Imaginary Worlds: The Status of Simulation Modeling in Claims for Cost-Effectiveness In Diabetes Mellitus
Over the past 20 years a number of simulations or models have been developed as a basis for tracking and evaluating the impact of pharmacological and other interventions in type 1 and type 2 diabetes mellitus. These models have typically tracked the natural course of these diseases generating long-term composite claims for cost-effectiveness. These claims can extend over the lifetime of the modeled patient cohort. Set against the standards of normal science, however, these claims lack credibility. The claims presented are all too often either immune to failure or are presented in a form that is non-testable. As such they fail to meet the key experimental requirements of falsification and replication. Unfortunately, there is a continuing belief that long-term or lifetime models are essential to decision-making. This is misplaced. The purpose of this review is to argue that there is a pressing need to reconsider the needs of health system decision makers and focus on modeled or simulated claims that are meaningful, testable, reportable and replicable in evaluating interventions in diabetes mellitus.

Medication Adherence Survey: A First Year Pharmacy Immersion Students’ Perspective
First year pharmacy Immersion students from University of North Carolina Eshelman School of Pharmacy used a three question survey during their rotation at Moses H. Cone Hospital that analyzed patients’ medication adherence. Data collection revealed common trends that have been shown in the literature and areas for improvement. This method of evaluation was used by Phase I Immersion students to gain perspective on the problems we continue to have with medication adherence.

Medical Spanish in U.S. Colleges and Schools of Pharmacy
Objective: To determine characteristics of Medical Spanish education provided to pharmacy students in schools and colleges of pharmacy in the United States.
Methods: A survey of U.S. pharmacy schools and colleges was performed to determine availability of Medical Spanish in pharmacy curriculum, course(s) containing Medical Spanish education, and characteristics of Medical Spanish courses. Additional follow-up questions were asked if a school did not offer Medical Spanish.
Results: 61 out of 138 institutions completed the survey (response rate = 44%). 36% (22/61) of respondents reported Medical Spanish education was offered in their curriculum. The most common barrier to offering a Medical Spanish course included a lack of personnel to teach the course (n=21, 54%) or no room in the curriculum (n=15, 38%).
Conclusion: While there is a limited number of institutions that provide Medical Spanish education to their pharmacy students, results of this survey provide a basic description of Medical Spanish education in schools and colleges of pharmacy in the United States. Data obtained from this survey can be used to refine or initiate Medical Spanish courses, including the teaching and assessment methods used.

Developing Students as Advocates through a Pilot Advocacy Curricular Thread within a PharmD Curriculum
There is a need for education about and development of advocacy skills within student pharmacists. Pharmacy literature describes experiences incorporating advocacy education within curricula; however, these are often singular, limited experiences. To increase student pharmacists’ awareness, knowledge, and skills, effective advocacy curricular threads are necessary within curricula. This paper provides rationale for the need for a greater emphasis on development of these skills within student pharmacists, evidence of curricular experiences surrounding advocacy from the literature, initial observations from a piloted curricular thread at one school, and implications for the academy.

An Interdisciplinary Experience focused on Pharmacogenetics: Engaging pharmacy and physician assistant students in conversations about antiplatelet therapy with respect to CYP2C19 genotype
Objective: The goals of the interdisciplinary laboratory were to educate and engage pharmacy and physician assistant (PA) students in a discussion focused on the collection, interpretation, and application of pharmacogenetic data.
Design: Interdisciplinary teams participated in a one-hour, case-based discussion and provided a therapeutic recommendation using the Clinical Pharmacogenetics Implementation Consortium guidelines.
Assessment: All students were surveyed before and after the laboratory on knowledge and application of pharmacogenetics and working in interdisciplinary teams. The interdisciplinary laboratory successfully enhanced the student’s knowledge about sample collection and interpretation of pharmacogenetic information. Additionally, the laboratory improved student confidence in working in interdisciplinary teams to apply pharmacogenetic information to clinical decision making. Furthermore, the majority of students indicated that the interdisciplinary laboratory is valuable and useful in healthcare curriculums.
Conclusion: The laboratory highlighted the differences between pharmacy and PA education regarding PGt, and brought to light several important uncertainties: (1) What is the depth of PGt knowledge that healthcare practitioners need? (2) What are best practices for conveying PGt information?

