Cancer and quality of life in spousal dyads: spillover in couples with and without cancer-related health problems
Abstract Purpose Poor health of one spouse can adversely influence the partner's health outcomes (Bspillover^). This study aimed to estimate quality of life spillover among spouses and to determine how presence of cancer influenced these effects. Methods We examined data on husband-wife dyads with cancer-related health problems, medical events, or disabilities (n=910) and matched comparison dyads from the 2004-2012 Medical Expenditures Panel Survey, a population-based survey of the USA. Mental and physical health-related quality of life and depressed mood were reported at two time points (T1 and T2, 11 months apart on average). Dyadic multilevel models evaluated the cross-lagged impact of HRQoL and depressed mood at T1 on spouses' HRQoL at T2, controlling for sociodemographics and health conditions. Results Small but statistically significant spillover was observed for mental and physical HRQoL among couples with cancer. Spillover occurred from both the spouse to the survivor and from survivor to spouse. Depressed mood, in particular, showed stronger spillover effects from the spouse to the survivor than the inverse. Similar effects were not observed in dyads without cancer. Conclusions Screening for and treating poor HRQoL and depressed mood concurrently in both cancer survivors, and their spouses may positively influence HRQoL outcomes. Future research is needed to further elucidate these findings and determine whether a concurrent approach to psychosocial care in survivors and their spouses may improve long-term outcomes

Understanding Care Integration from the Ground Up: Five Organizing Constructs that Shape Integrated Practices
Purpose: To provide empirical evidence on key organizing constructs shaping practical, real-world integration of behavior health and primary care to comprehensively address patients' medical, emotional, and behavioral health needs. Methods: In a comparative case study using an immersion-crystallization approach, a multidisciplinary team analyzed data from observations of practice operations, interviews, and surveys of practice members, and implementation diaries. Practices were drawn from 2 studies of practices attempting to integrate behavioral health and primary care: Advancing Care Together, a demonstration project of 11 practices located in Colorado, and the Integration Workforce Study, a study of 8 practices across the United States. Results: We identified 5 key organizing constructs influencing integration of primary care and behavioral health: 1) Integration REACH (the extent to which the integration program was delivered to the identified target population), 2) establishment of continuum of care pathways addressing the location of care across the range of patient's severity of illness, 3) approach to patient transitions: referrals or warm handoffs, 4) location of the integration workforce, and 5) participants' mental model for integration. These constructs intertwine within an organization's historic and social context to produce locally adapted approaches to integrating care. Contextual factors, particularly practice type, influenced whether specialty mental health and substance use services were colocated within an organization. Conclusion: Interaction among 5 organizing constructs and practice context produces diverse expressions of integrated care. These constructs provide a framework for understanding how primary care and behavioral health services can be integrated in routine practice. (J Am Board Fam Med 2015;28: S7-S20.
Inspection of specular and painted surfaces with centralized fusion techniques
Abstract This contribution presents new strategies to inspect specular and painted surfaces. Structures on such surfaces are normally only visible, if patterns of the environment are reflected in them. Thus, conventional approaches only yield a little information gain from a single measurement. In our approach, different intensity patterns are systematically generated in the environment of the surface such that these are reflected in the surface and captured by a camera. Following, the recorded images are processed simultaneously by a centralized fusion technique. Since the fused information is closer to the source, a better exploitation of the raw data is achieved. The fusion problem is formulated with an energy function. Its minimization yields the desired surface defects. The methodology is illustrated with two case studies: the analysis of machined surfaces, and the inspection of painted free-form surfaces. In both cases, a reliable yet cost-efficient inspection is attained matching the needs of industry
Electric Field Distribution Perpendicular to the Surface of Mid-infrared Antennas
Abstract The electric field in the normal direction was experimentally investigated by using mid-infrared antennas that were formed on a thin Al2O3 layer/Si substrate. The Al2O3 layer could be as thin as a single nm-order employing the atomic layer deposition, and was varied from 2 nm to 50 nm. The field distribution was estimated by observing the reflectivity change. It was found that the electric field decreased rapidly until a 6 nm depth from the antenna plane, and that the degree of attenuation became relaxed in the deeper region
