La scienza della psicoterapia che amplia le possibilità di azione (parte 2): Implementazione pratica della ricerca
No full textDie Handlungsmöglichkeiten-erweiternde Psychotherapiewissenschaft (HEP) stellt einen eigenen psychotherapiewissenschaftlichen Ansatz dar. Aufbauend auf einem erweiterten wissenschaftstheoretischen Konzept, das auf Ernst von Glasersfelds Radikalem Konstruktivismus basiert, wird eine Praxeologie formuliert. In der forschungspraktischen Umsetzung werden zunächst ein Phänomen möglichst allgemein, aber dennoch exakt definiert, mehrere psychotherapeutische Ansätze ausgewählt und schliesslich das Phänomen aus der jeweiligen Perspektive betrachtet und die Handlungsmöglichkeiten erarbeitet. Und nicht zuletzt werden die Ergebnisse zusammengefasst und hinsichtlich der persönlich eingeschätzten Viabilität reflektiert.The action-possibility-expanding psychotherapy science represents a separate psychotherapy science approach. Building on an extended concept of science theory based on Ernst von Glasersfeld’s Radical Constructivism, a praxeology is formulated. In the research-practical implementation, a phenomenon is first defined as generally as possible, yet precisely, several psychotherapeutic approaches are selected, and finally the phenomenon is considered from the respective perspective and the possibilities for action are elaborated. And last but not least, the results are summarized and reflected upon with regard to the personally assessed viability.La scienza della psicoterapia che si espande con l’azione rappresenta un approccio scientifico separato alla psicoterapia. Partendo da un concetto esteso di teoria della scienza basato sul costruttivismo radicale di Ernst von Glasersfeld, viene formulata una prassi. Nell’implementazione della ricerca-pratica, un fenomeno viene prima definito nel modo più generale possibile, ma preciso, vengono selezionati diversi approcci psicoterapeutici, e infine il fenomeno viene considerato dalla rispettiva prospettiva e vengono elaborate le possibilità di azione. Infine, ma non per questo meno importante, i risultati vengono riassunti e riflettuti in relazione alla fattibilità valutata personalmente

No full textDer Einsatz von Expositionstherapien ist durch den teilweise hohen Aufwand limitiert, die In-vivo-Konfrontationen bedürfen. VRET (Virtual Reality Exposure Therapy) bietet praktikable Alternativen durch kontrollierte Simulation der Reize. Die VRET findet daher immer mehr Eingang in Psychotherapie und Psychiatrie. Der Evidenzgrad von VRET bei spezifischen Phobien ist insgesamt als sehr hoch einzustufen. Eine grosse therapeutische Herausforderung stellt die Behandlung von Suchterkrankungen dar. Der klinische Nutzen von VRET in Zusammenhang mit Süchten wurde daher in den letzten Jahren zunehmend untersucht. In verschiedenen Studien konnte dahingehend ein proof of concept erbracht werden, als dass Suchtkranke auf die virtuelle Konfrontation mit Alkohol und anderen Substanzen sowie Glücksspiel mit erhöhtem Suchtdruck reagieren. Konkrete therapeutische Konzepte werden aktuell entwickelt und untersucht. Die positive Evidenzlage wie auch technische Weiterentwicklungen und geringere Anschaffungskosten sprechen für die Verbreitung der Anwendung von VRET in psychiatrischen Kliniken und psychotherapeutischen Praxen.Absract: The use of exposure therapies is limited by the sometimes high effort that in vivo confrontations require. VRET (Virtual Reality Exposure Therapy) offers viable alternatives through controlled simulation of stimuli. VRET is therefore increasingly finding its way into psychotherapy and psychiatry. The overall level of evidence for VRET in specific phobias can be classified as very high. A major therapeutic challenge is the treatment of addictive disorders. The clinical benefit of VRET in the context of addictions has therefore been increasingly investigated in recent years. In various studies, a proof of concept could be shown that addicts react to the virtual confrontation with alcohol and other substances as well as gambling with increased addictive pressure. Concrete therapeutic concepts are currently being developed and investigated. The positive evidence base as well as technical advancements and lower acquisition costs speak for the spread of the application of VRET in psychiatric clinics and psychotherapeutic practices.L’utilizzo di terapie di esposizione è limitato a causa dell’impegno in parte elevato richiesto, dal momento che presuppongono confronti in vivo. La VRET (Virtual Reality Exposure Therapy), terapia di esposizione alla realtà virtuale, offre alternative praticabili grazie alla simulazione controllata degli impulsi ed è quindi sempre più utilizzata in psicoterapia e psichiatria. Il grado di evidenza della VRET in presenza di fobie specifiche è in linea generale molto elevato. Il trattamento delle dipendenze rappresenta una grande sfida terapeutica e negli ultimi anni l’utilità clinica della VRET per il trattamento delle dipendenze è stata oggetto di un numero crescente di studi. Molti di questi hanno fornito un proof of concept, dimostrando come chi soffre di dipendenza non reagisca con un aumento di desiderio quando viene posto davanti a un confronto virtuale con alcolici e altre sostanze, così come al gioco d’azzardo. Al momento si stanno sviluppando e analizzando interventi terapeutici concreti. Le evidenze positive, accanto agli sviluppi tecnici e ai costi di acquisto inferiori, sono tutti elementi a favore della diffusione dell’utilizzo della VRET nelle cliniche psichiatriche e negli studi di psicoterapia
