The Courts Encourage It, So We Do It: Police Excessive Force Against Minorities
A continuing problem of American criminal justice is the frequency of violence directed against minorities by police. One cause of this is the leniency of courts, especially the Supreme Court, toward police officers accused of using excessive force. This position paper reviews the legal decisions that have granted a large degree of immunity to police in their use of force. It describes three doctrines: Qualified immunity, in which a public official is shielded from a lawsuit unless his/her actions violated a “clearly established” constitutional right; summary judgment, a judicial decision based on paper evidence without testimony; and limitations on respondeat superior liability, by which a municipality is largely excused from the principle that an employer is responsible for the torts of employees. For each doctrine, its original intent is described, and the history of judicial decisions is reviewed that led to the unintended consequence of excusing excessive police use of force
Barber Shops, Salons, and Spas: The Complexity – and Simplicity – Of Implementing Outreach and Enrollment Contracts Under The Affordable Care Act
This article examines the implementation of The Patient Protection and Affordable Care Act (ACA) with a particular focus on the states’ contracted outreach and enrollment services. Having collected administrative and perceptual data from key informants, we examined outreach and enrollment contracts in six states that have reduced the uninsured rate by at least 20% of ACA-eligible population. Our findings suggest that the complexity of the law and its structure, as well as the characteristics of contracting were, in effect, “drowned out” by clear patterns of highly collaborative implementation that involved extensive chains of diverse outreach/enrollment actors. These networks – ranging from experienced health advocacy organizations to community members’ recruiting in local churches, schools and barbershops – were able to put into effect comparatively simple systems with clear, shared goals among on-the-ground actors who broke through the implementation complexity, thereby creating new capacity and achieving significant enrollment gains
Determining access to financial services among the young and poor in American communities
This paper investigates access to financial services in American communities. It analyzes the access gap that the young and poor face relative to more economically advantaged and older groups in society. The paper uses data from the World Bank’s Global Financial Inclusion survey to create a multi-dimensional index of financial access and estimates a menu of quantitative models to gauge financial inclusion. Results show that young and poor people face substantial gaps in access to financial services when compared with older people, higher income individuals, more educated people, and individuals who are less vulnerable to future economic shocks. These findings expand understanding of the challenges that young and poor people face in U.S. financial markets and provide insights that can inform financial and social policies for greater financial inclusion and more sustainable communities
Pre-clinical pharmacokinetics, tissue distribution and physicochemical studies of CLBQ14, a novel methionine aminopeptidase inhibitor for the treatment of infectious diseases
Introduction: CLBQ14, a derivative of 8-hydroxyquinoline, exerts its chemotherapeutic effect by inhibiting methionine aminopeptidase (MetAP), the enzyme responsible for the post-translational modification of several proteins and polypeptides. MetAP is a novel target for infectious diseases. CLBQ14 is selective and highly potent against replicating and latent Mycobacterium tuberculosis making it an appealing lead for further development. Methods: The physicochemical properties (solubility, pH stability and lipophilicity), in vitro plasma stability and metabolism, pre-clinical pharmacokinetics, plasma protein binding and tissue distribution of CLBQ14 in adult male Sprague-Dawley rats were characterized. Results: At room temperature, CLBQ14 is practically insoluble in water (\u3c0.07 mg/mL) but freely soluble in dimethyl acetamide (\u3e80 mg/mL); it has a log P value of 3.03 ± 0.04. CLBQ14 exhibits an inverse Z-shaped pH decomposition profile; it is stable at acidic pH but is degraded at a faster rate at basic pH. It is highly bound to plasma proteins (\u3e91%), does not partition to red blood cells (B/P ratio: 0.83 ± 0.03), and is stable in mouse, rat, monkey and human plasma. CLBQ14 exhibited a bi-exponential pharmacokinetics after intravenous administration in rats, bioavailability of 39.4 and 90.0%, respectively from oral and subcutaneous route. We observed a good correlation between predicted and observed rat clearance, 1.90 ± 0.17 L/kg/h and 1.67 ± 0.08 L/kg/h, respectively. Human hepatic clearance predicted from microsomal stability data and from the single species scaling were 0.80 L/hr/kg and 0.69 L/h/kg, respectively. CLBQ14 is extensively distributed in rats; following a 5 mg/kg intravenous administration, lowest and highest concentrations of 15.6 ± 4.20 ng/g of heart and 405.9 ± 77.11 ng/g of kidneys, respectively, were observed. In vitro CYP reaction phenotyping demonstrates that CLBQ14 is metabolized primarily by CYP 1A2. Conclusion: CLBQ14 possess appealing qualities of a drug candidate. The studies reported herein are imperative to the development of CLBQ14 as a new chemical entity for infectious diseases
Hypotensive and antihypertensive effects of an aqueous extract from Guinep fruit (Melicoccus bijugatus Jacq) in rats
