Análisis socioantropológico en pacientes con cirugía bariátrica: análisis preliminar
Using socio-anthropological analysis, this paper presents preliminary results of an inter-disciplinary investigation entitled “A nutritional and anthropological study of patients with type III and IV obesity undergoing bariatric surgery.” The medical ethnographic approach employed in this study has two goals: to study patients’ cultural context before and after surgical treatment and to analyze perceptions, representations, social inte-gration, stigmatization, and social control before and after surgery. The results show potential discrimination against these patients based on their group identification and a permissive environment; these ideas are manifested in the concepts of the “obesogenic society” and the “obese victim.” On the other hand, the process of medicalization provides a mechanism capable of both social inclusion and exclusion. The results of this preliminary analysis are informing ongoing work between patients and professionals in this nutritional study.Desde un análisis socioantropológico se presentan algunos resultados preliminares en el marco del proyecto interdisciplinar: Estudio nutricional y antropológico en pacientes diagnosticados de obesidad tipo III y IV y sometidos a cirugía bariátrica. A través de la etnografía médica se realiza la aproximación a estos pacientes con dos objetivos: estudiar el contexto cultural en que se desenvuelve el paciente antes y después del tratamiento quirúrgico y analizar las percepciones, las representaciones, la incorporación a la vida social, el proceso de estigmatización y control social antes y después de la intervención. Los resultados advierten por un lado, el posible acoso a estos pacientes desde una óptica basada en el grupo y en el entorno que consiente, presentándose unos claros perfiles de “sociedad obesogénica” y “víctima obesa”. Por otro lado, el proceso de medicalización en estos pacientes se presenta como mecanismo idóneo de inclusión y exclusión social. Diagnóstico sociocultural de la alimentación de los jóvenes en Comitán, Chiapas
This study, based on group interviews with adolescents (pre-university level students) from various educational institutions and socio-cultural backgrounds in Comitán, Chiapas (Mexico), was undertaken to identify the settings for the dietary practices of young people. Using a multi-dimensional analysis, we found that the predominant paradigm in public health, based on concepts such as risk factors and lifestyle, tends to obscure the complexity of social processes and to place responsibility for health on each individual. The dietary practices of adolescents are determined by the dynamics established in school and family environments and occur in a context of time pressures as well as the poor provision and choice of foods. We noted a discrepancy between knowledge on the one hand and dietary practices, weight control and perceived risks or expectations concerning body image on the other.Con el objetivo de identificar los diferentes ámbitos en los que tienen lugar las prácticas alimentarias de los jóvenes de Comitán, Chiapas, México, se realizó un estudio basado en entrevistas grupales con jóvenes, estudiantes de nivel medio superior, adscritos a diferentes instituciones educativas y contextos socioculturales de Comitán, Chiapas. A través de un análisis multidimensional se encontró que la visión predominante en salud pública, con conceptos tales como factores de riesgo y estilos de vida, tiende a ocultar la complejidad de procesos sociales y a delegar la responsabilidad de cada individuo por su salud. La alimentación de los jóvenes está mediada por las dinámicas instituidas en los ámbitos escolares y familiares, toda vez que se encontró una falta de disponibilidad en términos de tiempo, así como de oferta y elección de los alimentos. Sistemas de salud sobrecargados hacia el tercer nivel y calidad de la atención médica: una encuesta de la Escuela de Medicina y Hospital RG Kar, India
The paper is based on a primary survey of 1095 patients from the Medicine, General Surgery and Cardiac departments in R.G. Kar Medical College and Hospital (a major public sector health care institution in the metropolitan city of Kolkata, India). The results show that public health care institutions remain a vital life support system of the poorer sections of the population. However, this is not a matter of choice, but necessity. The breakdown of the three tiered referral system in the region has resulted in the Hospital, originally conceived as a super specialty hospital and referral health care institution for patients from the adjacent district of North 24 Parganas, functioning as a diagnosis unit. The resultant pressure has exceeded the carrying capacity of the institution and led to poor quality of health care in the Hospital. Revitalizing lower level institutions is therefore essential not only to increase accessibility of health services, but also to ensure efficiency in higher level health care institutions.Este trabajo está basado en una encuesta aplicada a 1,095 pacientes de los Departamentos de Medicina General, Cirugía y Cardiología de la Escuela de Medicina y Hospital R.G. Kar (una institución de atención médica importante del sector público en el área metropolitana de Calcuta, India). Los resultados muestran que las instituciones de atención sanitaria públicas siguen siendo un sistema de apoyo vital para los sectores más pobres. Sin embargo, esto no es una cuestión de elección, sino de necesidad. El desmantelamiento del sistema de tres niveles de atención en la región ha tenido como consecuencia que el hospital cumpla las funciones de una unidad de diagnóstico (en un principio había sido concebido como un centro de derivación de tercer nivel para pacientes del distrito vecino North 24 Parganas). La presión consiguiente ha excedido la capacidad de la institución y ha empobrecido la calidad de la atención en el hospital. La revitalización de las instituciones de los niveles más bajos es, por ende, necesaria, no nada más para aumentar la accesibilidad a los servicios, sino también para asegurar la eficiencia en los niveles más altos de las instituciones de atención.

