Effectiveness of dragon fruit juice for glycemic control in elderly patients with type 2 diabetes mellitus
Background: Type 2 Diabetes Mellitus (T2DM) is a global health challenge with a rising prevalence, particularly among the elderly. Its management requires a holistic approach, including medical nutrition therapy. Red dragon fruit (Hylocereus polyrhizus) is rich in fibre and antioxidants, which may help regulate glucose metabolism. This study aimed to evaluate the efficacy of red dragon fruit juice on reducing random blood glucose levels in elderly patients with T2DM.
Methods: A quasi-experimental study with a pretest-posttest control group design was employed. Thirty-two elderly T2DM patients within the working area of Karo District General Hospital were selected via purposive sampling and allocated into an intervention group (n=16) and a control group (n=16). The intervention group consumed 250 ml of red dragon fruit juice once daily for seven consecutive days, while the control group received routine care only. Random blood glucose levels were measured using a glucometer before and after the intervention. Data were analysed using descriptive statistics and the Wilcoxon Signed Ranks Test.
Results: At baseline, the mean random blood glucose was 231.88 ± 99.37 mg/dL in the intervention group and 250.31 ± 63.32 mg/dL in the control group. Post-intervention, the intervention group exhibited a significant decrease to 226.25 ± 96.99 mg/dL (p=0.017). Conversely, the control group showed a non-significant increase to 252.75 ± 63.66 mg/dL (p=0.027). Rank analysis indicated that 75% of subjects in the intervention group experienced a decrease in blood glucose, whereas 81.25% of subjects in the control group experienced an increase.
Conclusion: The administration of 250 ml of red dragon fruit juice daily for seven days was effective as an adjuvant therapy in reducing random blood glucose levels in elderly patients with Type 2 Diabetes Mellitus. Red dragon fruit may be considered a safe and affordable complementary nutritional intervention for T2DM management in the elderly population
Associations between lifestyle factors and acute respiratory infection incidence at Mayen Primary Health Clinic
Background: Acute Respiratory Infections (ARI) remain a significant global health burden with high morbidity, particularly in developing nations. Modern lifestyle shifts—characterised by smoking habits, poor dietary patterns, physical inactivity, and compromised sleep quality—alongside environmental pollution, are hypothesised to contribute to susceptibility to these infections. Objective: This study aims to analyse the association between lifestyle factors (smoking, diet, physical activity, sleep quality) and environmental pollution exposure with the incidence of ARI among patients at the Mayen Primary Clinic in 2023–2024. Methods: This study employed an analytic observational design with a cross-sectional approach. The research was conducted at the Mayen Primary Clinic, Medan, involving 82 respondents selected via consecutive sampling. Data were collected using medical records and validated lifestyle questionnaires, followed by analysis using the Chi-Square test. Results: The findings revealed that the majority of respondents were suffering from ARI (85.4%). Bivariate analysis demonstrated significant correlations between ARI incidence and smoking habits (p<0.001), dietary patterns (p=0.001), sleep quality (p<0.001), and air pollution exposure (p<0.001). However, no significant association was observed between physical activity and ARI incidence (p=0.601). Conclusion: Unhealthy lifestyles and environmental pollution are primary determinants of ARI incidence in primary care settings, necessitating health interventions focused on behavioural modification and environmental control
The Changes of Retinal Nerve Fiber Layer (RNFL) in Cataracts undergoing Phacoemulsification using Spectral Domain Optical Coherence Tomography
Retinal nerve fiber layer (RNFL) is one of the vital layers of the retina with regard to the incidence and subsequent treatment of glaucoma. The damage done by glaucoma specifically involves this layer. Optical coherence tomography (OCT) is widely used by ophthalmologists for diagnosing and following retinal diseases such as diabetic retinopathy (DR), and OCT measurement of RNFL thickness is valuable for glaucoma screening and detection of progression. Cataract affects the measurement of retinal nerve fiber layer thickness as measured on OCT; and cataract surgery enhances the ability of OCT to measure the changes in retinal layers. We need to pay attention about the effects of phacoemulsification on RNFL thickness and its consequences to preserve the eye delicate tissues and structures and assure its functions and the knowledge will be beneficial for patients’ treatment in future
Implementation of Hypertension Service Monitoring Policy at Siulak Gedang Community Health Center, Kerinci Regency
The regencies in Jambi Province with the highest prevalence of hypertension are Kerinci Regency with a figure of 37.74%, Sungai Penuh Regency 33.92%, and Tanjung Jabung Timur 33.82%. The incidence of hypertension in the elderly has increased over the past three years, according to data from the Siulak Gedang Health Center. According to data from January-May 2022, the prevalence of hypertension in the elderly increased significantly in 2022, reaching 1,501 people. The informants in this study were the Head of the Siulak Gedang Health Center, Hypertension Officers, and Hypertension Patients. This study was conducted in October-December 2023. From the triangulation data with informants through monitoring and evaluation carried out, it was found that the implementation of the monitoring and evaluation policy for hypertension services at the Siulak Gedang Health Center had been carried out properly and in accordance with the Regulation of the Board of Directors of the Health Social Security Administration Agency Number 0118 of 2014 concerning Clinical Guidelines for the Management of Hypertension Disease, however, the implementation of hypertension services has not been carried out optimally and of course still requires maximum monitoring and evaluation by related parties in the implementation of hypertension services, so that hypertension officers are able to carry out promotive efforts in hypertension services properly and optimally in order to improve the health welfare of hypertension sufferers
Determinants Of Consumers' Purchase Decisions For Modern Retail In Medan City
The increase in retail business in a company is caused by many factors. Therefore, it is necessary to test variables that are considered appropriate to support the progress of retail business. Like this study will test whether production quality, service quality, and emotional affect purchases and loyalty by consumers in retail companies in Medan city. In this study, quantitative methods were used to collect data. The results of the study indicate that product quality has an influence on consumer purchases and loyalty; service quality also has a significant and positive influence on consumer purchases and loyalty; and emotional variables also have a significant influence on consumer purchases and loyalty, making them continue to buy the same product
