    Mobility Moves Patients Home

    To combat the negative effects of inpatient immobility, the 2DSU implemented a patient mobility preservation program aimed at increasing discharges to home with home care versus rehabilitation facilities. The program involved a twice-daily mobilization goal, tracked via progress sheets posted on patient doors and collected weekly. While initial results showed improvement six months into the program, the most significant positive impact on reducing rehab discharges occurred within the last three months. Given this success, recommendations for integrating the program into routine practice include shift-based mobility planning by RNs and PCAs, incorporating functional assessment scores into mobility plans, discussing individual patient progress during handoffs and IDRs, and adjusting mobilization timing based on individual patient needs and medical appropriateness

    Improving Interprofessional Communication Within The Neonatal ICU

    The 2024 National Database of Nursing Quality Indicators (NDNQI) survey highlighted an opportunity to improve interprofessional communication in the NICU, where the department scored lower (3.94) compared to similar units in Magnet hospitals (4.03). The NICU currently uses nurse-led rounds and whiteboard updates to facilitate communication, but a survey conducted by the NICU Collaborative Care Council (CCC) identified barriers to more effective communication. With 24 nurse respondents, the survey found that while most nurses (75%) feel empowered in-patient care decisions, there is room for improvement in communication, particularly between nurses and providers. Recommendations to enhance communication include increasing interdisciplinary engagement, implementing regular forums, and using virtual platforms, while also emphasizing a more inclusive approach to decision-making
