    The global current situation of the obesity as great public health problem and its implications all over the world is reviewed. The project DRECE (Diet and Risk of Cardiovascular Diseases in Spain) and some of its contributions to the knowledge of obesity and its complications in Spain are presented. Finally, an approximation is made to the actual knowledge of obesity’s pathophysiology.Se repasa la situación global actual de la obesidad como gran problema de salud pública y sus implicaciones en todo el mundo. Se presenta el proyecto DRECE (Dieta y Riesgo de Enfermedades Cardiovasculares en España) y algunas de sus aportaciones al conocimiento de la obesidad y sus complicaciones en España. Finalmente se hace una aproximación al conocimiento fisiopatológico de la obesidad

    Papel del péptido insulinotrópico dependiente de glucosa en la programación nutricional del síndrome metabólico

    Early nutritional restriction has been associated with increased incidence of metabolic syndrome-associated pathologies in adulthood. However, the underlying mechanisms that determine the development of these diseases are not yet fully known. In the present work, we explored the relevance of glucose-dependent insulinotropic polypeptide (GIP) in the development of these pathologies in a model of Wistar rats. Two groups of dams were fed ad libitum (C) or food-restricted (U) during pregnancy and suckling. At that time, rats were refed a high-fat diet (HFD; CHF and UHF) for 22 weeks. Both male and female UHF rats showed an obese phenotype characterized by hyperphagia, visceral fat accumulation and adipocyte hypertrophy, which was more pronounced than in CHF rats. Oral glucose tolerance tests showed that female UHF rats experienced glucose intolerance, insulin hypersecretion and an exacerbated GIP secretion. Administration of the GIP receptor antagonist, (Pro3)GIP, to UHF female rats markedly reduced visceral fat mass and adipocyte hypertrophy, and these changes were accompanied by improvement of glucose tolerance and insulin sensitivity. In conclusion, the exacerbated production and secretion of GIP seems to represent the stimulus for insulin hypersecretion and insulin resistance shown by UHF female rats, suggesting that GIP may play a critical role in the development of metabolic disturbances related to nutritional rehabilitation.La restricción nutricional precoz ha sido asociada con una mayor incidencia de patologías relacionadas con el síndrome metabólico durante la edad adulta. Sin embargo, los mecanismos subyacentes que determinan el desarrollo de dichas patologías aún no se conocen en su totalidad. En el presente trabajo, se analizó el papel del péptido insulinotrópico dependiente de glucosa (GIP) en el desarrollo dichas patologías en un modelo de rata Wistar. Las ratas gestantes fueron alimentadas ad libitum (C) o sometidas a restricción nutricional (S) durante el embarazo y la lactancia, al final de la cual las crías fueron realimentadas con dieta grasa (CR, SR) durante 22 semanas. Tanto los machos como las hembras SR mostraron un fenotipo obesogénico caracterizado por hiperfagia, acumulación de grasa visceral e hipertrofia adipocitaria, de manera más pronunciada que la población CR. Los test de tolerancia oral a la glucosa mostraron que las hembras SR experimentaron intolerancia a la glucosa e hipersecreción de insulina y GIP. La administración del antagonista del receptor de GIP, (Pro3)GIP, a las hembras SR dio lugar a una significativa reducción del tejido adiposo y del tamaño adipocitario, junto a una mejora de la tolerancia a la glucosa y de la sensibilidad a la insulina. En conclusión, la exacerbada secreción de GIP parece representar el estímulo para la hipersecreción de insulina y el desarrollo de resistencia a la misma en las hembras SR, lo que sugiere que GIP jugaría un papel esencial en el desarrollo de alteraciones metabólicas asociadas a la rehabilitación nutricional

