Dynamic Angular Petrissage as Treatment for Axillary Web Syndrome Occurring after Surgery for Breast Cancer: a Case Report
Background: In the context of breast cancer, axillary web syndrome (AWS), also called lymphatic cording, typically presents in the weeks after axillary surgery. This painful condition, likely lymphofibrotic in origin, restricts upper extremity range of motion (ROM). There is no established treatment, although physical therapy and other approaches have been used to variable effect. This report describes treatment of a female client with AWS, who had recently undergone a unilateral simple mastectomy with sentinel node biopsy plus axillary dissection.Methods: The client presented with pain upon movement (self-reported as 5 on the 0–10 Oxford Pain Scale), visible cording and restricted use of the ipsilateral upper extremity. Clinical assessment included determining the extent of AWS cording (taut, from axilla to wrist) and measuring glenohumeral joint ROM (140° flexion by goniometer). A therapeutic massage with movement protocol, termed dynamic angular petrissage, was administered over two sessions: Swedish massage combined with dynamically taking the limb through all possible angles of movement (passive ROM), controlling stretch and tension while simultaneously and segmentally applying petrissage and non-petrissage techniques to the underlying soft tissue. Careful attention was taken to not break the cord. Home care consisted of prescribed exercises performed by the patient.Results: After Session One, pain was reduced (to 0/10), ROM improved (to 170° flexion), and cording was visibly reduced. After Session Two the cord was residually apparent only on hyperextension, with no ROM restrictions in glenohumeral joint flexion. Follow-up at three months revealed absence of visual or palpable evidence of cording, unrestricted glenohumeral joint ROM, and absence of movement-associated pain.Conclusion: The signs and symptoms of AWS were quickly and effectively eliminated, without causing any pain or discomfort to the client. We propose that dynamic angular petrissage may be an efficient and safe treatment approach for reducing the pain, mobility restrictions, and cording of AWS
Impact of Massage Therapy on Fatigue, Pain, and Spasticity in People with Multiple Sclerosis: a Pilot Study
Background: Multiple sclerosis (MS) is a chronic, immune-mediated, inflammatory disease that leads to fatigue, pain, and spasticity, as well as other sensorimotor and cognitive changes. Often traditional medical approaches are ineffective in alleviating these disruptive symptoms. Although about one-third of surveyed individuals report they use massage therapy (MT) as an adjunct to medical treatment, there is little empirical evidence that MT is effective for symptom management in people with MS.Purpose: To measure the effects of MT on fatigue, pain, spasticity, perception of health, and quality of life in people with MS.Setting: Not-for-profit long-term care facility.Participants: Twenty-four of 28 enrolled individuals with MS (average age = 47.38, SD = 13.05; 22 female) completed all MT sessions and outcome assessments.Research Design: Nonrandomized, pre–post pilot study.Intervention: Standardized MT routine one time a week for six weeks.Main Outcome Measure(s): Modified Fatigue Index Scale (MFIS), MOS Pain Effects Scale (MOS Pain), and Modified Ashworth Scale (MAS). Secondary outcome measures: Mental Health Inventory (MHI) and Health Status Questionnaire (HSQ).Results: There was a significant improvement in MFIS (p < .01), MOS Pain (p < .01), MHI (p < .01), and HSQ (p < .01), all with a large effect size (ES) (Cohen’s d = -0.76, 1.25, 0.93, -1.01, respectively). There was a significant correlation betweenchange scores on the MFIS and the MOS Pain (r = 0.532, p < .01), MHI (r = -0.647, p < .01), and subscales of the HSQ (ranging from r = -0.519, to -0.619, p < .01).Conclusions: MT as delivered in this study is a safe and beneficial intervention for management of fatigue and pain in people with MS. Decreasing fatigue and pain appears to correlate with improvement in quality of life, which is meaningfulfor people with MS who have a chronic disease resulting in long-term health care needs

Utilizing Chair Massage to Address One Woman’s Health in Rural Ghana West Africa: a Case Report
Background and Objectives: There is limited access to health care in rural Ghana and virtually no rehabilitative services available. This situation presents a unique opportunity to utilizechair massage in addressing women’s health in rural Ghana, particularly when it comes to muscle pain and fatigue from heavy labor. The objective of this case report is to determine the results of chair massage as a strategy to reduce neck, shoulder, and back pain, while increasing range of motion.Case Presentation: The patient is a 63-year old Ghanaian female, who was struck by a public transport van while carrying a 30–50 pound load on her head, two years prior. The accident resulted in a broken right humerus and soft tissue pain. A traditional medicine practitioner set the bone, however there was no post-accident rehabilitation available. At the time of referral, she presented complaints of shoulder, elbow, and wrist pain. In addition, she was unable to raise her right hand to hermouth for food intake.Results: The results of this case report include an increase in range of motion, as well as elimination of pain in the right shoulder, elbow, and hand. Visual assessments showed an approximate increase of ROM within the ranges of 45–65 degrees in the right arm, as well as 10–15 degrees in 4th and 5th fingers. There was also a decrease in muscle hypertonicity in the thoracic and cervical areas, and a profound increase in quality of life for the patient.Discussion: This case report illustrates how therapeutic chair massage was utilized to address a common health concern for one woman in rural Ghana. It also demonstrates that pre-existing musculoskeletal disorders and pain may be eliminated with massage intervention. Massage therapy may be important to ameliorating certain types of health problems in remote rural villages in low income countries
A Standardized, Evidence-Based Massage Therapy Program for Decentralized Elite Paracyclists: Creating the Model
