    Get PDF
Objectives: This study aims to identify the risk factors associated with massive transfusion in obstetric hemorrhage and evaluate their predictive value to facilitate early intervention and improve maternal outcomes. Methods: A retrospective cohort study was conducted on 1305 women who received blood transfusions for obstetric hemorrhage at Etlik City Hospital between October 2022 and December 2024. Patients were categorized into two groups: those who required massive transfusion (?10 units of blood products) and those who received at least one unit of transfusion. Demographic, clinical, obstetric, and hematological parameters were analyzed. Statistical comparisons were performed using chi-square, Fisher's exact, Mann-Whitney U, and independent sample t-tests. Results: A history of recurrent miscarriage, two or more previous cesarean sections, congenital uterine abnormalities, and uterine rupture were significantly associated with massive transfusion (P<0.01). Severe anemia (Hb<9 g/dL) and thrombocytopenia were also identified as critical hematological predictors (P<0.001). Socioeconomic factors, including low educational attainment and unemployment, were significantly correlated with the need for massive transfusion (P<0.001). Obstetric complications such as postpartum atony (P<0.001) and premature rupture of membranes (P=0.04) also contributed to increased transfusion risk. Conclusions: Identifying key risk factors for massive transfusion can aid in early recognition and timely intervention for obstetric hemorrhage. Integrating these predictors into clinical protocols may enhance maternal care by improving preparedness and reducing maternal morbidity and mortality

Aim: Acute Stress Disorder is defined as the presence or worsening of involuntary symptoms, negative mood, dissociation, avoidance, and arousal symptoms that occur after encountering a highly traumatic stressor. The aim of this study is to evaluate Acute Stress Disorder in patients presenting to the emergency department after an earthquake. Material and methods: Patients who presented to the tertiary emergency department between the 2nd and 30th days following the Düzce earthquake on November 23, 2022, were included in the study. The total number of patients who visited the emergency department was 11,091, and 562 of them were identified as being affected by the earthquake. The records of 376 patients, who met the inclusion criteria, were retrospectively reviewed. Results: In our study, the average age of the participants was 42.54±6.93 years, with a higher proportion of female patients (54.3%). When comparing anxiety with all variables, it was found that patients with anxiety showed a statistically significant difference in age (p=0.028), tachycardia (p=0.001), and having a prior psychiatric diagnosis (p=0.002) compared to those without anxiety. However, no statistically significant difference was observed in gender (p=0.08) or dizziness (p=0.673). Conclusion: Earthquakes are a powerful source of stress that threatens a person’s life, property, and sense of meaning. To prevent acute stress disorder from evolving into post-traumatic stress disorder, early diagnosis and supportive treatment are essential. Despite the heavy workload in the emergency department, it is crucial not to overlook the need to refer patients to psychiatry to initiate their treatment process in such cases

In this study, a method is proposed for the preliminary estimation of school building costs. For this purpose, the projects of 96 school buildings, which were put out to tender by public institutions and the result announcement was published, were provided from the Electronic Public Procurement Platform. These projects are divided into 81 training data and 15 test data. The construction cost of educational buildings through projects may affect the output vectors; “Number of Classrooms, Construction Duration, Total Number of Floors, Floor Height, Building Height, Building Height Class, Basement Height, Earthquake Design Class, Floor Class, Ground Safety Stress, Bed Coefficient, Concrete Class, Number of Elevators, Wet Area, Raft Foundation Height, Floor Area, Basement Area, Total Area" parameters were determined and used in modeling and analysis. Analyzes were carried out with “SPSS Statistics 26” software. Using these parameters, Regression Analyzes (RA) were performed and equations were developed to estimate the costs of educational structures. Approximate Costs and Contract Prices were tried to be estimated with the developed models. The model in which all parameters were used with the created equations was the model with the best correlation level, with the determination coefficient R²=0.900 for the Approximate Cost Price and R²=0.927 for the Contract Price. An error rate of 17,5% was found between the approximate cost estimates obtained using the model and the actual costs. It was determined that there was an 18,2% error between the estimated contract prices and the actual contract prices. The Durbin-Watson criterion was used to check the consistency between predicted results and actual results. As a result, an approach that can estimate the approximate costs and contract prices of school buildings of different types and coefficients with error rates lower than 20% has been created. Both public institutions and contracting construction companies will be able to make realistic cost estimates by benefiting from these modeling, providing time savings. Conducting similar studies by increasing the number of data may be a solution to minimize the error rate in subsequent modeling.Bu çalışmada, okul binası maliyetlerinin ön tahmini için bir yöntem önerisinde bulunulmuştur. Bu amaçla, Kamu Kurumları tarafından ihaleye çıkılmış ve sonuç ilanı yayınlanmış 96 adet okul binasına ait projeler Elektronik Kamu Alımları Platformu (EKAP)’tan sağlanmıştır. Bu projeler 81 adet eğitim, 15 adet test verisi olacak şekilde ayrılmıştır. Projeler üzerinden eğitim yapılarının inşaat maliyeti çıktı vektörlerini etkileyebilecek; “Derslik Sayısı, Yapım Süresi, Toplam Kat Sayısı, Kat Yüksekliği, Bina Yüksekliği, Bina Yükseklik Sınıfı (BYS), Bodrum Yüksekliği, Deprem Tasarım Sınıfı (DTS), Zemin Sınıfı, Zemin Emniyet Gerilmesi, Yatak Katsayısı, Beton Sınıfı, Asansör Sayısı, Islak Alan, Radye Temel Yüksekliği, Kat Alanı, Bodrum Alanı, Toplam Alan” parametreleri belirlenerek modelleme ve analizlerde kullanılmıştır.

