    Depressione – un sistema dinamico complesso

    Der vorliegende Beitrag befasst sich mit neueren Befunden zur Depressionsforschung, vor allem zur psychotherapeutischen Behandlung von Depression. Auf Wunsch der Redaktion sollte damit auch eine Synopse der wissenschaftlichen und praktischen Entwicklungen gegeben werden, die am Institut für Synergetik und Psychotherapieforschung der Paracelsus Medizinischen Privatuniversität Salzburg und der kooperierenden Forschergruppen stattfinden. Ein Schwerpunkt liegt in der Personalisierung der Psychotherapie und betrifft sowohl neuere Methoden der Fallkonzeption (z.B. idiografische Systemmodellierung und deren computerbasierte Visualisierung) als auch Verfahren des hochfrequenten Prozessmonitorings und Prozessfeedbacks. Hierfür wurde das Internetund App-basierte Synergetische Navigationssystem (SNS) entwickelt und erprobt. Die Verläufe der Psychotherapie depressiver Patient*innen zeigen diskontinuierlich Sprünge dynamischer Muster (sogenannte Ordnungsübergänge), die von einer erhöhten kritischen Instabilität (dynamische Komplexität) und zeitlich begrenzter Synchronisation der Items eines Prozessbogens eingeleitet werden. Erhöhte kritische Instabilität ist zudem prädiktiv für besseren Therapieerfolg. In einer Studie an komorbid depressiven Patient*innen mit Zwangsstörungen wurde deutlich, dass sich neben klinischen Verbesserungen auch die funktionelle Konnektivitätsdynamik neuronaler Netze verändert. Wiederholte fMRT-Scans zeigten, dass Flexibilität und Fluidität der Neurodynamik mit der Psychopathologie (z.B. Depressionsintensität, BDI) negativ assoziiert sind, aber im Verlauf von Psychotherapie zunehmen. Abschliessend erfolgt eine Einschätzung von Möglichkeiten, nichtinvasive Neurostimulation mit Psychotherapie zu kombinieren.This article focuses on some new developments in the treatment of Major Depressive Disorder with the question of how to optimize psychotherapeutic procedures. Following the intention of the editor of this journal, a short synopsis is given on the scientific and practical developments at the Institute of Synergetics and Psychotherapy Research at the Paracelsus Medical University Salzburg. One focus is on the personalization of psychotherapy, which concerns new methods of case formulation (e.g., Idiographic System Modelling and computer-based tools for its visualization) and technologies for high-frequency process-monitoring and -feedback. For this reason, we developed and validated the Synergetic Navigation System (SNS) which is an Internetand App-based application for data collection, data analysis, and visualization of change dynamics and outcome. Based on this methodology, it became evident that the psychotherapeutic processes of depressive disorders are characterized by discontinuous patterns, so called «order transitions». The precursors of these discontinuous transitions are critical fluctuations represented by increased dynamic complexity, and an increase in the synchronization of the inter-item dynamics of the therapy process questionnaire. The amount of critical instabilities occurring before order transitions revealed to be predictive for treatment outcome. Beside clinical improvement also neural connectivity patterns are changing by psychotherapy, as could be shown in repeated fMRI scans during inpatient treatment of comorbid depressive patients with obsessive compulsive disorder. The functional connectivity dynamics of 119 brain regions was assessed during the resting state periods of the repeated scans, showing that neural rigidity (a loss of flexibility and fluidity) is associated with psychopathology (e.g., depression, BDI), but flexibility increases during psychotherapy. Finally, the option of combining psychotherapy with noninvasive neurostimulation will be discussed.il presente studio si focalizza sulle ultime scoperte della ricerca sulla depressione, con particolare riguardo al trattamento di tale patologia in ambito psicoterapeutico. Su richiesta della redazione, verranno passati in rassegna anche gli sviluppi scientifici e pratici registrati presso l’Institut für Synergetik und Psychotherapieforschung (Istituto di sinergetica e ricerca psicoterapeutica) della Paracelsus Medizinischen Privatuniversität (Università medica privata Paracelsus) di Salisburgo e dei gruppi di ricercatori che vi collaborano. Uno dei temi principali è rappresentato dalla personalizzazione della psicoterapia e riguarda sia i più recenti metodi di concettualizzazione del caso del paziente (ad es. modellazione sistemica idiografica e relativa visualizzazione computerizzata) sia la metodologia di monitoraggio (ad alta frequenza) e il feedback dei processi neuronali che intervengono durante la terapia. A questo riguardo è stato sviluppato e testato un sistema di navigazione sinergetico basato su app e internet (SNS). Lo svolgimento delle sedute di psicoterapia di pazienti con depressione ha mostrato cambiamenti discontinui dei modelli dinamici (i cosiddetti passaggi ad altro stato di ordine), che sono generati da un’instabilità critica elevata (complessità dinamica) e da