Abstract 22 
Introduction: In severe intra-articular fractures of the distal tibia, two-stage surgery 23 
with an external fixator is the standard procedure. In most cases, a modular external 24 
fixator is used; however, we have used the Ilizarov external fixator as a simple fixation 25 
method for temporary fixation. The purpose of this study was to report the results of 26 
two-stage surgery using the Ilizarov external fixator for intra-articular fractures of the 27 
distal tibia.  28 
Materials and Methods: The subjects were 39 patients treated with an external fixator 29 
as temporary fixation for intra-articular tibial fractures. Patients were divided into the 30 
Ilizarov group (27 cases, 27 limbs) and the Modular group (12 cases, 14 limbs). The 31 
time to definitive surgery and external fixation, Burwell criteria, and postoperative 32 
complications were compared between the groups.  33 
Results: There was no statistically significant difference in the time to definitive 34 
surgery and external fixation between the groups. The Burwell criteria were 35 
significantly better in the Ilizarov group than in the Modular group for both objective 36 
and subjective criteria (P = 0.037 and 0.022, respectively).  37 
Conclusions: The Ilizarov external fixation as a simple fixation method for temporary 38 
fixation in severe intra-articular fractures of the distal tibia provided sufficient fixation 39

Introduction 44 
The treatment of severe ankle and pilon fractures is often difficult 1-3). Temporary 45 
fixation with an external fixator promotes dislocation reduction by ligamentotaxis and 46 
healing of soft tissue injuries, and it plays an important role in the waiting period for 47 
definitive surgery and in the local treatment of open wounds 4). There are two main 48 
types of external fixators: modular external fixators and ring external fixators, such as 49 
the Ilizarov fixator 5,6). A modular external fixator was pin-bar system, in which pins 50 
inserted into the tibia and calcaneus were connected by a bar to apply an opening force 51 
to the ankle joint, and was used in the temporary fixation of ankle and pilon fractures 52 
because the technique with a modular fixator is simpler than that with the Ilizarov 53 
external fixator 2, 7-9) (Fig. 1-A). The Ilizarov external fixator was a wire-ring system and 54 
more commonly applied in deformity correction because of their ability to apply high 55 
levels of compression and control distraction and orientation in three dimensions 6) (Fig 56 
1-B). We had performed temporary fixation with a modular fixator, and also with the 57 
Ilizarov external fixator. However, we considered the technique of the Ilizarov external 58 
fixator excessive as the temporary fixation of ankle and pilon fractures. Therefore, we 59 
have performed a simplified technique of the Ilizarov external fixator as the temporary 60 
fixation of those fractures since 2010 (Fig. 1-C). Conventional Ilizarov external fixator 61

is generally used four rings, on the other hand, the simplified Ilizarov external fixator is 62 
used two rings. To our knowledge, there have been no reports of the clinical results 63 
using simplified fixation with the Ilizarov external fixator as temporary fixation for 64 
distal tibial fractures.  65 
The purpose of this study was to report the clinical results of patients who underwent 66 
definitive surgery for intra-articular fractures of the distal tibia after temporary fixation 67 
with a simplified Ilizarov external fixator. We also compared the clinical results using a 68 
Modular external fixator for those fractures. 69

Materials and Methods 71 
This study was approved by our institutional review board (U20-12-012). We 72 
retrospectively evaluated consecutive patients who were treated for intra-articular 73 
fractures of the distal tibia, namely ankle and pilon fractures, with an external fixator as 74 
temporary fixation at two centers between December 2010 and December 2018. 75 
Inclusion criteria consisted of patients with an age 20 years or older who treated for 76 
those fractures at our institution with a modular external fixator or other institution with 77 
the simplified Ilizarov external fixator. In total, 47 limbs in 45 patients were identified. 78 
Six patients (24%) were excluded from the study because of incomplete medical records 79

