    Taiwan’s Experience in Managing COVID-19 and the Impact on Medical Students : An ounce of prevention is worth a pound of cure.

      In the past several months, the globe was overwhelmed by the novel coronavirus (COVID-19) pandemic. As rising number of documented cases and mortality toll continued to be reported, it is imperative to reexamine the current healthcare system as well as consider the repercussion on medical society. The condition in Taiwan, although expected critical regarding close intertwine with epicenter, was relatively stable thus far given multiple efforts paid, suggesting the experiences here may serve as a reference for other countries. In this review, the progression of COVID-19 was illustrated. Additionally, four aspects of implemented measures were introduced, and their respective implications were discussed. First, national team production and name-based allocation, followed by map-directed and online pre-order system, ensured the public to obtain enough surgical masks. Second, constant advocacy of the quarantine policies and personal hygiene, in conjunction with regular press conference by officials, accomplished message transparency and citizens’ literacy upon the situation. Third, establishment of the traffic control bundle successfully antagonized nosocomial infection and prevented subsequent clusters. Forth, biomedical research not only propelled the elucidation of the COVID-19 pathogenesis but also facilitated the development of potential medications and even vaccination. Furthermore, the impact on medical education was delineated. Reorganization of training curriculum, distance learning, and the practice of virtual meeting reshaped the landscape of clinical training. In conclusion, through conscientious appraisal on how we confronted COVID-19 will the community be better prepared for the next pandemic

    Antimicrobial Photodynamic Therapy as an Alternative Therapeutic Modality for Drug Resistant Tuberculosis

    High tuberculosis (TB) disease burden has been a perennial problem for many countries for centuries. As of 2014, there were approximately 9.6 million incident cases of TB worldwide, 1.5 million of which, resulted in deaths. Established treatment regimens for Tuberculosis such as the HRZE regimen (Isoniazid, Rifampicin, Ethambutol and Pyrazinamide), have proven to be effective in eradicating the disease even in developing countries. However, new strains of drug resistant Tuberculosis have been discovered, they are the Multi-Drug Resistant (MDR) and Extreme Drug Resistant (XDR) Tuberculosis. These strains have shown resistance to even second line drug regimens and their incidence has progressively increased since 2003. In search of a new alternative mode of treatment, researchers have discovered a new technology called antimicrobial Photodynamic Therapy (aPDT), which is already in use to treat cancerous lesions to date and shows promise for application with Tuberculosis patients

    Comprehensive Assessment of Risk Factors Associated with Methicillin-Resistant Staphylococcus aureus (MRSA) Infection in Asia: A Systematic Review

    Background Antibiotic resistance persists as a global health challenge, among which methicillin-resistant Staphylococcus aureus (MRSA) is the most common and serious threat. Although certain control strategies have been implemented, the incidence and prevalence of MRSA remain high, especially in Asia Objective To analyze the risk factors associated with MRSA infection in Asia Methods A systematic review was conducted through PubMed, Scopus, and EBSCOhost, searching for observational studies which analyze the risk factors associated with MRSA infection in Asia. Studies selected were reviewed and assessed with STROBE’s criteria. Results Sixteen observational studies were included, with a total of 10,253 subjects, consisting of 4 cohort studies, 4 cross-sectional studies, and 8 case-control studies. We find that certain social behaviors, such as sharing of personal items, limited daily activities and exposure to post-surgery individuals increases the risk of MRSA infection. Antibiotics use, especially when used extensively, is also found to elevate the risk. Furthermore, invasive medical procedures ease the entry of MRSA into the body, thus increasing risk of infection. Conclusion The knowledge of these factors is hoped to help in establishing guidelines for infection prevention and control strategies, helping to halt the rise in antibiotics resistance, as well as reducing the prevalence of MRSA infection worldwide

    Targeted Cellular Delivery of CRISPR/Cas9 with RNA Aptamer-Cationic Liposome As A Potential Therapy For Metastatic Prostate Cancer

    Prostate cancer is the sixth deadliest cancer worldwide due to its metastatic ability, rendering conventional treatment futile. Editing of target oncogenes with the help of CRISPR/Cas9 is gaining recognition as promising treatment modality. However, implementation of CRISPR/Cas9 in clinical setting remains difficult due to challenges in delivery to target tissue. Therefore, this literature review focuses on exploring nanocarrier suitable for CRISPR/Cas9 delivery. The method used to assemble this literature review is by performing a comprehensive literature search with corresponding keywords. Results shows that targeted delivery system of CRISPR/Cas9 using cationic liposome conjugated with RNA aptamer is an excellent candidate. Modification of cationic liposome, a well-established nanocarrier, with PEG and RNA aptamer as targeting ligand increases specificity, efficiency, stability, and circulation time in bloodstream. However, further research is still needed to confirm these findings for future implementation of this technology as prostate cancer therapy.     Modulation of Gut Microbiota through Mediterranean Diet as a New Insight for the Alzheimer's Disease Therapy