una sincronizzazione limitata nel tempo degli elementi dello schema del processo terapeutico. Un’elevata instabilità critica è inoltre predittiva di migliori risultati nel corso della terapia. In uno studio svolto su pazienti depressi e inoltre affetti da disturbi compulsivi, oltre a miglioramenti clinici, si sono evidenziati anche cambiamenti nella dinamica di connettività delle reti neuronali. Ripetuti esami effettuati tramite fRMN hanno evidenziato l’associazione negativa della flessibilità e della fluidità delle dinamiche neuronali con la psicopatologia (ad es. intensità della depressione, BDI), tuttavia con un loro aumento nel corso della psicoterapia. Infine segue una valutazione delle possibilità di combinare la neurostimolazione non invasiva alla psicoterapia

    Der Verlust des Arbeitsplatzes geht mit erheblichen gesundheitlichen Folgen einher, Arbeitslose sind von Depressionen häufiger betroffen als Erwerbstätige. Der Beitrag geht der Frage nach, inwiefern der Zusammenhang zwischen Arbeitslosigkeitserfahrung und Depression durch soziale Unterstützung vermittelt wird. Dazu werden bevölkerungsweite Querschnittsdaten des Zusatzmoduls «Psychische Gesundheit» der Studie zur Gesundheit Erwachsener in Deutschland (DEGS1-MH, 2008–2011) verwendet und depressive Störungen anhand der DSM-IV-Kriterien des psychiatrischen Dia­gnoseinterviews «Composite International Diagnostic Interview» (DIA-X/M-CIDI) gemessen. Die Fallzahl für multivariate Analysen beträgt n=2.806 im Alter zwischen 18 und 64 Jahren. Frauen und Männer mit Arbeitslosigkeitserfahrung sind etwa doppelt so oft von Depressionen betroffen wie Erwerbstätige ohne Arbeitslosigkeitserfahrung in den letzten fünf Jahren. Der Erklärungsanteil sozialer Unterstützung am Zusammenhang zwischen Arbeitslosigkeitserfahrungen und Depression liegt bei Frauen bei 20,8% (p=0.008), bei Männern bei 15,7% (p=0.140) Die Analysen betonen die Bedeutung sozialer Ressourcen für den Zusammenhang zwischen Arbeitslosigkeit und Depressionen.Job loss is associated with considerable health consequences; unemployed people are affected by depression more often than employed people. The paper investigates the extent to which the connection between unemployment and depression is mediated by social support. To this end, population-wide cross-sectional data from the supplementary module «Mental Health» of the study on the health of adults in Germany (DEGS1-MH, 2008–2011) are used and depressive disorders are measured using the DSM-IV criteria of the psychiatric «Composite International Diagnostic Interview» (DIA-X/M-CIDI). The number of cases for multivariate analyses is n=2,806 between the ages of 18 and 64. Women and men who experience unemployment are roughly twice as likely to suffer from depression than employed persons who have not experience unemployment in the last five years. The explanatory share of social support in the connection between the experience of unemployment and depression is 20.8% for women (p=0.008) and 15.7% for men (p=0.140). The analyses emphasize the significance of social resources for the connection between unemployment and depression.La perdita del posto di lavoro comporta conseguenze gravi per la salute. I soggetti disoccupati sono maggiormente colpiti da depressione rispetto alla popolazione degli occupati. Lo studio analizza la questione di quanto il rapporto tra disoccupazione e depressione sia mediato e influenzato dal supporto in ambito sociale. A tale scopo si ricorre all’impiego dei dati trasversali registrati nella popolazione nell’ambito del modulo aggiuntivo «Salute psichica», facente parte dello Studio sulla salute degli adulti in Germania (DEGS1-MH, 2008–2011), e alla valutazione dei disturbi depressivi, sulla base dei criteri DSM-IV e dell’intervista diagnostica psichiatrica «Composite International Diagnostic Interview» (DIA-X/M-CIDI). L’analisi multivariata prende in esame 2806 soggetti di età compresa tra 18 e 64 anni. La percentuale di donne e uomini disoccupati colpiti da depressione è il doppio rispetto a quella degli occupati negli ultimi 5 anni. La percentuale dei casi in cui il supporto in ambito sociale influenza la correlazione tra disoccupazione e depressione è risultata pari al 20,8% nelle donne (p=0,008) e al 15,7% negli uomini (p=0,140).     Die Elektrokonvulsionstherapie (EKT) ist Teil des multimodalen Behandlungskonzepts in der Behandlung insbesondere depressiver Erkrankungen. Die therapieresistente Depression ist als ein wesentlicher Indikationsbereich anzusehen. Nach jahrzehntelanger Erfahrung erweist sich EKT weiterhin als sehr wirksame und aufgrund der heutigen Standards und Technik als sichere und relativ nebenwirkungsarme Behandlungsmethode. Daher überrascht ihre vorurteilsbeladene und negative Wahrnehmung und Bewertung. Auch die sehr häufige Einschätzung der EKT als eine «Ultima ratio» führt dazu, dass sie in vielen Fällen erst zu einem sehr späten Zeitpunkt im Krankheitsverlauf angewandt wird – trotz anderslautender Empfehlung bspw. der S3-Leitlinie Unipolare Depression der DGPPN. Der Indikationsbereich für EKT ist klar umrissen und es