(1 patient), loss to follow-up after less than 6 months (4 patients), and neurological 80 
disorders caused by trauma affecting gait and lower limb motion (1 patient). A final 81 
total of 41 limbs in 39 patients were included. Two patients had bilateral injuries. The 82 
mean age of the 39 patients was 48.8 ± 15.4 years, and the patients comprised 31 males 83 
and 8 females. In our study, the main surgical indication for two-stage surgery was poor 84 
joint congruency after closed dislocation reduction. Patients were divided into two 85 
groups based on the external fixator used: Ilizarov external fixator or modular external 86 
fixator. The Ilizarov external fixator group (group I) comprised 27 limbs in 27 patients, 87 
and the modular external fixator group (group M) comprised 14 limbs in 12 patients. 88 
Four experienced surgeons treated the patients. Two of the four experienced surgeons 89 
used a simplified Ilizarov external fixator and the other two experienced surgeons used 90 
a modular external fixator. The evaluation parameters were Arbeitsgemeinschaft für 91 
Osteosynthesefragen and Orthopaedic Trauma Association (AO /OTA) classification, 92 
the wait time from injury to external fixation, wait time from injury to definitive surgery, 93 
fixation method for definitive surgery, Burwell criteria, and postoperative complications. 94 
AO/OTA classification classifies and codes fractures and consists of bone and segment 95 
number and fracture morphology: bone and segment number 43, distal end segment of 96 
the tibia; segment number 44, malleolar segment of the tibia/fibula; fracture 97

morphology for end segment type A, extra-articular facture; type B, partial articular 98 
fracture; type C, complete articular fracture. The Burwell criteria are based on three 99 
separate scales (good, fair, poor) for subjective (pain, walking ability) and objective 100 
criteria (range of motion of the ankle joint) 10). 101 
1. Surgical techniques 102 
With fixation using the simplified Ilizarov external fixator, we used two rings instead of 103 
the usual four rings. Under general anesthesia, two wires were passed through the 104 
calcaneus from the distal ring, and after applying traction to the distal ring with both 105 
hands and reduction of the fracture by ligamentotaxis, one wire and one half-pin were 106 
inserted into the distal tibia from the proximal ring for fixation (Fig. 2).  107 
The modular external fixation was constructed with a uniplanar frame. A through-pin or 108 
half-pin was inserted into the calcaneus, and two half-pins were inserted into the 109 
anterior medial side of the tibia and fixed after traction.  110 
2. Statistical analysis  111 
Statistical analysis software (JMP version 16.0.1; SAS Institute, Cary, NC, USA) was 112 
used for the statistical analyses. The Mann–Whitney U test was used to compare the 113 
wait times from injury to external fixation and from injury to definitive surgery between 114 
the groups. The chi-square test was used to compare the Burwell criteria and the 115

presence of postoperative complications between the groups.  116

Results 118 
Table 1 shows the breakdown of the fractures in groups I and M. There were 16 ankle 119 
fractures and 25 pilon fractures. There were no statistically significant differences in age 120 
at surgery, AO/OTA classification, and wait times from injury to external fixation and 121 
from injury to definitive surgery between the groups. The mean duration of follow-up 122 
was significantly longer in group M than in group I. All patients underwent internal 123 
fixation as definitive surgery. 124 
Table 2 shows the Burwell criteria and complications in groups I and M. Objective and 125 
subjective Burwell criteria were significantly better in group I than in group M (P = 126 
0.037, 0.022). Post-traumatic osteoarthritis was significantly more frequent in group M 127 
than in group I (P = 0.013). There were no significant differences in other postoperative 128 
complications (skin blister, skin necrosis, surgical site infection, delayed union, nerve 129 
palsy, revision surgery requirement) between the groups.  130

