Reexamining the Impact of Medicaid Expansion in a Post-Affordable Care Act Environment from a Critical Race Perspective
Get PDFThe passage of the Patient Protection and Affordable Care Act (ACA) in 2010 drastically transformed the health care system in the United States. This paper examines the factors influencing state decisions relative to Medicaid expansion in a post-ACA environment through the lens of Critical Race Theory. This study incorporates economic, geographic and health variables into a model of post-ACA-Medicaid decision-making by using logistic regression to examine State Medicaid expansion from 2010 to 2014. The size of the minority population in state, tobacco use and southern distinctiveness are significant predictors of decision making relative to Medicaid expansion. Findings support that racialized decision-making, particularly in the South, continue to play a significant role in state-level policymaking
Determining the Level of Cultural Competence of College Police Departments: A Study of Three Different Campuses
Get PDFThis research examines the understanding that police chiefs on three college campuses have about cultural competence and determines how organizationally integrated cultural competence is in their police departments. This article uses semi-structured interviews and surveys to determine the levels and organizational integration of cultural competence. The findings indicate that there are varying degrees of cultural competence across the three departments. This was primarily based on three factors: the level of understanding of cultural competence, the emphasis placed on promoting cultural competence by departmental leadership, and administrative or legal barriers. Given the complexity of cultural competence, the article concludes by offering suggestions for researchers to use multiple tools in assessing cultural competence of service delivery organizations
The Flipped Class: Experiential Learning Manifested
Get PDFThrough the “flipped class,” students are introduced to new material outside the classroom before the traditional in-class lecture occurs. Ideally, flipping the class challenges students to engage in the lower levels cognitive work (remembering and understanding) outside of class. Students, then, are free to focus on higher order cognitive work (application, analysis, synthesis, and/or evaluation) in class, where they have the support of their peers and the instructor. To drive the requisite higher order cognitive work inside the classroom, it is important that in-class teaching and learning methodologies extend beyond simple information dissemination. Experiential learning, the process of transforming information into knowledge is critical to the success of the “flipped class.” As such, the “flipped class” must challenge students to find, interpret, communicate and turn knowledge into action. Peer-instruction is an important component of the process. Researchers suggest that experiential pedagogy similar to the “flipped class” have the potential to effectively engage today’s college students, aptly called “GenMe’ers,” because they expect to learn from each other rather than a professor whose authority they don’t respect, anyway

Optimal piperacillin-tazobactam dosing strategies against extended-spectrum-lactamase-producing Enterobacteriaceae
No full textPiperacillin-tazobactam has been proposed as an alternative to carbapenems for the treatment of infections caused by extended-spectrum-lactamase (ESBL)-producing Enterobacteriaceae. However, limited understanding of optimal dosing strategies for this combination may curtail its utility. In this study, we correlated various exposures of piperacillin-tazobactam to efficacy, using a modified pharmacokinetic/pharmacodynamic index. Using a clinical Klebsiella pneumoniae isolate expressing CTX-M-15, piperacillin MIC values were determined with increasing tazobactam concentrations and fitted to a sigmoid inhibitory maximum effect (E max ) model. A hollow-fiber infection model (HFIM) was used to evaluate the efficacy of escalating tazobactam dosing with a fixed piperacillin exposure. Simulated drug concentrations from the HFIM were incorporated in the E max model to determine the percentage of free time above instantaneous MIC (%fTMICi) associated with each experimental exposure. The target %fTMICi associated with growth suppression was prospectively validated using an SHV-12-producing isolate of Escherichia coli and 2 other CTX-M-15-producing K. pneumoniae isolates. Based on our reference isolate, piperacillin-tazobactam exposures of %fTMICi of 55.1% were associated with growth suppression. Despite underlying differences, these findings were consistent with prospective observations in 3 other clinical isolates. Our modeling approach can be applied relatively easily in the clinical setting, and it appeared to be robust in predicting the effectiveness of various piperacillin-tazobactam exposures. This modified pharmacokinetic/pharmacodynamic index could be used to characterize response to other -lactam/-lactamase inhibitor combinations
