Scissors bite – epidemiology, etiology and methods of treatment
The study presents the literature review of the epidemiology, etiology and methods of treatment of the scissors bite. This maloocclusion is recognized when there is no contact between the occlusal surfaces of the posterior teeth in occlusion and when palatal surfaces of the upper teeth contact with buccal surfaces of the lower teeth. The frequency of this malocclusion ranges from 1,8% to 19% and it depends on the study population. The cause of such a malocclusion is a discrepancy between the width of the maxilla and mandible and axial malpositioning of the lateral teeth. This malocclusion is usually accompanied with elongation of the teeth in scissors bite, buccal tipping of the upper teeth and lingual tipping of the lower teeth. In the treatment methods which allow to balance the discrepancy between the width of the bony bases and dental arches and obtain both correct axis and intrusion of the elongated teeth are used. Removable functional appliances, palatal and lingual arches, miniscrews, dragon helix spring, modified orthodontic crown, corticotomy, osteodistraction and appropriate surgery methods are used. The choice of the treatment methods depend on the range and severity of the defects, age, cooperation with the patient and possibile side effects
Evaluation of interdental spacing in patients with shortened dental arch in relationship with prosthetic treatment needs
Aim of the study. The aim of the study was to analyze the interdental spacing in patients with different amount of dental arch reduction in relationship with prosthetic treatment needs. Material and methods. The study group included 120 individuals of both gender, aged 41–65, selected from the patients admitted to The Department of Prosthodontics, Wrocław Medical University and Academic Dental Policlinic, Krakowska Street 26, Wrocław, in years 2007–2009. Patients were divided into two study groups depending on amount of dental arch reduction and control group including individuals with complete natural dentition. All patients underwent interview and general oral cavity examination according to the protocol worked out in The Department of Prosthodontics, Wrocław Medical University. Assessment of interdental spacing was carried out with metal gauges by Pol-Intech, 0,05 milimeters thickness. The obtained data underwent statistic analysis. Results. The results of study on interdental spacing revealed more spacing with reduction of dental arch. Conclusions. Individuals with extremely shortened dental arches revealed the largest loss of dental arch integrity, thus this group of patients has the highest treatment needs
Evaluation of shear bond strength of various orthodontic brackets bonded to bovine enamel with use of Grengloo adhesive. An in vitro study
Introduction. Durability of the bond strength between the orthodontic bracket and the enamel is crucial to perform successful treatment with fixed appliance. Contemporary, apart from wide-spread and well-known metallic brackets, ceramic brackets become more popular in orthodontic practice. Aim of the study. The purpose of the current study was to calculate shear bond strength of metal, polycrystalline and monocrystalline orthodontic brackets bonded to bovine enamel with composite resin. Material and methods. Sixty bovine incisors with intact buccal surfaces were used in this study. The teeth were randomly divided into three equal groups. In group 1 stainless steel Victory Series (3M Unitek) brackets were bonded to the enamel. In group 2 polycrystalline Clarity (3M Unitek) brackets were used and in group 3 monocrystalline Radiance (American Orthodontics) brackets were used. Shear bond test was performed 24 hours after polymerization of Grengloo (Ormco) orthodontic adhesive. Results. The highest shear bond strength was found in Victory Series group (mean 10.65 MPa ± 5.2), followed by Clarity group (mean 9.15 MPa ± 1.91) and Radiance group (mean 6.65 MPa ± 1.71). Statistical analysis revealed significantly higher shear bond strength of Victory Series brackets and Clarity brackets in comparison with Radiance brackets. The difference between Victory Series brackets and Clarity brackets was not significant
Aspergillosis of a maxillary sinus – a case study
The symptoms of fungal infections of the nasal sinuses aren't typical, and often are confused with bacterial infections. The aim of study is to present the case of fungal infection in the maxillary sinus as a complication of endodontic treatment in upper second left premolar. The factors and mechanism which have influence on development of fungal infection in the maxillary sinus was presented in the discussion
Legal and medical considerations of consequences of extracting the wrong tooth
This paper presents the conclusions of a forensic medical team in a dental negligence case involving both medical civil liability and civil proceedings.The defendant dentist removed the wrong tooth from a patient (second premolar instead of the upper right second molar). The insurance company paid out compensation on medical civil liability grounds and the patient sought further redress through civil proceedings for personal infringement and prosthetic treatment costs.The analysis of the act, apparently clear matter in the ground lay a medical problem, took surprising turn. Although dental negligence involving the unreliable conduct of medical records and commit his mistake of removing the wrong tooth, the key for medico-legal opinion was a medical records from another study, in which the patient is treated in several days earlier. From her records showed that there were absolute indications for extraction of two teeth. The opinion found that only the order of a physician turned tooth extraction, resulting in medical malpractice proved to be a neglience causing no injury for the patient. Use of fractal analysis for bone assessment