Discussion 132 
Temporary external fixation is commonly used in the treatment of intra-articular 133

fractures of the distal tibia until soft tissue healing is achieved 2, 4, 7-9). Staged surgery 134 
using external fixation reduces the rate of deep infection associated with pilon fractures, 135 
and good results have been reported even in patients with severe soft tissue damage of 136 
Ruedi type 3 11-12). Staged surgery has been recommended as a safe treatment for ankle 137 
fractures without compromising the clinical results 13). 138 
The incidence of osteoarthritis was lower in group I than in group M in this study. 139 
Although the mean duration of follow-up was shorter in group I than in group M, this 140 
difference was not considered to have an effect because previous reports have shown 141 
that most post-traumatic osteoarthritis develops within 1 year 14). The reason for the 142 
lower incidence of osteoarthritis in group I may be that the fractures in group I were 143 
better aligned at the time of temporary fixation than those in group M. The reason for 144 
the lower frequency of osteoarthritis with the simplified Ilizarov external fixation 145 
compared to the Modular external fixation was speculated to be that the patient 146 
maintained better aligned even during the waiting time from injury to definitive surgery. 147 
Group I showed significantly better results regarding the Burwell criteria at the final 148 
follow-up than group M. Because the incidence of osteoarthritis was higher in group M, 149 
and the duration of follow-up after definitive surgery was longer in group M than in 150 
group I, osteoarthritis may have progressed further, leading to worse results in group M. 151

The wait time from injury to definitive surgery was not significantly different between 152 
the groups. In previous reports, the wait time for definitive surgery was 12–15.7 days, 153 
and the present study showed comparable results 7, 15-16).  154 
In this study, there were no significant differences in skin disorders and wound infection. 155 
In previous reports on pilon fracture, 8%–20% of patients with skin blisters, superficial 156 
necrosis, and superficial infection improved with conservative treatment, such as wound 157 
care and antimicrobial administration, and 2%–16% of patients with deep infection 158 
improved with conservative treatment 17-20).  159 
Our study had several limitations. First, the severity of the fracture type was not 160 
compared, which may have affected the clinical results. By observing the fracture 161 
severity, it is possible to identify the fracture types that would benefit from an Ilizarov 162 
external fixator. Second, we were unable to compare the rigidity of the fixation force 163 
using the simplified Ilizarov external fixator with that of other fixators (e.g., regular 164 
Ilizarov external fixator, modular external fixator). However, it may be possible to 165 
assess the results using the finite element method or other methods. Third, the clinical 166 
results were evaluated using only the three-step scale of the Burwell criteria. It is 167 
possible that a more multifaceted assessment could have been performed if other 168 
clinical scores had been used. 169

The fixation method using a simplified Ilizarov external fixator in this study was 170 
associated with a lower incidence of osteoarthritis after definitive surgery than that 171 
using a modular external fixator, and there were no differences between the groups 172 
regarding the time from injury to definitive surgery and the incidence of complications. 173 
These results suggest that external fixation using the simplified Ilizarov fixator is a 174 
useful method. 175

要旨  
序論：プロテイナーゼ 3 抗好中球細胞質抗体 (PR3-ANCA)は，多発血管炎性肉芽腫症の血
清学的マーカーである．しかし最近の研究では潰瘍性大腸炎 (UC)の診断マーカーとして
の役割も示されている．本研究は，UC の疾患重症度，病変範囲および臨床経過における
PR3-ANCA の臨床的役割を評価するために行われた．  
方法：血清 PR3-ANCA は，1 ヵ月以内に診断された 77 人の新規発症患者を含む 173 人の
UC 患者，110 人のクローン病患者，48 人の炎症性腸疾患以外の腸疾患患者および 71 人
の健常対照者で測定された．PR3-ANCA 力価と，疾患の重症度，病変範囲および臨床経
過などの臨床データとの関連を評価した．PR3-ANCA 測定の臨床的有用性は，受信者動
作特性 (ROC)解析により評価された．  
結果：PR3-ANCA≧3.5U/mL は，全対象患者における UC 診断に対して感度 44.5%，特異
度 95.6%を示した．また PR3-ANCA 陽性率は 77 人の新規発症 UC 患者でより高かった
(58.4％ )．新規発症群では，PR3-ANCA 陽性群の方が陰性群よりも疾患の重症度と病変範
囲が高度であった (p <0.001)．治療後，partial Mayo score は PR3-ANCA 力価とともに有意
に低下した．寛解導入療法にステロイドを要した患者の割合は，PR3-ANCA 陽性群で陰
性群患者より有意に高かった．ROC 解析では，新規発症 UC 患者におけるステロイド導
入に対し PR3-ANCA≧3.5U/mL は感度 75.0%％，特異度 69.0%％であった．  
結論：本研究の結果から，PR3-ANCA 測定は UC 診断だけでなく，疾患の重症度や病変範
囲拡大の評価，臨床経過の予測にも有用であることが示唆された．  
 