    Despite the fact that Alzheimer’s disease (AD) is one of the most common form of dementia, its underlying pathogenesis has not yet fully known. As it is an intricate and multifactorial disease, finding a potential therapy intercepting its pathological process is still out of reach. With an increase in evidence of the success of gut microbiota modulation as a therapy to other diseases, one should consider the same possibility in AD. Therefore, this literature review focuses on exploring the possibility of dietary intervention as an innovative therapy for AD by modulating the gut microbiota composition, based on recent researches. The method used to assemble this literature review is by performing a comprehensive literature search with corresponding keywords. The result is that Mediterranean dietary intervention, characterized by the high intake of omega-3 polyunsaturated fatty acids, antioxidant nutrients, and vitamins, has shown to be beneficial for brain functions development, targeting the gut-brain axis. However, a higher level of evidence-based research is still needed to more confirm the findings in hope for a future implementation of this new insight as a therapy for AD and to unfold the gut microbiota role in it.     IMPACT OF INTERNET GUIDED SELF DIAGNOSIS ON DOCTOR PATIENT RELATIONSHIP- A QUALITATIVE STUDY

    INTRODUCTION The patient-physician relationship has been noted to be second only to family relationships in terms of importance. This relationship is based on trust of the patient on his physician. The growth of the Internet has enabled the public to more readily access information about health and disease. Patients use of the Internet in searching for and gathering health information is growing and has now become somewhat commonplace. Apart from many other issues created the central issue is the issue of trust on the doctor. An earlier study done on the patient compliance related to internet guided self-diagnosis revealed online self-diagnosis has significant effects on patient compliance. So through this qualitative study we hope to explore all aspects of the internet related self-diagnosis in medical practice OBJECTIVES The OBJECTIVE OF THE RESEARCH IS TO STUDY THE IMPACT OF INTERNET GUIDED SELF DIAGNOSIS ON DOCTOR PATIENT RELATIONSHIP.THE OBJECTIVES ARE STUDIED UNDER THE HEADINGS OF- DIAGNOSISREACTIONPROBLEMS IN FURTHER MANAGEMENTCOMPLIANCE TO PRESCRIPTIONPERCEPTIONS/ FEELINGSANY RECOMMENDATIONS MATERIALS AND METHODS The sampling frame consisted of 20 participating Doctors. The study participants were asked to fill a structured survey form, which consisted of ideal, interpretative and leading questions focussed on recording experience, opinions and perceptions. The recurrent views were identified and these similar views were used to answer the research questions. OBSERVATION AND RESULTS From this study it was found that all the study participants have encountered such a patient who came with an internet guided self-diagnosis. About three fourth of these doctors said that the self-diagnosis was incorrect. Further, according to the physicians majority of them said this lead to problems in further management. But surprisingly most of the physicians said the patients were compliant to their doctor’s prescription even after doing the self-diagnosis. Our qualitative study also reviled a vast array of perceptions about the topic of study, these included absurdness, frustration, feeling irritated and many others. CONCLUSION Thus, from our study it can be said in conclusion that the internet guided self-diagnosis does not help the patient in any way instead in addition to causing problems in management also causes the straining of the doctor patient relationship. But still upon being counselled patient being compliant to the doctor reveals that the patients still trust the doctor more than the internet which is accordance with an earlier quantitative research which we studied in our study of literature. Thus, from our study it can be said in conclusion that the internet guided self-diagnosis does not help the patient in any way instead in addition to causing problems in management also causes the straining of the doctor patient relationship.     Diabetes and obesity represent an underestimated threat to the health ofpopulation worldwide. During the past few decades, we have witnessed an epidemic of typetwo diabetes and obesity. Currently, at least 300 million adults are obese and 422 millionadults are living with diabetes. Moreover, the prevalence is expected to increase worldwide,particularly in low-income and middle-income countries; which imposes a serious health andeconomic concerns in every region. There is a strong association between diabetes and obesity since obesity is one ofthe most notable risk factor of type two diabetes. Additionally, both diabetes and obesitygive rise to serious premature illnesses and deaths in many countries and also imposefinancial burdens to the society. As health practitioners, we are confronted with multi-layered treatment issues andcomplications involved in the care of diabetes and obesity. To meet the challenge, newapproaches in management and prevention of obesity, diabetes and their associatedcomplications are urgently needed. This conference will serve as a platform to createawareness amongst medical students, exchange potential solutions and, hopefully, make animpact on the future healthcare system