Since Mandelbrot announced the concept of the fractal geometry, fractal analysis has been used in the description of various structures found in nature and biosignals. It has been used to interpret biological phenomena such as nervous tissue, vascular system, the electrical conductivity of the heart and EEG. In dentistry, there have been many attempts to analyze the structure of jaw bones to assess periodontal disease and osteoporosis. Many authors indicate that bone tissue has a fractal structure, and that the fractal analysis can be inexpensive and convenient screening instrument in the diagnosis of systemic diseases that cause changes in the image of bones. The use of electromyography in orthodontics – a systematic study review
Electromyography (EMG) is the most objective technique for evaluating muscle function and efficiency by detecting and analyzing their electrical activity. Due to the method of recording EMG signals stand out an intramuscular electromyography in which needle electrodes are inserted through the skin and surface electromyography (sEMG) which uses surface electrodes. The aim of this study was to review the possibility of EMG uses in orthodontics. The material were researches published in 2000–2011 in the National Center for Biotechnology Information (NCBI) – MEDLINE – PubMed database. Analysis of the stomatognathic system by the use of EMG expands our knowledge about the anatomy, physiology and pathology of the stomatognathic system, allow for the identification patients with temporomandibular disorders and allow for objective orthodontic treatment monitoring
Etiology and diagnosis of dentine hypersensitivity – a literature review
Along with caries dentine hypersensitivity is a frequent ailment, which makes patients see the dentist. They are forced to do it because of sharp, shooting pain of mild or moderate intensity, which appears as a result of thermal, chemical, dehydrative, osmotic or mechanical stimuli. The study is based on the latest literature and it presents the causes and diagnostics of dentine hypersensitivity. The ailment has a multi-factor aetiology. Patients suffering from periodontal diseases are particularly predisposed to dentine hypersensitivity. The diagnostics of the ailment uses both the patient’s subjective feelings in response to stimuli and clinical examination with the application of objective methods. In order to develop a full therapy plan it is necessary to make an appropriate diagnosis and specify aetiological factors
The clinical significance of two-level PVT Periotest® measurements – in vivo endosteal implant stability study
Introduction. A number of investigations was published with different clinical and scientific approach to Periotest® and Ostell® measurements in the last decade. Attempts were made to assess Ostell® and Periotest® prognostic measurement values in terms of the success of implant treatment as one value or threshold level measurement. Aim of the study. The aim of this study was to evaluate the clinical usefulness of PTV measurements as a method of secondary implant stability and the quality of implant bone zone assessment. Material and methods. Thirty two implant fixtures of five different systems were evaluated during prosthetic phase after second stage surgery. The study was conducted using Periotest® (Siemens AG, Benshein, Germany). The following parameters were taken into account: location of an implant in lower or upper dental arch, selection of located supragingivally and occlusally abutment measurement points and length with diameter of the implant body. Abutment measurement points were chosen in two distant clinically accessible locations: on the occlusal free end margin and on the most gingival accessible surface of the connected abutment. Results. The lowest and highest measured PTV value were -8 and +8. The average of differences between gingivally and occlusally measured PTV values for maxilla and mandible was 2.95 ± 1.6 and 3.33 ± 2.0 respectively. Conclusions. The use of PTV Periotest® measurements in two extremely distant points on the implant abutment is clinically objective, non-invasive method of assessing implant bone zone quality before functional load. Smaller calculated PTV difference can predict better prognosis for occlusal load planning protocol
Comparison of intraoral digital photographs quality performed with a simple compact digital camera (using lighting from the dental unit lamp) and a Digital Single Lens Reflect camera with macrophotography set
Introduction. In dental surgery digital cameras play a major role. High quality intraoral pictures are useful in scientific and didactic activity. Aim of the study. The aim of the study was to compare the quality of intraoral photographs performed with a simple compact digital camera using lighting from the dental unit lamp and an advanced set based on a DSLR camera in patients under surgical treatment. Material and methods. The study was conducted in the Department of Dental Surgery at the Medical University of Lodz, Poland, from 24.10.2012 to 15.01.2013. The first study arm consisted of 37 adult patients undergoing dental surgical treatment. Intraoral photographs were performed in two series using different equipment sets: 1) using simple compact digital camera and lighting from the dental unit lamp, 2) using a DSLR camera with a makro lens and a ring flash. The photographs were gathered in photograms, printed in high quality and presented to the group of 10 dentists for evaluation according to 3 criteria. Results. According to our study, the quality of photographs performed with a simple compact digital camera using lighting from the dental unit lamp is worse than the quality of photographs performed with advanced set basing on a DSLR camera in all 3 criteria. Conclusion. In situations, where high quality of intraoral photographs is required, using advanced photographic set basing on DSLR camera is highly recommended
