain was the waist (n = 100, 44.8%) and the lowest incidence of pain was in the heel (n=2.9%). The pain in the lower limbs was higher in women (30.9% vs. 16.9%, p=0.006). Chronic trauma of the upper limbs in individuals less than 45 years old (22.4%) was more than those aged 65-45 years (17.1%) and over 65 years old (1.7%) (p = 0.002). ). The neck and waist pain in the group of upper the diploma (69.7%) was higher than the diploma (59%) (p = 0.004). Lower limb pain in housewives (34.5%) was higher than employees (18.3%) and those with free or agricultural occupation (18.9%) (p=0.018).
CONCLUSION: According to the results, chronic traumatic pain had a considerable prevalence rate. The most common location of the pain was the low back
Experimental Models of Thrombocytopenia in Laboratory Animals and their Application in Identifying the Complications of Chemotherapy Drugs
BACKGROUND AND OBJECTIVE: Thrombocytopenia is one of the complications of chemotherapy drugs that may cause death. Different animal models of thrombocytopenia are used for clinical research and identification of its causes, each with advantages and disadvantages. The aim of this review article is to investigate the methods of thrombocytopenia induction in laboratory animals and their advantages and disadvantages.
METHODS: This systematic review was conducted using the keywords “thrombocytopenia platelet”, “chemotherapy”, “animal model”, in PubMed, Science Direct and Scopus databases from 1990 until October 2017. The title and abstract of several articles were reviewed, and after excluding the unrelated items, final articles were selected and reviewed.
FINDINGS: Animal models of thrombocytopenia are of two types of immune and non-immune. Non-immune models reduce platelet production through bone marrow suppression. Antiplatelet antibodies are used in immune models. The immune and non-immune thrombocytopenic models have some advantages and limitations and are selected according to the current therapeutic goals. Mice and rats are commonly used as laboratory animals, and cyclophosphamide and carboplatin are the most commonly used drugs.
CONCLUSION: According to the results of this study, due to the limitations of human subject research in diseases that lead to thrombocytopenia, there is a need to develop appropriate animal models for studying and identifying the factors affecting thrombocytopenia
Association of Limb-Girdle muscular dystrophy with multiple sclerosis: A case report
Background: The association of limb-girdle muscular dystrophy (LGMD) with other neurological disorders is uncommon.
Case presentation: We report a 25-year-old female with LGMD who suffered from slowly progressive proximal muscular weakness and atrophy since she was 12 years of age. The patient recently presented with acute loss of left side visual acuity. After evaluation, findings were suggestive of multiple sclerosis.
Conclusions: This is the first report of LGMD in association with MS. The simultaneous occurrence of MS with myopathies may be incidental but there may be a genetic susceptibility for both diseases. This comorbidity may influence the treatment of MS
Alterations in serum levels of trace element in patients with breast cancer before and after chemotherapy
Background: Breast cancer is the most common serious disease around the world. The trace elements have a vital role in the metabolism and chemotherapy may change the level of metal ions. Due to the ambiguity of the existence in this regard, the study examined the trace element serum levels in women with breast cancer before and after chemotherapy .
Methods: Sixty patients were studied undergoing specialist. First sampling was taken before chemotherapy (after 4 weeks of surgery) and second sampling was taken after the completion of 3 courses of chemotherapy, approximately 9 weeks after the first chemotherapy. The patients took Adriamycin 60mg/m2 Cytoxan 600mg/m2. Serum zinc and iron levels were measured using standard spectrophotometric method. Measurement of serum copper was done by atomic absorption spectroscopy.
Results: Serum zinc and iron levels in women after chemotherapy significantly decreased (p<0.001), however, the serum level of copper increased but was not significant (P=0.676).
Conclusions: Our findings demonstrate significant decrease in zinc and iron levels in breast cancer patients after 3 courses of Adriamycin and Cytoxan chemotherapy. Prescribing zinc supplements can be useful after chemotherapy
A rare case of hepatic subcapsular biloma after laparoscopic cholecystectomy and subsequent endoscopic retrograde cholangiopancreatography
Background: Biloma is a rare abnormal localized accumulation of bile out of biliary tree due to an injury that usually occurs postoperatively from an injured cystic or bile duct. While most bilomas collect in the subhepatic space, we describe a rare case of hepatic subcapsular biloma after laparoscopic cholecystectomy and endoscopic retrograde cholangiopancreatography (ERCP) which was successfully done one week after surgery treated by percutaneous drainage and placement of plastic stents in common bile duct (CBD).
Case presentation: A 30-year-old woman with right upper quadrant pain referred to our center. Four weeks ago, she had a history of laparoscopic cholecystectomy and two weeks ago she had a history of ERCP. Abdominal ultrasonography showed a mass in her right liver lobe. The patient underwent the fluid aspiration under ultrasonography guidelines. Chemical analysis of aspirated fluid proved that the liquid was bile. A drainage catheter was placed in site of bile accumulation to drain the liquid. The next follow up ultrasonography showed that the mass disappeared and no recurrence was detected.
Conclusions: Diagnosis of biloma in a patient with the history of laparoscopic cholecystectomy or ERCP should be considered for proper interventional treatment to reduce morbidity and mortality
