FITT Odyssey: A Scoping Review of Exercise Programs for Managing Rotator Cuff–Related Shoulder Pain
OBJECTIVE: To summarize the FITT (frequency, intensity, time, type), components of exercise programs included in randomized controlled trials (RCTs) that compared 2 or more programs for managing rotator cuff–related shoulder pain (RCRSP).
DESIGN: Scoping review.
LITERATURE SEARCH: Electronic searches were conducted up to May 2023.
STUDY SELECTION CRITERIA: RCTs comparing the effects of 2 or more types of exercise programs, differing in prescription according to the FITT principle, in people with RCRSP.
DATA SYNTHESIS: We extracted data from each trial report so that we could answer items 1 to 10 and 13 to 15 from the Consensus on Exercise Reporting Template (CERT). Descriptive analysis of the exercise programs was performed by summarizing and presenting the FITT characteristics, and other relevant CERT characteristics (material, provider, delivery, tailoring).
RESULTS: FITT characteristics from 46 exercise programs included in 22 trials were extracted. The exercise programs were divided into 4 categories (defined in accordance to the original authors’ description and proposed rationale): motor control (n = 8), scapula-focused (n = 7), eccentric (n = 8), and nonspecific exercise programs (n = 28). Five programs were allocated to 2 different categories. The different program types had similar parameters. Exercise programs frequency ranged from 2 to 7 times per week, dose ranged from 1 to 3 sets and 4 to 30 repetitions per sets, and exercise program duration ranged from 4 to 16 weeks.
CONCLUSION: There was considerable variability in the parameters used to prescribe exercises for RCRSP. Clinicians seeking guidance on FITT parameters derived from trials should do so cautiously because there was no one-size-fits-all approach

Propranolol As a Treatment Option for Chylous Effusions and Chylous Ascites in Fetuses and Neonates: A Systematic Review
OBJECTIVE
Chylous effusion and chylous ascites are rare but serious conditions that affect both fetuses and neonates. Previous studies have documented chylous effusions or chylous ascites treatment with medications as an adjunct to respiratory support and dietary modifications, but no formal recommendations have been made. New literature suggests propranolol as an effective and safe treatment option, though no randomized clinical studies have been published to date. This review aims to assess the efficacy and safety of propranolol in the treatment of chylous effusion and chylous ascites in fetuses and newborns from case reports.
METHODS
A comprehensive search of 10 databases and grey literature was completed. The inclusion criteria for articles were age at diagnosis less than 40 days old and case report/series. Articles were excluded if they were animal studies or not published in English.
RESULTS
Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, 4 articles were ultimately included in the study for a total of 10 reported cases. Propranolol administered to mother and neonates was effective in 100% of cases. The most common oral dose for mothers was 20 mg, 4 times daily, titrated to 40 mg, 4 times daily. The maximum dosage varied for administration orally to neonates, with the median being 3 mg/kg/day. Side effects, including bradycardia and transient hypoglycemia, were seen in 20% of the cases and resolved with dose adjustment.
CONCLUSION
Propranolol is a relatively effective and safe treatment option for chylous effusion and chylous ascites that can be administered prenatally or to neonates
Dietary Fatty Acids and Flight-Training Influence the Expression of the Eicosanoid Hormone Prostacyclin in Songbirds
Diet shifts can alter tissue fatty acid composition in birds, which is subsequently related to metabolic patterns. Eicosanoids, short-lived fatty acid-derived hormones, have been proposed to mediate these relationships but neither baseline concentrations nor the responses to diet and exercise have been measured in songbirds. We quantified a stable derivative of the vasodilatory eicosanoid prostacyclin in the plasma of male European Starlings (Sturnus vulgaris, N = 25) fed semisynthetic diets with either high (PUFA) or low (MUFA) amounts of n6 fatty acid precursors to prostacyclin. Plasma samples were taken from each bird before, immediately after, and two days following a 15-day flight-training regimen that a subset of birds (N = 17) underwent. We found elevated prostacyclin levels in flight-trained birds fed the PUFA diet compared to those fed the MUFA diet and a positive relationship between prostacyclin and body condition, indexed by fat score. Prostacyclin concentrations also significantly decreased at the final time point. These results are consistent with the proposed influences of precursor availability (i.e., dietary fatty acids) and regulatory feedback associated with exercise (i.e., fuel supply and inflammation), and suggest that prostacyclin may be an important mediator of dietary influence on songbird physiology
Relative Impact of Economic Policy Uncertainty and Mortgage Rates on the Supply of New Homes for Sale in the United States
This paper analyzes the changeable impact of economic policy uncertainty and mortgage rates on the supply of new homes for sale in the United States. The homes supply is proxied by the ratio of new homes for sale to new houses sold. The analysis utilizes monthly data from January 1985 to November 2023. The relationship is assessed using least square regression with second order autoregressive correction as well as Bai-Perron multiple breakpoint regression for structural breaks and Markov switching tests for state-dependency. The results demonstrate a significant direct correlation between economic policy uncertainty and housing supply, but only during the February 2009 – January 2013 subperiod, i.e. in the environment of very low interest rates. Responsiveness of housing supply to economic policy uncertainty is particularly high during episodes of financial crises. These results may provide valuable insights for policymakers and other stakeholders to make informed decisions regarding the sources of fluctuations in the housing market
Network Modulation at Stable States
Advances in microarray and sequencing technologies have made possible the interrogation of biological processes at increasing levels of complexity. The underlying biomolecular networks contain large numbers of nodes, yet interactions within the networks are not known precisely. In the absence of accurate models, one may inquire if it is possible to find relationships between the states of such networks under external changes, and in particular, if such relationships can be model-independent. In this paper we introduce a class of such relationships. The results are based on the observation that changes to the equilibrium state of a network due to an alteration in an external input are “small” compared to the change in the input, a phenomenon we refer to as network modulation. It relies on the stability of the state. One consequence of network modulation is that response surfaces containing expression profiles of different mutants of an organism are low-dimensional linear subspaces. As an example, the expression profile of a double-knockout mutant generally lies close to the plane defined by the expression profiles of the wild-type and those of the two single-knockout mutants. This assertion is validated using experimental data from the sleep-deprivation network of Drosophila and the oxygen-deprivation network of Escherichia coli. The linearity of response surfaces is crucial in the design of a feedback control algorithm to move the underlying network from an initial state to a prespecified target state