Impact of Sales Growth, Profitability, Firm Size, and Leverage on Earnings Management with Corporate Governance as Moderation
This study examines the impact of sales growth, profitability, company size, and financial leverage on earnings management, with corporate governance as a moderating factor. The sample consists of 20 food and beverage companies listed on the Indonesia Stock Exchange (IDX) from 2018 to 2023, with a total of 120 data points. Using panel data analysis via SmartPLS version 4.1.0.8, the results show that sales growth and company size positively affect earnings management, while profitability has a negative impact. Financial leverage has no direct effect. Corporate governance weakens the effects of sales growth, company size, and leverage, while strengthening the negative effect of profitability. Financial Fundamental Factors On Company Value Stock Price As An Intervening Variable In A Goods Consumer Company On The Indonesian Stock Exchange
The consumer goods sector is a sector that has been targeted by many investors, given the rapidly growing needs of the Indonesian population which has significantly become the main attraction for investors to invest in this sector. The more rapid development of consumer goods companies in Indonesia, making these companies compete with each other to create the latest innovations in the consumer goods sector, to meet the needs of the Indonesian population, this makes companies compete with each other in order tomaintain and increase company value. The purpose of this research is to examineand analyze the effect of Price To Book Value, Return On Equity, and Debt To Equity Ratio on company value through stock prices. The method used in this research is quantitative, and the nature of the research is explanatory. The data collection technique used is documentation. The type and source of data is secondary data in the form of financial reports of companies in the consumer goods sector which are published on the Indonesia Stock Exchange for the 2017-2021 period. The research data was tested using Partial Least Square (Software Smart PLS). The conclusion of this study is that price to book value has no significant effect on firm value, Return On Equity has no significant effect on firm value, Debt On Equity Ratio has no significant effect on firm value, Stock price is able to mediate the effect of debt on equity ratio on firm value. The research data was tested using Partial Least Square (Software SmartPLS). Chronic renal failure (CKD) occurs when kidney function declines gradually and cannot recover as before where the body loses the ability to maintain a normal balance of metabolism, fluids and electrolytes resulting in uremia or azithaemia. The following study aims to determine how the description of body image in patients who carry out haemodialysis therapy in the haemodialysis room of RSU Royal Prima Medan. The results of the study of body image in 57 patients with GGK who underwent haemodialysis therapy. It was found that the majority of respondents showed a positive body image, namely as many as 20 respondents or 35.1%, this shows that more than a third of patients have a good perception of their body even though they are undergoing haemodialysis therapy
Neuroleadership as a Tool for Combating Unethical Behaviour in the South African Public Sector: Addressing Historical Trauma and Leaders
This paper is a narrative review exploring how an understanding of how brain function underlying leadership behaviour and decision making -neuroleadership- can be utilised to address unethical behaviour, including nepotism, corruption and fraud, in the South African public sector, aggravated by various circumstances, one of which is historical trauma from the apartheid era. Notwithstanding countless legislative efforts to curb unethical practices, South Africa continues to experience widespread corruption in its public sector, particularly within municipalities. We suggest unresolved historical trauma as further fuelling unethical practices among leaders who feel disadvantaged by previous political systems. This paper investigates how neuroleadership principles, including self-awareness, emotional regulation and social cognition have the potential to address these behaviours. The findings are that neuroleadership offers a novel strategy for studying and teaching ethical leadership in the South African public sector by enhancing among others, emotional regulation and bias awareness, with the potential to address the severe challenges municipalities face, such as financial mismanagement, and procurement irregularities. Integrating neuroleadership into ethics training programs for public officials is recommended to promote emotional intelligence and cognitive flexibility, while addressing underlying historical traumas is suggested to help mitigate unethical behaviour and foster more inclusive leadership practices
Cultural Intersection in the Novel "Kuli" by M.H. Szekely Luloofs
This article discusses the representation of cultural intersection in M.H. Szekely-Lulofs' novel Kuli, a colonial text that records the lives of indentured laborers on Deli plantations. Starting from the multi-ethnic historical context of North Sumatra, this research highlights how cross-ethnic encounters—Javanese, Sundanese, Madurese, Betawi, Malay, and even Chinese—are constructed through stereotypes, conflict, and power relations. Lulofs, a Dutch-descended female writer, uses her personal experiences in Deli as both the setting and the medium to depict a fragmented social reality. Utilizing the perspective of literary sociology, the analysis is directed towards two main aspects: first, ethnic diversity as the foundation of Deli society, shaped by migration and colonization; and second, intersectional conflicts arising as a consequence of differences in ethnic identities and representations within the narrative. The novel Kuli presents ethnic stereotypes in layers: Betawi people are depicted as arrogant, Madurese as tough, Sundanese as polite, and even Chinese who are socially and economically marginalized. This stereotype is not only an identity marker but also a source of conflict that culminates in intergroup violence. Inter-ethnic intersection is depicted ambiguously—on one hand, it gives rise to "ship brother" solidarity, while on the other, it triggers exclusion and criminalization. The novel Kuli, therefore, presents plurality as an arena of tension, where ethnic representations operate within a colonial framework that reinforces social hierarchie