    Get PDF
Aim: To continuously monitor midwifery students' physiological stress during high-fidelity perinatal simulation using a wearable device (Empatica (c) E4 wristband) and assess the impact of a stress-focused debriefing on their physiological and psychological stress and satisfaction. Background: High-fidelity simulation is an innovative training method that improves satisfaction and learning in healthcare, including midwifery. However, students might experience pressure and this can result in stress during simulations. Design: A two-group randomized controlled. Methods: The study was conducted at a University College's High-Fidelity Perinatal Simulation Centre in Belgium. Participants included fifty-six second- and third-year midwifery students. The intervention group received a stress-focused debriefing in addition to a standard debriefing, while the control group received routine standard debriefing. Physiological stress was continuously monitored using the E4 wristband, which measures electrodermal activity, heart rate, blood pressure and temperature. Measurements were divided into time periods: pre-simulation (T-0), scenario-running (T-1), debriefing (T-2) and post-simulation (T-3). Psychological stress was assessed at T-0 and T-3 using the Short Stress State Questionnaire and satisfaction was evaluated the Satisfaction with Simulation Experience Scale at T-3. Results: The intervention group showed a significant reduction in psychological stress and higher satisfaction than the control group (p < 0.001). Electrodermal activity at T-2 and heart rate at T-3 were significantly lower in the intervention group than the control group (p < 0.05). Conclusions: A stress-focused debriefing effectively reduces midwifery students' psychological and physiological stress while enhancing satisfaction. Electrodermal activity and heart rate parameters offered valuable insights into the students' physiological responses during perinatal simulation.Scientific and Technological Research Council of Turkey (TUBITAK) [1059B192100834]This study was conducted as part of the SimStress Project (Stress Measurement during Simulation Training) and was funded by the Scientific and Technological Research Council of Turkey (TUBITAK) (Grant ID: 1059B192100834) in the framework of 2219-International Postdoctoral Research Fellowship Program

Cyclical hybrid imputation technique for missing values in data sets

    Get PDF
The problem of missing data in data sets is the most important first step to be addressed in the preprocessing phase. Because incorrect imputation of missing data increases the error in the modeling phase and reduces the prediction performance of the model. When it comes to health, it is inevitable to choose models that show a higher success rate. In cases where there is missing data, the performance of machine learning models may differ depending on the amount of data contained in the data set. The presence of missing data and this high rate affects the accuracy and reliability of analysis and modeling studies because it will affect the complete amount of data in the data set. Estimating and filling in the missing data very precisely, close to its real value, will provide a significant visible performance increase in the modeling phase, which is the next stage. After imputing the missing data with an artificial intelligence model rather than a random method, it is obvious that the accuracy of the model trained with this data is higher than the model trained with data filled with classical filling methods such as mean and mode. In this study, we propose a new algorithm that has been tested on many datasets to address the problems caused by missing data imputation in the dataset. The algorithm aims to impute missing values more effectively by using row-based and column-based imputation techniques together and cyclically. The algorithm takes into account individual missing values using column-based imputation features and the overall data structure using row-based imputation features. The proposed algorithm achieved 100% accuracy with some row and column-based imputation techniques on 3 different datasets used in the study. Higher accuracy was achieved compared to other imputation techniques

Weighted Fractional Euler-Maclaurin-Type Inequalities by Various Function Classes

    Get PDF
In this paper, we establish weighted Euler-Maclaurin-type inequalities for various classes of functions by employing Riemann-Liouville fractional integrals. To begin with, we derive a fundamental integral identity using a positive weight function, which serves as the basis for our main results. Utilizing this identity in combination with Riemann-Liouville fractional integrals, we present several weighted Euler-Maclaurin-type inequalities that are applicable to a broad range of function classes, including differentiable convex functions, bounded functions, Lipschitzian functions, and functions of bounded variation. These results offer a deeper understanding of Euler-Maclaurin-type inequalities and suggest potential avenues for future research. The findings presented herein extend and generalize previous results available in the literature

Effect of Oral plus Topical and Only Topical Tranaxamic Acid Application on Blood Loss and Postoperative Transfusion in Primary Total Hip Arthroplasty

    Get PDF
Objectives: Total hip arthroplasty is one of the most common procedures performed to reduce pain and improve hip functions in patients with advanced hip osteoarthritis, but perioperative blood loss, acute anemia and transfusion requirement increase the risk of morbidity and mortality during and after surgery and negatively affect functional recovery. We aimed to present the comparative results of oral + topical and only topical tranexamic acid application to reduce blood loss and postoperative transfusion in primary total hip arthroplasty. Methods: We retrospectively evaluated the patients who applied to the Orthopedics and Traumatology outpatient clinic with complaints of hip pain and limited movement between January 2014 and December 2020, who underwent primary total hip arthroplasty with the diagnosis of coxarthrosis and who were administered oral + topical and only topical tranexamic acid before and during surgery, in terms of blood loss and transfusion requirement. Results: No statistically significant difference was observed between the preoperative, day 0 and day 1 hemoglobin means in those that were applied oral + topical tranexamic acid and those that were applied only topical (p > 0.05). However, the second- and third-day hemoglobin means in those that were treated with topical medication alone were found to be statistically significantly lower than in those that were treated with oral + topical tranexamic acid (p = 0.032, p = 0.0001). Conclusions: Oral + topical tranexamic acid application in total hip arthroplasty surgery is more effective than topical applications alone when it comes to reducing blood loss, hemoglobin and hematocrit decrease without increasing the risk of thromboembolic diseases and wound complications
