Effects of a Trunk Harness on Lumbopelvic Stability and Muscle Activity during Prone Hip Extension Exercise
Get PDFBackground: Therapeutic exercise is essential in patients with low back pain and lumbopelvic instability. Intra-abdominal pressure is necessary for ideal dynamic stabilization patterns. However, accurate performance of such exercises is a challenge. A trunk harness can help stabilize and alter muscular patterns. This study aimed to examine the effects of using a trunk harness on lumbopelvic stability and muscle activity during prone hip extension in healthy individuals and patients with low back pain. Methods: Sixteen patients with low back pain and 15 healthy individuals performed prone hip extension under control, Dynamic Neuromuscular Stabilization(DNS)-maneuver, and DNS-maneuver + harness conditions. Lumbopelvic kinematic data, muscle onset time of the bilateral erector spinae, semitendinosus, gluteus maximus, low back pain severity, and difficulty performing prone hip extension were evaluated. Repeated-measures two-way analysis of variance was performed for each measurement item. The significant level was set at 5%. Results: The lumbar lordosis angle was significantly lower in the DNS-maneuver and DNS-maneuver + harness conditions. The anterior pelvic tilt angle was significantly lower, and muscle onset of the gluteus maximus and contralateral erector spinae occurred earlier in the DNS-maneuver + harness condition. The difficulty of performing prone hip extension was significantly lower in the DNS-maneuver and DNS-maneuver + harness conditions and was considerably lower in the DNS-maneuver + harness condition than in the DNS-maneuver condition. Conclusion: Wearing a trunk harness could help stabilize the lumbopelvic region and change muscle activity patterns

Lung Cancer Masquerading as Thoracolumbar Pain Detected in an Outpatient Physical Therapy Setting
Get PDFStudy Design: Case Study. Background: This case report describes the clinical course of a patient with right low back pain (LBP) and the use of clinical evaluation findings to initiate medical referral resulting in a diagnosis of cancer presenting as mechanical low back pain. Diagnosis: A 59-year-old male with primary complaints of progressive right-sided LBP affecting his job, sleep, and daily activities was seen by a physical therapist in an outpatient setting. The components of the patient’s history and physical examination were consistent with mechanical thoracolumbar dysfunction. However, clustered examination findings suggested the possibility of a more serious underlying disease. Considering a review of evaluation findings, the patient was referred to a physician for medical evaluation and diagnostic imaging to rule out a non-mechanical cause of symptoms. Medical diagnostic imaging discovered metastatic cancer as the primary cause of this patient’s thoracolumbar pain. Discussion: Physical therapists must perform systems screening and understand the clinical findings associated with the presence of serious underlying disease. In managing LBP systems, screening can provide an accurate differential diagnosis and guide the clinician in deciding if a physician referral is warranted
Measurement of Transmission Efficiency of Blue Light Blocking Devices
Get PDFTechnology and light sources have experienced a revolution in recent years leading to the production of light emitting diode (LED) bulbs. White-light LED bulbs undergo degradation over time, leading to a rise in color temperature and a proportional increase in the emission of blue light from these bulbs. The small size of LEDs makes them the optimal choice for electronic devices because of their limited screen size. This means that blue light now exists in red, green, and blue solid-state illumination systems that did not exist a decade ago. It is debated if blue light induces toxic effects on the retina, but researchers agree that blue light negatively impacts the body’s circadian rhythm. Rather than focusing on electronic devices, individuals should be more concerned about blue light emission from the sun as it has been deemed the dominant source of low wavelength radiation. Blue light blocking devices have been introduced into the market claiming to prevent blue light induced retinal damage, improve sleep quality, and to lessen eye fatigue symptoms. However, these lenses may have been introduced to the market just as a commercial sales pitch if blue light is not toxic to the retina. This project suggests that the blue light lens is effective in blocking blue light significantly more than other filtering devices. Additionally, to combat disrupting the circadian rhythm, individuals should refrain from using electronic digital devices for two to three hours before going to bed or use a blue light filtering device