gibt nur wenige absolute Kontraindikationen; auch eine Kombination von EKT mit anderen Behandlungsverfahren ist möglich und sinnvoll. Zentrales Anliegen dieses Artikels ist die notwendige Enttabuisierung: EKT sollte im Rahmen einer umfassenden Therapieplanung als mögliche Behandlungsoption angeboten werden. Oftmals ermöglicht erst sie die Überwindung depressiver Blockaden, die Auseinandersetzung mit biografisch relevanten Konfliktthemen, dysfunktionalen Bewältigungsmechanismen, traumatischen Erfahrungen und auch dem «existenziellen Thema der verlorenen Lebenszeit» infolge einer therapieresistenten Depression.Electroconvulsive therapy (ECT) is part of the multimodal treatment concept in the treatment of depressive disorders in particular. Therapy-resistant depression is regarded as a major indication. After decades of experience, ECT continues to prove to be a very effective treatment method that is safe and has relatively few side effects due to current standards and technology. Its prejudiced and negative perception and evaluation is therefore surprising. The very frequent assessment of ECT as a last resort means that in many cases it is only used at a very late stage in the course of the disease – despite recommendations to the contrary, for example the S3 Guideline Unipolar Depression of the DGPPN. The indication range for ECT is clearly defined and there are only few absolute contraindications; a combination of ECT with other treatment procedures is also possible and sensible. The central concern of this article is the necessary removal of taboos: ECT should be offered as a possible treatment option as part of a comprehensive therapy plan. In many cases, it is the only way to overcome depressive blockages, to deal with biographically relevant conflict issues, dysfunctional coping mechanisms, traumatic experiences, and also the «existential issue of lost lifetime» as a result of therapy-resistant depression.La terapia elettroconvulsivante (TEC) è parte del piano di trattamento con approccio multimodale nella cura delle patologie soprattutto di natura depressiva. La depressione resistente alle terapie va considerata come un’indicazione chiave. Dopo decenni di esperienza la TEC si è rivelata essere un metodo di trattamento ancora molto efficace e, a fronte degli standard e delle tecnologie attuali, più sicuro e con effetti collaterali relativamente contenuti. Perciò sorprende che tale metodo sia ancora percepito e valutato in modo così pregiudizievole e negativo. Anche la posizione molto frequente secondo cui la TEC debba essere utilizzata in ultima ratio comporta che il ricorso a tale metodo nel trattamento avvenga solo in uno stadio molto avanzato della patologia, nonostante le diverse raccomandazioni, come la linea guida nazionale S3 per il trattamento della depressione unipolare della DGPPN, Deutsche Gesellschaft für Psychiatrie und Psychotherapie, Psychosomatik und Nervenheilkunde (Società tedesca di psichiatria e psicoterapia, psicosomatica e neurologia). Le indicazioni per il ricorso alla TEC sono chiaramente definite e sussistono esigue controindicazioni che possano escluderne in modo assoluto l’impiego. Inoltre è possibile e sensato combinare la TEC con altre metodologie di trattamento. La questione centrale del presente articolo è rappresentata da un tentativo di smantellamento dei tabù legati a tale metodo: la TEC dovrebbe essere proposta come possibile opzione nell’ambito di un trattamento terapico a 360°. Spesso è proprio il ricorso a tale metodo che rende possibile il superamento dei blocchi causati dalla depressione, il confronto con tematiche conflittuali rilevanti nella biografia del paziente, i meccanismi per riuscire ad affrontare le situazioni più problematiche, le esperienze traumatiche e anche il «tema esistenziale del tempo ormai trascorso e perso» a causa di una patologia depressiva resistente alle terapie

    Man unterscheidet Trauer und depressive Erfahrung, um damit das Kernstück des Leidens einer Person zu beschreiben, die sich in einer Depression befindet. In der Trauererfahrung wird eine Person oder Situation unerreichbar, und darin liegt der erlittene Verlust. Die Erfahrung einer melancholischen Depression ist anders: verlorengegangen ist das, was das Subjekt mit dem Gewebe verankert, das es mit der Welt verbindet. Es wird eine radikal relationale Herangehensweise zu Depressionen eingeführt, bei der untersucht wird, wie Klient*in und Therapeut*in sich im Hier und Jetzt der Therapiesituation gegenseitig deprimieren.L’esperienza del lutto e l’esperienza depressiva si distinguono per evidenziare il nucleo della sofferenza delle persone che soffrono di depressione. Nell’esperienza del lutto, una persona o una situazione specifica diventa irraggiungibile, e in essa risiede la perdita subita. L’esperienza della depressione malinconica è diversa: quello che viene perso è ciò che lega il soggetto alla struttura che lo collega al mondo. Si introduce un approccio radicalmente relazionale alla depressione, in cui il cliente e il terapeuta sono visti come depressivi qui e ora nella situazione della terapia