序論  
炎症性腸疾患 (IBD)は、潰瘍性大腸炎 (UC)とクローン病 (CD)からなる慢性の腸管炎症を
引き起こす疾患である．UC および CD は，特徴的な内視鏡的所見および組織学的所見に
よって診断される．しかし UC および CD の疾患特異的な血清学的マーカーが特定されれ
ば，侵襲的検査による診断手順を回避できる可能性が報告されている．CD の診断マーカ
ーとしては抗 Saccharomyces cerevisiae 抗体 (ASCA)，Pseudomonas fluorescens 関連配列 I2
に対する抗体，Escherichia coli 外膜ポリン C に対する抗体，細菌フラジェリンに対する
抗体，CD ペプチドに対する抗体などが報告されている．UC の診断マーカーとしては核
周囲抗好中球細胞質抗体 (p-ANCA)が報告されてはいるものの決定的なマーカーがないの
が現状である．  
ANCA には p-ANCA と細胞質 ANCA(c-ANCA)があり，後者は多発血管炎性肉芽腫症の
特異的マーカーである．アズール顆粒中のセリンプロテアーゼであるプロテイナーゼ
3(PR3)は c-ANCA の抗原である．2013 年に PR3-ANCA 測定が UC 診断に有用であること
が初めて報告され，我々の以前の研究でも PR3-ANCA のカットオフ値 (3.5 U/mL)は UC 診
断の感度が 39.2%，特異度が 96.1%であった．さらに最近では PR3-ANCA 測定が世界中で
UC 診断に有用であることが報告されている．しかし PR3-ANCA が疾患の重症度，病変範
囲および臨床経過にどのように影響しているかは不明である．  
本研究では，2 施設での UC 患者を対象に PR3-ANCA の診断能を検討し，PR3-ANCA 力
価と疾患の重症度，病変範囲および臨床経過との関連を分析した。  
 
方法  
対象

本研究は 2015 年 7 月 1 日から 2020 年 6 月 30 日まで福岡大学病院と久留米大学病院で
行われた．173 人の UC 患者の血液検体が用いられ，77 人の新規発症 UC 患者の血液サン
プルは初回もしくは 2 回目の受診時に，それ以外の 96 人の維持治療中 UC 患者の血液サ
ンプルは臨床経過中のランダムな時期に採取された．なお UC の診断は本邦の診断基準に
基づき臨床所見と内視鏡的所見および組織病理学的所見により評価された．UC の疾患重
症度は，173 人の患者の partial Mayo score(0–1 ; 寛解，2–4 ; 軽症，5–6 ; 中等症，7–9 ; 
重症 )および血液検体の採取 2 週間以内に下部消化管内視鏡検査を受けた 135 人の UC 患
者の Mayo score(0-2 ; 寛解，3-5 ; 軽症，6-10 ; 中等症，11-12 ; 重症 )に従って定義され
た．  
またコントロールとして，CD(n = 110)， IBD 以外の腸疾患 (n = 48)および健常対象者 (n 
= 71)から血液検体が採取された．  
本研究は，福岡大学および久留米大学の倫理委員会によって承認された．  
 
自己抗体測定法  
血清 PR3-ANCA 力価は化学発光酵素免疫測定キット (STACIA®MEBLux™テスト ; Medical 
and Biological Laboratories，愛知県，日本 )を使用して測定された．カットオフ値は> 3.5 
U/mL とした．  
 