    Genética y epigenética de la obesidad

    Although obesity is the result of the interaction between genetic predisposition and environmental factors, we still have very little knowledge about their contribution. In recent years, genome-wide association studies (GWAs) have allowed us to identify many genes associated with common obesity, among them the following: FTO, MC4R, KCTD15, MTCH2, NEGR1, BDNF, FAIM2, TMEM18, etc. Moreover, several gene-environment interaction studies, have allowed us to know that the influence of genetic variants in these genes is not deterministic but is modulated by environmental factors among which diet and physical activity are the most relevant. In parallel, epigenetics has also developed, based on modifications and regulations that do not involve changes in the DNA sequence. Among the epigenetic regulators, the most important are methylation and microRNAs. Studies of genome-wide methylation have also allowed us to identify differentially methylated genes associated with obesity. In this review we will analyze the main genetic and epigenetic factors associated with obesity as well as its environmental modulation.Aunque la obesidad es el resultado de la interacción entre la predisposición genética y los factores ambientales, todavía tenemos un conocimiento muy escaso sobre su contribución. En los últimos años, los estudios de asociación de genoma completo, conocidos como GWAs, nos han permitido identificar muchos genes asociados con la obesidad común, entre ellos destacan los siguientes: FTO, MC4R, KCTD15, MTCH2, NEGR1, BDNF, FAIM2, TMEM18, etc. Varios estudios de interacción gen-ambiente, nos han permitido conocer que la influencia de las variantes genéticas en esto genes no es determinista sino que está modulada por factores ambientales entre los que destaca la dieta y la actividad física. Paralelamente, se ha desarrollado también la epigenética, basada en modificaciones y regulaciones en el ADN que no implican cambios de secuencia. Entre los reguladores epigenéticos, los más importantes son la metilación y los microRNAs. Los estudios de metilación de genoma completo nos han permitido también identificar genes diferencialmente metilados asociados con obesidad.     Aim: Chitosan nanoparticles have been use on different pharmaceutical formulations, however there is not data important that show if these structures are toxic. In this work we studied the acute toxicity of chitosan nanoparticles. Methods: We used mice BALB/c, the chitosan nanoparticles was administrated intraperitoneal via by 15 day after these, the mice was sacrifice by cervical dislocation and the organs were treated for histological analysis using HE stain. Results: The histological results showed that chitosan nanoparticles causes damage on heart, liver, spleen, intestine and kidney in mouse BALB/c male. Conclusion: The exposure at chitosan nanoparticles causes histological damage on heart, liver, spleen, intestine and kidney.Objetivo: Las nanopartículas de quitosano han sido usadas recientemente en diversas formulaciones farmacéuticas, este trabajo evaluó la toxicidad que pueden presentar estas estructuras.Material y Métodos. Se utilizaron ratones de la cepa BALB/c administrados durante 15 días con nanopartículas de manera intraperitoneal posterior  a lo cual se sacrificaron, obtuvieron diversos órganos y trataron para su análisis.Resultados: Los resultados muestran que las nanopartículas de quitosano presentan toxicidad en ratones machos, ya que pueden dañar Corazón, Intestino, Riñón, Bazo e Hígado Conclusión: La exposición a nanopartículas de quitosano causó daño histológico sobre Corazón, hígado, bazo, intestinos y riñón

    Pirfenidone experience in mild-to-moderate Idiopathic Pulmonary Fibrosis in a General Hospital.

    This article present the experience and outcomes of patients treated with pirfenidone.FVC and DLCO parameters during 12 months were collected in patients treated with pirfenidone.Eight of the ten patients continued treatment until month 12. 7 patients presented at 12 months an adequate response treatment, 1 patient did not achieve therapeutic targets established (improvement or stability).At week 52, our patients had a mean change in FVC(%) of -2.38±6.93% versus -5.2% of clinical trials and -8.3% on placebo. Higher incidence of adverse effects were observed in absolute terms, than clinical trials.Our results show that pirfenidone is a well tolerated drug, whose toxicity can be controlled by dose adjustment, and it is effective in mild-moderate IPF within one year. In this context, no proven effectiveness and safety medium / long term and the high economic impact, it is necessary to identify those patients who may derive greater clinical benefit