Melicoccus bijugatus Jacq (Mb) has been reported to have cardiovascular modulatory effects. In this study, we evaluated the antihypertensive effects and mechanism of action of Mb on NG-Nitro-l-arginine Methyl Ester (l-NAME) and Deoxycorticosterone Acetate (DOCA) rat models. Aqueous extract of Mb fruit (100 mg/kg) was administered for 6 weeks to rats by gavage and blood pressure was recorded. Effects of the extract on vascular reactivity was evaluated using isolated organ baths, and tissues were collected for biochemical and histological analysis. The systolic blood pressure (SBP), diastolic blood pressure (DBP) and mean arterial pressure (MAP) were significantly (P \u3c 0.05) reduced with extract (100 mg/kg) administration and treatment compared to the hypertensive models. Mb (100 µg/mL) reduced the vascular contractility induced by phenylephrine (PE), and caused a dose-dependent relaxation of PE-induced contraction of aortic vascular rings. The vasorelaxation properties seemed to be endothelium dependent, as well as nitric oxide (NO) and guanylyl cyclase, but not prostaglandin dependent. Histomicrograph of transverse sections of the ventricles from the Mb group did not show abnormalities. The extract significantly (P \u3c 0.05) reduced an l-NAME induced elevation of cardiac output and Creatine Kinase Muscle-Brain (CKMB), but had no significant impact on the activities of arylamine N-acetyltransferase. In conclusion, Mb significantly decreased blood pressure in hypertensive models. The extract possesses the ability to induce endothelium dependent vasodilation, which is dependent on guanylyl cyclase but not prostaglandins
Trends and associated characteristics for Chagas disease among women of reproductive age in the United States, 2002 to 2017
Background: American trypanosomiasis, commonly referred to as Chagas disease, is caused by a single cell protozoan known as Trypanosoma cruzi (T. cruzi). Although those affected are mainly in Latin America, Chagas has been detected in the United States (US), Canada and in many European countries due to migration. Few studies have explored the epidemiology of Chagas within the US or changes in disease burden over the past decade. The objective of this study was to explore the trends and associated characteristics for Chagas disease among hospitalized women of reproductive age in the US. Methods: We analyzed admissions data including socio-demographic and hospital characteristics for inpatient hospitalization for women of reproductive age (15–49 years) in the US from 2002 through 2017. We employed Joinpoint regression analysis to determine trends in the prevalence of Chagas disease over this period. Results: A total of 487 hospitalizations of Chagas disease were identified, corresponding to 3.7 per million hospitalizations over the study period. The rate statistically increased from 1.6 per million in 2002 to 7.6 per million hospitalizations in 2017. Chagas was most prevalent among older women, Hispanics and those in the highest zip income bracket. The in-hospital mortality rate was about 10 times greater among women with Chagas compared to those without the condition (3.1% versus 0.3%), and the condition tended to be clustered in women treated at large, urban teaching hospitals in the Northeastern region of the US. Conclusion: Chagas disease diagnosis appears to be increasing among hospitalized women of reproductive age in the US with a 10-fold elevated risk of mortality
Highlights of clinical practice guideline for the management of community-acquired pneumonia: 2019 update by the american thoracic society and infectious diseases society of America
Pneumonia is the leading cause of morbidity and mortality in the United States. The American Thoracic Society and The Infectious Diseases Society of America recently published updated guidelines for the diagnosis and treatment of community-acquired pneumonia. Initial antibiotic therapy should cover for Streptococcus pneumoniae, Haemophilus influenzae, and other gram-negative bacilli. Patients who have risk factors for the development of antibiotic resistant pathogens, such as methicillin-resistant Staphylococcus aureus and Pseudomonas aeruginosa should have appropriate empiric coverage for these pathogens. The recommended duration of treatment of community-acquired pneumonia is a minimum of 5 days in conjunction with clinical improvement, which has remained consistent from previous guidelines
An image processing method for extraction of the stresswave reflection period
The stress wave reflection method is widely used in the detection of structure size and integrity due to its advantages of low environmental impact and convenience. The detection accuracy depends on the accurate extraction of the stress wave reflection period. The traditional peak-peak method (PPM) measures the time interval between the first two peaks of the reflected waves to extract the reflection period. However, human interpretation is not avoidable for identifying the weak peak due to signal energy leaks into the surrounding environment. This paper proposes an algorithm for automatic extraction of the stress wave reflection period based on image processing to avoid human interference. The image is the short-time Fourier transform (STFT) spectrogram of the reflected wave signal after applying wavelet denoising and quadratic self-correlation operations. The edge detection method of image processing is used to extract the periodically occurring trough in the image. Graying and filtering are performed to eliminate interference. The frequency of the trough distribution is calculated by using the fast Fourier transform (FFT), and then the reflection period of the stress wave is obtained. The effectiveness and accuracy of the proposed method are validated by measuring the different lengths of two buried metal piles in soil. Comparing with the existing method of extracting the stress wave reflection period, this new algorithm comprehensively utilizes the time-frequency domain information of the stress wave reflection signal
