Quantitative genetics of microbiome mediated traits
Multicellular organisms host a rich assemblage of associated microorganisms, collectively known as their microbiomes . Microbiomes have the capacity to influence their hosts\u27 fitnesses, but the conditions under which such influences contribute to evolution are not clear. This is due in part to a lack of a comprehensive theoretical framework for describing the combined effects of host and associated microbes on phenotypic variation. Here we address this gap by extending the foundations of quantitative genetic theory to include host-associated microbes, as well as alleles of hosts, as factors that explain quantitative host trait variation. We introduce a way to partition host-associated microbiomes into components relevant for predicting a microbiome-mediated response to selection. We then apply our general framework to a simulation model of microbiome inheritance to illustrate principles for predicting host trait dynamics, and to generalize classical narrow and broad sense heritabilities to account for microbial effects. We demonstrate that microbiome-mediated responses to host-level selection can arise from various transmission modes, not solely vertical, but that the contribution of non-vertical modes can depend strongly on host life history. Our work lays a foundation for integrating microbiome-mediated host variation and adaptation into our understanding of natural variation
Characteristics of prepharmacy undergraduate institutions as a predictor of first-year pharmacy school success
Objective: To evaluate the impact of undergraduate institutional characteristics on first-year pharmacy (P1) success at a private, 4-year program.
Methods: A retrospective review of students matriculating from 2020 to 2023 was conducted. Characteristics of each student\u27s primary undergraduate institution, such as admission rate, highest degree offered, enrollment size, and other factors, were obtained from the US Department of Education Scorecard. Student academic metrics and outcomes in the P1 year were also collected. A mixed-effects model was used to identify independent undergraduate institutional predictors of P1 success, defined as passing all courses without remediation and progressing on time to the second year.
Results: A total of 162 students from 82 undergraduate institutions were included. Univariate analysis demonstrated that undergraduate institutions offering a bachelor\u27s degree or higher, having a lower admission rate (or not having an open admissions policy), and having at least 50% full-time faculty were predictors of P1 success. Student metrics such as prematriculation grade point average (GPA) and achieving a bachelor\u27s degree were not predictive of P1 success, although prematriculation GPA was independently associated with P1 GPA. Collinearity among predictive undergraduate institution characteristics was common. In a mixed-effects model, P1\u27s success was best predicted by the undergraduate institution\u27s admission rate.
Conclusion: Multiple undergraduate institution characteristics significantly predict P1 success, whereas student academic metrics were not predictive in our analyses. These findings challenge the reliance on traditional academic metrics, particularly GPA, and suggest that future research should focus on holistic admissions approaches that include both institutional and nonacademic factors
Reliability of a student patient education assessment rubric for continuous glucose monitor initiation
Objective: To validate a student patient education assessment rubric for initiation of a continuous glucose monitor (CGM) by measuring the inter-rater reliability (IRR).
Methods: Second and third-year pharmacy students at two institutions were evaluated on their ability to counsel a patient on the initial set up and use of a CGM device, either Dexcom G6 or FreeStyle Libre 2, during a practical assessment. Utilizing a standardized rubric, faculty evaluators evaluated each student. After course completion, three additional evaluators reviewed a recording of each session and evaluated the students utilizing the same rubric. The intraclass correlation coefficient [ICC(2,k)] was calculated to determine the IRR of the overall rubric and its four main sections: Introduction, Counseling Competency, Closing and Communication Ability.
Results: A total of 54 students completed the counseling practical assessment; one video recording was excluded for poor audio quality. Each student was graded using the standardized CGM rubric by four total faculty evaluators. The average student score was 23.1 out of 25 possible points. Median scores were slightly lower in students counseling on the Dexcom G6 device compared to Libre 2 (p = 0.005). The ICC(2,k) value among four evaluators was good [0.86, 95 % CI (0.71-0.93)], indicating a high level of agreement for the total rubric score.
Conclusion: A patient education assessment rubric for CGM device initiation demonstrated good inter-rater reliability across two institutions and may be a useful tool for institutions evaluating CGM counseling activities