The biologist studies man as a physical organism existing in a space-time world of measurable dimensions. This organism has the ability to adapt to changes in its environment; to the processes ofgrowth, development and aging; and to disease. The main objectives of medicine have been to help both adaptation and defence. Prevention through hygiene and immunisation, drugs, surgical removals, and replacements-all these make their contributions, which have increased in complexity and expense, disappointing Beveridge's hope in 1949 that a National Health Service plus rehabilitation (then a novelty) would so improve health that the Service would wither away.
To avoid and prevent ill health are worthy aims; to promote health is also worthy, but something different. It was to study healthy living that Dr Scott Williamson, Dr Innes Pearse, and their colleagues devised the Peckham experiment, of which this book is at once a reminder and a memorial. They wanted to study Homo sapiens in his natural habitat and they began their exploration as biologists. In an initial family club of 150 families they noted that disorders, however skilfully treated, recurred in the environment out of which the disorder had arisen. So, in 1935, they designed a laboratory in which human beings could be observed growing up and creating new families. The centre, purpose-built, combined maximum visibility throughout, flexibility, and free access to all members. By the outbreak of war 800-900 families had grown their own lively social life. Entrevista a Sir Michael Marmot
In August of 2008 the WHO Commission on the Social Determinants of Health concluded its work with the publication of a report entitled: “Closing the gap in a generation: Health equity through action on the social determinants of health.” The Commission’s chair, Sir Michael Marmot, was kind enough to answer our questions about the Commission’s recommendations. This interview was conducted by email in May of this year.En agosto de 2008, la Comisión de Determinantes Sociales de la Salud de la OMS concluyó su trabajo con la publicación de un informe titulado: “Cerrando la brecha en una generación: Equidad en salud a través de la acción sobre los determinantes sociales de la salud”. "This is the first in a series of pamphlets published by the Health Policy Advisory Center (Health-PAC). It analyzes the forces in the health system that prevent most Americans from getting good health care. How can that system be changed so that good health care is a right for all? Around the country emerging community and worker groups are working to transform the present health system into one that will provide high quality, low-cost, accessible health care for all. Health-PAC, an independent, non-profit research and education organization, works with and services these groups. Reformas de gobiernos socialistas a las políticas de salud en Bolivia y Ecuador: el potencial subestimado de la Atención Primaria Integral de Salud para impactar los determinantes sociales en salud
Background
Selective vertical programs prevailed over comprehensive primary health care in Latin America. In Bolivia and Ecuador, socialist governments intend to redirect health policy. We outline both countries’ health system’s features after reform, explore their efforts to rebuild primary health care, identify and explain policy gaps, and offer considerations for improvement.
Methods
Qualitative document analysis.
Findings
Earlier reform left Bolivia’s and Ecuador’s population in bad health, with limited access to a fragmented health system. Today, both countries focus their policy on household and community-based promotion and prevention. The negative effects on access to care of decentralization, dual employment, vertical programming and targeting are largely left unattended. Neglecting care is understandable in the light of particular interpretations of social medicine and social determinants, international policy pressures, reliance on external funding and institutional inertia. Current policy choices preserve key elements of selective care and consolidate commodification. It might not improve health and worsen poverty.