統計分析  
データは JMP Pro バージョン 16 統計分析ソフトウェア (SAS Institute Inc.，ノースカロ
ライナ州ケアリー，アメリカ )を用いて解析した．スチューデントの t 検定，分散分析，
マン・ホイットニーU 検定，クラスカル・ウォリス検定，カイ 2 乗検定，フィッシャーの
直接確率検定，対応のある t 検定およびウィルコクソンの符号付き順位検定が適切に使用
された．相関の強さは相関係数 r によって評価された．PR3-ANCA 力価が UC の寛解導入
療法におけるステロイドの必要性を予測できるか，受信者動作特性曲線 (ROC)を解析し
た．統計学的有意性は P <0.05 とした．  
 
結果  
UC の PR3-ANCA 陽性患者と陰性患者の臨床的特徴の違い  
全 UC 患者の臨床的特徴を表 1 に示す．UC 患者 173 人中，男性が 90 人，女性が 83 人
で，平均年齢は 40.4 歳 (範囲 12〜84 歳 )であった．表 2 にコントロール群を含む対象者の
臨床的特徴を示す．CD， IBD 以外の腸疾患および健常対象者と比較して，PR3-ANCA は
UC で高い陽性率であった．UC 対非 UC の感度は 44.5%，特異度は 95.6%であった．図 1
は各群の PR3-ANCA 力価を示しており，UC の PR3-ANCA 力価は他の群よりも有意に高
かった．次に PR3-ANCA 陽性患者と陰性患者の臨床的特徴を表 3 に示す．Partial Mayo 
score に基づく疾患重症度は，PR3-ANCA 陰性患者に比べ陽性患者で有意に高値であった
(p <0.001)．図 2 は Partial Mayo score による重症度別の PR3-ANCA 力価を示したものであ
る．寛解と軽症，中等症，重症および軽症と重症の比較では，重症度が高いほど PR3-
ANCA 力価は有意に高値を示した．さらに血液検体採取の前後 2 週間以内に下部消化管内
視鏡検査を受けた 135 人の患者を対象に PR3-ANCA 力価と Mayo score の相関関係を評価
した．図 3 は PR3-ANCA と Mayo score の間に軽度の相関関係があることを示している (r

= 0.3555，p <0.001)．  
 
新規発症 UC の PR3-ANCA 陽性患者と陰性患者の臨床的特徴の違い  
次に，治療因子によるバイアスを除外するために，1 か月以内に UC と診断され，未治
療もしくは 5-アミノサリチル酸 (5-ASA)のみで治療されている 77 人の患者の PR3-ANCA
力価を評価した (図 4)．77 人の新規発症 UC 患者の 45 人 (58.4%)で PR3-ANCA 陽性であっ
た．以前の研究で報告されているように PR3-ANCA は未治療群でより高い陽性率であ
り，PR3-ANCA 測定が UC の診断に有用であることが示唆された．表 4 は新規発症例での
PR3-ANCA 陽性患者と陰性患者の臨床的特徴の違いを示しており，PR3-ANCA 陰性患者
に比べ陽性患者は有意に病変範囲が広く (p =0.029)，Mayo score に基づく重症度が高かっ
た (p <0.001)．図 5 では新規発症例における PR3-ANCA 力価と Mayo score に基づく重症度
との相関を評価したが，明らかな相関関係は認めなかった．  
 
PR3-ANCA 力価は個々の患者における UC の疾患重症度を表している  
PR3-ANCA 陽性の UC 患者を対象に，治療前後の PR3-ANCA 力価と Partial Mayo score
を検討した．173 人の患者のうち，治療介入 1 年以上後に再度 PR3-ANCA を測定し得た
21 人の患者 (そのうち新規発症患者が 15 人を含む )における Partial Mayo score および PR3-
ANCA 力価の変化を図 6 に示す．21 人の患者の多くで Partial Mayo score が改善し，PR3-
ANCA 力価も低下した．PR3-ANCA 力価は治療前よりも治療後の方が有意に低かった (p = 
0.018)．この結果から PR3-ANCA 力価が測定時における個々の患者の重症度を表すことが
示唆された．  
 
