Migrants during the COVID-19 Crisis in Japan

論文内容の要旨 
Aim: The COVID-19 pandemic has exacerbated the pre-existing vulnerabilities of 
migrants. In Japan, with the continuously increasing number of migrants, a concern for 
disaster nurses is to ensure migrants’ timely access to health information and services. 
This study aims to identify barriers to healthcare access for Nepalese migrants during the 
COVID-19 crisis in Japan as well as identify associated facilitating factors in a mixed 
data integration process.  
Methods:  This study used exploratory sequential mixed methods study design to 
delineate barriers and facilitators. A total of 11 focus group discussions (with 89 Nepalese 
migrants) were carried out for qualitative data and then an online survey (937 
respondents) was conducted to quantify the identified variables. Using the ‘building 
approach’ for integration, qualitative themes of barriers and facilitators were used to 
develop survey items. Integration of qualitative and quantitative phases was reported via 
a ‘joint display’.  
Results: From the survey, four significant barriers as predictors to healthcare access were 
confirmed: lack of COVID-19 hotline services; language barriers; unfamiliarity with the 
Japanese medical system, and fear of losing a job. As for significant facilitators that can 
predict better health access were found to be free COVID-19 medical care provided by 
the Japanese government and mutual aid. Non-significant but the most agreed barriers 
were lack of medical interpreters and price transparency in healthcare. Except for poor 
Japanese language ability and very-affected income, sociodemographic factors like 
gender, length of stay, immigration status, education, occupation, family status, and good 
English language ability did not predict their perceived healthcare access. Other 
qualitative variables unique to the current pandemic: temporary disruptions in the health 
care system, perceived denial, perceived delay in care, inefficacy of low-dose drugs were 
the least-agreed barriers in the quantitative survey. In contrast, health insurance and the 
COVID-19 information from Nepali health workers were the most agreed facilitators 
while the availability of medical interpreters and telehealth services were the least agreed 
facilitators perceived by the Nepalese migrants.  
Conclusion: Despite numerous barriers identified in the qualitative focus groups, the 
survey revealed that the Nepali migrants perceived access to healthcare in Japan during 
the COVID-19 crisis to be facile. Without mixed-methods data integration, identifying 
important but not-so-significant or least agreed barriers such as inefficacy of low-dose 
drugs and telehealth services would have been shadowed. The impact of low-dose drugs 
and the access to the global availability of free Nepali telehealth services during the 
COVID-19 crisis cannot be ignored, and both of these findings deserve further in-depth 
investigations. Similarly, as price transparency was the second-mostly agreed barrier, 
medical professionals should carefully consider consumers’ barriers in price shopping for 
healthcare. This study suggests an establishment of a crisis information hub by the Nepal 
embassy or concerned authorities, flexible policies for active deployment of Nepalese 
health workers and volunteers, accessible hotlines in the Nepali language, and 
incorporation of Nepali telehealth services in Japan.

審査結果の要旨 
問題意識：移住者は社会的弱者・災害時要配慮者と考えられている中、COVID-19 パンデミ
ックは、その脆弱性を悪化させた。日本では、移住者の数は継続的に増加しており、不確実
な社会の中で、移住者が健康に関する情報やサービスをタイムリーに受けられることを探
求することは、災害看護の重要な課題となった。 
目的： COVID-19 危機におけるネパール人移住者の保健医療アクセスの障壁となるものを
特定し、その障壁に対抗するための要因を明らかにすることを目的とした。 
研究方法：質的データの分析による障壁および促進要因の解明と、アンケート調査による定
量化の混合アプローチを行った。 フォーカス・グループ・ディスカッションの質的分析に
おいては、障壁と促進要因のテーマを特定し、オンライン調査によって量的にその根拠を示
すこととした。 
結果・考察：質的調査では、89 人の日本在住ネパール人を対象に 11 のフォーカス・グルー
プ・ディスカッションが行われ、経済的障壁、構造的障壁、認知的障壁、文化的障壁、心理
的障壁、法的障壁の 6 つのカテゴリーが上げられた。量的調査では、特定された要因を定量
化したオンラインフォームを作成、実施し、1234 の回答から有効回答 937 件を分析し、質
的データと総合的に考察、比較することで最も重要な障壁と促進要因を特定し、要因間の関
係を確認することを試みた。これらの障壁に対する促進要因としては、健康保険の加入、相
互扶助、医療通訳の利用可能性、遠隔医療サービス、ネパール人医療従事者やボランティア
からの COVID-19 情報などが挙げられた。 
結論：ネパール人移住者は医療へのアクセスに多様な種類の困難を認識しており、彼らを助
けた多くの促進要因もあった。最も重要な促進要因は、日本政府による無料の COVID-19
医療であるが、遠隔医療サービス、相互扶助、ネパール人医療従事者からの情報も役に立っ
た。本研究で得られた知見に基づいて、日本の柔軟な政策の下で多様な人的社会的資源を活
用することや、遠隔医療サービスを利用することは、ネパール人移住者を支援するための将
来の危機に対する重要なイノベーションとなり得ると結論づけた。 
研究成果の貢献について：防災の観点から見てもこのテーマに関して多くのデータ収集と
課題の本質に迫った研究は稀であると評価された。申請者による研究概要の発表に続いて
質疑があり、それらに的確に応答した。災害のたびに情報弱者となり災害時要配慮者という
課題が残り続けていながらグローバル化・デジタル化する社会において、今回の研究の発端
となったような ICT を利活用したリスクコミュニケーション、健康相談などの開発、健康
危機管理への貢献を促進することが急務であり、成果をグローバル化した日本での公衆衛
生や災害研究に還元することで、今後の地域防災と健康課題の改善に向けたソリューショ
ンが増えることと期待できる。 
本論文は、5 大学共同災害看護学専攻の博士論文の審査基準に則って審査したところ、博
士（看護学、災害看護グローバルリーダー）の学位を申請できるものと判断された。