Interpretation
Care can be considered as a social determinant on its own. Key to the accomplishment of primary care is an integrated application of family medicine, taking advantage of individual care as one of the ways to act on social determinants. It deserves a central place on the policy-makers’ priority list, in Bolivia and Ecuador as elsewhere.Antecedentes: en Latinoamérica los programas verticales selectivos han prevalecido sobre la atención primaria en salud (APS). En Bolivia y Ecuador los gobiernos socialistas tienen la intención de redirigir la política sanitaria. En este artículo bosquejamos las características del sistema de atención de la salud de ambos países después de la reforma, exploramos sus esfuerzos por reconstruir la APS, identificamos y explicamos las brechas en las políticas y exponemos algunas reflexiones para su mejoramiento. Métodos: Análisis cualitativo de documentos. Hallazgos: Las reformas previas dejaron a las poblaciones de Bolivia y Ecuador en mal estado de salud y con acceso limitado a sistemas de salud fragmentados. Hoy en día, ambos países centran sus políticas en la prevención y la promoción de la salud basadas en el hogar y la comunidad. Sin embargo, los efectos negativos de la descentralización, del doble empleo, de la programación vertical y de la focalización sobre el acceso a la atención en salud han quedado desatendidos en gran parte. Esta desatención se entiende bajo la luz de interpretaciones particulares de la medicina social y los determinantes sociales, las presiones de políticas internacionales, la dependencia a financiamientos externos y la inercia institucional. Las políticas actuales preservan los elementos clave de la atención selectiva en salud y consolidan su mercantilización, por lo que es posible que no mejoren la salud y profundicen la pobreza. Conclusión: la atención en salud puede ser considerada como un determinante social por sí mismo. La aplicación integrada de la medicina familiar (aprovechando la atención individual como uno de los modos para actuar sobre los determinantes sociales) es clave para el logro de la atención primaria, se merece un lugar central en la lista de prioridades de los hacedores de políticas en Bolivia, en Ecuador y en cualquier otro lugar
Reflexiones y consideraciones sobre la influenza A/H1N1 en México
1. Mexico needs to rebuild an efficient epidemiological monitoring system which can react in a timely manner to unusual infectious outbreaks. SINAVE, the National Epidemiologic Monitoring System, showed itself incapable of responding opportunely and effectively to the increase of pneumonia cases which was reported as early as February 2009. Nor did SINAVE pay sufficient attention to the inhabitants of Gloria, Perote, Veracruz when they complained of an outbreak of respiratory illness. 2. Priority should be given to forming teams with the technical capacity to identify the earliest cases of any new outbreak and then to immediately establish quarantine procedures. These teams should be housed within the National Public Health System and should be properly structured, well organized, and coordinated on the national level. There can be no doubt that the recent public health emergency in Mexico demonstrated the disorder and disorganization which have resulted from dismantling of Mexico’s national public health system. The deficiencies in Mexican public health result from several decades of disinvestment in the public sector and of favoring privatization and technological dependency on transnational companies. The lack of effective and consistent communication, guidance, and policies in response to the emergency made clear just how the current system reinforces social exclusion by not responding to the needs of the population. In October 2008 the Latin American Social Medicine Association (ALAMES) organized an international workshop entitled “The Social Determinants of Health.” Representatives of ALAMES’ seven regions participated in discussions of the various consultative papers prepared by the working groups of the WHO Commission on the Social Determinants of Health as well as the Commission’s final report. The workshop considered how ALAMES should respond to the work of the Commission. In this paper we summarize the main points outlined in the position paper prepared by the Organizing Committee1 as well as a synopsis of the main contributions made by each of the workshop’s study sections.En octubre de 2008 año se realizó el Taller de Discusión sobre Determinantes Sociales de la Salud organizado por ALAMES, con la participación de representantes de todas las regiones de la Asociación.