PR3-ANCA 力価と臨床経過との関連  
次に新規発症患者 77 人の臨床経過を調査した．表 5 に新規発症例における PR3-ANCA
力価ごとの治療法の比較を示す．PR3-ANCA 力価が 3.5 U/mL 以上の 80.0% (36/45)で寛解
導入療法にステロイドを導入したのに対し，1.1〜3.4 U/mL では 50%(8/16)，1.0 U/mL 以
下では 25% (4/16)であった．PR3-ANCA 高力価は，ステロイド導入と有意に関連していた
(p <0.001)．さらに  UC 患者における寛解導入療法中のステロイド導入に対する PR3-
ANCA 力価の臨床的有用性について ROC 解析を用いて評価した．図 7 に示す ROC 解析で
は，ROC 曲線下面積は 0.759 で，PR3-ANCA 力価が 3.5 U/mL 以上の場合，感度 75.0%，
特異度 69.0%であった．従って UC 患者における PR3-ANCA 力価が 3.5 U / mL 以上の場
合，臨床経過中のステロイド導入の予測マーカーとなり得ることが示唆された．  
 
考察  
血清学的バイオマーカーは，CD，UC， IBD 以外の腸疾患，健常人の鑑別だけでなく，
疾患の重症度や臨床経過の予測にも有用であると期待されている．  PR3-ANCA が UC の
診断バイオマーカーであることが近年明らかになった．欧米では，PR3-ANCA は CD 患者
よりも UC 患者で多く検出された (感度 52.1%，特異性 97.3%）．スイスの 61 人の小児患者
において，PR3-ANCA は UC 診断において高い感度と特異度を示した (それぞれ 58%と
93%）．中国では，PR3-ANCA の感度は 57.1%，特異度は 98.9%であった．また本研究で

は，新規発症 UC 患者における PR3-ANCA の陽性率は 58.4%であった．PR3-ANCA は世
界中の UC 診断において高い感度と特異度を示している．  
これまでの研究では，血清学的バイオマーカーは IBD 患者の重症度評価や治療反応性
の予測には十分ではないことが示されていた．しかし，PR3-ANCA 力価は疾患の重症度
と相関し，全結腸炎型の患者では他の病型の患者よりも高い陽性率であった．さらに，
PR3-ANCA 力価の変化は，個々の患者における臨床経過中の疾患の重症度と関連してい
た．これらの結果から，PR3-ANCA 力価は UC 診断，疾患の重症度および病変範囲の評価
のための血清学的バイオマーカーとなり得ることが示唆された．  
PR3-ANCA 力価は UC の診断バイオマーカーとして報告されているが，PR3-ANCA 力価
と臨床経過との関連性は不明である．本研究では，PR3-ANCA 力価が 3.5 U/mL 以上の
UC 患者の 80%(36/45)が寛解導入にステロイドを要したが，一方で PR3-ANCA 力価が 1.0 
U/mL 以下の患者でステロイドを要したのは 25%(4/16)であった．また UC 患者におけるス
テロイド導入の必要性の予測因子として PR3-ANCA 力価の ROC 解析を行った結果，PR3-
ANCA 力価 3.5 U/mL は感度 75.0％と特異度 69.0％であり，PR3-ANCA 力価は臨床経過を
予測する可能性が示唆された．  
本研究にはいくつかの限界がある．UC 患者の腸管の炎症は便中カルプロテクチンや血
清 C 反応性タンパク質では評価されなかった．再発までの時間，ステロイドフリーの期
間，手術や入院率などの他の臨床経過については評価していない．また治療前後の PR3-
ANCA 力価の変化は 21 人の患者でしか評価されなかった．今後は複数の人種を対象とし
た前向き研究において，PR3-ANCA 力価と他の評価法での重症度や様々な臨床経過との
相関を評価し，PR3-ANCA 力価の重症度や治療に伴う変化を検討する必要がある．   
 
結語  
PR3-ANCA 陽性率は新規発症の UC 患者で高かった．PR3-ANCA 力価は疾患の重症度，病
変範囲および臨床経過と関連していた．本研究の結果，PR3-ANCA は UC の診断だけでな
く，疾患の重症度と病変範囲を評価し，ステロイド導入の臨床経過を予測し得る有用な血
清学的マーカーである可能性が示唆された．