Get PDFThe Establishment Clause of the First Amendment has steadily been eroded over the past ten years by the Supreme Court, but it all began with the infamous Lemon test in 1971. In _Lemon v. Kurtzman_, the Court established the Lemon test to create a “clearer” standard to determine if a government policy or action directly or indirectly advanced or accommodated religion in compliance with the Establishment Clause. Over time, the Lemon test has slowly be whittled away, leaving the opportunity for revision in _Kennedy v. Bremerton_. The Court in Bremerton interpreted the clause “by reference to historical practices and understandings.” However, this opinion only opened the door to many more unanswered questions.Recently, cases such as _Trinity Lutheran v. Comer_, _Espinoza v. Montana Department of Revenue_, and _Carson v. Makin_ have challenged the notion of government neutrality when it comes to the Establishment Clause and the Free Exercise Clause. The divided Court in _Trinity Lutheran_ applied the most stringent analysis to determine whether the law that infringed on a party’s right was used to further a compelling interest and whether the interest is furthered by narrowly tailored means. Justice Sotomayor, joined by Justice Ginsburg, strongly dissented, asserting that the Court had changed the relationship between religious institutions and the civil government, or between church and state. As if by premonition, this is exactly what is happening in the current landscape of religious charter schools. Following _Trinity Lutheran_, along came _Espinoza v. Montana Department of Revenue_ nullifying the remaining Blaine amendments in state constitutions. These remaining amendments or “no-aid” provisions prevented state money from being given based on religious use of the funds. Finally, _Carson v. Makin_ came crashing in to scramble the First Amendment even further. The Court held that Maine’s “nonsectarian” requirement for their tuition assistance program for students violated the Free Exercise Clause since the “program operated to identify and exclude otherwise eligible schools on the basis of their religion.”Public education is now on the chopping block as parochial public charter schools may become a real possibility as a result of the Supreme Court’s decision in _Trinity Lutheran_. Currently, only Oklahoma, has approved an application for a parochial school, St Isidore of Seville Virtual Catholic School. In summer 2024, however, the Oklahoma Supreme Court, ruled that a parochial charter school was unconstitutional. This will likely not be the end of the road for St. Isidore. Furthermore, Oklahoma is likely to be only the first state to challenge the unconstitutionality of parochial charter schools.States are stuck between a rock and a hard place in trying to prevent public funding from being disbursed to religious charter schools. States have only three options: completely eradicate charter schools and other tuition assistance programs; argue that as state actors, the Establishment Clause prevents religious charter schools; or argue that as private actors, charter schools are in a league of their own and not confined to the reasoning in _Trinity Lutheran_. The only way to prevent funding for parochial schools is to take the necessary measure of repealing state statutes allowing for charter schools, voucher programs, and tuition assistance. States should reinforce the separation between church and state by putting a nail in the coffin of charter schools to prevent the disastrous consequences of allowing parochial charter schools to devastate the public school system
Use of the Erector Spinae Plane Block as an Option for Perioperative Pain Management in Cardiac and Thoracic Surgical Patients: An Educational Model
Get PDFProblem Statement: Cardiothoracic surgical patients are at increased risk for inadequate pain control in the perioperative period due to a significant level of surgical pain and limitations in multimodal pain management. A strong level of evidence supports use of the erector spinae plane block (ESPB) over thoracic epidural analgesia (TEA) and thoracic paravertebral block (TPVB) in cardiothoracic surgery; however, adoption of this technique into practice is lacking.
Purpose: The primary project aim was to increase knowledge and awareness of ESPB in nurse anesthetists.
Methods: An educational intervention along with pre- and post-testing were utilized to assess for change in participants’ attitudes and knowledge of ESPB. The educational intervention highlighted block overview, technique, performance in literature, risks, benefits, & indications.
Sample: Nurse anesthetists were recruited from Lincoln Memorial University Nurse Anesthesia clinical sites, Council on Accreditation’s program directory, and professional contacts already known to the primary investigator.
Analysis: Paired t-testing was conducted, and P-values \u3c 0.05 indicated improved attitude and increased knowledge occurred. Shapiro-Wilk was conducted to assess normality. Non-parametric testing, Wilcoxon signed rank, was also incorporated due to a smaller sample size (n=13).
Implications for Practice: This project supports the online educational intervention as an effective way to increase awareness and knowledge of ESPB in nurse anesthetists. Increased outreach, targeted at community hospitals and rural practice settings is warranted. Future studies evaluating practice barriers in rural settings and incorporation of ESPB into regional training for nurse anesthesia training programs is warranted

The school-to-prison pipeline systematically removes minority children from the classroom. This process facilitates their entry into the justice system through school punishments, the use of school resource officers, and the harsh conditions of the juvenile justice system which has devastating effects on the minority youths in America. To correct this problem, action must be taken at every step along the pipeline to completely alter the behavior that destines the minority youth of America to incarceration. These key steps include reforming school discipline, removing school resource officers from the schools, and educating and encouraging juvenile defense attorneys to delve deeper into their juvenile clients and take a holistic approach to representation