Transitions of Care Operations at a Family Medicine Clinic: Patient Follow Up and Financial Outcome Analysis
Introduction: Coordinating smooth patient transitions remains a growing area of interest among healthcare professionals in light of the addition of the Readmission Reductions Program to the Affordable Care Act, which enables CMS to penalize hospitals for 30-day readmissions due to myocardial infarction, pneumonia, heart failure, and in due time, COPD. 2 Many facilities have tested varying TOC models previously piloted at other institutions, focusing on ensuring the proper measures are in place before patients depart. However, there is a significant lack in data quantifying the rate of timely patient follow up after discharge, despite the evidence supporting the value of this visit. The purpose of this study is to analyze TOC outcomes at a local family medicine clinic to assess potential lack of billing utilization and gaps of care related to patient follow up.
Methods: The investigators were provided with emergency department discharge charts from the local hospital affiliated with the family medicine clinic; discharge dates ranged from April 2014 to August 2014. Discharge charts were analyzed to establish medical complexity. Investigators used electronic medical records to extract descriptive data to analyze patient and financial outcomes. A subgroup analysis was performed utilizing a subset of patients identified as “established” at the clinic.
Results: A total of 317 unique discharge reports were evaluated, with 104 of those in the “established” patient subgroup. During the study period, 8.8% of the total group of patients demonstrated timely follow-up. Additionally, rate of incorrect billing techniques was 79%. A consequence of the low percentage of patient follow up and improper billing is missed revenue opportunity for the clinic; financial consequences range from 67,150 over the 5 month period.
Conclusion: A coordinated outpatient TOC procedure cannot be determined from this study. However, need for further analysis of outcomes at outpatient facilities has been identified.

Review of Drug Quality and Security Act of 2013: The Drug Supply Chain Security Act (DSCSA)
The Drug Supply Chain Security Act (DSCSA) signed into law in November 27, 2013 by president Obama creates a uniform national standard for tracing drug products through the supply chain. The goal of DQSA is to enhance FDA’s ability to help protect consumers by detecting and removing potential dangerous products from the pharmaceutics distribution supply chain. A new electronic, interoperable system will identify and trace only prescription drugs in the finished form for human use while distributed in the United States. The purpose of this review was to shed light on a complex and complicated process that it will require cooperation between FDA and drug manufactures, wholesale drug distributors, repackagers and dispensers. The implementation of the DSCSA is based on several law requirements and FDA has developed a schedule with time frames for each of them to be executed over a 10-year period. From this review, FDA recommendations are provided through the FDA Guidance on Identifying Suspect Product document to help trading partners and provide information about the risk of suspect drugs entering the supply chain. Moreover, FDA organized on April 5-6, 2016 in Silver Spring, MD a public workshop to gather valuable feedback from stakeholders who shared their input about the implementation of the new electronic system and its requirements. By the end of 2023, a unified system will provide easier data exchange and less errors, and will increase the safety and security of the pharmaceutical distribution supply chain.

Impact of Independent Pharmacist Prescribing on the Future of Pharmacy
Pharmaceutical care as a philosophy is neither universal nor sustainable. Pharmacist prescribing will be the most unifying and transformative innovation in pharmacy practice, globally. Scope expansion supported by prescribing rights for pharmacists will impact pharmacy and other professions: community pharmacy will evolve into an independent profession; and pharmacy and medicine will merge. Important decisions in pharmacy education and practice policy must be made in view of these future changes.

The Relationship Between Treatment Adherence and Non-picked Up Medications in Community Pharmacies
Medication adherence is one of the major factors that determine an outcome of a treatment. Despite the effort of healthcare providers to improve the adherence rate, it still remains a serious issue in our health care system today. Many patients do not take their medications as directed or just simply do not want to fill or refill their prescriptions. A small survey performed by pharmacy students at Temple School of Pharmacy showed that there is a significant number of patients did not pick up their medications. Cardiovascular medications are the most common non-picked up drugs. There are various reasons that patients use to justify for this non-adherence behavior. However, they are unaware that their actions create a tremendous negative effect not only on the treatment itself but also on the entire health care system. The consequences include expensive therapies, inaccurate research data, and poor treatment outcomes. Many solutions have been utilized to solve this problem, but it is still the major problem for healthcare providers to keep in mind when planning a drug regimen. Although many people believed that pharmacists are responsible for solving this problem, it should be a multidisciplinary effort of all healthcare providers to improve the medication adherence.

Building Health Literacy and Cultural Competency Through Video Recording Exercises
Video creation skills would allow pharmacists to create content tailored to their patients’ needs and include relevant aspects of cultural competency to achieve the goal of improved health literacy. Video recording exercises can provide students with an opportunity to build their drug knowledge and health literacy skills.