    Tratamiento quirúrgico de la obesidad: pros y contras

    Obesity is a first magnitude public health problem. Because of its multifactorial pathogenesis, treatment of obesity is complex and requires a multidisciplinary approach. The first step is an adjustment and re-education of eating habits and healthy lifestyle. Diet and physical activity are pillars so that, once achieved weight loss, it remains within the normal range. As alternatives for severe cases there are pharmacological, endoscopic and surgical treatments that can achieve significant weight reductions and maintained over time. In this sense, the surgery has proven, in combination with changes in lifestyle and dietary adjustments, the best available treatment for morbid obesity, achieving significant improvement in health status and quality of life of these patients for their great general health benefits. However, it should be borne in mind the possibility of negative effects arising such as global nutritional deficiencies, as well as specific vitamin and mineral deficiencies, and even an increased risk of alterations in bone mineral density and alcoholism. Both a weight re-gain as an increased risk of suicide after some time of surgery have been described and should be considered. In women of childbearing age necessary to add the possible problems arising from an increased prevalence of preterm birth and low birth weight.La obesidad representa un problema de salud pública de primera magnitud. Debido a su patogenia multifactorial, el tratamiento de la obesidad es complejo y requiere un abordaje multidisciplinar. El primer paso consiste en un ajuste y re-educación de los hábitos dietéticos y estilo de vida saludable. La dieta y la actividad física son pilares para que, una vez conseguida la pérdida de peso, ésta se mantenga dentro de la normalidad. Como alternativas para los casos graves existen tratamientos farmacológicos, endoscópicos y quirúrgicos que permiten conseguir reducciones de peso más importantes y mantenidas en el tiempo. En este sentido, la cirugía ha demostrado ser, en combinación con los cambios de estilo de vida y ajustes dietéticos, el mejor tratamiento disponible para la obesidad mórbida, logrando alcanzar mejoría importante en el estado de salud y en la calidad de vida de estos pacientes por sus grandes beneficios en todo el organismo. Sin embargo, debe tenerse presente la posibilidad de que surjan efectos negativos tales como carencias nutricionales globales, así como déficits minerales y vitamínicos específicos e, inclusive un mayor riesgo de alteraciones en la densidad mineral ósea y de alcoholismo. Tanto una reganancia ponderal como un aumento del riesgo de suicidio transcurrido algún tiempo de la cirugía han sido descritos y deben ser tenidos en cuenta. En mujeres en edad fértil resulta necesario añadir los posibles problemas derivados de una mayor prevalencia de parto prematuro y bajo peso al nacer

    All members of the families of the neuroglia have the ability to adopt a reactive state ("gliosis" in general, "astrogliosis" "oligodendrogliosis" and "microgliosis" in its specific form) face to situations where importantly is modified homeostasis of the nervous tissue and / or normal functional dynamics of neurons and their accompanying neuroglial cells (trauma, neurotoxicity, infections, neurodegenerative processes). In this reactive state, neuroglial cells become "new cells with new functions" because a significant change of their cellular functions occurs, mainly by the expression of new genes. Thus they become major players on the different stage of the new SN pathological situations. Activation of neuroglia cells is initially beneficial, aiming to solve the pathological changes, but it can turn detrimental to nerve tissue. Also, in recent years, it has been found that also the lack of reactive responses or the existence of “maladaptative responses” of the different neuroglial cells as well as the involution or regression of these cells, are important sources of nervous pathology. Studies of these different types of reactive glial cells have significantly expanded the knowledge we had about the pathogenesis of diseases of the SN. Moreover, many studies are revealing that they may be important to define new therapeutic drug targets or create new cell therapies to control the progress of the "pathogenic cascades", through activating neuroprotection-neurorepair and/or inhibiting the neurotoxicity-neurodegeneration."Todos los miembros de las familias de la neuroglía tienen la capacidad de adoptar un estado reactivo (“gliosis” de manera general; “astrogliosis”, “oligodendrogliosis” y “microgliosis”, en su forma más específica) ante situaciones en que se modifica de manera importante la homeostasis del tejido nervioso y/o la dinámica funcional normal de las neuronas y sus células gliales acompañantes (traumas, intoxicaciones, infecciones, neurodegeneraciones). En este estado reactivo se convierten en “nuevas células con nuevasfunciones” pues cambian de manera importante sus funciones celulares con la expresión de nuevos genes. De esta forma pasan a ser actores principales en el escenario de las nuevas situaciones patológicas del Sistema Nervioso (SN). La activación de las células de neuroglia en principio es beneficiosa, tendente a solucionar los cambios patológicos, pero puedeconvertirse en perjudicial para el tejido nervioso. Asimismo, en los últimos años, se ha comprobado que también es causa de patología la falta de respuesta reactiva de las distintas células neuróglicas o la involución de estas células. Los estudios sobre estos diferentes tipos de células gliales reactivas han ampliado de manera muy importante los conocimientos queteníamos sobre la patogenia de las enfermedades del SN, y también nos están dando a conocer que pueden ser apoyos fundamentales para definir dianas farmacológicas terapéuticas o crear nuevas terapias elulares que ayuden a controlar el avance de las cascadas patogénicas” por la vía de activar la europrotección-neurorreparación y/o inhibir la neurotoxicidad-neurodegeneración
