Understanding Barriers to Access and Opportunities for Telemedicine in Underserved Urban Communities
Loss to follow-up and decreased access to timely care have health consequences that often lead to increased emergency service utilization for benign, treatable, and preventable conditions. As such, significant medical intervention is necessary. Specifically in the District of Columbia (DC), the health of Medicaid beneficiaries of Wards 7 and 8, overwhelmingly underserved communities, necessitates alternative means for more accessible, convenient, and consistent care, which can be achieved by integrating telemedicine services into current care modalities. Utilizing survey responses from the pre-COVID-19 pandemic, we identified a need for a community-based telemedicine service centered at the Pennsylvania Avenue Baptist Church, designing a threefold project to build knowledge, capacity, and to test feasibility. To build knowledge, we assessed demographics, telemedicine knowledge, current and past telemedicine use, health care utilization, access and improvement issues, telemedicine layperson utilization, and opinions and desire for telemedicine services. A total of 223 responses were gathered from residents residing in Wards 7 and 8 as well as in the neighboring DC communities. An array of data results demonstrates that integrating telemedicine services into community care centers ultimately satisfies identifiable needs in DC\u27s low-income communities for mental health resources, better coordination of care, more convenient and culturally attuned care, and greater health literacy. Integrating telemedicine services into community care centers satisfies the need to decrease costs of care and improve the access and quality of care. Effective evaluation strategies and outcome measures that indicate benefits beyond cost savings could provide useful information on how to integrate sustainable telehealth systems in health care delivery models

Morbidity prediction in conservatively managed rib fracture patients
PURPOSE: Rib fractures, common in blunt chest trauma, affect 10% of trauma patients and are linked to increased pulmonary morbidity and mortality. This study applies machine learning to identify predictors of complications in conservatively managed rib fracture patients. METHODS: Data from the 2013-2021 American College of Surgeons\u27 Trauma Quality Improvement Program included adults (≥ 18 years) with isolated thoracic injury from blunt trauma and conservatively managed rib fractures. Variables included demographics, comorbidities, injury severity, injury patterns, admission vitals, and complications. The permutation importance method identified top predictors of in-hospital complications. RESULTS: Of 321,355 rib fracture patients, 183,303 (57.0%) had isolated rib fractures. The five primary predictors of complications in all rib fracture patients were age, Glasgow Coma Scale (GCS) on admission, Revised Cardiac Risk Index (RCRI), chronic obstructive pulmonary disease (COPD), and alcohol use disorder. For isolated rib fracture patients, the same predictors applied but in the order: age, RCRI, GCS, COPD, and alcohol use disorder. A logistic regression model using these predictors showed acceptable discriminative capacity for complications in the full cohort [AUC (95% CI): 0.72 (0.71-0.72)] and isolated rib fracture patients [AUC (95% CI): 0.72 (0.71-0.73)]. CONCLUSION: Cardiovascular risk, age, and level of consciousness on admission are key predictors of complications in conservatively managed rib fracture patients. Though complication rates remain low overall, elderly patients with multiple cardiovascular risk factors face a heightened risk of deterioration
Perspectives on the current diagnostic and treatment paradigms in secondary hemophagocytic lymphohistiocytosis (HLH)
Improved awareness of hemophagocytic lymphohistiocytosis (HLH) among clinicians has led to an increase in its diagnosis. Often diagnosis is made based on the HLH- 2004 criteria. While these criteria have considerable strengths, they lack specificity and may be fulfilled in the setting of many pro-inflammatory disorders. Genetic defects affecting cellular cytotoxicity cause familial (primary) HLH. On the other hand, secondary HLH is more a pathophysiologic process common to many conditions, rather than a singular disease entity. Improved genetic, immunologic, and functional testing have changed not only the way we diagnose HLH, but also how we treat it. In 2004, there were few active agents and regimens. In 2024, there are multiple safe and effective targeted therapies. We have begun to understand that routine and immediate use of etoposide-based therapy in secondary HLH is likely not appropriate, and emerging cytokine-directed therapies may be more rational interventions. Moreover, it is recognized that identifying and treating the driver of secondary HLH is at least as important as treating the cytokine storm and immune dysregulation. Unfortunately, over-reliance on, and narrow interpretation of, the HLH- 2004 criteria can lead to overdiagnosis, misdiagnosis, and unneeded exposure to drugs that can be harmful. It is important that clinicians understand the limitations of the current diagnostic paradigms for secondary HLH, and the shortcomings of reflexive use of etoposide-based therapy. Herein we will discuss the pros and cons of the current paradigm for the recognition, diagnosis, and treatment of secondary HLH