Background and Purpose: Evidence suggests that para-athletes are injured more often than able-bodied athletes. The benefits of massage therapy for these disabled athletes are yet to be explored. This paper documents the process followed for creating a massage program for elite paracycling athletes with the goal to assess effects on recovery, rest, performance, and quality of life both on and off the bike.Setting: Massage therapists’ private practices throughout the United States.Participants: A United States paracycling team consisting of 9 elite athletes: 2 spinal cord injury, 2 lower limb amputation, 1 upper limb amputation, 1 transverse myelitis, 1 stroke, 1 traumatic brain injury, and 1 visually impaired.Design: The process used to develop a massage therapy program for paracyclists included meetings with athletes, coaching staff, team exercise physiologist, and sports massage therapists; peerreviewed literature was also consulted to address specific health conditions of para-athletes.Results: Team leadership and athletes identified needs for quicker recovery, better rest, and improved performance in elite paracyclists. This information was used to generate a conceptual model for massage protocols, and led to creation of the intake and exit questionnaires to assess patient health status and recovery. Forms also were created for a general health intake, therapist information, and a therapist’s SOAAP notes.Discussion: The conceptual model and questionnaires developed herein will help to operationalize an exploratory study investigating the feasibility of implementing a standardized massage therapy program for a decentralized elite paracycling team
Concussion Treatment Using Massage Techniques: a Case Study
Background: Concussion, its recognition, diagnosis, and treatment is a growing public health issue. Massage practitioners who specialize in rehabilitation deal with a variety of injury cases that involve concussion, including those incurred by falls, motor vehicle incidents, and sports injuries.Purpose: This case study presents a unique massage therapy approach to concussion trauma treatment.Participant: Male 23-year-old intramural soccer player diagnosed with postconcussion syndrome resulting from a fall.Intervention: Assessment and treatment were completed in two sessions of 45 minutes spaced two days apart. Massage therapy techniques were applied to injury areas by a Licensed Massage Practitioner.Results: Using the Balance Error Scoring System (BESS) and self-report, the outcome measures showed diminished concussion symptoms and regained ease in range of motion in the cervical area.Conclusion: Positive results for this case highlight the potential importance of massage therapy work to reduce headache, dizziness, and nausea in concussion recovery. In the presence of such outcomes, massage therapy may also have a supportive role in a person’s return to function after concussion

Perceptions of Massage Therapists Participating in a Randomized Controlled Trial
Background: Clinical practice and randomized trials often have disparate aims, despite involving similar interventions. Attitudes and expectancies of practitioners influence patient outcomes, and there is growing emphasis on optimizing provider–patient relationships. In this study, we evaluated the experiences of licensed massage therapists involved in a randomized controlled clinical trial using qualitative methodology.Methods: Seven massage therapists who were interventionists in a randomized controlled trial participated in structured interviews approximately 30 minutes in length. Interviews focused on their experiences and perceptions regarding aspects of the clinical trial, as well as recommendations for future trials. Transcribed interviews were analyzed for emergent topics and themes using standard qualitative methods.Results: Six themes emerged. Therapists discussed 1) promoting the profession of massage therapy through research, 2) mixed views on using standardized protocols, 3) challenges of sham interventions, 4) participant response to the sham intervention, 5) views on scheduling and compensation, and 6) unanticipated benefits of participating in research.Conclusions: Therapists largely appreciated the opportunity to promote massage through research. They demonstrated insight and understanding of the rationale for a clinical trial adhering to a standardized protocol. Evaluating the experiences and ideas of complementary and alternative medicine practitioners provides valuable insight that is relevant for the implementation and design of randomized trials
Comparing the Effects of Rest and Massage on Return to Homeostasis Following Submaximal Aerobic Exercise: a Case Study
Introduction: Postexercise massage can be used to help promote recovery from exercise on the cellular level, as well as systemically by increasing parasympathetic activity. No studies to date have been done to assess the effects of massage on postexercise metabolic changes, including excess postexercise oxygen consumption (EPOC). The purpose of this study was to compare the effects of massage recovery and resting recovery on a subject’s heart rate variability and selected metabolic effects following a submaximal treadmill exercise session. Methods: One healthy 24-year-old female subject performed 30 minutes of submaximal treadmill exercise prior to resting or massage recovery sessions. Metabolic data were collected throughout the exercise sessions and at three 10 minute intervals postexercise. Heart rate variability was evaluated for 10 minutes after each of two 30-minute recovery sessions, either resting or massage. Results: Heart rate returned to below resting levels (73 bpm) with 30 and 60 minutes of massage recovery (72 bpm and 63 bpm, respectively) compared to 30 and 60 minutes of resting recovery (77 bpm and 74 bpm, respectively). Heart rate variability data showed a more immediate shift to the parasympathetic state following 30 minutes of massage (1.152 LF/HF ratio) versus the 30-minute resting recovery (6.91 LF/HF ratio). It took 60 minutes of resting recovery to reach similar heart rate variability levels (1.216 LF/HF) found after 30 minutes of massage. Ventilations after 30 minutes of massage recovery averaged 7.1 bpm compared to 17.9 bpm after 30 minutes of resting recovery. Conclusions: No differences in EPOC were observed through either the resting or massage recovery based on the metabolic data collected. Massage was used to help the subject shift into parasympathetic activity more quickly than rest alone following a submaximal exercise session.
